Alteraciones de la prueba ergométrica en pacientes con fibromialgia Alterations of the ergometric test in patients with fibromyalgia

Abstract: La fibromialgia es uno de los síndromes reumáticos más comunes, caracterizado por dolores musculoesqueléticos y puntos dolorosos sin existir sinovitis ni miositis. Se estudiaron 20 pacientes con diagnóstico de fibromialgia para conocer, mediante la prueba ergométrica, las alteraciones del sistema nervioso autónomo. Se observó, en el grupo control, un incremento normal de la frecuencia al adoptar el ortostatismo lo cual refleja activación del sistema simpático, mientras que en las pacientes con fibromialgia no existió dicho reflejo. Se comprobó incremento adecuado de la tensión arterial en el 87 % de las pacientes con fibromialgia, infradesnivel del ST mayor de 1 mm en el 20 % de las pacientes, ausencia de arritmia y síntomas de cardiopatía en todas las pacientes y disminución de la capacidad física en 4.Se concluyó que las pacientes con fibromialgia presentan una disfunción del sistema nervioso autónomo. Fibromyalgia is one of the commonest rheumatic symptoms, characterized by musculoskeletal pain and painful points with neither synovitis nor myositis. 20 patients with diagnosis of fibromyalgia were studied to know the alterations of the autonomous nervous system by the ergometric test. In the control group, it was observed a normal increase of frequency on adopting orthostatism, which reflects the activation of the sympathetic system. This reflex did not exist among the patients with fibromyalgia. An adequate rise of the arterial pressure in 87 % of the patients with fibromyalgia, an infraunevenness of the ST over 1 mm in 20 % of the patients, an absence of arrhytmia and symptoms of heart disease in all patients, and a reduction of physical capacity in 4, were proved. It was concluded that patients with fibromyalgia present a dysfunction of the autonomous nervous system.

Algunos factores de riesgo y fenómenos psicológicos relacionados con el estado de salud bucal

Abstract: a descriptive study was carried out to 67 fellows older than 15 years assisted by the neighbourhood clinic no. 28 from the previsora community policlinic in the period of april-november in 2006, to determine risk factors and psychological phenomena related to the state of mouth health problems. prevalence was obtained with 33 patients (49.2%) within the ages 30 to 44 years and the state of fairly favorable and unfavorable dentistry problems represented 37,3%. the risk factor that influenced the most in the state of mouth health problems was the biological one (77.6%). when relating the dentistry diagnostic with the perception of risk of that illness, most of the patients perceived it, but they were not able to go to the orthodontist to receive treatment. 61.2% of the patients were afraid and 50,8% corresponded to the state of fairly favorable and unfavorable mouth health problems. the perception of risk to get sick involved 76.1% of the patients

Abstract: 71 patients of both sexes aged 60-75 who were diagnosed extracranial carotid damage, but were asymptomatic from the neurological and cardiovascualr point of view, were studied. all of them underwent high resolution color carotid echodoppler and were divided into categories, according to the vascular damage: 30 with intima media thickness >1,1 mm and 41 con stenosis, in addition to a control group of 30 subjects with vascular risk factor without carotid injury. ekg and an ergometric test were performed in all the individuals to define the diagnosis of silent ischemic heart disease. this diagnostic tool was modified for the elderly. recommendations of conditioning and preventive measures were included for any complication.the variables were codified in a computerized database, and contingency (doble entry) tables were made. chi square test was used for the qualitative variables to evaluate the association between the variables of interest, whereas the t students' test was applied to the quantitative variables.the results were considered significant when the probability of association of the tests was < 0.05. the time of action of the vascular risk factors showed a close relation to the carotid damage, mainly smoking (38.2 years as an average). the behavior of the action of the risk factors on the coronary arteries was evidenced in this same order of frequency by the ergometric test. the coincident relation between the carotid and the coronary damage was proved. there was a higher percentage of positive ergometric tests among the elderly with carotid stenosis (57.7 %).

Abstract: a prospective, descriptive and cross-sectional study the behaviour of infant mortality in a period of 4 years at the health municipalities of plaza de la revolución and playa, in havana city, was carried out. the infant mortality rates due to genetic causes in children under 1 were analyzed. both were similar in the 2 municipalities with a light predominance in the municipality of playa (2.8 per 1 000 live births). infant mortality rate from genetic cause had a trend to decrease in time in these municipalities. on studing infant mortality according to the genetic etiology the deaths produced by multiple factors were the most frequent, which coincides with the reference literature.
Assessment of a telemetry monitoring system (Movicorde) developed in Cuba

