Comparison between Preoperative and Postoperative Sublingual Misoprostol for Prevention of Postpartum Hemorrhage during Cesarean Section: A Randomized Clinical Trial  [PDF]

Abstract: Background: Blood loss is one of the important complications during cesarean section (CS). Previous reports have shown that misoprostol is effective in reducing blood loss during and after CS. However, the optimum time for its administration to decrease the amount of PPH is still under discussion. Objective: To compare the effect of preoperative and postoperative administration of sublingual misoprostol (400 μg) in reducing the amount of blood loss during and 24 hours after CS. Setting: Obstetrics and Gynecology Department, Faculty of Medicine, Assiut University, Assiut, Egypt, between January 2017 and July 2018. Study Design: A prospective, randomized clinical trial. Methods: Four-hundred thirty women fulfilling the inclusions criteria: elective lower segment CS at term (≥37 weeks) with normal fetal heart tracing who accepted to participate in the study. Patients were divided into two groups; Patients assigned to group 1 received 400 μg sublingual misoprostol immediately after urinary catheterization and before skin incision, while patients assigned to group 2 received sublingual misoprostol immediately after skin closure. The primary outcome was the estimation of intraoperative and postoperative blood loss for 24 hours. Results: There was a significant reduction in the intraoperative blood loss in group 1 compared with group 2 (403.51 ± 72.99 vs. 460.99 ± 74.66 ml, respectively). Abstract: Objective: To compare the accuracy of different ultrasonographic modalities; two-dimensional ultrasound (2D-US), color Doppler and three-dimensional power Doppler (3D-PD) in the antenatal diagnosis of the morbidly adherent placenta. Setting: Obstetrics and Gynecology Department, Faculty of Medicine, Assiut University, Assiut, Egypt. Study Design: A cross-sectional study. Methods: All patients fulfill the inclusions criteria: gestational age > 28 weeks, previous one or more cesarean delivery, previous uterine surgery, placenta previa, vitally stable patient and women accepted to participate in the study were included. All patients were evaluated using 2D-US, color Doppler and 3D-PD before delivery. The final diagnosis was established by laparotomy and by histopathology of hysterectomy sample if hysterectomy would be done. Results: One-hundred fifty patients were enrolled in the study. 2D-US has higher sensitivity (86.96%) than 2D color Doppler (84.06%) and 3D-PD (79.71%) in the diagnosis of placenta accreta. Abstract: Introduction: Hemorrhoids are a frequent and familiar concern of patients in the general practice and surgery settings. Colonoscopy is both diagnostic and therapeutic. In Qatar, There are little data available about coincidental pathology in elderly patients with hemorrhoids. Objective: To assess the outcome of colonoscopy in management patients with hemorrhoidal disease and average-risk for colorectal cancer and its clinical significance. Method: A retrospective study of 200 patients with hemorrhoids with average risk colorectal cancer (anal bleeding and anal symptoms) and subjected to colonoscopic examination at Al Khor hospital’s endoscopy unit during the period from May 2005 till August 2012. Patients below 50 years and high risk patients for colorectal cancer; with the following alarm signs: Positive personal history of colorectal neoplasms or Inflammatory Bowel Disease (IBD), positive first degree family history of colorectal neoplasms, history of altered bowel habits, recent significant weight loss, presence of iron deficiency anemia, and history of previous colonoscopy were excluded. All significant endoscopic co-findings (diverticuli, polyps, cancer, angiodysplasia and varices, or colitis) were recorded. Results: There were 200 patients; 134 male, 66 female; Mean patient age was 56.3 years (range, 50 to 82 years), who met the eligibility criteria. 200 colonoscopies were performed. Evaluation of these patients revealed Polyps in 16 patients (13 benign and 3 malignant), seven malignant lesions(one annular lesion and 6 fungating masses), Ulcerative colitis in 5 patients, no crohn’s disease, diverticular disease in 4 patients, no Vascular malformations, bleeding piles in 3 patients controlled by injection, and 65.5% of patients were free from any additional pathology. Colonoscopy changed the treatment plan in 58 patients (29%). No complications were encountered. Conclusion: Coincidental abnormalities in colon and rectum in elderly patients with hemorrhoids are common. Benign polyps, followed by diverticular diseases, anal fissures then colorectal cancer were the commonest findings. Endoscopic evaluation of elderly patients with hemorrhoids and average risk for colorectal cancer is advocated. Omitting endoscopy in these patients can lead to major doctors’ delay.

