Greater Occipital Nerve Block by Bupivacaine/Methyl Prednisolone Vs. Bupivacaine/Normal Saline in Medication Overuse Headache

Abstract: Background: Medication overuse headaches (MOH) include headaches that last more than 15 days a month and usually occur after long-term use of analgesic. Most methods of medical treatment are ineffective. In some studies, great occipital nerve block is used to treat the headaches.Methods: This double-blind experimental study was performed in adult patients with MOH who attended Sina Hospital in Tehran from June 2009 to June 2011. Greater occipital nerve (GON) block was done in the two groups of patients by administering a combination of 3 ml of 0.5% bupivacaine and 2 ml of 5% saline or 3 ml of 0.5% bupivacaine and 80 mg of methyl prednisolone. We evaluated headache severity by Visual analog scale (VAS) and recorded days without headache. If patients were taking pain medications, preventive medications were given as usual before and after the injection. Student's t-test was performed for statistical analysis and a P<0.05 was considered significant.Results: Overall, 13 male and 19 female patients with a mean age of 39.88±8.76 years participated in the study. The mean reduction in headache severity one hour after injection was 5.56±1.03 and 4.63±1.92 in the first and second groups, respectively. Average days without headache one month after injection were 8.75 and 4.75 days in the first and second groups, respectively. There were no significant differences between the two groups.Conclusion: Both methods seem to be effective in medication overuse headaches treatment. This finding is important as non-responders to conventional methods will otherwise have a decreased quality of life.

Abstract: Statement of Problem: Tooth extraction is always considered as the final treatment option in dentistry."nConsidering the numerous advances in dentistry, nowadays the preservation of the permanent teeth until old"nage is common. However, in most economically poor countries or those without security service insurance,"nthe high rate of extraction, particularly among restorable teeth, is regrettable."nPurpose: The aim of the present study was to determine the reasons for tooth extraction among patients"nreferred to the faculty of Dentistry, Tehran University of Medical Sciences in 2002."nMaterials and Methods: This descriptive and cross-sectional study was conducted on 320 patients. The"ninformation about patient's general knowledge, oral health status, tooth location and causes of extraction were"ncollected and recorded in a questionnaire. The data were submitted to statistical Chi-Square test."nResults: No statistically significant difference was found between two genders in their mentioned causes for"nextraction. The most prevalent reasons were as follows: Caries (50%), Periodontal diseases (16.6%). Absence"nof an acceptable occlusion, prosthetic problems, patient's request, etc... make up the remaining 33.4% of the"nreasons."nConclusion: According to this study, it is suggested to investigate extraction etiology at the society level and"nif similar results are obtained, necessary steps should be taken to prevent caries and periodontal problems as"nthe major mentioned causes for tooth extraction.
The use of tissue expander in repairing skin and hair lesions of the head

Abstract: Of 14 cases who underwent this operation only in one of them because of infection in operation site, we had to extract the expander. Also, in the first day of the operation that we injected serum inside the expander of two other patients, the wound opened up and the expander appeared and came out automatically. The remaining ten patients have tolerated the operation after treatment period without any complications and good result was achieved. This method in covering up skin lesions and in eliminating burn as well as old wound scars was very effective and the patient become satisfied because there was no need to remove skin from other parts of the body and thus no new scars would have been created. The only problems in this method of treatment were its duration which is needed for injection inside the expander as well as bad appearance in operation region and these problems cause the patients to become depressed and to become isolated from the public for a long period. For the past three years that we used this method for covering skin lesions in different parts of the body and even in treating extensive spots and the hemantrium we achieved excellent results and the report will subsequently be submitted.

Abstract: Single coronary artery is a coincidental finding during coronary angiography or at autopsy. Although it is a rare condition and most of time has an asymptomatic clinic; prognosis varies. We would like to report a case about percutaneous coronary intervention in a patient who has anomalous single coronary artery arising from right sinus valsalva.
Sepsis Risk Factors in Children: a Brief Report

