Four Cases of Chylous Ascites following Robotic Gynecologic Oncological Surgery

Abstract: Chylous ascites is an uncommon form of ascites characterized by milky-appearing fluid caused by blocked or disrupted lymph flow through chyle-transporting vessels. The most common causes of chylous ascites are therapeutic interventions and trauma. In this report, we present four cases of chylous ascites following robot-assisted surgery for endometrial staging and the treatment strategies that we used. After retroperitoneal lymph node dissection, leaving a drain is very useful in diagnosing chylous ascites and observing its resolution; furthermore, the use of octreotide in conjunction with TPN appears to be an efficient treatment modality for chylous ascites and should be considered before any invasive intervention. 1. Introduction Chylous ascites is an uncommon form of ascites characterized by milky-appearing fluid caused by blocked or disrupted lymph flow through chyle-transporting vessels. The most common causes of chylous ascites are therapeutic interventions and trauma. The increase in the incidence of chylous ascites is attributed to the longer survival of patients with cancer and more aggressive abdominal, retroperitoneal, and cardiothoracic interventions, despite the lack of recent large studies [1]. The recommended treatment options for chylous ascites are therapeutic paracentesis, dietary control with a high-protein, low-fat, medium-chain triglyceride- (MCT-) based diet, total parenteral nutrition (TPN), somatostatin, and surgical intervention [2]. In this study, we present four cases of chylous ascites following robot-assisted surgery for gynecologic malignancies and their management. 2. Case Presentations Case 1. A 58-year-old woman, gravida 6, para 6, with a two-month history of vaginal bleeding was referred to our hospital. Transvaginal ultrasonography revealed a solid 12 × 17?mm lesion in the endometrial cavity. The patient was diagnosed with a carcinosarcoma with heterologous elements following an endometrial biopsy. The patient underwent robot-assisted endometrial staging via the da Vinci S surgical system (Intuitive Surgical Sunnyvale, CA). The procedure included hysterectomy, bilateral salpingo-oophorectomy (BSO), and pelvic and paraaortic lymph node dissection (PPLN) up to renal vein. The patient was discharged on the third postoperative day without any complication. The final pathological finding was a stage IB grade III carcinosarcoma with heterologous elements. Abstract: Objectives. There are limited clinical data revealing the relationship between mitral annular calcification (MAC) and systemic inflammation. The goal of the present study was to compare high-sensitivity C-reactive protein (hs-CRP) levels in patients with and without MAC and investigate the relationship between MAC and hs-CRP. Methods. One hundred patients with MAC who underwent transthoracic echocardiography (TTE) and 100 age-matched controls without MAC who underwent TTE were included in our study. Hs-CRP levels were compared between groups. Results. Prevalence of female gender, hypertension, and coronary artery disease were significantly higher in the MAC group than in the control group (64% versus 45%, =0.007, 42% versus 28%, =0.03 and 37% versus 18%, =0.003, resp.). On multivariate analysis, age, gender, and coronary artery disease were the only independent predictors of MAC. The levels of hs-CRP were higher in the MAC group than in the control group (2.02±0.35 versus 1.43±0.47 mg/dl, <0.001). This increase in hs-CRP levels in the MAC group persisted in patients without hypertension, coronary artery disease, and in male patients when compared to the control group. Conclusions. Our study demonstrated that hs-CRP, which is a sensitive marker of systemic inflammation, increased in patients with MAC.
Affective Factors That Influence Chemistry Achievement (Motivation and Anxiety) and the Power of These Factors to Predict Chemistry Achievement-II

Abstract: In this research, motivation and anxiety for Chemistry course of 819 high school students attending 10 different high schools located in the city center of Mersin, were investigated. Anxiety and motivation scales constituted and developed for Chemistry course by researchers were used as an instrument to collect the data. To provide evidence reliability and validity of scales, test re-test reliability and Cr?, correlation between items and total test scores and factor analysis have been utilized on data. In order to find reach our research problem and sub problems, descriptive statistic, t tests, simple and multiple lineer regression analysis and variance analysis have been performed. In this study, it was determined that while grade 10 students have the highest motivation for this lesson, grade 9 students possess the highest anxiety level for chemistry lesson, as well. Also, it was found out that the motivation and anxiety for chemistry lesson, on their own, is a significant predictor of chemistry achievement.

