Current management of gastric cancer

Abstract: gastric cancer is a disease with high incidence and mortality in our population. the prognosis of patients with this disease is closely related to the neoplasm stage at diagnosis, including the following characteristics of the tumor: extension into the gastric wall thickness, spread to locoregional lymph nodes and the ability to generate distant metastases, as described by the tnm classification. for localized tumors characterized only by invasion of mucosa or submucosa at diagnosis, survival at 5 years is between 70 and 95% with exclusive surgical management; however, when extension into the gastric wall is higher and/or there is locoregional nodal involvement, survival decreases to 20-30% at 5 years. currently, at high-volume centers, the extent of gastrectomy is individualized based on several parameters, which in an increasing number of cases allows a total gastrectomy with d2 lymphadenectomy and preservation of the spleen and pancreas. this improved procedure increases the chance of r0 surgery and improves the relationship between resected and affected lymph nodes, resulting in a decreased risk of the long-term locoregional recurrence. to improve these results, different therapeutic strategies combining chemotherapy or chemoradiotherapy with surgery have been tested. previously, the intergroup 0116 clinical trial, published in 2001, which changed clinical practice in the united states, showed that adjuvant chemoradiotherapy improved survival (from 26 to 37 months overall survival) of these patients. in europe, perioperative chemotherapy has been considered the standard treatment, since the publication of two randomized phase iii trials showed an increase at 5 years survival in the group treated with chemotherapy.

Abstract: The aims were to describe the behavior of the subtalar joint and foot in the race walk, and seek for correlations between them and the footprint. 12 race walkers participated in the study. The arch index was calculated on their footprints. Plantar pressures were measured and 3D photogrammetry used on a single support while they race walked at their own competitive speed. Maximum pressures were calculated in each region of the foot and also the maximum and minimum values of the three angles that describe the subtalar joint. The maximum pronation was higher than that described in the walking gait and similar to that of the running gait (-13.6 ± 3.90). In the beginning of the support the subtalar joint was between the walking and the running gaits. This suggests that the cushioning mechanism of this joint is adjusted according to the type of locomotion. The region of the foot that registered higher pressures was the external rearfoot (21.02 kPa/kg) followed by the internal forefoot (13.12 kPa/kg), showing a different behaviour to that of the running gait, in which both present similar maximum pressures. The subjects with lower arches tended to support with the internal face of the foot (r=-0.713) and with the leg inclined medially (r=0.874). Likewise, the racewalkers with higher arches registered higher pressures in the external part of the rearfoot, whereas the lowest ones did it in the internal part of the midfoot. Los objetivos fueron describir el comportamiento de la articulación subastragalina y el pie en la marcha atlética y buscar correlaciones entre estos y la huella plantar. Participaron 12 marchadores. Se calculó el índice del arco sobre sus huellas plantares. Se registraron presiones plantares y se aplicó fotogrametría 3D durante un apoyo mientras marchaban a velocidad individual de competición. Se calcularon las presiones máximas en cada región del pie y los valores máximos y mínimos de los tres ángulos que describen la articulación subastragalina. La máxima pronación ha mostrado ser mayor que la descrita en la marcha común y similar a la de la carrera (-13.6±3.90). Al inicio del apoyo, la articulación subastragalina se encuentra entre la marcha y la carrera, sugiriendo un ajuste en su mecanismo amortiguador según el tipo de locomoción. La zona que registra mayores presiones es el retropié externo (21.02 kPa/kg) yseguida lu ego del antepié interno (13.12 kPa/kg), a diferencia de la carrera, que presenta presiones máximas similares. Abstract: This paper presents the maps of solar radiation on the surface of south-east region of Spain named Región de Murcia. These maps are the result of treatment, study, and correlations obtained by data of 35 weather stations distributed throughout the region. These stations have collected data for 6 to more than 25 years. The paper presents the work performed for the treatment of collected data, the correlations used for the adjustment of the data, and the parameters obtained as a result of this adjustment. The weather stations are equipped with various sensors to collect several parameters, the measures covered and used for the study were global and diffuse radiation (in case that the station has pyranometer diffuse) and ambient temperature. The results were used to produce maps of horizontal radiation (global and diffuse) and ambient temperature at the surface of the region studied. To achieve the irradiation maps for this extended region has been used “r.sun” program, technical program that with the help of GRASS program uses geographic information systems (GISs) to evaluate the influence of geographical variables captured on horizontal plane at the surface. Using the results of the program for theoretical data and/or generic, and with the feedback obtained by the correlations of measures covered from weather stations, we have studied the incident radiation on the region. The main result of this study was to produce maps of irradiation on horizontal plane, correction for inclined surfaces, and also maps of temperature of the studied region. On this basis, results have been produced maps and tables of monthly solar radiation (mean maximum and minimum) for each of the 45 municipalities of the Region of Murcia. Thus, was obtained a useful tool for the calculation of available energy and thermal needs at the design process of plants using solar thermal and photovoltaic. 1. Introduction The energy from the sun is responsible for the vast majority of the types of energy that transforms and use the humanity. Besides, this source of energy is inexhaustible on a human scale, so solar energy has a special place in the field of renewable energy. To enable the collection and use of solar energy is necessary to make a well-designed catchment facility following a series of theoretical and technical criteria, right inclination to Ecuador, design of solar tracking systems, correct choice of materials to be used by recruiters, and so forth.