Abstract:

We have developed a genetic algorithm approach for automatically generating expert advisors, computer programs that trade automatically in the financial markets. Our system, known as GenFx or Genetic Forex, evaluates evolutionarily generated expert advisors strategies using predetermined fitness functions to automatically prioritize parents for breeding. GenFx simulates several key factors in natural selection. It employs a multiple generation breeding population, a notion of gender, and the concept of aging to maintain diversity while providing many breeding opportunities to highly successful offspring. The approach is also especially efficient running in a multiple processor, multiple selection-strategy mode using multiple settings. We found out that a multi-processor gender-based running of the system outperformed all single runs of the system. This system is inspired by GenShade, a previous system that we have developed for evolutionary generating procedural textures. The methods described in this paper are not limited to the Forex market or financial problems only but are applicable to many other fields.

Flavobacterium columnare / Myxobolus tilapiae Concurrent Infection in the Earthen Pond Reared Nile Tilapia (Oreochromis niloticus) during the Early Summer

Abstract: Results: Flavobacterium columnare (F. columnare), the dermotropic Gram negative yellow pigmented bacteria was isolated from different sites of skin ulcerations in the Nile tilapia (Oreochromis niloticus) and Nile catfish (Clarias gariepinus) collected from an earthen pond located at an aquaculture station in Sharkiya Province, Lower Egypt during an acute episode of mass kills during the early summer of 2009. An acute infection with F. columnare was behind the emergent event of mass mortalities among both populations. Many of the Nile tilapias exhibited typical signs of hole - in- the head like lesions from which F. columnare together with the myxosporean spore, Myxobolus tilapiae (M. tilapiae) were retrieved. Most of the cohabitating infected Nile catfishes exhibited severe form of saddle back like ulcer. The identities of the retrieved isolates were confirmed using morphological, biochemical and molecular tools. Conclusion: The current study reports on the seasonal resurgence of mass mortalities due to F. columnare infection in the polycultured Nile tilapia and Nile catfish during the early summer. Further, the endemic existence of the myxosporean spore M. tilapiae concurrently with some environmental stresses (abrupt temperature rise, polluted pond water) and faulty management (high ammonia, high pH and low dissolved oxygen) are the potential initiating factors for such outbreaks.
Obesity and eating habits among college students in Saudi Arabia: a cross sectional study

Abstract: A total of 357 male students aged 18-24 years were randomly chosen from College of Health Sciences at Rass, Qassim University, KSA for the present study. A Self-reported questionnaire about the students' eating habits was conducted, and their body mass index (BMI), body fat percent (BF%), and visceral fat level (VFL) were measured. Data were analyzed using SPSS statistical software, and the Chi-square test was conducted for variables.The current data indicated that 21.8% of the students were overweight and 15.7% were obese. The total body fat exceeded its normal limits in 55.2% of the participants and VFL was high in 21.8% of them. The most common eating habits encountered were eating with family, having two meals per day including breakfast, together with frequent snacks and fried food consumption. Vegetables and fruits, except dates, were not frequently consumed by most students. Statistically, significant direct correlations were found among BMI, BF% and VFL (P < 0.001). Both BMI and VFL had significant inverse correlation with the frequency of eating with family (P = 0.005 and 0.007 respectively). Similar correlations were also found between BMI and snacks consumption rate (P = 0.018), as well as, between VFL and the frequency of eating dates (P = 0.013).Our findings suggest the need for strategies and coordinated efforts at all levels to reduce the tendency of overweight, obesity and elevated body fat, and to promote healthy eating habits in our youth.Obesity is often defined as a condition of abnormal and excessive fat accumulation in adipose tissue to the extent that health may be adversely affected [1]. Abstract:

