终末期肾病患者血液透析期间低血压的原因及对策

Abstract: 为探讨终末期肾病(end-stagerenaldisease,ESRD)患者血液透析期间低血压发生的原因及对策,回顾性分析行血液透析的30例ESRD患者临床资料。结果,ESRD30例患者血液透析2428例次,发生低血压206例次,发生率为8.5%,发生低血压的最重要原因是体液超滤过多过快(占63.6%),其次为心功能不全、自主神经功能紊乱等。应避免体液超滤过多,设定合理的干体重,积极监控和预防血液透析中自主神经功能紊乱及其他引起低血压的相关因素。更多还原

Abstract: 摘要:目的 检测维持性血液透析患者(MHD)、非透析终末期肾脏疾病患者(ESRD)血清中铁调素(hepcidin)和血清转铁蛋白受体(serum transferring receptor, sTfR)的水平变化,探讨其在肾性贫血铁缺乏中的作用。方法 选择维持性血液透析患者80例、非透析终末期肾脏疾病患者80例,健康体检者80例作为对照组。ELISA法检测血清Hepcidin、sTfR、总铁结合力(TIBC),同时测定血清铁(Fe)、铁蛋白(FER)、血清转铁蛋白(TRF),比较组间差异。结果 ①维持性血液透析患者和非透析终末期肾脏疾病患者血清hepcidin和sTfR水平均明显升高,差异有统计学意义(P<0.01)。②与非透析ESRD组相比,MHD组的血清hepcidin和sTfR水平显著升高,差异有统计学意义(P<0.05)。③相关分析显示,血清hepcidin与Scr、hs-CRP、血清铁(Fe)、铁蛋白(FER)呈正相关,r分别为0.515、0.312、0.290、0.286,P均<0.01;与HGB、eGFR、HCT、TRF、TIBC呈负相关,r分别为-0.466、-0.362、-0.285、-0.248、-0.224,P<0.05。④血清sTfR水平与铁蛋白、转铁蛋白饱和度(TSAT)有相关性,相关系数分别为-0.764、-0.519,P均<0.01;与血红蛋白和红细胞压积有相关性,相关系数分别为0.378和0.514,P均<0.01。⑤血清hepcidin、sTfR联合铁蛋白或转铁蛋白饱和度诊断缺铁时,绘制受试者工作特征曲线(receiver operating characteristic curve, ROC),曲线下面积为90.8%,对应的灵敏度为93.3%,特异度为77%。 结论 血清hepcidin、 sTfR 联合铁蛋白或转铁蛋白饱和度对于ESRD患者合并肾性贫血铁缺乏的评估及治疗有重要意义。
ABSTRACT: Objective To detect the changes of serum hepcidin and serum transferrin receptor (sTfR) levels in patients on maintenance hemodialysis (MHD) and those end-stage renal disease (ESRD) patients not on dialysis so as to explore their role in renal iron-deficiency anemia. Methods A total of 80 patients on MHD, 80 ESRD patients not on dialysis, and 80 healthy controls were enrolled in this study. Serum hepcidin, sTfR and total iron-binding capacity (TIBC) were measured by ELISA. Serum Fe, ferritin (FER) and transferrin (TRF) were measured by routine methods. Data were compared among the groups. Results ① Serum hepcidin and sTfR levels were significantly higher in patients on MHD and ESRD patients not on dialysis than in the healthy controls (P<0.01). ② Serum hepcidin and sTfR were significantly higher in patients on MHD than in ESRD patients not on dialysis (P<0.05). ③ Pearson correlation analysis revealed that serum hepcidin was positively correlated with Scr (r=0.515), hs-CRP (r=0.312), serum Fe (r=0.290) and FER (r=0.286) (all P<0.01); but negatively correlated with HGB (r= -0.466), eGFR (r=-0.362), HCT(r=-0.285), TRF(r=-0.248) and TIBC (r=-0.224) (P<0.05). ④ Pearson correlation analysis revealed that sTfR was positively correlated with HGB (r=0.378) and HCT (r=0.514) (both P<0.01), but negatively correlated with FER (r= -0.764) and TSAT(r=-0.519) (both P<0.01). ⑤ The area of serum hepcidin, sTfR and FER or TSAT under the ROC curve was 90.8%, and the sensitivity and specificity were 93.3% and 77%, respectively. Conclusion Serum hepcidin, sTfR and ferritin or transferrin saturation is important for assessing and treating ESRD patients with renal iron-deficiency anemia

