Showing that the student has a specific scientific competence, This document is historical certificate and evidence from knowledge of the teachers to the Prophet. In this article, we studied eyhzade (dervish’s son) Ahmed Ziya Efendi’s diploma recieved from Muderris (proffesor) Hasan Kuds Efendi that was Islamic jurist and lived in the last period of Ottoman State. This sertificate is an example part of the general sertificate for mental and transplants covered by all the sciences and given in detail for the scientific diploma. Firstly in this article we mentioned Ahmed Ziya Efendi’s and Hasan Kuds Efendi’s short biographies. Secondly we presented information about authorization/icazet after that assessed this sertificate. Thirdly we translated this authorication to Turkish.

Abstract: One of the roads that led to diplomatic and commercial relationships between the Northern and the Southern countries passed through Dagestan. In the XVIth century, the effectiveness of this and commercial road began to increase. In the XVIIth century it was one of the main roads followed by diplomats travelling from Europe to Iran. The Western merchants came after the Western diplomats. However, in comparison with the Russian and the Iranian merchants who activated there, the effectiveness of the Western merchants had never been same. After the Russian occupation of Astrakhan, travelling of Dagestanians to Astrakhan and its north areas was restricted. The Dagestanians merchants achieved to pass over this restriction in first half of the XVIIth century. In this paper, the trade road passing through Dagestan in the XVIIth century, place of the Dagestan economy of the towns on thsi trade road and the Dagestanian merchants activating in the Idil-Caspian road have been examined.

Abstract: We have trained Artificial Neural Network (ANN) with back propagation algorithm by using five different foliages. After training process , we have tested 15 similar foliages for each class and classification is desired by ANN. Then we have tried to classify 6 different foliage which are not trained before. The ANN, tried to classify these samples and it classify them to the nearest trained foliage class of the 5 different trained class.
Ga-67 Uptake: a Predictor of Post-Therapy Active Residual Disease and Clinical Outcome in Patients with Diffuse Large Cell Lymphoma

Abstract: Purpose: Diagnosis and treatment of post-therapy active residual disease (PTARD) is essential in patients with lymphoma. After treatment, Ga-67 scan is considered as a useful technique for monitoring response in these patients. Material and Methods: Between December 1998 and January 2004, 63 patients histopathologically diagnosed with Diffuse Large Cell Lymphoma (DLCL) were studied. Patients were evaluated before and after treatment with a whole body Ga-67 scan in addition to other imaging techniques. International Prognostic Index (IPI), and clinical variables were determined according to criteria reported by the International non-Hodgkin’s Lymphoma Prognostic factors Project. Results: Among the patients with positive computed tomography (CT) scan, the 5-year relapse-free and overall survival rates were 70% and 78% for those with negative scans compared with 23% and 35% for gallium-positive patients, respectively (p< 0.004, p<0.008). Furthermore, the 5-year relapse-free and overall survival rates were 92% and %91 for those with negative scans compared with 33% and 40% for gallium-positive patients (p< 0.001, p< 0.004), among the patients with negative CT scan. All patients were divided into two groups according to the IPI index after therapy and the 5-year relapse-free survival rate for negative Ga-67 scan is 75%, as compared with 42% for restaging positive Ga-67 scan (p<0.004) in the patients with low IPI score.Conclusion: Ga-67 scan is capable of detecting PTARD that remains undetected at CT and it appears to be a better predictor of outcome than previously evaluated pretreatment risk factors in patients with DLCL.
Acral myxoinflammatory fibrablastic sarcoma: Report of six cases

Abstract: Acral myxoinflammatory fibroblastic sarcoma was first described in 1998. The tumor typically involves distal extremities; especially fingers and toes. It is usually located in subcutaneous tissue. The age range is broad (mean age 40). The size of tumor varies between 1-8 cm. Microscopically, myxoid and hyaline zones are characteristic. Inflammatory cells are abundant and there are large cells resembling Reed-Sternberg or ganglion cells and multivacuolated cells having lipoblast-like appearance.We present 6 cases of acral myxoinflammatory fibroblastic sarcoma, locations were ankle, 1st toe and hand. The most presenting symptoms were swelling with or without pain and they have lasted for 3,5 months-4 years.Immunohistochemical studies of our cases, revealed vimentin (5/5), CD68 (5/5), CD34 (1/3) positivity. Ki-67 proliferation indices varied between 2-10%. Five patients underwent tumor excision, while in one patient amputation had been performed.This tumor is a low grade sarcoma but has the potential of recurrence and even metastasis, though it is rare. In one of our cases, recurrence was observed; in none of our cases metastasis has been determined.It included many tumoral and non-tumoral lesions in differential diagnosis.
Transient Osteoporosis of the Hip: Case Report

