Abstract: informatic is an important tool for health professionals, since the use of the technologies of information and communication facilitates their acting in research, teaching and medical care. nowadays, informatics leans toward the migration to free software, hence in the course 2010-2011 a new program for the medical informatics subject was put into use. this intends a flexible approach to this change, wherever the conditions are present. however the implementation of the teaching of free software requires the preparation of the professors and that of textbooks approaching these topics. due to the present lack of literature, an electronic book was created in order to support the teaching of the libreoffice ofimatic package.
Diamel Therapy in Polycystic Ovary Syndrome Reduces Hyperinsulinaemia, Insulin Resistance, and Hyperandrogenaemia

Abstract: For to determine the effect of Diamel on the insulin resistance, insulin sensitivity, and sexual hormones results in women with polycystic ovary syndrome (PCOS). A study was carried out on 37 patients with this disorder. A triple-blind clinical trial was designed in which the Diamel food supplement was compared with a placebo. The women with reproductive ages were randomly distributed in two groups, with 18 and 19 women respectively, and they took Diamel or placebo and were followed up during 6 months with clinical and biochemical evaluation. A significant decrease in the HOMA-IR from the initial value at six months was observed in the group with Diamel. The insulin sensitivity improved considerably in this group. The rate of menstrual recovery was higher in the group with Diamel, and two patients from this group obtained pregnancy. The hormone levels shows a significant decrease in testosterone at 3 months in the group with Diamel compared with the control group. The LH also decreases in the same group when comparing the start with 6 months.We concluded that the Diamel decreases insulin resistance and improves sensitivity to this hormone in women with PCOS, with improvement in the levels of LH and testosterone. 1. Introduction Polycystic ovary syndrome (PCOS) is a heterogeneous disorder characterized by chronic anovulation and the cutaneous effects of hyperandrogenism [1–3]. There is a high prevalence of PCOS in women with type 2 diabetes mellitus, and there is evidence that insulin resistance and associated compensatory hyperinsulinaemia play a central role in the pathogenesis of PCOS in some women [4–7]. The metabolic aspects of this syndrome include insulin resistance, obesity, lipid abnormalities, and an increased risk for impaired glucose tolerance and type 2 diabetes mellitus [7–9]. Hyperinsulinaemia and insulin resistance are more common among women with PCOS than women without the condition regardless of whether they are obese or not [1, 8]. High insulin levels can increase the amount of androgens produced by ovarian theca cells in synergism with LH. It has been suggested that hyperandrogenism and insulin resistance occur early on in life and might even originate during foetal life and detected clinically before puberty [9, 10]. A potential mechanism for insulin resistance in PCOS women increased serine phosphorylation of the insulin receptors [8]. Abstract: an updated review on a new group of drugs that act by different ways and that regulate postprandial hyperglycemia in diabetic patiens by different mechanisms of action was made. the roles played by repaglinide, an insulin secretagogue agent independent of sulphonilureas that helps to restore the physiology of the failure in the initial insulin secreting peak in type 2 diabetics, and by acarbose, an intestinal alpha-glucosidase inhibitor, were stressed. as postprandial hyperglycemia is determinant in the appearance of vascular complications, it was concluded that these groups of drugs may help to attain a better metabolic control and that they represent new drug options to treat type 2 diabetes mellitus.

Abstract: Se hizo una revisión actualizada sobre un nuevo grupo de drogas que actúan por vías diferentes y por distintos mecanismos de acción regulan la hiperglucemia posprandial en los pacientes con diabetes mellitus. Se destacó la repaglinida, agente secretagogo insulínico independiente de las sulfonilureas, que ayuda a restaurar la fisiología del fallo en el pico secretor insulínico inicial, en pacientes con diabetes tipo 2 y de la acarbosa, agente inhibidor de las alfas glucosidadas intestinales. La hiperglucemia posprandial es determinante en la aparición de las complicaciones vasculares, por lo cual se concluyó que estos grupos de medicamentos ayudarían a alcanzar un mejor control metabólico y representan nuevas opciones medicamentosas en el arsenal terapéutico para la diabetes mellitus tipo 2. Abstract: La retinopatía diabética constituye un importante problema de salud por la discapacidad visual que provoca. El tratamiento de elección continúa siendo la fotocoagulación láser, pero en ocasiones hay formas avanzadas en que la respuesta es pobre y la evolución tórpida. Se continúan buscando otras alternativas de tratamiento que puedan mejorar la evolución, como son los esteroides y antiangiogénicos. Con el objetivo de reagrupar información actual sobre este grupo de medicamentos (análogos de la somatostatina), los cuales tienen efectos antiangiogénicos y han sido usados en el tratamiento de la retinopatía diabética proliferativa y el edema macular diabético, se realizó esta revisión. Los análogos de la somatostatina han mostrado ser eficaces, fundamentalmente, en el control de los fenómenos hemorrágicos y proliferativos en la retinopatía diabética proliferativa, y al mejorar también la integridad de la barrera hematoretiniana, ofrecen una alternativa de tratamiento para el edema macular diabético, fundamentalmente, cuando la respuesta al láser no resulta favorable. The diabetic retinopathy is an important health problem due to visual disability provoked. The choice treatment still remains the laser photocoagulation, but sometimes there is advanced ways in which the response is poor and the course is torpid. Other alternatives of treatment are look for to improve the course including the steroids and anti-angiogenic. The aim of present review was to regroup the current information on this group of drugs (somatostatin analogues), which has a anti-angiogenic effect and used in the treatment of proliferative diabetic retinopathy and the diabetic macular edema. The somatostatin analogues have shown to be effective, mainly in the control of hemorrhagic and proliferative phenomena and to also improve the integrity of the hematoretinal barrier, offer an alternative of treatment for diabetic macular edema, mainly when the response to laser is not favorable.