The integration between advanced techniques for groundwater exploration is necessary to protect and to manage the vital resources. Enhanced Thematic Mapper Landsat (ETM+) images, a geographic information system (GIS), hydrological modeling and direct current (DC) resistivity geoelectrical techniques were used in integrated manner to identify the groundwater potentialities in the study area. The study area is approximately 1195 km2, located at the western portion of south Sinai. From the results of the eight thematic layers as input to GIS model, the suitable locations for dams could be estimated in the two main drainage basins Matulla and Tayiba.

Abstract:

The feasibility of employing stand-alone solar energy systems to meet demand-side loads depends strongly on providing appropriate solar energy storage. The present paper presents an efficient and economical, underground, thermal storage design to store hot water at a temperature of around 180?C required for running a double effect absorption chiller to cool a zero-energy-house in a desert environment. The performance of the design is evaluated employing a specially developed efficient mathematical model, for simulating the steady state radiation, convection and conduction processes occurring within the storage unit. The model is presented and analyzed, and employed to investigate the effects of various design parameters on storage efficiency. It is demonstrated that high storage efficiency may be reached, providing that appropriate insulation materials are used. It is also revealed that the soil conductivity has little effect on storage efficiency.

A new method for classification of Brachiopods based on the radon transformation

Abstract: Brachiopods have a lateral outline which is quite important in systematic studies. It is often assessed by a qualitative evaluation and linear measurements, which are not clear enough and precise for describing the shape of the shell and its changes In this paper we propose a new method for classification of fossils based on the radon transform from their greyscale image. We take the case of brachiopods which has Complex shapes. We use an adaptation of Radon transform called R-transform which is invariant to common geometrical transformations. Each shape is described by R3D transform. We consider the grayscale image as a set of cuts obtained from successive binarization for each gray level in image, and for each segmentation we compute the R-transform then we obtained the R3D transform. The advantages of the proposed method are robustness to noise, and invariant to common geometrical transformations scale, translation and rotation.
Gene Expression Analysis of the Pleiotropic Effects of TGF-β1 in an In Vitro Model of Flexor Tendon Healing

Abstract: Flexor tendon injuries are among the most challenging problems for hand surgeons and tissue engineers alike. Not only do flexor tendon injuries heal with poor mechanical strength, they can also form debilitating adhesions that may permanently impair hand function. While TGF-β1 is a necessary factor for regaining tendon strength, it is associated with scar and adhesion formation in the flexor tendons and other tissues as well as fibrotic diseases. The pleiotropic effects of TGF-β1 on tendon cells and tissue have not been characterized in detail. The goal of the present study was to identify the targets through which the effects of TGF-β1 on tendon healing could be altered. To accomplish this, we treated flexor tendon tenocytes cultured in pinned collagen gels with 1, 10 or 100 ng/mL of TGF-β1 and measured gel contraction and gene expression using RT-PCR up to 48 hours after treatment. Specifically, we studied the effects of TGF-β1 on the expression of collagens, fibronectin, proteoglycans, MMPs, MMP inhibitors, and the neotendon transcription factors, Scleraxis and Mohawk. Area contraction of the gels was not dose-dependent with the TGF-β1 concentrations tested. We observed dose-dependent downregulation of MMP-16 (MT3-MMP) and decorin, and upregulation of biglycan, collagen V, collagen XII, PAI-1, Scleraxis, and Mohawk by TGF-β1. Inter-gene analyses were also performed to further characterize the expression of ECM and MMP genes in the tenocyte-seeded collagen gels. These analyses illustrate that TGF-β1 tilts the balance of gene expression in favor of ECM synthesis rather than the matrix-remodeling MMPs, a possible means by which TGF-β1 promotes adhesion formation.
