Tratamento das fraturas do ter?o distal do rádio pela fixa??o externa e enxerto ósseo

Abstract: the author presents a prospective study in which he uses the external fixation method associated with the autogenous bone graft for the management of articular and metacarpal distal radius fractures. thirty-six patients with a mean age of 52,2 years were treated. the follow-up had na average duration of 36,2 months. the stability of the reduction and its maintenance were assured by the external fixation with the autogenous bone graft. in the patients who underwent a densitometric analysis of the bone mass, the presence of the autogenous mass showed statistically steady in the long run. with this technique the rehabilitation could be antecipated had began in the immediate postoperative period, thas favoring the mobilization. in the fourth week the external fixation device is removed, this restoring the free movement of the wrist, except for the extension which is hampered by an splint of dorsal situation for two additional weeks. in the review of the data resulting from the treatment the anatomical and functional features were considered. the anatomical findings were obtained from radiographic examinations on the patients. the analysis of these outcomes were based on the scheck method (1962) and were graded excellent in 72% of the cases, and good in 28%, satisfactory as a whole. the data related to the function obtained were evaluated based on the green and o'brien system (1978) modified by cooney et al.(1987). in the 24th week, 14% were considered insatisfactory and 86% satisfactory. at 12 months and in december,1999, they were considere satisfactory as a whole. the complications detected during the treatment were: pine site infections in 8,31% of the cases, and transient symptoms of post-traumatic sympathetic dystrophy in 8,33%, all of them thoroughly resolved with appropriate therapy. in 33% of the patients there were also identified signs of post-traumatic arthristis of the ulnal styloid process which however evolved asymptomatically in all cases under review.

Abstract: the aim of this paper was to analyse the anatomical variations of the extensor carpi radialis longusand brevis in sixty male adults cadaver upper limbs. the extensor carpi radialis longus and brevis muscles and tendons had a few anatomical variations, but very important about surgical aspects. the supernumerary tendons were found in 3 hands (all in relation to the extensor carpi radialis longus tendon). the presence of tendinous connections between the two tendons was found in 17 upper limbs. the presence of an accessory tendon making the union between the tendons of extensor carpi radialis brevis and longus was registered in 4 dissected hands. the insertion of the extensor carpi radialis longus tendon was in the radial side of the base of second metacarpal bone and the extensor carpi radialis brevis in the radial side of the base of third metacarpal bone in all dissected hands. the more common pattern of the extensor carpi radialis longus and brevis was one muscle and one tendon of the extensor carpi radialis longus and brevis, localized in the second dorsal wrist compartment, and with a constant insertion in the radial side of bases of the second and third metacarpal bones

Abstract: we recorded with this study that the epl muscle and its tendon show a few anatomical variations. any accessories tendons either the absence of this muscle or its tendon was found. in all preparation the passage of the tendon through the third osteofibroses compartment, crossing on the erbc and erlc going to the thumb was seen. the change of the tendinuous trajectory, deviating itself to the radial direction in one of the preparations was checked (1,6%). the partial duplication of the proximal tendon to the metacarpophalangeal joint (amp) was seen in three observations (4,8%), one bilateral and distal to the articulation in five (8,3%), 2 bilaterally. the bone insertion at the base of distal (bfd) phalanx in all observations were found. during its passage through metacarpophalangeal joint we recorded its insertion in the joint capsule in 14 observations (23,3%), however the bone insertion there was not observed in this place.. the most common pattern about this (epl) extensor pollicis longus muscle was the presence of a tendon, passing through the osteofibroses dorsal of wrist compartment with a bone insertion at the base of distal pollicis phalanx.

Abstract: En el análisis de los tendones de los músculos extensores radiales largo y corto del carpo, basados en la disección anatómica de 60 miembros superiores de cadáveres adultos de raza blanca y sexo masculino, registramos que las variaciones anatómicas no son muy frecuentes. La presencia de tendones accesorios ocurrió en apenas 3 manos disecadas, todas del músculo extensor radial largo del carpo. La presencia de vínculos tendinosos uniendo los tendones, fue registrada en 17 piezas. La presencia de un tendón intermedio uniendo los dos tendones, fue verificada en 4 piezas. La inserción del tendón del músculo extensor radial largo del carpo ocurrió, invariablemente, en el lado radial de la base del segundo metacarpiano y del músculo extensor radial corto en el lado radial de la base del tercer metacarpiano. Basados en nuestras disecciones podemos afirmar que la conformación anatómica normal de los músculos extensores radiales largo y corto del carpo, es la presencia de un cuerpo muscular y un tendón de cada uno de los músculos extensores radiales que son cubiertos por los músculos abdutor largo y extensor corto; pasan por el segundo compartimiento dorsal del pu o y se insertan, el músculo extensor radial largo en el lado radial de la base del segundo metacarpiano y el músculo extensor radial corto en el lado radial de la base del tercer metacarpiano, invariablemente The aim of this paper was to analyse the anatomical variations of the extensor carpi radialis longusand brevis in sixty male adults cadaver upper limbs. Abstract: Registramos pelo presente estudo que o músculo (ELP) e seu tend o apresentaram poucas varia es anat micas. N o registramos a presen a de tend es acessórios e nem a ausência desse músculo ou de seu tend o. Verificamos em todas as prepara es a passagem do tend o pelo terceiro compartimento osteofibroso, cruzando sobre os extensor radial curto (ERCC) e longo (ERLC) do carpo e dirigindo-se ao polegar. A altera o no trajeto tendinoso, desviando-se no sentido radial foi verificada em uma das prepara es (1,6%). A duplica o parcial do tend o proximal a articula o metacarpofalangica (AMFP) foi verificada em 3 observa es (4,8%), sendo 1 bilateral, e distal à articula o em 5 (8,3%), 2 bilateralmente. A inser o óssea na base da falange distal (BFD) foi encontrada em todas as observa es. Durante sua passagem pela articula o metacarpofalangica, registramos sua inser o na cápsula articular em 14 observa es (23,3%), porém n o registramos a inser o óssea nesse local. O padr o mais comum em rela o ao músculo ELP foi a presen a constante de 1 tend o, passando pelo terceiro compartimento osteofibroso dorsal do punho com inser o óssea na base da falange distal do polegar.
Fractures of the distal radius (Colles' fracture)

