El consentimiento informado en el tratamiento integral del ni o con fisura labio-alveolo-palatina Informed consent in the comprehensive treatment of children with labial-alveolar-palatal clefts

Abstract: Introducción: el Servicio de Cirugía Maxilofacial del Hospital Pediátrico Universitario "Juan Manuel Márquez" de La Habana adoleció de un proceso estandarizado de consentimiento informado. Se tuvo además como propósito el analizar los criterios de los representantes válidos sobre la información que le brindaría el consentimiento informado. Métodos: se estudió a los 32 integrantes del equipo multidisciplinario y a 9 representantes válidos de los pacientes para así constatar el grado de conocimiento sobre los principios de ética Médica y Bioética. Resultados: se mostró que la mayoría tuvo un nivel bajo en cuanto a ética Médica y niveles medio o alto en la Bioética. Los representantes válidos presentaron un nivel bajo. Se confirmó el reconocimiento de la necesidad e importancia del consentimiento informado aunque el equipo de salud refirió que no lo utiliza adecuadamente. Se elaboró una propuesta metodológica del proceso de consentimiento informado. Conclusiones: el nivel de conocimiento sobre los principios de la ética Médica y la Bioética de los profesionales no se correspondió con la percepción que ellos tuvieron ni con la expresión práctica de dichos elementos. Introduction: the Maxillofacial Surgery Service at Juan Manuel Marquez Children's University Hospital in Havana did not have a standardized informed consent procedure. Another purpose was to analyze the opinions of valid representatives about the information to be provided by such informed consent. Methods: a study was conducted of the 32 members of the multidisciplinary team and 9 valid representatives of the patients to verify their awareness of the principles of medical ethics and bioethics. Results: most showed low awareness of medical ethics, and medium to high awareness of bioethics. Valid representatives showed a low level of awareness. Acknowledgement of the importance of and need for informed consent was confirmed, but the medical team admitted that they do not use it appropriately. A methodological proposal was developed for the process of informed consent. Conclusions: awareness of the principles of medical ethics and bioethics by professionals was not consistent with their perception of such principles or their implementation.
Osteogénesis por distracción en defectos mandibulares y su estabilidad esquelética a los 2 a?os de tratados

Abstract: to show the results of the skeletal stability after 2 years of to be operated on from the first patients presenting with mandibular hypoplasias treated with osteogenic distraction in our country. eleven hemi-mandibles in 7 patients using mandibular osteogenic distraction with leibinger distraction device. latency period was of 72 hours, mandibular distraction was applied at a rate of 1,0 mm each 12 hours and fixation was of 8 weeks. then, postsurgical orthodontic treatment was started. there were significant postoperative skeletal changes. results were assessed by cephalometry studies before intervention, after and at 2 years of treatment. also, there were significant postoperative skeletal changes in snb and anb with p< 0,05. the arithmetic mean of the osteodistraction achieve in branch was of 7,5 mm and in the body it was of 8,0 mm. at 2 years there remain a mandibular bone stability of 88,8 % qualified as good. occlusal changes were favorable. there were not complications involving the good course of patients treated. it was shown that the bone distraction is an effective method for treatment of mandibular defects.

Abstract: El presente estudio tiene como objetivo mostrar los resultados de la estabilidad esquelética a los 2 a os de operados, de los primeros pacientes con hipoplasias mandibulares tratados con distracción osteogénica en nuestro país. Se trataron 11 hemimandíbulas en 7 pacientes con distracción osteogénica mandibular, con distractores marca Leibinger. El periodo de latencia fue de 72 horas, la distracción mandibular se aplicó a razón de 1,0 mm cada 12 horas y la contención fue de 8 semanas. Luego se inició el tratamiento ortodóncico posquirúrgico. Se evaluaron los resultados a través de estudios cefalométricos antes de la intervención, luego y a los 2 a os del tratamiento hubo cambios esqueléticos posoperatorios significativos en las mediciones angulares cefalométricas SNB y ANB, con p< 0,05. La media aritmética de la osteodistracción obtenida en rama, fue de 7,5 mm y en el cuerpo de 8,0 mm. A los 2 a os existió una estabilidad ósea mandibular de 88,8 % evaluada de buena. Los cambios oclusales fueron favorables. No existieron complicaciones que afectaran la buena evolución de los pacientes tratados. Se demostró que la distracción ósea es un método eficaz para el tratamiento de defectos mandibulares. Abstract: El tumor odontogénico adenomatoide es un tumor poco frecuente derivado del epitelio odontontogénico, que contiene estructuras canaliculares con modificaciones inductivas de intensidad variable en el tejido conjuntivo. Es una lesión de crecimiento lento y poco invasiva pero que se puede asemejar a otras lesiones odontógenas de mayor agresividad como el quiste dentígero y el ameloblastoma entre otros. Su localización clásica (área de caninos superiores) nos orienta al diagnóstico y su patrón histológico ductiforme es muy propio de este tumor. Otros tumores que se encuentran dentro de este grupo son el fibroma ameloblástico, el odontoameloblastoma, el quiste odontógeno calcificante y los odontomas compuesto y complejo. Este grupo de lesiones puede o no tener formaciones de tejido duro dental dentro de ellos. Por esta razón, se presenta un paciente con este tipo de tumor, al que se le realizó estudio histopatológico, se revisó la literatura acerca de este tumor odontogénico benigno y sus características clínicas, radiográficas, tratamiento, así como los diagnósticos diferenciales que se deben tener en cuenta. The adenomatoid odontogenic tumor is an uncommon neoplasm derivative of the odontogenic epithelium containing canalicular structures with inductor modifications of variable intensity in the conjunctival tissue. It is a slow growth lesion and no much invasive but that may to be similar to other odontogenic lesions more aggressive including the dentigerous cyst and the ameloblastoma among others. Its classical location (upper canine area) guides us to diagnosis and its duct histological pattern is very typical of this tumor. Other tumors included in this group are the ameloblastic fibroma, the ameloblastic odontoma, the calcified odontogenic cyst and composed and complex odontomas. This group of lesions may or not to have formations of hard tissue inside. Thus, authors present the case of a patient presenting with this type of tumor making a histopathology study, a literature review on this benign odontogenic tumor and its clinical radiographic features, treatment, as well as the differential diagnoses to be into account.