Abstract: 摘要:目的 探讨促红细胞生成素(EPO)对肾性贫血大鼠肾脏的保护作用及其可能的作用机制。方法 将实验大鼠随机分为对照组(NC组,n=18)和实验组(n=72),采用腺嘌呤注射建立肾性贫血大鼠模型,再随机分为单纯贫血组(SA组)、铁剂+促红素组(IR+EPO组)、促红素组(EPO组)、铁剂组(IR组)。药物干预4周后,处死大鼠(心脏取血),自动化血液分析仪检测血清肌酐(Scr)、尿素氮(Bun)、血红蛋白(Hb)、血细胞比容(Hct);ELISA法检测铁调素(Hepcidin)。免疫组织化学染色法和Western blot法分别检测大鼠肾脏骨形态蛋白-6(BMP-6)、丝/苏氨酸激酶受体-1(SMAD-1)和SMAD-4的表达。结果 ①一般指标的比较:与SA组相比较,IR+EPO组和EPO组BUN、Scr显著降低(P<0.01);IR+EPO组和EPO组Hb、Hct显著升高(P<0.01)。②ELISA法检测示IR+EPO组和EPO组铁调素显著降低(P<0.01)。③病理学观察提示EPO可以减轻大鼠肾脏病理及功能改变。④免疫组织化学染色示各组大鼠肾小球细胞胞质和肾小管上皮细胞内均表达SMAD-4、SMAD-1、BMP-6。⑤Western blot进一步提示,与SA组相比,IR+EPO组和EPO组SMAD-4蛋白表达量显著下调(P<0.01),IR+EPO组和EPO组SMAD-1蛋白表达量下调(P<0.05),IR+EPO组和EPO组BMP-6蛋白表达量显著上调(P<0.01)。结论 EPO通过BMP/SMAD传导通路,抑制铁调素的表达及分泌,纠正贫血,发挥肾脏保护作用。
ABSTRACT: Objective To investigate the protective effect of erythropoietin (EPO) on the kidney of renal anemia rats and further explore the renoprotective action and possible mechanisms. Methods A total of 90 SD rats were randomly divided into 5 groups: normal control rats (NC), simple anemia group (SA), iron + EPO group (IR+EPO), erythropoietin group (EPO), and iron group (IR) with 18 in each. The rats were sacrificed after 4 weeks?? treatment with intragastrically-injected adenine. Serum creatinine (Scr), blood urea nitrogen (Bun), hemoglobin (Hb), hematocrit (Hct), and hepcidin were measured with automated hematology analyzer. The expressions of bone morphogenetic protein-6 (BMP-6), a serine/threonine kinase receptor-1 (SMAD-1) and serine/threonine kinase receptor-4 (SMAD-4) in the kidney were detected by immunohistochemistry and Western blot. Results ① The results of general indicators: BUN and Scr in IR + EPO group and EPO group were lower than those in SA group (P<0.01). Hb and Hct in IR + EPO group and EPO group were increased significantly compared with those in SA group (P<0.01). Hepcidin was significantly increased in simple anemia group (P<0.01). ② ELISA results showed that Hepcidin was lower in IR + EPO group and EPO group than in SA group (P<0.01). ③ The results of pathological observation indicated that EPO could reduce rat renal pathology and function. ④ Immunohistochemistry revealed that SMAD-4, SMAD-1 and BMP-6 were expressed in the rat glomerular cells and renal tubular epithelial cells. ⑤ Western blot further indicated that the expression of SMAD-4 was significantly decreased in IR+EPO group and EPO group (P<0.01). Abstract: 植物病毒可通过影响植物形态和生理特性从而对媒介昆虫和寄生性天敌产生作用。然而,在植物媒介昆虫寄生蜂三营养级关系研究中有关植物病毒的影响很少被考虑。本研究测定和分析了番茄植株感染番茄黄化曲叶病毒(tomatoyellowleafcurlvirus,TYLCV)后叶毛密度的变化及对烟粉虱Bemisiatabaci(Gennadius)重要寄生性天敌海氏桨角蚜小蜂EretmocerushayatiZolnerowichandRose行为与适合性的影响。结果表明:携带TYLCV病毒番茄植株叶毛密度显著增加,为健康植株叶毛密度的1.8倍。海氏桨角蚜小蜂在带毒植株叶片上的寄主处置时间和寄主块停留时间显著长于其在健康植株叶片上的时间,分别为其2倍和1.5倍,但寄生蜂的寄生率、羽化率及发育历期差异不显著(P>0.05)。本文首次报道了双生病毒侵染可引起叶毛密度的增加,对理解植物-双生病毒-烟粉虱-寄生蜂四方关系提供了新的数据。
钩藤总碱对焦虑模型行为学和脑组织单胺类神经递质的影响