Abstract: Background: Sepsis is a leading cause of death in infants and children. In this study, we determined sepsis risk factors in children admitted in Bahrami Hospital. Methods: Medical records of 94 patients with septicemia and a positive blood culture were reviewed in this study. The patients had been admitted during 2010-2011 in different wards of Bahrami Pediatric Hospital. Variables including age, gender, underlying diseases, causative agents and use of medical devices were extracted from the medical records and analyzed statistically. Results: 54.3% of the cases were male and 51% were below 1 year of age. 54.3% had underlying diseases including malignancy (18.1%) and renal disease (11.7%), which were the most common causes. Failure to thrive (FTT) was detected in 34% of the cases. 82% of the causative bacterial agents were gram positive bacteria and the most common organisms were coagulase negative staphylococci (63.8%) and staphylococcus aureus (10.6%). IV-catheters (100%), NG tubes (9.6%), urinary catheters (4.3%) had been used in the patients. The mortality rate was 6.3%. Conclusion: This study showed that septicemia was more prevalent in infants and the most common underlying diseases were malignancy and renal disease, respectively. Gram positive bacteria were the most common cause of septicemia.
Landslide Susceptibility Mapping in Sajaroud Basin Using Logistic Regression Model

Abstract: In this research, logistic regression analysis was used to create a landslide hazard map for Sajaroud basin. At first, an inventory map of 95 landslides was used to preduce a dependent variable, which takes a value of 0 for absence and 1 for presence of landslides. Ten factors affecting landslide occurence such as elevation , slope gradient, slope aspect, slope curvature, rainfall, distance from fault, distance from drainage, distance from road , land use and geology were taken as independent parameters. The effect of each parameter on landslide occurrence was determined from the corresponding coefficient that appears in the logistic regression function. The interpretation of the coefficients showed that road network plays the most important role in determining landslide occurrence. Elevation, curvature, rainfall and distance from fault were excluded from the final analysis because these variables did not significantly add to the predictive power of the logistic regression. After transferring final probability function into Arc/view 3.2 software, landslide susceptibility map was prepared. The results of accuracy assessment showed that overall accuracy of produced map is 85.3 percent. Therefore, 53% of the area was located in very low hazard, 18.3% in low hazard, 21% in moderate hazard and 7.7 % residual area is located in high hazard regions. Model and then susceptibility map verity was assessed using -2LL, Cox and Snell R2, Nagelkerk R2, and was validated.
High Speed Capacitor-Inverter Based Carbon Nanotube Full Adder

Abstract: Carbon Nanotube filed-effect transistor (CNFET) is one of the promising alternatives to the MOS transistors. The geometry-dependent threshold voltage is one of the CNFET characteristics, which is used in the proposed Full Adder cell. In this paper, we present a high speed Full Adder cell using CNFETs based on majority-not (Minority) function. Presented design uses eight transistors and eight capacitors. Simulation results show significant improvement in terms of delay and power-delay product in comparison to contemporary CNFET Adder Cells. Simulations were carried out using HSPICE based on CNFET model with 0.6 V VDD.
Litigation Procedure in Oil Pollution Incidents

Abstract: After the occurrence of oil pollutions and damages to persons, properties and the environment, important issues will be raised in connection with the litigation process. Victims of oil pollutions are innumerable people and need to be identified in order to be represented in a court of law. Recently, environmental organizations have also been added to the list of plaintiffs. Furthermore, because of the vast dimensions of the oil spills, it should be determined that according to the general principles of private international law, if the oil pollutions occur at high seas i.e. areas outside the territorial jurisdiction of states, which is the competent court to address the issue and which law should be applied. In the following we try to briefly examine different aspects of litigation procedure in oil pollution incidents.

Abstract: Background: Her2/neu is one of the epidermal growth factor receptors families and seems to have prognostic significance of some solid tumors. The objective of this study is to evaluate the possibility of Her2 expression in gastric cancers and the possible relationship of Her2 with tumor’s clinicopathologic parameters and also its prognostic role.Methods: This study was performed on 100 cases of gastric carcinoma with stage I b to III (according to TNM staging). Survival, recurrence date of patients, grade and lymph nodes involvement were assessed. Her2/neu expression was determined by immunohistochemical method on received sample blocks. Survival of patients with or without Her2-neu expression were evaluated by Kaplan- Meier method and compared with the log-rank test followed by multivariate analysis using Cox regression.Results: Seven cases were 3+ membranous Her2 reactivity, 5 cases were 2+ and13 cases were 1+; also 75% of cases demonstrated no reactivity. Regardingrelationship between tumor grade and membranous Her2 , all patients with poorly differentiated tumors were Her2 negative but patients with moderate and well differentiated tumor had 18.1% and 19.6% Her2 reactivity respectively; there were no significant difference between groups statistically(P>0.05). Median overall survival was 27.25 and 46 months in Her2 negative and her2 positive cases respectively; there were no significant difference between groups statistically as well (P>0.05).Conclusion: Her2 reactivity has not relationship with tumor grade and lymph node involvement as well as tumor stage. From the other point of view no significant correlation is found between Her2 expression and disease free survival or overall survival of gastric cancer patients.
