Ideal ward round making in neurosurgical practice.

Abstract: The success of a perfect ward round lies in the role of the consultant leading the ′round making group′ (RMG) as well as the hallmark of effective questioning and participation of each member. Twelve senior consultants with more than 10 years′ experience in neurosurgical practice at three different university hospitals were observed during round making by a participant observer. Observations were made on the group climate of the RMG, the leadership pattern and language expressed by the clinician conducting the round and the effectiveness in his performance as a leader during clinical discussions. The group climate showed evidence of good productivity and flexibility with 92% and 75% consultants, pleasantness of climate was above average with only 50% (6/12) and poor objectivity with 42% (5/12) consultants. Forty two percent of the consultants were not always very well comprehensible, while only 50% (6/12) spoke exactly fitting the occasion. Only 33% (4/12) of the consultants used humour effectively, while 42% (5/12) spoke unnecessarily in between discussion and were poor in introducing the problems of patient to the round making group. Ward round making in neurosurgical practice needs a holistic approach with motivation, planning, leadership skills and structured curriculum to fulfill its objectives.

Abstract: Trypanosomiasis induces loss of body condition in pregnant animals leading to birth of offspring s, with low birth weights foetal and neonatal losses, besides production losses in lactating animals. The consequences of trypanosomiasis are less severe in better-nourished animals but good nutrition does not by itself provide protection. In situ formed phase transited drug delivery system of ketoprofen for achieving osmotic, controlled and level a in vitro in vivo correlation

Abstract: A dry process induced phase transited, non disintegrating, controlled release, in situ formed asymmetric membrane capsular system for poorly water soluble drug, ketoprofen, was developed and evaluated both in vitro and in vivo for osmotic and controlled release of the drug. In situ formed asymmetric membrane capsules were prepared using fabricated glass capsule holders via dry, phase inversion process. Effect of varying osmotic pressure of the dissolution medium on drug release was studied. Membrane characterization by scanning electron microscopy showed an outer dense region with less pores and an inner porous region for the prepared asymmetric membrane. In vitro release studies and statistical test for all the prepared and marketed formulation were done at P > 0.05. The drug release was found to be independent of the pH, but dependent on the osmotic pressure of the dissolution medium. In vivo pharmacokinetic studies showed a level A correlation (R 2> 0.99) with 39.24 % relative bioavailability compared to immediate release tablet of ketoprofen. Excellent correlation achieved suggested that the in vivo performance of the phase transited in situ formed AMCs could be accurately predicted from their in vitro release profiles and could a means for controlled delivery of drugs with varying solubility.
Simultaneous derivative spectrophotometric analysis of doxylamine succinate, pyridoxine hydrochloride and folic acid in combined dosage forms

Abstract: Two UV spectrophotometric methods have been developed, based on first derivative spectrophotometry for simultaneous estimation of doxylamine succinate, pyridoxine hydrochloride, and folic acid in tablet formulations. In method I, the concentrations of these drugs were determined by using linear regression equation. Method II is also based on first derivative spectrophotometry however simultaneous equations (Vierdot′s method) were derived on derivative spectra. The first derivative amplitudes at 270.0, 332.8 and 309.2 nm were utilized for simultaneous estimation of these drugs respectively by both methods. In both the methods, linearity was obtained in the concentration range 2.5-50 μg/ml, 1-40 μg/ml and 1-30 μg/ml for doxylamine succinate, pyridoxine hydrochloride, and folic acid respectively. The developed methods show best results in terms of linearity, accuracy, precision, LOD, LOQ and ruggedness for standard laboratory mixtures of pure drugs and marketed formulations. The common excipients and additives did not interfere in their determinations.

