Abstract: Klebsiella pneumoniae productora de β-lactamasa de espectro expandido (BLEE) ha jugado un papel importante como causa de infecciones en la unidad de alto riesgo neonatal (UARN) del Instituto Autónomo Hospital Universitario de Los Andes (IAHULA). En el presente trabajo se describe un brote ocasionado por esta bacteria en los neonatos hospitalizados en dicha unidad durante el mes de febrero 2007, así como también, cepas aisladas en los meses siguientes al brote y además, se estudia el ambiente y el personal, como posible fuente de esta bacteria. Las cepas de K. pneumoniae aisladas del brote eran del mismo fenotipo de resistencia, productoras de (3LEE tipo TEM y SHV y pertenecían al mismo genotipo que las cepas aisladas de las manos y de las soluciones jabonosas, posible fuente de infección, lo cual indica que se trataba del mismo clon. El brote se resolvió usando dos importantes medidas: reforzando el lavado de manos y con la indicación oportuna de imipenem a los neonatos afectados. Klebsiella pneumoniae as a producer of extended spectrum beta-lactamase (ESBL) has played an important role as a cause of infection in the neonatal high risk unit (NHRU) of the Autonomous Hospital Institute of the Universidad de Los Andes (AHIULA). In this paper an outbreak caused by this bacterial specie that affected neonates hospitalized in this unit during February 2007 is described. Besides, the environment and the personnel were studied as possible sources of this organism. The strains of K. pneumonia isolated from the outbreak had the same resistance phenotype, produced ESBL type TEM and SHV and belonged to the same genotype as the isolated strains from the hands and the soapy solutions, possible sources of infection. This indicates that it was the same clone. The outbreak was resolved using two important measurements: reinforcing hand washing and with the opportune treatment of neonates with imipenem.
Propiedades probióticas de Lactobacillus spp aislados de biopsias gástricas de pacientes con y sin infección por Helicobacter pylori

Abstract: background: gastric microbiota is a complex ecosystem, composed by diverse microorganisms, where h pylori and lactobacillus are relevant species. aim: to study the probiotic properties of lactobacillus spp isolated from gastric biopsies of subjects bearing or not h pylori. materials and methods: gastric biopsies from 197 individuals that consulted for gastrointestinal disorders were cultivated in media specific for h pylori and lactobacillus spp. probiotic properties of lactobacilli isolates and histological diagnosis of h pylori colonized patients were performed. results: among 197 individuals evaluated, hpylori was detected in 43%, lactobacillus spp in 24%, while 8% presented both bacteria simultaneously. forty one percent of the lactobacillus spp strains produced hydrogen peroxide, s'3% presented a highly hydrophobic surface and 32% had an inhibitory effect upon hpylori atcc 43504, even greater than lactobacillus lgg or lal reference strains. the main pathological diagnosis of patients ivas chronic non-atrophic gastritis (73%) followed by intestinal metaplasia (19%). a similar proportion of both conditions were observed in patients colonized by lactobacillus spp, while not colonized individuals developed preferentially chronic non-atrophic gastritis (p =0.002). conclusions: lactobacillus spp and h pylori were mutually excluded, suggesting a competitive colonization model between probiotic bacteria and pathogens .

Abstract: introduction: the serial assessment of head circumference (hc) is a valuable tool that gives us early warning about potential growth alterations and central nervous system development disturbances. aim: to know the rate of health controls where hc is measured, diagnosed and if a concordant therapeutical behavior was taken. method: 424 health control records of infants were reviewed in 2 health centers of colina from july 2002 to present. it was recorded hc measurement, diagnosis and indications about hc. all measurements were compared with nchs charts and then compared with the diagnosis registered in medical records. results: 85,3% of controls had hc measurement record. 17,7% of them had a diagnosis, being concordant in 92% with the researcher team. 14% of the controls without diagnosis had an abnormal hc. non behavior was registered in the only abnormal hc record found by health team. conclusion: hc adequately measure is adequately in almost all health controls, although its interpretation is underused. it seems that standard behaviors are needed to get, control and refer the infants with abnormal hc to optimize the value of this clinical tool

Abstract: El artículo que se presenta a continuación analiza los primeros a os de la transición chilena (1987-1988) desde la perspectiva de la memoria histórica, considerando la forma en que los recuerdos de lo que habían sido los gobiernos de Frei y de la Unidad popular, así como los del golpe de Estado y la dictadura, marcaron el desarrollo político y social durante el periodo se alado, es decir, el proceso plebiscitario de 1988. Biología reproductiva de la tagua común (Fúlica armillata) y la tagua de frente roja (F. rufifrons) en un área agroforestal del centro-sur de Chile

Abstract: between the breeding seasons 2008 and 2009 we studied the nesting and breeding of fulica armillata and f. rufifrons inhabiting a natural lagoon in an agro-forestry area of central-south chile. we studied the differences in characteristics of nest, eggs, incubation, hatching success and characteristics of the young, with the aim provide basic data on breeding ecology of coots in chile. in both species the breeding period started in late september, extending into late january. nesting platforms of both coots were similar in size and built with dried crossed juncus, had a circular form and shallow nest-cup. clutch sizes of f. rufifrons were almost two-fold those of f. armillata. the eggs of f. armillata distinguished from the eggs of f. rufifrons by size, weight and color pattern. incubation time in f. armillata and f. rufifrons was 27 and 26 days, respectively. f. armillata had a hatching success of 46% (88.5%, traditional method) and 62% (96%) for 2008 and 2009, respectively; while for f. rufifrons was 85% (98%) for 2008. although chicks of both species born with black downs, they exhibited an evidently different coloration pattern. in general, our observations agree with previous reports on the breeding biology of the studied coots. we believe that the description of eggs and chicks made here may become a useful guide to recognize nests of f. armillata and f. rufifrons. in addition, some of our results may have potential utility for management and conservation planning of freshwater ecosystems.
Producción de ?-lactamasas de espectro extendido (BLEE) en cepas de Acinetobacter baumannii aisladas en hospitales de la VIIIa Región, Chile

Abstract: the resistance of acinetobacter baumannii to ?-lactam antibiotics is mainly due to the synthesis of ?-lactamases. from a clinical point of view, this bacteria and others, grouped under the acronym space (s: serratia, p: pseudomonas, a: acinetobacter, c: citrobacter, e: enterobacter) are essentially amp-c ?-lactamases producers. there is no local information about esbl presence in acinetobacter. we studied esbl production using the ho and col. technique modified by adding cloxacillin as chromosomal ?-lactamases inhibitor. from 69 isolates, with resistance to at least one third generation cephalosporin, only 7 showed positive synergy test. four of these amplified for tem family gene, and one of these amplified also for the oxa family. our study found a low esbl production percentage, which agrees with the premise of amp-c as the main mechanism of resistance to ?-lactam antibiotics in a. baumannii. however, the esbl description in these bacteria emphasizes the capacity of expressing multiple resistance mechanisms
