Abstract: introduction: generally hospitalization affects familiar relationships and it also obvies to take certain changes in families’ routine. that is why family caregivers have acquired an important role, as they fulfil the needs of the patients. most of the times, this activity is performed by women. aims: the aims of this article are: to explain family care givers profile and the kind of care they provide in the following units: maxilofacial, neurology, neurosurgery and traumatology in the university hospital traumatología y rehabilitación in granada.this article will analyse different needs and problems family caregivers find within the hospital and inform about nurses opinion concerning the role of family caregivers. design: a qualitative design has been used by means of systematic observation and focal group. surveys and in-depth interviews have been carried out within 2 different groups: nurses and auxiliary nurses. the results obtained have been analysed by the programme atlas/ti 2.4. results: the results mentioned above were the appropriate for an hospital in relation with family caregivers as far as activities, level of information, and requests are concerned. conclusion: on the whole atmosphere in hospitals is somehow hostile to family caregivers. for this reason it will be convenient to set up a different kind of professional relations between health staff and family caregivers. moreover, it is necessary to recognize the importance of family caregivers role in the hospital.
Efecto del cultivo hidropónico de tomillo (Thymus vulgaris L.) en la calidad y rendimiento del aceite esencial

Abstract: thyme (thymus vulgaris l.) is an aromatic plant, whose consumption is increasing at rates that are two or three times the world's population growth, which is generated for the essential oil use in food, cosmetic and pharmaceutical industries. under this perspective and with the need for obtaining higher yields of essential oil than those obtained in open fields, the aim of the present work is to evaluate both essential oil quality and yield of thyme cultivated in a greenhouse under an open hydroponic system. three growing densities were evaluated: d1, d2 and d3 (14, 28 and 71 plants·m-2, respectively) and two concentrations of the steiner nutrient solution (s1: 100 % and s2: 50 %). a 3x2 factorial experiment with four replicates was used for each treatment. the results show that higher values for plant height were presented with s1 d2, and s2 d1, for the stem diameter with s1 d2 and s1 d3; no statistical difference was presented among the treatments for fresh and dry weight, although the largest number of leaves was presented with s1 d2 and s2 d1; the largest area leaf was only reached with s1 d2. in the analysis of the oils four components were identified (ρ-cymene, α-terpinene, carvacrol and thymol) of which the most abundant was thymol with 23.331 %. the use of hydroponic crops allows cultivation of high sowing density species and to obtain a high quality product, recommending s1 d2 as the best treatment where thyme ideal growth is obtained in all the variables.
Aplicación de la Cromatografía de Gases Bidimensional en el Análisis de Derivados Azufrados y Nitrogenados en Gasolina

Abstract: the comprehensive two-dimensional gas chromatography (gc×gc) is used as a powerful tool for an improved characterization in sulphur and nitrogen speciation in gasoline. there are shown the results obtained of the connection between gc×gc and a specific detector, sulphur (scd) and nitrogen (ncd). this technique is based on the separation of two orthogonal dimensions, achieved by two chromatographic capillary columns with different polarity. the final chromatogram obtained was structured according to the volatility and polarity of hydrocarbons which facilitates its identification. the results are a very promising alternative in the characterization of these types of compounds in middle distillates. this work illustrates the high resolution power of the gc×gc in combination with a selective detection of sulphur or nitrogen compounds in gasoline when a specific detector is used.

Abstract: Se emplea la Cromatografía de Gases Bidimensional (GC×GC) como una poderosa herramienta en el análisis de derivados con azufre y nitrógeno presentes en gasolinas. Se presentan los resultados obtenidos del acoplamiento de la GC×GC a un detector especifico, azufre (SCD) y nitrógeno (NCD). La técnica se basa en la separación en dos dimensiones ortogonales que son el resultado de la asociación de dos columnas de GC de diferente polaridad. El cromatograma final obtenido ésta estructurado en función de las propiedades de volatilidad y polaridad de los hidrocarburos, lo que facilita su identificación. Los resultados son muy prometedores en la caracterización de éstos compuestos en mezclas complejas. En este trabajo se ilustra el potencial que tiene el alto poder resolutivo de la GC×GC combinado con una detección selectiva de azufre y nitrógeno en la caracterización de gasolinas al utilizar un detector especifico para éstos contaminantes. Estado nutricional de ni?os con enfermedad renal crónica en la consulta de nefrología pediátrica del Hospital Universitario del Valle, Cali

Abstract: introduction: chronic renal disease (crd) can compromise nutritional status in children. nutritional assessment should be performed in early stages of crd in order to maintain growth, as well as to prevent malnutrition and to diminish progression of renal diseases and metabolic consequences of uremia. objective: evaluation of nutritional status of children with moderate crd stages 2- 4 by means of anthropometry, biochemical profile and dietary survey by records of last three days. methods: a descriptive observational study was performed to a total of 17 patients that attended to pediatric nephrology service at the hospital universitario del valle, from december 2007 to march 2008. results: of the children 65% were males, mean age was 6.2 for males and 10.3 for females. causes of crd were reflux nephropathy in 41%. malnutrition under-2 standard deviation for height and weight for the age was found in 30% of patients. the measurement of triceps fold, showed moderate undernourishment in 75% of the children. according to the biochemical data 5 patients (29%) had serum albumin <3.5 mg/dl. dietary records evidence diminution in the consumption of calories, calcium, iron, zinc. sodium ingestion was over the recommended values. conclusions: these results support that an early nutritional intervention, is essential in children with of crd in early stages; nutritional deficiencies were found in early stages of disease. it is essential to consume 100% of calories and other nutrients to avoid growth deficit and other important alterations.
Issues to Consider in Designing Immunotherapy Clinical Trials for Glioblastoma Management  [PDF]