Comparative evaluation of the effect of topical fluorides on the microhardness of various restorative materials: An in vitro study

Abstract: Context: Topical fluorides can recharge the fluoride content of exhausted glass ionomer cements, converting them into fluoride reservoirs. However, the high reactivity of fluoride agents used in topical fluorides may result in the deterioration of surface properties of these restorations. Aim: To evaluate and compare the effect of topical fluorides on the microhardness of conventional glass ionomer cements (Fuji II,GC Corporation,Tokyo, Japan and Ketac Fil Plus,3M ESPE, Seefeld, Germany), high viscosity conventional glass ionomer cements (Ketac Molar Easymix, 3M ESPE, Seefeld, Germany and Fuji IX GP, GC Corporation, Tokyo, Japan), and resin-modified glass ionomer cements (Vitremer, 3M ESPE, St.Paul, MN, USA and Fuji II LC, GC Corporation, Tokyo, Japan). Materials and Methods: Twenty-one pellets were made of each material and stored in distilled water at 37°C for 48 h. These were then randomly divided into 3 subgroups of 7 pellets each. One subgroup was treated by 4 min application of 1.23% acidulated phosphate fluoride (APF) gel, other subgroup with 2% sodium fluoride (NaF) gel, and the third subgroup was used as control. Thereafter, all the pellets were subjected to microhardness testing (load = 100 g for 15 s). Results: APF gel of 1.23% produced a statistically significant decrease in microhardness (P<0.05) of all the restorative materials as compared with restorative materials used as control. The decrease in the microhardness was more pronounced in conventional glass ionomer cements and least pronounced in resin-modified glass ionomer cements. No statistically significant difference (P > 0.05) in microhardness was found after NaF treatment in all the restorative materials tested as compared with control subgroups. Conclusion: The use of 1.23% APF gel may be detrimental to the long-term durability of glass ionomer restorations.
Effect of gabapentin premedication on preoperative anxiety and postoperative pain

Abstract: Background : In addition to chronic pain and anxiety disorders, few studies have found promising role of gabapentin in relieving acute postoperative pain as well as acute anxiety too. Objectives : To evaluate the effect of gabapentin premedication on preoperative anxiety and postoperative pain in patients undergoing elective open cholecystectomy. Methods : A prospective, randomized, double blind and placebo controlled study. Eighty adult patients of ASA I and II were divided into 2 groups of 40 each. Patients in group 1 and group 2 received capsules Gabapentin (1200mg) or identical placebo capsules 2 hours prior to surgery respectively. Preoperative anxiety and 12 hours postoperative pain was assessed using Anxiety and Pain VAS score respectively along with the observation of side effects. Postoperatively, intravenous pethidine 0.5 mg/kg was given when pain VAS > 40mm and time to first pethidine injection and total pethidine consumption in 12 hours was recorded. Results : Anxiety VAS scores after one hour of drug intake (35.75±20.11 versus 46.63±12.73) and just before induction of anaesthesia (45.75±30.27versus 68.13±29.84) along with postoperative Pain VAS scores were significantly lower in gabapentin group. Time to the first pethidine demand was significantly longer (145.34±194.54 min versus 26.30±51.02min) and cumulative pethidine consumption throughout study period was also significantly lower (35.91 ± 16.61 versus 57.84 ± 20.72mg) in gabapentin group. Conclusions : 1200 mg gabapentin premedication in open cholecystectomy patients significantly reduced preoperative anxiety, postoperative pain and total pethidine consumption with negligible side effects. Abstract: Authentication of medicinally important plants is essential for increasingly demands of herbal remedies worldwide. DNA barcoding technology is currently gaining importance as a reliable tool for plant species identification, although one barcode gene is not enough in the exceptions. Short sequence diversity of standardized specific coding gene regions of rbcLa and matK of plastid genome together with noncoding ribosomal internal transcribed spacer 2 (ITS2) marker is used as barcode to compare and differentiate plant species. The success of obtaining sequences of the 29 analyzed plants distributed in 21 families using three different barcode genes rbcLa, matK and ITS2 were 97%, 79% and 75% respectively. Modified Grover's Search Algorithm in O(M+logN)Steps

Abstract: The present letter proposes a modification in the well known Grover's search algorithm, which searches a database of $N$ unsorted items in $O(\sqrt{N/M})$ steps, where $M$ represents the number of solutions to the search problem. Concurrency control techniques and extra registers for marking and storing the solutions are used in the modified algorithm. This requires additional space but it is shown that the use of extra register and marking techniques can reduce the time complexity to $O(M+\log N)$.
