OBESIDAD PREGESTACIONAL Y RIESGO DE INTOLERANCIA A LA GLUCOSA EN EL EMBARAZO Y DIABETES GESTACIONAL

Abstract: Antecedentes: La obesidad en mujeres en edad reproductiva es cada vez más frecuente y complica el embarazo aumentando el riesgo de enfermedades como la diabetes mellitus gestacional (DMG). Objetivo: Determinar el riesgo de presentar DMG e intolerancia a la glucosa (ITG) de acuerdo con el índice de masa corporal pregestacional (IMCPG). Método: Muestra de 489 pacientes, se clasificaron por IMCPG y se les realizó una curva de tolerancia oral a la glucosa (CTOG) de 100 g de 120 minutos. Se definió DMG con dos valores alterados en la CTOG e ITG con un solo valor alterado. Se aplicó prueba de chi cuadrada para determinar diferencia entre grupos y se obtuvo un Odds Ratio (OR), con intervalo de confianza del 95%. Resultados: 9 pacientes con bajo peso, 194 con peso normal, 158 con sobrepeso y 128 con obesidad, edad gestacional promedio 31,3 ± 5,6 semanas. El 13% de las pacientes presentaron DMG, el 10,6% ITG. Las mujeres con sobrepeso presentaron un OR de 3,81 para desarrollar ITG (IC95% 1,62-8,95) y un OR de 3,7 para DMG (IC95% 1,65-8,38), mientas que las pacientes con obesidad pregestacional presentaron un OR de 6,6 para desarrollar ITG (IC95% 2,83-15,66) y un OR de 8,8 para DMG (IC95% 4,05-19,51), comparadas con mujeres con peso pregestacional normal. Conclusión: La población mexicana tiene mayor riesgo de DMG que otras poblaciones y mayor prevalencia de sobrepeso y obesidad, por lo que se debe realizar un diagnóstico y tratamiento oportuno de DMG, previniendo complicaciones y diabetes mellitus tipo 2. Does renal replacement therapy increase mortality in the ICU?

Abstract: A recent retrospective study published in Critical Care has questioned the use of RRT in critically ill patients, including those with severe AKI [4]. Compared with conservative treatment, RRT patients showed a higher mortality. The indication for RRT was at the discretion of the physician, and patients receiving RRT presented higher severity and renal impairment scores. After multivariate analysis correcting for severity of illness (Acute Physiology and Acute Chronic Health Evaluation II score and Sequential Organ Failure Assessment score), mortality remained higher in RRT patients. In a multivariate analysis in septic shock patients, however, we observed that both severity scores are not related to mortality, contrary to the maximum Sequential Organ Failure Assessment score [5]. The decision of starting RRT in many patients during their ICU stay may have been related to evolving factors not included in the analysis (worsening severity, overload, progressing hypoxemia, and so forth), which affect mortality but are not detected in the initial snapshot assessment. For instance, it is inconceivable that anuric patients did not receive RRT. On the other hand, the use of RRT in some patients with mild renal impairment may have increased morbidity.The study underlines the idea that RRT is not necessary in many patients presenting mild AKI in the ICU, but may be misleading if we accept that this therapy is deleterious in general.Monique M Elseviers and Robert L LinsWe appreciate the comments of Dr Pesta?a and understand his concerns about a possible misinterpretation of the results by considering our findings in too general a manner.First, however, we have to rectify that the overall Stuivenberg Hospital Acute Renal Failure results published in this journal did not derive from retrospective observations but from a prospective study of 1,303 AKI patients consecutively admitted to the ICU. Abstract:

The nineteenth century’s quest for the missing matter (Vulcan) ended with the publication of Einstein’s General Theory of Relativity. We contend that the current quest for the missing matter is parallel in its perseverance and in its ultimate futility. After setting the search for dark matter in its historic perspective, we critique extant dark matter models and offer alternative explanations—derived from a Lorentz-invariant Lagrangian—that will, at the very least, sow seeds of doubt about the existence of dark matter.