Abstract: context and objective: although colles' fracture is a common clinical situation for the orthopedist, we did not find any information in the literature that would allow safe decision-making on the best treatment for each fracture type. the aim of the present study was to investigate brazilian orthopedists' opinions regarding the main aspects of treatments for colles' fracture. design and setting: cross-sectional study conducted during the 34th brazilian congress of orthopedics and traumatology. methods: five hundred questionnaires containing 12 items were randomly distributed to orthopedists who were attending the congress; 439 were filled out correctly and were considered in this study. results: the main factors in making decisions on interventions in fracture cases were whether the fracture was intra-articular, the existence of shortening of the distal radius and the patient's age. the classification method most used was frykmann. the closed reduction method most used was manual reduction. the principal surgical interventions were percutaneous pinning (39%), external fixation (27%) and volar plate (19%). most of the interviewees only used bone grafts for osseous gaps in special cases. the most frequent complications were impairment of joint mobility and residual pain. conclusions: brazilian orthopedists have concordant opinions regarding conservative treatment methods and the use of bone grafts. there were conflicting opinions regarding surgical treatment methods, classification types and complications.
Estudo da regenera o nervosa em nervos tibiais de ratos wistar utilizando o Fluoro-Gold como marcador neuronal

Abstract: Os autores estudaram em ratos a regenera o nervosa no enxerto nervoso tradicional, comparando-os com grupo controle, através da contagem no nível da medula espinhal, entre L3 e S1, de motoneur nios marcados por meio de exposi o do nervo tibial ao Fluoro-Gold (FG). No grupo controle, ambos os nervos tibiais foram expostos ao FG , e em 48 horas, após a perfus o, os motoneur nios foram contados. No grupo experimental, foi ressecado um segmento de 8mm do nervo tibial criando uma falha segmentar. O segmento de nervo de um foi usado para reparar o lado contralateral, utilizando sutura epineural. Após quatro meses, o nervo tibial direito foi exposto ao FG após o enxerto nervoso e, o esquerdo, antes do enxerto. Os motoneur nios foram contados após a perfus o. Ambas as medulas foram cortadas em segmentos de 40mm. Para a análise estatística foram utilizados testes de Wilcoxon e Student. O grupo controle apresentou um significativo aumento de motoneur nios quando comparado ao experimental. No grupo experimental, o número de motoneur nios foi significantemente maior, quando a exposi o do nervo tibial ao FG era feita distalmente ao enxerto nervoso. O enxerto nervoso funciona como um bloqueio parcial para a migra o dos ax nios em regenera o.
Artroplastia de excis?o do trapézio e interposi??o tendinosa na rizartrose: estudo prospectivo

Abstract: objective: to prospectively evaluate the results from a series of patients who underwent surgical treatment for rhizarthrosis using the technique of trapezium resection associated with interposition of yarn from the long abductor tendon of the thumb. methods: from may to august 2005, ten patients underwent surgical treatment for rhizarthrosis. patients with primary osteoarthrosis of the trapezium-metacarpal joint, in stages ii, iii and iv of the eaton classification, with persistent pain that was refractory to clinical treatment, were included. for the functional assessment, the visual analogue scale, dash questionnaire and buck-gramcko score were used. for the overall assessment on the patients, strength measurements were made for palm grip, pulp to pulp pinch, lateral pinch, three-point pinch, opposition and radial and palmar abductions. the migration index for the first metacarpal was also determined on radiographs at rest and under stress. results: the pain relief was considered to be good (p = 0.005), with functional improvements in modules 2 (p = 0.02) and 3 (p = 0.022) of dash. the buck-gramcko score showed one excellent and three very good results. there was an improvement in almost all of the overall assessment and was only non-significant regarding lateral pinch and abduction. for all patients, there was migration of the first metacarpal. conclusion: trapeziectomy associated with interposition of yarn from the long abductor tendon of the thumb was shown to be a relatively simple and effective technique for pain relief and functional improvement.
