Additional factors, such as age, lifestyle, and uremia can also influence the hemorrhagic risk and should be carefully assessed before decision-making on the appropriate management of patients with less severe forms of ITP. The recent introduction of new classes of therapeutic agents such as rituximab and the thrombopoietic growth factors has had a major impact on the management of ITP. Updated treatment guidelines have recently been made available but they are based largely on expert opinion rather than on high-quality clinical trial evidence. This structured review is focused on the management of adults with chronic ITP, including the use of new classes of agents.

JVLA Observations of IC 348SW: Compact Radio Sources and their Nature

Abstract: We present sensitive 2.1 and 3.3 cm JVLA radio continuum observations of the region IC 348 SW. We detect a total of 10 compact radio sources in the region, of which seven are first reported here. One of the sources is associated with the remarkable periodic time-variable infrared source LRLL 54361, opening the possibility of monitoring this object at radio wavelengths. Four of the sources appear to be powering outflows in the region, including HH 211 and HH 797. In the case of the rotating outflow HH 797 we detect at its center a double radio source, separated by $\sim3"$. Two of the sources are associated with infrared stars that possibly have gyrosynchrotron emission produced in active magnetospheres. Finally, three of the sources are interpreted as background objects.

First measurements of 15N fractionation in N2H+ toward high-mass star forming cores

Abstract: We report on the first measurements of the isotopic ratio 14N/15N in N2H+ toward a statistically significant sample of high-mass star forming cores. The sources belong to the three main evolutionary categories of the high-mass star formation process: high-mass starless cores, high-mass protostellar objects, and ultracompact HII regions. Simultaneous measurements of 14N/15N in CN have been made. The 14N/15N ratios derived from N2H+ show a large spread (from ~180 up to ~1300), while those derived from CN are in between the value measured in the terrestrial atmosphere (~270) and that of the proto-Solar nebula (~440) for the large majority of the sources within the errors. However, this different spread might be due to the fact that the sources detected in the N2H+ isotopologues are more than those detected in the CN ones. The 14N/15N ratio does not change significantly with the source evolutionary stage, which indicates that time seems to be irrelevant for the fractionation of nitrogen. We also find a possible anticorrelation between the 14N/15N (as derived from N2H+) and the H/D isotopic ratios. This suggests that 15N enrichment could not be linked to the parameters that cause D enrichment, in agreement with the prediction by recent chemical models. These models, however, are not able to reproduce the observed large spread in 14N/15N, pointing out that some important routes of nitrogen fractionation could be still missing in the models.
La transformation des relations entre état et associations au centre d’une nouvelle dynamique institutionnelle : le cas de la politique de la ville en France

Abstract: Les associations uvrant pour la politique de la ville constituent le principal mode d’intervention de l’état dans cette politique publique. Il serait pourtant erroné de ne considérer ces associations que comme de simples auxiliaires de la puissance publique. On assiste en fait à une dynamique de plus grande ampleur qui tout en inscrivant les associations dans une nouvelle configuration institutionnelle participe aussi de l’émergence d’un état médiateur . Celui-ci emprunte à l’idée associative son positionnement au sein de la société caractérisé par la valorisation de la proximité et de la cohésion sociale qui en fait une sorte de corps intermédiaire entre l’état et la société civile. Réciproquement, l’état médiateur intègre à lui, par des dispositifs partenariaux, certaines associations, celles notamment qui de petite taille, peuvent justifier de la plus grande proximité avec les populations des quartiers de la politique de la ville. L’état médiateur ne désigne donc en aucune manière une phase de retrait de l’état mais un redéploiement de celui-ci, marqué par un fort interventionnisme sur la société en même temps que l’adoption d’un discours et de pratiques qui furent longtemps l’expression exclusive des associations.

Abstract: Male and female mice of ACA strain were infected with 200 and 2,000 bloodstream trypomastigotes from Dm 28c clone of Trypanosoma cruzi. All the animals were susceptible to the infection regardless of their sex and dose of initial inoculum used. Both male and female, infected with 2,000 parasites showed significantly higher level of parasitemia than mice infected with the lower dose. At the same time, males developed higher parasitemia level than females, when they were infected with both 200 and 2,000 parasites. Despite of experimental animals behaved as susceptible, the survival period was significantly longer in males infected with 200 trypomastigotes when compared to males infected with 2,000 parasites. Females infected with 2,000 survive a longer time than males infected with the same dose of parasites. No significant differences were observed during the survival period when comparing females infected with 200 and 2,000 parasites, neither in the case of male and female infected with 200 parasites. Apparently, the parasitemia level is the main characteristic affected by the variables sex of the host and initial parasite dose.
Concordancia de las pruebas de tuberculina e Interferón gamma en población reclusa Concordance of tuberculin tests and Interferon gamma release assays in the prison population