Abstract: Glioblastoma (GBM) is the most common primary brain malignancy in adults and has a poor prognosis despite standard of care treatment. The mainstay of GBM treatment has relied on maximum surgical resection and chemotherapy and radiation. Cancer immunotherapy has made great strides since the advent of anti-PD-1, anti-CTLA-4, and other immune checkpoint inhibitors. With the advancement of novel therapeutics, more clinical trials for patients have opened as well. An important future direction of clinical trials is the ability to identify appropriate patients to optimize treatment response and minimize toxicities. This review describes considerations in designing future GBM clinical trials in not only immunotherapy but also with other promising treatments. We will discuss factors, such as pseudoprogression, genetic and circulating biomarkers, and the commensal microbiome of patients in the setting of clinical trial design.
Septic shock in pregnancy due to pyogenic sacroiliitis: a case report

Abstract: After the onset of unspecific acute pain in the left buttock region, a 31-year-old pregnant woman developed septic shock due to pyogenic sacroiliitis. The medical and obstetric management, treatment applied and patient's experience are described.The correct diagnosis and treatment of pyogenic sacroiliitis during pregnancy may avoid joint and bone destruction in addition to maternal and fetal complications.The function of the sacroiliac joint is to reduce pelvic stress caused by changes in weight due to body movement. Hormonal effects of pregnancy permit relaxation of the ligaments supporting the sacrum and the pelvic bones. It has been hypothesized that pregnancy sacroiliitis is associated with microscopic areas of injury on the joint surfaces produced by the changes during pregnancy. Pyogenic sacroiliitis has also been related to immunosuppression during pregnancy.Sacroiliac joint disease usually presents with lower back pain that increases with ambulation. The majority of cases represent non-specific and uncomplicated arthritis. Nevertheless, sometimes this joint can be seeded after bacteraemia, resulting in a pyogenic process. The prognosis depends on prompt diagnosis and early start of treatment.A 31-year-old woman, gravida-1 (23 weeks' gestation), para-0, presented with acute severe pain in her left buttock region radiating to the leg and increasing with ambulation. No underlying pathologies or drug abuse were present and no systemic symptoms were encountered. Backache was initially attributed to nerve compression. Nonsteroidal anti-inflammatory drugs and rest were prescribed.After 4 days, the pain became worse. Physical examination revealed a temperature of 39.2 oC, pulse rate of 111 beats/minute, respiratory rate of 43 breaths/minute and blood pressure of 100/50mmHg. The laboratory test results were significant for leukocytes of 5400/mm3 with left shift (92%), haematocrit of 24%, D-Dimer of 946.8μg/L and platelet count of 85,000/mm3. Chest X-ray showed images

Abstract: Se realizó un estudio experimental, del tipo intervención comunitaria, que incluyó a 12 cuidadoras de pacientes con demencia que pertenecían al Policlínico “Armando García” de Santiago de Cuba, en el periodo del 1 de septiembre del 2005 al 30 de igual mes del 2006. Los cuidadores fueron seleccionados por un muestreo no probabilístico intencional por juicio en el que se incluyeron aquellos que vivían más próximos al local donde se realizó la instrucción (consultorio 12), que tuvieran otro familiar o cuidador para quedarse con el paciente en el horario de la intervención, que no tuvieran limitaciones físicas o mentales que dificultaran su asistencia o aprendizaje y que estuvieran dispuestos a participar en el estudio una vez explicadas las características del mismo y los beneficios que reportarían para él y su paciente, así como que se mantuvieran sin ausencias durante la intervención. Se utilizó la prueba de Mc Nemar como método estadístico y el porcentaje como medida de resumen. Entre los resultados más relevantes puede decirse que predominaban los viejos-viejos en el cuidado de sus familiares, mujeres con nivel escolar medio y alto que conocían bastante bien qué es la demencia y sus síntomas, pero desconocían algunas complicaciones y medidas contra el estrés y cuidado del enfermo. An experimental study of the community intervention type, including 12 care givers of patients with dementia that belonged to "Armando García" Polyclinic, was carried out from September 1st, 2005 to September 30th, 2006. Care givers were selected by a judgment intentional non probabilistic sampling that included those living next to the place where the instruction was given (doctor's office 12), those having another relative or caregiver to stay with the patient during the intervention, those not having physical or mental limitations that hindered their attendance or learning and who were willing to participate in the study once its characteristics were explained and benefits to them and their patients were reported, as well as they were present during the intervention. The Mc Nemar test was used as a statistical method, and the percent as a summary measure. Among the most outstanding results the elderly prevailed in caring their relatives, women with middle and higher school level that knew well about dementia and its symptoms, but they ignored some complications and measures against the stress and care of the patient.

Etiología de la neumonía asociada a ventilación mecánica en un hospital clínico: Asociación con co-morbilidad, uso previo de antimicrobianos y mortalidad

Abstract: ventilator-associated pneumonia (vap) is a complication with an increased risk of morbidity and mortality. inadequate antibiotic treatment is a risk factor of mortality which can be improved. for this reason it is important to know the local etiology of vap. during a one year-period we investigated the etiology of vap in a teaching hospital. forty eight vap were included, of which 19 were women. the median age was 59.5 (range 17-91 years), twelve vap were early onset. methicillin resistant staphylococcus aureus (mrsa) was the main microorganism isolated, regardless of timing of diagnosis of vap, followed by polimicrobial etiology, acinetobacter sp and p. aeruginosa. etiology was not associated with comorbidity; however previous antibiotic use was related with mrsa and polymicrobial etiology. mortality was 35% and was mainly associated with p. aeruginosa isolation. conclusion: mrsa was the main cause of vap regardless of the timing of its occurrence