QUEILONASO Y ESTAFILORRAFIA PRECOZ EN UN TIEMPO QUIRURGICO: RESULTADOS POSTOPERATORIOS. 1990-2006

Abstract: the combined cleft palate fissures are among the most difficult to treat morph physiological abnormalities. in the last decades, there has been a valuation in whether to treat precociously the soft palate along with the lip with the purpose to obtain better aesthetical and functional results. this investigation has the reason to evaluate the results of the application of palatorrhaphy in patients with cleft lip (plap) or their combinations, who were treated in our service in the last 15 years, regarding: sample presentations, maxillary growth and development, occlusion, phonetic and aesthetic results. a descriptive study was performed in a universe of 207 patients with cleft lip and palate fissures (clpf) that were surgically intervened in our service, the sample was made out of the 122 who were submitted to such technique. the 40,2% of the patients had clpf out of which the 69,4% was unilateral and the rest bilateral. the 56, 5%, isolated non syndromic palate fissure and only the rest were complete. the total of the sample had a normal growth and development. the 97% of the sample were evaluated as aesthetically satisfactory and the 91% was phonetically evaluated as favorable. results speak in favor of this technique for the treatment of these fissures.

Abstract: Las fisuras labio-alveolo-palatina combinadas entre sí, constituyen alteraciones morfofisiológicas y estéticas muy difíciles de tratar. En las últimas décadas se ha valorado la conveniencia de realizar el cierre del paladar blando muy temprano conjuntamente con el cierre del labio con el propósito de obtener mejores resultados estéticos y funcionales. La presente investigación tiene el propósito de evaluar los resultados de la aplicación de Queiloestafilorrafia precoz en los pacientes con fisuras labio-alveolo-palatina (PLAP) o sus combinaciones, tratados en nuestro Servicio, en los últimos 15 a os, en cuanto a: presentación de la muestra, crecimiento y desarrollo maxilar, oclusión, resultados estéticos y foniátricos. Se realizó un estudio descriptivo en un universo de 207 pacientes con FLAP operados en nuestro Servicio, cuya muestra la constituyó los 122 que se sometieron a la mencionada técnica. El 40.2% de la muestra presentó FLAP, de los cuales 69.4% fueron unilaterales y el resto bilaterales. El 56.5% presentó Fisura palatina no sindrómica aislada y sólo la mitad de ellas eran completas. La totalidad de la muestra presentó un crecimiento y desarrollo y oclusiones normales. El 97% de la muestra fueron evaluados como estéticamente satisfactorios y el 91% fue evaluado foniátricamente como favorable. EL empleo de la queiloestafilorrafia precoz constituye un abaratamiento para el Estado, del gasto per cápita en pacientes figurados de $ 6 033.25 MN. Abstract: El presente trabajo tiene como propósito proponer a los cirujanos maxilofaciales dedicados a la cirugía maxilofacial pediátrica una metodología de tratamiento en la atención multidisciplinaria al paciente fisurado labio alveolo palatino, de manera tal que se unifiquen los criterios de tratamiento y sistematice la atención a este tipo de paciente en todo el país. Se realizó una revisión bibliográfica en la Internet, en la búsqueda de artículos originales que abordaron esta temática y se encontró que en algunas instituciones de salud en México, Chile, Colombia y Brasil existen protocolos de tratamiento para la atención multidisciplinaria del fisurado labio-alveolo-palatino.