Abstract: Objetivos: Estudiar en población penitenciaria la concordancia de la prueba de la tuberculina (PT) y las pruebas de interferón gamma (IFG). Material y métodos: Estudio prospectivo realizado en una prisión en mayo-junio de 2009. Se estudian los ingresos sin antecedente de tuberculosis (TB) o con PT previa negativa o no realizada. Se realizó IDR de Mantoux (positivo 3 10 mm) y extracción sanguínea para prueba de IFG (QuantiFERON -TB Gold). En los infectados, se realizó despistaje de TB. Se pasó un cuestionario y se solicitó consentimiento informado. El estudio fue aprobado por un Comité ético ajeno a instituciones penitenciarias. La concordancia entre PT e IFG se basó en el índice Kappa. Resultados: Se incluyeron 181 casos. El 62% eran extranjeros, el 17% vacunados por BCG, el 8,4% UDI y el 4% VIH+. En los extranjeros había más vacunados, menos UDI y menos infectados por VIH que en autóctonos (p=0,02, p=0,02, y p=0,01, respectivamente). La PT fue positiva en el 24% y la IFG en el 26%. Hubo información de ambas en 149 (82%) casos. El 15,8% fueron discordantes. El índice Kappa fue de 0,6 (0,4-0,7). La concordancia varió según subgrupos, siendo mayor en autóctonos (kappa= 0,8) y menor en vacunados (kappa=0,4) e inmigrantes (kappa=0,5). Conclusión: La concordancia global fue moderada-buena, pero en vacunados e inmigrantes fue menor. El nivel de discordancia aconseja ampliar el estudio, así como evaluar que prueba predice mejor el riesgo de progresión a TB y el coste-beneficio de ambas en la población reclusa de nuestro país. Objective: To study the agreement of Tuberculin Skin Tests (TST) and Interferon Gamma Release Assays (IGRA) when screening tuberculosis infection amongst inmates recently admitted to prison. Materials and methods: Prospective study conducted in a prison during the months of May and June 2009. Inmates without a TB history, with previous TST negatives or without prior TSTs were included. Participants signed an informed consent form and the study was approved by an independent Ethical Committee. TST (positive 10 > or = mm) and IGRA (Quantiferon TB-Gold) were performed and standardized data collection was carried out. The agreement between both tests was analysed using the Kappa index. Results: A total of 181 people were included. 62% were foreign-born, 17% had previous BCG vaccination, 8.4% were IDUs and 4% HIV-infected. Foreign born subjects were more frequently vaccinated and presented less drug use and HIV infection than people born in Spain. (p=0.02, p=0.02 and p=0.01 respectively). First observation of flux avalanches in a-MoSi superconducting thin films

Abstract: We have observed the occurrence of dendritic flux avalanches in an amorphous film of Mo$_{84}$Si$_{16}$. These events are understood to have a thermomagnetic origin and involve the abrupt penetration of bursts of magnetic flux taking place within a limited window of temperatures and magnetic fields. While dc-magnetometry allows one to determine the threshold fields for the occurrence of the thermomagnetic instabilities, magneto-optical imaging reveals the spatial distribution of magnetic flux throughout the sample. Conducting appropriate experiments, typical for this goal, avalanches were confirmed to be a characteristic of this material, ruling out the otherwise admissible possibility of an experimental artifact or a feature related to defects in the film. After the present observation, a-MoSi can be included in the gallery of superconducting materials exhibiting flux avalanches when in the form of thin films, a characteristic that must be carefully taken into consideration when one plans to employ films of those materials in applications.
Radio monitoring of the periodically variable IR source LRLL 54361: No direct correlation between the radio and IR emissions

Abstract: LRLL 54361 is an infrared source located in the star forming region IC 348 SW. Remarkably, its infrared luminosity increases by a factor of 10 during roughly one week every 25.34 days. To understand the origin of these remarkable periodic variations, we obtained sensitive 3.3 cm JVLA radio continuum observations of LRLL 54361 and its surroundings in six different epochs: three of them during the IR-on state and three during the IR-off state. The radio source associated with LRLL 54361 remained steady and did not show a correlation with the IR variations. We suggest that the IR is tracing the results of fast (with a timescale of days) pulsed accretion from an unseen binary companion, while the radio traces an ionized outflow with an extent of $\sim$100 AU that smooths out the variability over a period of order a year. The average flux density measured in these 2014 observations, 27$\pm$5 $\mu$Jy, is about a factor of two less than that measured about 1.5 years before, $53\pm$11 $\mu$Jy, suggesting that variability in the radio is present, but over larger timescales than in the IR. We discuss other sources in the field, in particular two infrared/X-ray stars that show rapidly varying gyrosynchrotron emission.