Abstract: Transient osteoporosis of the hip is an uncommon, self-limited entity of unknown cause. The condition was first described by Curtiss and Kincaid in 1959 as a syndrome of transient demineralisation of the hip in the third trimester of pregnancy. Generally, the patients are women in the third trimester of pregnancy or middle-aged men who are seen initially with acute progressive hip pain. The other factors that may cause synovitis and osteopenia do not accompany the disease. Joint space has usually been protected. Transient osteoporosis of the hip, which has a benign pattern, usually improves spontaneously in a few months . In this article, we aimed to review transient osteoporosis of the hip with a case who had right hip pain and was diagnosed as transient osteoporosis of the hip. Turk J Phys Med Rehab 2004;54:122-4.

Abstract: In a recent paper by authors (Ziarati and Ucan, January 2001) a Back Propagation-Artificial Neural Network (BP-ANN) was adapted for predicting the required car parts quantities in a real and major auto parts supplier chain. It was argued that due to the learning ability of neural networks, their speed and capacity to handle large amount of data, they have a potential for predicting components requirements and establishing associated scheduling throughout a given supply chain system.This paper should be considered a continuation of the first paper as the neural network approach introduced in this paper replaces the BP-ANN by a new method viz., Genetic Cellular Neural Network (GCNN). The latter approach requires by far less stability parameters and hence better suited to fast changing scenarios as in real supply chain applications.The model has shown promising outcomes in learning and predicting material demand in a supply chain, with high degree of accuracy.
Differences between Epilepsy Patients Under Politherapy and Epilepsy Patients Under Monotherapy

Abstract: Objective: This study aimed to evaluate demographical variables, clinical features, neurological examination and cranial imaging results which cause or determine the necessity for politherapy treatment in epilepsy patients.Patients and Methods: The patient files of 785 epileptic patients were followed by Marmara University Epilepsy Outpatient Clinic were scanned retrospectively and a questionnaire was filled in for each file. Patients were grouped as monotherapy or politherapy, considering the number of antiepileptics they used. The demographical variables, clinical features, neurological examination and imaging results of the two groups were compared. The results were analyzed by Student's-t test and chi-square tests.Results: A history of craniotomy and intracranial tumors is more frequent in politherapy patients. Also, simple partial seizure, convulsive status epilepticus, pathological neurological examination findings, abnormal EEG, cranial imaging (MRI/CT) and SPECT results were found to be higher in politherapy patients.Conclusion: Politherapy is an important step in epilepsy treatment. However, because of drug-drug interactions and drug side effects it is not the first choice. Determining the differences between patients who use politherapy and monotherapy will help recognizing the clinical data which may lead to politherapy need. (Marmara Medical Journal 2012;25:123-7)
Civil Society and Civic Consciousness = Sivil Toplum ve Sosyal Bilin

Abstract: The meta-narratives of modern times are considered as the most significant factors impeding the development of the civil society' that coincided with the western definition of democracy. Nevertheless, this article suggests that the real problem was institutional inadequacy, which was produced by lack of civic consciousness. The best evidence for this argument is the search by theoreticians for a civic consciousness despite their different ideological orientations. Indeed, the demise of meta-narratives in post-modern times does not tend to lead to a more civil society because of the vicissitude of this extant institutional inadequacy, which appears to be created by the decline of cultural understanding of civic consciousness.

Unlike that, it evaluates knowladge as a means of arranging human’s relationship with universe and God. The main aim of science and civilization from the view of the Holy Qur’an is to “read” creatures in universe and realitiesabout them; from this point, to try to know God’s existance, solidarity and dignity as HE wants.The Holy Qur’an values all mental, emotional and physical actions done to reach “knowledge of facts undoubtly as “producing knowladge” and “ thinking” activities. In addition, it also sees the efforts to reach the facts as worship. The Holy Qur’an gives place not only to the verses that human is acknowladged by God but also the human’s understanding these verses, commenting them and developing new points of views from them. In this article, knowladge which can be produced by human, usage of them and the concepts about thinking which may be displayed while fulfilling these activities by human will be explained.
