Cryopreservation of Parathyroid Glands

Abstract: The risk of permanent hypoparathyroidism following thyroid and parathyroid surgery is around 1% in the hands of experienced endocrine surgeons. Although this complication is rare, rendering a patient permanently aparathyroid has significant consequences on the health and quality of life of the patient. Immediate autotransplantation of parathyroid glands that are injured or unintentionally removed offers the best possibility of graft viability and functionality. However, since the majority of cases of hypoparathyroidism are transient, immediate autotransplantation can complicate postoperative surveillance in certain patients, especially those with primary hyperparathyroidism. Cryopreservation of parathyroid tissue is an alternate technique that was developed to treat patients with permanent hypoparathyroidism. This method allows for parathyroid tissue to be stored and then autotransplanted in a delayed fashion once permanent hypoparathyroidism is confirmed. This article provides a contemporary review on cryopreservation of parathyroid tissue and its current role in thyroid and parathyroid surgery. 1. Introduction Permanent postoperative hypoparathyroidism results from the unintentional removal or injury of parathyroid glands during thyroid and parathyroid surgery. Permanent hypoparathyroidism is defined as persistent hypocalcemia requiring calcium and vitamin D supplementation 6 months after surgery [1]. The development of hypoparathyroidism is contingent on the type of operation performed. The risk is nominal during a minimally invasive parathyroidectomy for a single adenoma but is greatest after a subtotal or total parathyroidectomy, thyroid resection and nodal dissection for large and extensive thyroid cancers, and reoperative neck operations [1]. Even though the risk of transient hypocalcemia can be high during an extensive neck dissection, the permanent hypoparathyroidism rate is typically around 1% in the hands of experienced endocrine surgeons at high-volume centers. The accidental onset of permanent hypoparathyroidism can be agonizing for the patient and the clinician alike. For the patient, its negative impact includes a reduced quality of life, expensive lifelong medication supplementation, frequent laboratory testing, and the potential for frequent hospital admissions. In addition, the persistent absence of parathyroid hormone (PTH) has long-term systemic effects on the body, such as the development of osteoporosis (due to the decreased function of osteocytes), premature cataracts, cardiac dysfunction, and neurologic dysfunction [1, 2]. Abstract: a health care relationship is based on classic premises of autonomy, benefit to the patient, justice and non-maleficence (or the "do no harm" principle). moral rules are principles that should guide professional conduct, they have no legal power but are self imposed by the group and oblige all professionals to act accordingly. although the capacity of any imprisoned individual to make decisions about medical treatment is restricted, there are no ethical or moral arguments to justify limitations on the exercise of autonomy where decision making processes that might directly affect him/her are concerned.

Abstract: yatago (trichanthera gigantea) is a specie with wide potential for the production of forages and as conservative of basins. however, the establishment is limited for high costs. in this sense, this investigation consisted on the evaluation of the effects of four propagation methods in field of t. gigantea with the purpose of analyzing some characteristics for the propagation, establishment and costs, in moprevats unet, san cristóbal, táchira state. the propagation methods were evaluated: t1: nursery plants in plastic bags, t2: stakes to naked root, t3: direct propagation of a stake and t4: direct propagation of two stakes and three repetitions and three repetitions.120 days after the sowing, the variables % of survival, number of buds, forage fresh weight and dry weight, and % of dry matter for treatment were measured. the establishment costs were analyzed. the % of survival did not present differences among the treatments (p = 0.933), while the variables number of buds, fresh weight, dry weight, and % of dry matter presented differences (p = 0.000). an economic evaluation with these results was carried out, being determined that the method of direct propagation of a stake for point reduced the costs in 22.3 %.

Abstract: This article seeks to analyse, from the case study of the Pro-K ln party, how Islamophobia is used as a resource by some parties on the extreme right in Europe to obtain political returns and electoral support. Using an analysis of the genesis of the party and its electoral program as a starting point, the study dismantles theargument that this party’s main objective is to fight against the Islamisation of Europe, and shows that this is only one means to articulate an ultra-rightist political discourse and that its attempt to stop the construction of the mosque in K ln is only window-dressing to conquer the votes of the citizenry.

Abstract: El yatago (Trichantera gigantea) es una especie con amplio potencial para la producción de forrajes y la conservación de cuencas. Sin embargo, su establecimiento esta limitado por los altos costos. En este sentido, esta investigación evaluó los efectos de 4 métodos de siembra de T. gigantea, bajo un dise o completamente aleatorizado, con el propósito de analizar características para su propagación, establecimiento y costos, en MOPREVATS UNET San Cristóbal estado Táchira. Los tratamientos evaluados fueron: T1: Plantas de vivero en bolsas plásticas, T2: Estacas a raíz desnuda, T3: Siembra directa de una estaca y T4: Siembra directa de dos estacas, con tres repeticiones. A los 120 días se evaluó el porcentaje sobrevivencia, número brotes, peso fresco y seco del forraje y el porcentaje de materia seca. El % sobrevivencia no mostró diferencias entre los tratamientos, mientras que el número de brotes, peso fresco, peso seco y % MS presentaron diferencias (p = 0,000) entre los métodos de propagación. Con estos resultados, se realizó una evaluación económica, determinándose que el método de siembra directa de una estaca por punto, reduce los costos en un 22,3%, lo cual lo hace apropiado para su rápido establecimiento. Abstract: La relación asistencial descansa sobre los pilares clásicos de autonomía, beneficio para el paciente, justicia y no maleficencia (no hacer da o). Las normas deontológicas son los principios que deben guiar la conducta profesional, no tienen fuerza jurídica, pero están autoimpuestas por el grupo y obligan a todos los profesionales. Aunque la capacidad de una persona encarcelada para tomar decisiones sobre las actuaciones médicas que sobre ella se pueden hacer está limitada, no hay argumentos éticos ni deontológicos suficientes como para poner cortapisas al ejercicio de la autonomía en la toma de decisiones clínicas que les afecten.