Abstract: Introduction The Movicorde system was designed in order to help cardiologists and physiotherapists atexercise training sessions,where constant supervision of relevant parameters is required.Objectives To evaluate the use of Movicorde in high-risk cardiac patients, including the presence of arrhythmias, ST segmentabnormalities and target heart rate.Methods A descriptive, cross-sectional study of 80 high-risk patients who attended the gymnasium of the Institute of Cardiology,from July 2009 through April 2011, in a Cardiac Rehabilitation (CR) program.The measurements taken by Movicordewere compared to those taken by the cardiologists and a data base was created for statistical analyses.Results Sixty two (62) patients had premature ventricular complexes (PVC). The high specificity (99.87 %), sensibility (80,33%)and the 83,42% positive predictive value (VPP) of the system to detect PVC, was described. There was total concordancebetween the equipment and the physicians in 57 patients (71,25%) with respect to ST segment depression.This had a very superior observed value to that expected and showed that the results were representative (p<0,005).Patients who had acute or recent Myocardial Infarction or chronic ischemic heart disease had intermediate total exercise-session duration and their target heart rate had a similar value which was lesser with respect to the rest of thepatients.Conclusions The Movicorde system showed high efficiency for the telemetric analyses of exercise training sessions and its use isrecommended in clinical practice.

Abstract: Se realizó un estudio descriptivo a 67 sujetos mayores de 15 a os pertenecientes al consultorio No. 28 del Policlínico Comunitario Previsora en el período de Abril-noviembre del 2006, para determinar algunos factores de riesgo y fenómenos psicológicos relacionados con el estado de salud bucal. Se obtuvo predominio en las edades de 30 a 44 a os con 33 pacientes (49.2 %) y el estado de salud bucal medianamente favorable y desfavorable representó un 32.8 % y 41.8 % . El factor de riesgo que más influyó en el estado de salud bucal fue el biológico (77.6%). Al relacionar el diagnóstico estomatológico con la percepción de riesgo de enfermedad bucal la mayoría de los pacientes percibían éste pero no eran capaces de acudir al odontólogo para recibir tratamiento. El 61.2% de los pacientes tenían miedo y el 29,9% correspondió al estado de salud bucal desfavorable. Calidad de la atención dispensarial a la cardiopatía isquémica

Abstract: Se realizó un estudio transversal sobre la calidad de la atención dispensarial en una muestra de 129 cardiópatas isquémicos, atendidos en los consultorios urbanos del Policlínico "Centro" del municipio de Camagüey durante 1998, a través de una encuesta confeccionada al efecto que contenía las variables a estudiar, aplicada por los autores y procesada por métodos automatizados. En la serie estudiada predominaron: el sexo femenino (55 %); el grupo de 45 a os y más (92,24 %); el sedentarismo (83,72 %), el antecedente patológico familiar de cardiopatía isquémica (82,17 %), y la hipertensión arterial (72,87 %) entre los factores de riesgo coronario, y la angina de pecho (52,71 %) entre las formas clínicas. Desde el punto de vista de la atención dispensarial, todos (100 %) estaban registrados, pero más de las 2/3 partes no recibieron los controles periódicos en consultas programadas y casi la mitad no mantuvo la adherencia al tratamiento. A pesar del alto porcentaje de controlados, no se consideró eficiente la atención dispensarial A crosswise study of the quality of classified care in a sample of 129 patients with ischemic heart disease, who were seen at the urban physician’s office of "Centro" polyclinics in Camagüey municipality during 1998, was performed by using an specially designed that included variables to be studied, was applied by the authors themselves and automatically processed. The following prevailed in the studied sample: females (55%), the 45 y and over age group (92.24%), the sedentary lifestyle (83.72%), the pathological family history of ischemic heart diseases (82.17%); and blood hypertension (72.87%) among the coronary risk factors and the angina pectoris (52.71%) among the clinical forms. From the classified medical care viewpoint, all the patients were registered, but two thirds were not systematically controlled in scheduled visits whereas almost half of them did not adhere to the medical treatment. In spite of the high percent of patients under control, the classified care was not considered to be efficient

Abstract: Realizamos un estudio prospectivo, descriptivo y transversal de la mortalidad infantil en un período de 4 a os, en los municipios de salud Plaza de la Revolución y Playa, en Ciudad de La Habana. Analizamos las tasas de mortalidad infantil en menores de 1 a o de edad por causa genética y éstas se mantuvieron similares en ambos municipios, con un predominio ligero en el municipio Playa (2,8 por 1 000 nacidos vivo). La mortalidad infantil por causa genética tuvo tendencia a disminuir con el tiempo en ambos municipios. Al analizar la mortalidad infantil según la etiología genética las muertes de causa multifactorial fueron las más frecuentes, lo cual coincide con la literatura de referencia.