Abstract: In this research, our aim was to determine students’ perception level of attitude and self efficacy towards chemistry and to put forth effects of these variables on chemistry achievement for consideration (in other words, to determine how the chemistry achievement were predicted by these variables). In this point of view the research was conducted with 819 students studying at the 1st, 2nd and the 3rd grade of 10 high schools which are located in the city center of Mersin. Addressed to research problems, data was analyzed via descriptive, correlation, linear and multiple regression statistical analyses. As a result it is determined that 2nd graders group of high schools has maximum attitude scores and the attitude towards chemistry lesson, on its own, is a significant predictor of chemistry achievement. it is also determined that 2nd graders group of high schools has maximum self efficacy scores and the self efficacy towards chemistry lesson, on its own, is a significant predictor of chemistry achievement.

Abstract: Knowledge-based economic approach becomes the main developmental leverage for the global economy. In this sense, increasing the productivity, the skill qualification of manpower, the national competitive advantages, and attaining sustainable development with stabilisation of growth rates depend upon the use of information and communication technologies within the realm of the targets of knowledge-based economic policies. Turkey has no other chance of fulfilling the requirements for the adequate transformation to the knowledge-based economic guidance. The formation of the information society is the primary requisite for the conduct of a knowledge-based economy, and this, in turn, implies an integration of social mechanisms, social policy and cultural environment into the new economic framework. Thus Turkey faces a difficult task to fulfil. At present, there exist even risks of missing the chance to play an effective part in the global knowledge circulation. Developing a national knowledge policy requires joint efforts of the organisations of social life and the government, at the mutual platform of which issues will be discussed thoroughly. In that respect, the creation of common national values depends upon the intensified global activities of the country in producing, processing, sharing and managing knowledge. A national knowledge policy would optimise the utility of gaining competitive advantages, stabilising economic growth and to make economic development of the country sustainable.
Binomial Additive Modeling for Nonlinear Relationships

Abstract: Objective: In most of the fields, researcher misapplies linear models, although there are more complex models needed. In this study generalized additive modeling of a real life data set is taken into account for binomial response case. Material and Methods: Generalized additive models (GAM) have become an elegant and practical option in model building. Those models represent an extension of generalized linear models (GLM) with a linear predictor involving a sum of smooth functions of covariates. GAMs allow for rather flexible specification of the dependence of the response on the covariates by specifying the model in terms of smoothing functions besides linear components. The advantage in that type of modeling is that the forms of the explanatory variables are not predetermined unlike in linear regression modeling but are constructed according to information derived from the data. This provides significant advantage for the nonparametric modeling over the linear regression modeling. Results: The models that are constructed for different components are interpreted and compared using Un-Biased Risk Estimator criterion. The model which best explains the structure of the dataset using splines in term of UBRE is given.
Two Live Births following Robotic-Assisted Abdominal Cerclage in Nonpregnant Women