Abstract: introduction: metastatic tumors of hypophyseal gland are infrequent. most are silent lesions discovered accidentally in necropsy. appearance of symptomatic metastasis is however, exceptional. development: we describe here clinical and radiological findings in a female patient aged 69, presenting with a non-differential carcinoma of lung, diagnosed two years a half ago, starting with headache and visual disorders without hypopituitarism and insipidus diabetes. we made a nuclear magnetic resonance and diagnosis was a hypophyseal lesion operated on by trans-esphenoidal route, and pathological anatomy service reports a metastasis of lung carcinoma. conclusions: patient receives chemotherapy, radiotherapy, and monoclonal antibody with a favorable evolution.

Abstract: introduction: present paper was conducted due to the high incidence of traumatic vertebral lesions with medullary lesion or not present in patients seen in the emergency department of "enrique cabrera" hospital. the aim of present paper was to verify a series of patients with unstable vertebral fractures and also to assess its clinical course. methods: an interventional prospective study was conducted in 15 patients presenting with unstable vertebral factures from 1990 to 2001. all patients underwent a lateral and preoperative and postoperative routine radiological study. according the frankel's classification the preoperative and postoperative clinical picture was registered. results: there was a male sex predominance (86.6%) and mean age was of 35,2 years. the more involved vertebral segment was the t12-l1 and the mechanisms causing the predominant lesions were the height falls and the road accidents. ten patients were operated on; in all there was instrumentation and in five we used decompression of nervous elements, as well as the luque instrumentation, transpedicular corpectomy and luque instrumentation, harrington-luque and anterior decompression with ao osteosynthesis. in six patients we achieved good results, regular in two and poor in other two patients. as complication were registered the dura mater and sepsis in the operative site in a case and a rupture in one of the luque wires in other patient. conclusions: early medullary decompression and spinal stabilization are very necessary.
Tuberculosis en la Ciudad de la Habana, 1995-1999

Abstract: OBJETIVO: La tuberculosis es un importante problema mundial de salud que recibe una atención priorizada del Sistema de Salud Cubano. Lo objetivo del trabajo es describir el comportamiento de los indicadores del Programa de Control de Tuberculosis cubano. MéTODOS: A partir de una revisión de los documentos de la vigilancia de la tuberculosis en los registros del Centro Provincial de Higiene y Epidemiología se expone la infraestructura sanitaria, las estrategias del Programa de Control en la ciudad, se describen las tasas de incidencia, indicadores de localización, diagnóstico y manejo de casos. RESULTADOS: Se hallaron 8 sintomáticos respiratorios por cada mil consultantes de medicina general; la tasa de incidencia de tunerculosis en todas sus formas descendió de 16,4 (1995) a 12,0x105 habitantes (1999); la tuberculosis pulmonar descendió de 15,1 a 10,45 habitantes mientras la tuberculosis extrapulmonar ascendió de 1,3 a 1,6 x 105 habitantes, en igual período. Del total de los casos nuevos, el 40-50% fueron identificados en los policlínicos, el 67% fueron diagnosticados por baciloscopías, el 15,2% por cultivos, el 13.8% sólo por evidencias clínicas y radiológicas; el 0,9% y el 1,5%, respectivamente, fueron diagnosticados por biopsia o hallazgos de necropsia. Los grupos de 15-64 a os incrementaron su incidencia en 1996-1997 y disminuyeron en 1998-1999; los casos >64 a os de edad disminuyeron progresivamente de 1995 a 1999; en general, la tasa de incidencia de casos disminuyó. La demora promedio entre primeros síntomas y diagnóstico mejoró de 42 días en 1995 a 28,6 en 1999. CONCLUSIONES: La reversión de la tendencia de la notificación de casos nuevos parece haberse detenido en 1996. La situación de los indicadores de tuberculosis revelan cambios satisfactorios en el período analizado.

Tuberculosis en la Ciudad de la Habana, 1995-1999

Abstract: objective: tuberculosis is a worldwide health problem getting a prioritized attention by the cuban national health system. to describe the main indicators of the cuban tuberculosis control program. methods: based on surveillance data from the provincial center of hygiene and epidemiology, the health care network and strategies of the tuberculosis control program were reviewed; incidence rates, case finding indicators, diagnosis and case management were described. results: eight subjects with respiratory symptoms were found per 1,000 attending general medical care services. the incidence rates of all tuberculosis types declined from 16.4 in 1995 to 12.0 x 105 people in 1999. pulmonary tuberculosis incidence rate was reduced from 15.1 in1995 to 10.4 x 105 in 1999, whereas extrapulmonary tuberculosis had an increment from 1.3 to 1.6 x 105 in the same period. of all new cases, 40-50 % were diagnosed at multispecialty clinics, 67.6% were diagnosed by positive smears, 15.2 % by positive cultures, 13.8 % by clinical and x-rays evidences only; and 0.9 % and 1.5 % were respectively diagnosed by biopsy and necropsy. there was an increase in the incidence rate in the age group 15-64 years in 1996 and 1997 but it declined again in 1998 and 1999. the age group 64 years and over showed a rate reduction from 1995 to 1999. in general, incidence rates diminished in the overall period. the average delay between onset of symptoms and diagnosis improved from 42 days in 1995 to 28.6 days in 1999. conclusions: there seems to be a halt in reporting trends of new cases in 1996. tuberculosis indicators reveal satisfactory changes in the study period.