Abstract: This article analyzes from a sequential perspective the possible pathways of people affected by one or another form of eating disorder. In this prospect, I consider three dimensions: the social resources, the market of available therapies and the specific effects that are accumulated along large trajectory within the available therapeutic universe. The article describes the process from the entrance in a therapeutic universe, experienced through exceptional way, toward its transformation into a routine where people acquires competences which are developed in the daily relational universes within fractions of the dominants classes. Este artículo analiza desde una perspectiva secuencial los recorridos posibles de las personas comprometidas con una u otra forma de trastorno alimentario. Para ello, tiene en cuenta tres dimensiones: los recursos sociales, el menú de terapias disponible y los efectos específicos que se acumulan en una trayectoria larga dentro del universo terapéutico disponible. El artículo describe el proceso desde el cual el ingreso, vivido de modo excepcional, en un universo terapéutico se acaba transformando en una rutina en la cual la persona adquiere competencias para desenvolverse en los universos relacionales cotidianos de ciertas fracciones de la clase dominante.

Abstract: In this paper I present, in the first place, the possible links between philosophy and social science, from, on the one hand, the auto-understanding ways of the philosophical work, and in the second hand, philosophical resources effects on the social scientific vocation. Subsequently, I analyse the philosophical protection against sociology. In third place, I study how sociology permits to improve the philosophical reflexivity from a thick description of the hierarchy that organizes the philosophical field: so, sociology of philosophy appears, not as an attack to philosophy, but as an increase of the philosophical work critical capacities. In fourth place, to finish, I explore philosophical nets as understanding criterion of intellectual creativity. The paper presents specific examples which came basically from the history of Spanish and French thought in the XX century. En este artículo se presentan, en primer lugar, los vínculos posibles entre la filosofía y las ciencias sociales, a partir de, por un lado, los modos de autocomprensión del trabajo filosófico y, por otro, los efectos de los recursos filosóficos en la vocación de científico social. Posteriormente, se analizan los modos de protección de la filosofía frente a la sociología. En tercer lugar, se estudia cómo la sociología permite mejorar la reflexividad filosófica a partir de una descripción densa de las jerarquías que organizan el espacio filosófico: la sociología de la filosofía aparece entonces no como un ataque a la filosofía sino como un incremento de la capacidad autocrítica del trabajo filosófico. En cuarto lugar y, para acabar, se exploran las redes filosóficas como criterio de comprensión de procesos de creatividad intelectual. el trabajo presenta en cada uno de sus momentos ejemplos concretos derivados fundamentalmente de la historia del pensamiento espa ol y francés del siglo XX.

Abstract: Dans ce texte, on analysera la trajectoire de trois intellectuels espagnols (Juan Carlos García-Borrón, Manuel Sacristán, Esteban Pinilla de las Heras) pour développer une sociologie de l’échec intellectuel. Pour analyser les différentes modalités d'échec présentes dans leurs récits, il faut distinguer trois p les de l'excellence intellectuelle. Le premier, on le nommera consécration institutionnelle ou capacité des agents à occuper, selon leurs ambitions personnelles, les postes institutionnels qui étaient à leur disposition. Le deuxième p le montre la reconnaissance spécifique que chacun a pu rencontrer auprès de ses pairs : on l'appellera consécration intellectuelle. La capacité à se donner des objectifs intellectuels selon des critères exclusivement délimités par des problèmes du champ intellectuel définit le troisième p le : on l’appellera autonomie créatrice. In this text I shall analyze the career of three Spanish intellectuals (Juan Carlos García-Borrón, Esteban Pinilla de las Heras, and Manuel Sacristán Luzón) from their autobiographic stories, with the aim of developing a sociology of intellectual failure. In order to analyze the different forms of failure in their narratives I shall distinguish between three poles of intellectual excellence: institutional reputation, intellectual reputation, and creative autonomy. So, with the experience of failure as a guide, I shall try to reconstruct the elements which directed their intellectual careers and the possible incompatibilities between the achievement of some and the loss of others.