Abstract: Introduction. To report the robotic-assisted abdominal cerclage performed in two nonpregnant women and the success of live birth outcomes. Presentation of Cases. A 36-year-old woman with a complaint of recurrent second trimester pregnancy losses and a 35-year-old patient with a complaint of preterm deliveries and cervical insufficiency underwent robotic assisted abdominal cervicoisthmic cerclage placement in nonpregnant period. The two patients had spontaneous pregnancy after the robotic-assisted abdominal cerclage and delivered healthy infants. Discussion. The limitations of traditional laparoscopic abdominal cerclage have been accomplished with robotic surgery advantages especially intuitive movements and increased range of motion. There are only a few studies in the literature including robotic assisted abdominal cerclage in nonpregnant women, and only five successful live birth outcomes were reported. In this paper, we reported the sixth and seventh cases of achieved live pregnancy after robotic assisted abdominal cerclage in the literature. Conclusion. Robotic assisted abdominal cerclage is a good alternative surgical method with successful pregnancy outcomes. 1. Introduction Cervical insufficiency is a condition which is characterized with painless cervical dilatation and is an important factor causing second trimester pregnancy loss. Cervical insufficiency occurs in 0.5–1% of all pregnancies [1]. Extensive cervical conization, routine dilation and curettage for diagnostic and therapeutic purposes, traumatic cervical lacerations during deliveries, and congenital or DES exposure-related abnormalities may lead to cervical insufficiency [2]. Cervical cerclage is a surgical procedure which involves suturing the neck of the cervix with a purse type stitch to keep the cervix closed for the treatment of cervical incompetence. The vaginal route is the most used surgical method for cervical cerclage. The abdominal approach is usually only done if the cervix is too short to attempt a standard cerclage, or if a vaginal cerclage has failed or is not possible. The minimal invasive techniques, such as laparoscopy, have been considered for the abdominal cerclage in the last few years [3–5]. Due to technical difficulties and limitations of the laparoscopic surgery, laparoscopic abdominal cerclage has not gained widespread popularity. Abstract: There are a lot of diseases that carry death risk when these diseases are infected to human body, if early measures are not taken. Thyroid cancer is one of them. In USA, number of thyroid cancer cases resulted in death in only 2013 shows necessity of early fight with this disease. This study aims performance improvement in diagnosis of thyroid cancer with machine learning techniques. Study consists of 3 phases. In the first phase, BayesNet, NaiveBayes, SMO, Ibk and Random Forest classifiers have been trained with thyroid cancer train dataset. In the second phase, trained classifiers have been tested with thyroid cancer test dataset and the obtained performance results have been compared. In the third and last phase, approaches named above have been integrated to algorithm AdaboostMI to show difference between of ensemble classifiers from conventional individual classifiers and first two phases have been repeated. With using ensemble approaches performance improvement has been achieved in diagnosis of thyroid cancer. Also, kappa, accuracy and MCC values obtained from these classifier models have been explained in tables and effects on diagnosis of the disease have been shown with ROC graphics. All of these operations have been carried out with WEKA data mining program.

Abstract: Keratoconus affecting young population is a progressiveeye disease. This disease may remain silent untilcomplain of visual impairment is appeared. Although the previous treatment methods initiallyprovided slightly improvement of visual acuity, couldnot prevent keratoconus progression. However, collagencross-link (CCL) treatment both halts the progression ofthe disease and provides an increased vision. Antifungal susceptibilities and subtype identifications of Candida species, isolated in blood cultures throughout one-year period

Abstract: Purpose: In intensive care units, 10-20% of nosocomial bloodstream infections are composed of Candida species. Candida species of normal flora omits the skin barrier by the way of invasive catheter application or endothracheal intubation and cause infections. In this study, we aimed to determine the distribution and the antifungal susceptibilities of Candida species isolated from blood cultures.Material Methods: Amphotericin B, fluconasole and itraconazole susceptibilities and Minimum Inhibitory Concentrations (MIC values) were measured with M27-A2 broth microdilution method which is recommended by National Committee for Clinical Laboratory Standards (NCCLS).Results: Candida spp were isolated in blood cultures of 23 patients throughout one-year period. Six of them were children and 17 were adults. 16 patients (69%) were admitted at intensive care unit. The most prevalent agent was C. albicans (52%). Amphotericin MIC values were found pretty low. Fluconazole susceptibility was detected 96% in accordance with the other studies.Conclusion: Due to the highest mortality rate of candidemia, especially candida infections emerged in intensive care units should be followed in case of appropriate and sufficient antifungal treatment and the emergence of resistant Candida species.
