"Research protocol: a synthesis of qualitative studies on the process of adaptation to dependency in elderly persons and their families"

Abstract: Objectives: 1) To present a synthesis of the qualitative evidence available on the process of adaptation to dependency in elderly persons and their families; 2) to conduct an in-depth study into the experiences and strategies developed by both to optimise their living conditions; 3) to enable standards of action/intervention to be developed in the caregiving environment.A synthesis of qualitative studies is projected with an extensive and inclusive bibliography search strategy. The primary search will focus on the major databases (CINAHL, MEDLINE, EMBASE, PsycInfo, PSICODOC, Cochrane Library, JBI, EMBASE, LILACS, CUIDEN, CUIDEN qualitative, CUIDATGE, British Nursing Index, SSCI). The secondary search will be conducted in articles taken from the references to studies identified in the articles and reports and the manual search in congresses and foundation papers. Article quality will be assessed by the guide proposed by Sandelowski & Barroso and data extraction done using the QARI data extraction form proposed by the Joanna Briggs Institute for Evidence-Based Practice.The synthesis of the findings will be based on the principles and procedures of grounded theory: coding, identification and relationship between categories, and synthesis using constant comparison as a strategy.This synthesis of qualitative evidence will enable us to detect health needs as perceived by the receivers in their own interaction contexts.Europe is currently, after Japan, the most aged region worldwide, with 16% of the population aged over 65 years [1]. International predictions also conclude that this ageing will increase more rapidly than in any other region in the world [2].According to these international predictions, Spain (where the present study is projected) will have the oldest population in the world by 2050 [1]. Effectiveness of participatory-action-research to put in practice evidence at a nursing onco-hematology unit

Abstract: the aim was to evaluate the effectiveness of evidence implementation through participatory-action research (par). a prospective quasi-experimental design with two non-equivalent and non-concurrent groups (2006 and 2008) was adopted. the research was conducted at the bone marrow transplant unit of a tertiary-level spanish hospital. to put the evidence in practice, par was adopted as an "intervention studied". the dependent variables were: professional performance and patient outcomes (psycho-emotional area and adverse effects). in total, 125 patients were recruited (1st period=56; 2nd period=69). the results in the second period show significant improvements in professional performance in terms of the quality of the registers of signs and symptoms. in the psycho-emotional area, the psycho-social adjustment improved significantly; without caregiver burden or satisfaction showing any clear tendencies. among the adverse effects, catheter-related thrombosis and catheter-related infection improved significantly; there were no significant differences in the level of pain or mucositis. through the par, evidence could be put in practice and the outcomes under analysis could be improved.

Abstract: within the qualitative research paradigm, the participatory-action- research (par) integrates knowledge and action. it is not necessary to wait for knowledge to be produced to transfer it into practice. it is a method used for decades in disciplines such as education and sociology; however it is emerging in the health field. there are a variety of definitions, classifications and par models; this article aims to shed light upon this method of research, its history, philosophy and its use, given its potential and innovation in the field of health sciences.

Es un método que se utiliza desde hace varias décadas en disciplinas como la educación o la sociología, sin embargo es emergente en el ámbito de la salud. Existen una gran variedad de definiciones, clasificaciones y modelos de IAP y con este artículo pretendemos arrojar luz sobre este método de investigación, su historia, su filosofía y su utilización, dados su potencial y novedad en el ámbito de las ciencias de la salud. Finding evidences on oncohematological patients (1st part): Mucositis, pain and satisfaction

Abstract: Evidence Based Practice (EBP) is used as a tool to update nursing knowledge. It increases nurses′critical appraisal skills, to use the best available investigations and improve quality care in the medium to long term.In order to evaluate the repercussion of the EBP on quality daily cares, it is necessary to asses the impact on patients’ health; in regards to oncohematological nursing, some health outcomes would be: secondary effects, psychosocial factors influence on the admission time and patients satisfaction with nursing carers.Oral Mucositis, and pain associated to it, is one of main secondary effects of treatments for cancer. This complication appears in approximately 40% of the cases.There are countless studies on the subject of Oral Mucositis. However, not much has been published about Nurse Care Patient Satisfaction and Psychosocial Impact.As a consequence, our objective has been: to find and summarize the evidences about Mucositis, Pain associated, and Satisfaction; in order to encourage the spread of knowledge and promote changes in practice.Databases as CINAHL, MEDLINE, Cochrane Library, EMBASE, Pascal Biomed, LILACS, CUIDEN, CUIDEN qualitative y CUIDATGE were revised in Spanish, French and English. No time restrictions were applied.

