Vigilancia del personal de laboratorio expuesto al VIH y otros virus de transmisión sanguínea

Abstract: the results of 15 years of surveillance of the laboratory personnel with different levels of exposure to hiv and other bloodborne viruses like hcv, hbv and htlv-1 were presented. various biosafety measures were applied and elisa was used as the main lab technique. 628 serum samples were analyzed for hiv whereas 354 for the rest of the viruses but no lab-acquired infection was detected. this indicates the low incidence of hiv infection in health care workers at risk and the great importance of biosafety measures in the prevention and early detection of these infections.

Abstract: Se reportaron los resultados de 15 a os de vigilancia del personal de laboratorio con diferentes grados de exposición al virus de inmunodeficiencia humana (VIH) y otros virus de transmisión sanguínea como el virus de hepatitis C, virus de hepatitis B y el virus linfotrópico de células T humanas (HTLV-I). Se emplearon diferentes medidas de bioseguridad y como técnica de laboratorio fundamentalmente el ELISA. De 628 muestras analizadas para VIH y 354 para el resto de los virus no se encontraron infecciones adquiridas del laboratorio, lo que confirma la baja incidencia de la infección por VIH en trabajadores de la salud expuestos y el gran valor de las medidas de bioseguridad en la prevención y detección temprana de estas infecciones. Pesquisaje de anticuerpos contra el virus linfotrópico de células T humanas tipo I (HTLV-I) en donantes de sangre y grupos de riesgo

Abstract: Se estudió la presencia de anticuerpos contra el virus linfotrópico de células T humanas tipo I (HTLV I) en 3 774 sueros; de ellos, 1 409 eran de donantes de sangre, 1 444 de pacientes que habían padecido recientemente alguna enfermedad de transmisión sexual (ETS) y 921 de enfermos politransfundidos. Se emplearon los sistemas DAVIH HTLV I (ELISA) y DAVIH BLOT HTLV I (Western Blot) producidos en Laboratorios DAVIH (La Habana, Cuba), para el pesquisaje y confirmación, respectivamente. De los 68 sueros reactivos en la prueba de ELISA, en 2 se confirmó la presencia de anticuerpos al HTLV I/II y 12 fueron considerados indeterminados por el Western Blot. Abstract: the results of the design and evaluation of davih vih-2 diagnosing system, an indirect elisa for screening of hiv-2 antibodies, which uses a hiv-2 glycoprotein gp36 synthetic peptide in its solid phase, were exposed. in the system evaluation using who reference panels, 100 % sensitivity, 99,81 % specificity, 99,81% efficacy and very good concordance level (kappa = 0.978) were attained. serum samples of 959 individuals with undetermined or negative results to the hiv-1 antibodies confirmation (davih blot) were evaluated by the davih vih-2 system. twenty four samples were reactive, six of which had confirmed hiv-2 antibodies. these results allowed recommending the introduction of this diagnostic kit in the hiv infection diagnosing algorithm in cuba.

Abstract: the case definition as well as the systems of classification of the infection due to hiv and aids used up to date were reviewed. the limitations of their application in the clinical practice were also mentioned since the first systems were created to establish an epidemiological surveillance of the infection and because of the appareance of aids. the role of the definition and of the simplified classification systems that are utilized in countries with limitations of resources where it is not possible to use adequate diagnostic aids is also analyzed here. it was stressed that the utilization of theses systems in medical attention and research leads to the necessity of complementing clinical criteria with the use of efficient progression markers that help to estimate the prognosis and to evaluate the efficiency of the antiretroviral therapeutics. finally, it was assessed the impact of the alternate immunological and virological markers on the improvement of classification systems.

Abstract: Se revisó la definición de caso y los sistemas de clasificación de la infección por VIH y SIDA empleados hasta la fecha y se mencionaron sus limitaciones para su aplicación en la práctica clínica porque los primeros sistemas fueron creados con fines de vigilancia epidemiológica de la infección y la aparición de casos de SIDA. Se comentó el papel de la definición y sistemas de clasificación simplificados que se usan en países con limitaciones de recursos, donde no es posible emplear medios de diagnóstico adecuados. Se hizo énfasis en que el uso de estos sistemas en la atención médica y la investigación implica la necesidad de complementar los criterios clínicos con el uso de marcadores de progresión eficientes, que ayuden a estimar el pronóstico y permitan evaluar la eficacia de la terapéutica antirretroviral. Finalmente se valoró el impacto de los marcadores sustitutivos inmunológicos y virológicos en el perfeccionamiento de los sistemas de clasificación. Confirmación de la presencia en Cuba del virus linfotrópico tipo I de las células T humanas mediante la reacción en cadena de la polimerasa

Abstract: Se estudiaron por reacción en cadena de la polimerasa los primeros casos diagnosticados en Cuba como seropositivos al HTLV-I/II (virus linfotrópicos de las células T humanas), con el objetivo de diferenciar el tipo de virus causante de la infección. Se utilizaron 3 juegos de oligonucleótidos cebadores y los productos de amplificación fueron detectados mediante hibridación con oligosondas específicas. El 100 % de los casos resultaron positivos para el HTLV-I, no se encontró positividad para el HTLV-II. Se confirmó la presencia en Cuba de este retrovirus, aunque la seroprevalencia es baja si se tiene en cuenta que el Caribe es una zona endémica para el HTLV-I. The first cases diagnoses in Cuba as HTLV-I/II seropositives (human T lymphotropic virus) were studied by polymerase chain reaction aimed at differentiating the type of virus causing the infection. 3 kits of primer oligonucleotides were used and the amplification products were detected by hybridization with specific oligoprobes. 100 % of the cases were HTLV-I positives. No HTLV-II positivity was found. It was confirmed the presence in Cuba of this retrovirus, eventhough the seroprevalence is low if it is taken into consideration that the Caribbean is an endemic zone for HTLV-1.

Abstract: the objective of this report was to evaluate oral fluid and urine as optional samples for hiv-1 confirmatory test using davih-blot system (laboratorios davih, la habana, cuba), for which they were compared with their corresponding serum samples in a group of 125 individuals. in band pattern analysis, predominant antibodies in positives oral fluid and urine samples against p34, p68, gp41, gp120, were no different from their corresponding sera according to the proportion comparison test (p<0,001). relative sensitivity and specificity of this system were 100 % in oral fluid and 98, 75% and 100% in urine samples respectively. these results support optional use of oral fluid and urine that, with slight modifications in the diagnostic system, can be applied for hiv-1 antibody confirmation testing.
Empleo del AMPLICOR HIV-1 DNA test v 1.5 en el diagnóstico de la infección perinatal por el VIH-1 en Cuba

Abstract: introduction: hiv-1 proviral dna amplification is the preferential method for hiv diagnosis infection in infants and it has been used in cuba since 1992. amplicor hiv-1 dna kit is an in vitro qualitative assay for the detection of hiv-1 proviral dna in the whole blood. objective: this paper showed the results of the use of this kit for the first time in cuba for the perinatal diagnosis of hiv-1 infection. methods: three hundred forty six whole blood samples from children of hiv seropositive women were analyzed by the amplicor hiv-1 dna kit in the period 2005-2007. results: among the tested samples, six were positive, and 340 negative. conclusions: the assay was reproducible under the cuban conditions and the achieved results made the diagnosis and follow up of children of hiv-1 seropositive mothers possible.