Abstract: 目的考察钩藤总碱(rhynchophyllatotalalkaloids,RTA)抗焦虑药效。方法采用国际通用的大鼠高架十字迷宫焦虑动物模型(elevatedplus-mazeanxietyanimalmodel,EPM)和小鼠期待性焦虑实验,观察RTAⅠ组[200mg/(kg?d)]、RTAⅡ组[400mg/(kg?d)]、RTAⅢ组[800mg/(kg?d)]3个剂量组对大、小鼠焦虑行为学的影响;采用酶联免疫吸附法(ELISA)测定RTA干预前后EPM大鼠的脑组织内去甲肾上腺素(NE)、多巴胺(DA)和5-羟色胺(5-HT)单胺类神经递质的含量。结果与空白对照组比较,RTA3个剂量组大鼠进入开放臂次数比例(OE%)和在开放臂滞留时间比例(OT%)有显著升高(P<0.05),RTA3个剂量组小鼠均无应激性体温升高,RTAⅢ组EPM大鼠脑组织中NE、DA、5-HT含量较空白对照组显著下降(P<0.05)。结论在大鼠EPM和小鼠期待性焦虑实验中,RTA显示出一定的抗焦虑作用,其作用机制可能与降低EPM刺激后大鼠脑组织中NE、DA和5-HT含量有关。

Abstract:
通过水面浮床栽培试验,研究了水芹菜(Oenanthejavanica (Blume) DC)的自然越冬存活率、生物量以及在富营养化水体中对氮、磷的吸收。研究表明,浮床栽培的水芹菜在武汉地区可以自然越冬,存活率为76%,植株的平均鲜量为108.22 g/株,其中水上部分占80.2%,水下部分占19.8%。在元月至4月生长期内,每平方米水面栽培的水芹菜可从水中直接吸收52.23 g的氮和11.51 g的磷。结果表明,水芹是一种适合在低温情况下进行水培的植物,并对富营养化水体有良好的净化效果。
By planting cress (Oenanthejavanica (Blume) DC) with the floating bed on water surface, survey was carried out on its overwintering livability, biomass and capability to absorb nitrogen and phosphorus in the eutrophicated water body. Research showed that the cress was able to live through the winter in Wuhan area with survey rate of about 76%. The individual biomass was as high as 108.22 g/ind., with upper water surface part of 80.2% and under water of 19.8%. From January to April, cress planted on water surface per square could absorb 52.23 g nitrogen and 11.51 g phosphorus. Results suggested that cress is a cold-resistant plant which can be planted on the floating bed. It has a high potential for clearing the eutrophicated water.
云南省小麦品种(系)抗秆锈性分析

Abstract: 为了解当前云南省小麦品种对小麦秆锈病的抗性,于2012-2013年用小麦秆锈菌生理小种21C3CTHQM、34MKGQM和混合小种对收集到的云南省102份小麦生产品种和68份农家品种进行了抗秆锈病鉴定。结果表明,凤麦39、靖麦10号、玉麦3号等65份生产品种及云0072、云0300、云0205等3份农家品种对小麦秆锈病表现抗性,分别占供试品种(系)的63.7%和4.4%。表明云南省小麦农家品种的抗性水平总体较差;生产品种对小麦秆锈病的抗性总体处于中等水平,一些新育成的优良品种以及大面积推广品种对小麦秆锈菌表现高度感病,这种情况应引起高度重视。

Abstract: Eu^2 激发的氯磷酸锶荧光粉一直是一种传统的灯用荧光粉,合成了蓝色荧光粉Sr5(PO4)3Cl:Eu^2 ,并研究了在电子激发下的发光特性,此外还对碱土金属Ca^2 ,Ba^2 等共同掺杂之后对发光性能的影响及原因进行了分析,当在中-低电压工作下,对其在不同电流密度的电子束激发下的发光亮度和饱和特性进行了测量。
欧克肖特:知识论视野中的现代性批判——以政治为例

Abstract: 欧克肖特“现代性”批判的矛头直指欧洲四百年思想史所体现出的一种“理性主义”的气质,究其根本,则源于传统对“知识”的定义只强调“技术知识”,而忽略了“实践知识”这另一重要的知识形态。欧克肖特所要做的,便是在知识论层面上张举并恢复后者应有的地位,同时,在政治实践领域,维护“传统”的价值以对抗“意识形态”。可以说,他的这一努力为广义知识论的建立做出了特有贡献,并同“默会知识论”一起汇入到20世纪复兴亚里士多德实践哲学传统的洪流中去。

Abstract: 朱湘的新诗有很明显的个人特征,他力争用复活的旧诗词语灌注的新诗语言系统来恢复古典诗歌的理想形态,个人化地试验诗歌,进行从旧向新的渐变。他模仿古诗的题材、主题、意境、词句和形式技巧,但艺术价值有限,主要原因在于其诗的“隔”、古典套语的烂熟、跨文化融合的排异与冲突。