Abstract: There is an enormous knowledge base in the field of health which has no application. Its growth is not limited by its discovery but by the lack of its implementation. The necessity of implementing conclusions is evident in evaluating the real repercussion on quality daily cares; as “strategies” and “outcomes”. In talking about oncohematological nursing, some health outcomes would be: the reduction of adverse events such as nosocomial infections and pressure ulcers. Consequently, our objective has been: to find and summarize the evidences about Venous Catheterization and Pressure Ulcers; in order to encourage the spread of knowledge and promote changes in practice.Method: Databases as CINAHL, MEDLINE, Cochrane Library, EMBASE, Pascal Biomed, LILACS, CUIDEN, CUIDEN qualitative y CUIDATGE were revised in Spanish, French and English. No time restrictions were applied.Findings: The main findings and recommendations were synthesised on a poster, next to suggestions for practical changes to implement, evidence levels used, and the clinic problem significance. The suggested changes arose from nursing staff based on evidences found that includes behaviours and attitudes changes, which should facilitate more rapid innovations diffusion.

Abstract: Our society keeps on achieving goals regarding individual liberties. This fact has been reinforced since the law 41/2002 (basic law on patient rights, duties and autonomy regarding health information and documentation) was approved on May 16th 2003. This law establishes the ethic boundaries not to be crossed regarding health attention. Additionally to the patient autonomy, this law includes the reference to the living wills, defined as an anticipated declaration about the desired care in case of loosing the capability to give an opinion about it when the end of life is coming.Literature makes clear that living wills are a phenomena in which not only the patients and the health professionals are involved, but the whole society.This article aims to revise the participation of the health and medical professionals in this process, as well as to think about the role of the health carer leading this process. The objective is not to reduce the caring process to a simple document signing, but to plan health decisions when the end of life is coming from a global conception of the caring process.
Establishment of a System for Monitoring and Control of Cross Contamination in the Laboratory of Microbiological Analysis of Food during 2009

Abstract: Within the activities for quality control in the laboratory, the final results of a particular analyteare now recognized as intermediates, due to the relevance given to quality assurance, as the ultimategoal of programs for quality management. This concept makes it necessary to establishmentof integral tools, to detect events such as cross-contamination, and measures to avoid affectingthe analysis method. Objective: to established a system for monitoring and control of crosscontaminationin the laboratory of microbiological analysis of food. Materials and methods:Flowcharts were developed to control the proceedings on the populations of aerobe mesophilicsand molds originated from pollution in the environment, surfaces, sterile material and culturemedia, which included a decision tree designed to perform control actions based on tolerance intervals,which were established as objective tool to decision-making leading to normalize countsof microbial populations in question. Results: Warning limits stricter were obtained for aerobicmesophilic and mold populations in the different controls, except for environment of media preparationand the corresponding for sterile material. Conclusion: The process developed allowedto complement the system of internal quality control in the laboratory, to provide of an objectivetool for closing nonconformities because of cross-contamination.

Abstract: the living will (lw) gathers the treatment preferences of a person. spain is starting the lw' implantation. in order to the tv will be accessible and useful for the older people, we have to know how it has been developed in other countries. in united states, tv has happened through different stages: from a formal and bureaucratic conception to an integral concept, denominated advance care planning. spanish old adults have a favourable opinion about tv, but it is necessary to include it in a strategy of communication and dialogue with its family and its professionals caregivers. review literature findings will facilitate the development of an effective organizational model that standardizes in spain the use of the tv by the old adults.

Abstract: similar to the idealistic representations of the youth condition, the predominant analysis on youth unemployment and the way in which it can be solved, considerates the phenomenon of isolation in a descriptive way, without repairing capitalistic contradictions and its influence on the institutional and political-judicial relations that govern the new forms of organizations and international divisions of work. nevertheless, it is the context of labor unemployment, with its multiple relations and contradictions, and the way as it is affected by the reconstruction of capitalism on a world-wide scale. in order to make it these questions evident and to reveal its dynamics, it is necessary to put the politics of juvenile work within the the frame of the world transtition of capitalism in latin america under the fordistic supremacy in the social organization of work, with its respective state of being, corresponding to its post-fordistic capitalism and its state form, momentarily differentiated by conditions of the accumulation of capital and specific mechanisms of the regulation of antagonism of classes.
