A phenomenological analysis of post operative pain experiences in three subcultures in Iran

Abstract: Introduction: Pain is a phenomenon with both personal and cultural meanings. As cultural diversity increases around the world, caregivers are more and more likely to care for clients from backgrounds different from their own. Culture affects the assessment and management of pain. The aim of this study was to explore the meaning of pain experiences in 3 subcultures in Iran. Materials and Methods: A phenomenological approach was used in order to explore the meanings of post operative pain experiences in participants. Data were gathered through series of semi structured interviews as well as goal-oriented observation and collected written narratives. Benner's interpretive method was used for data analysis. Results: From analysis of viewpoints of 15 patients from Khorram-Abad, 13 patients from Tehran and 8 patients from Gorgan, 4 themes emerged. Considering postoperative pain insignificant compared to primary pain, need for protection, increase in spirituality and insensitivity of healthcare providers were the emerged themes. Conclusion: While pain is a subjective and personal phenomenon and reaction to pain is influenced by cultural and personal values, in this study no difference was found in the pain experiences in 3 under-study subcultures. It seems that Iranian-Islamic culture acts as a conceptual umbrella to encompass the whole experiences and influences all subcultures.

Abstract: The core concept of nursing is care; although caring is a universal concept and a component of the philosophies guiding the practice of all health and caring professions. Human beings learn to care bybeing cared by other people. In primitive societies there was not a clear distinction between healer and carer. However, caring, healing, and curing include a meaning of getting control over and expressing power upon people who are receiving care. That was a crucial point for the separation of caring from healing and curing. Nursing evolved from the mother's role and is frequently identified in people's perception more as a woman's duty than a job. Nursing continues to struggle to overcome the stereotypes held for centuries. However, as nursing is maturing as a science, an emphasis on the meaning of care and the approach of nursing as a purely caring science is apparent in many scientific nursing books and papers. Caring is evolving as a new paradigm for nursing, as the profession seeks its rightful place in the modern societies.

Abstract: living with the human immunodeficiency virus and the acquired immunodeficiency syndrome usually becomes a negative personal experience, which translates into pain and suffering. this means a real challenge for the nursing care, which is based on a human interaction which complexity depends on the dialog and bilateral interaction between the nurse and the patient, and implies a reflection of moral kind, in all that it involves values, feelings and moral justifications; that in the practice have frequently been overlooked. this article intends to show a theoretical framework based on empirical research that allows exploring and understanding, the moral feelings that guide the relation nurse-person, who live with hiv / aids. in this way, it is necessary to resource to theoretical concepts of the moral philosophy of nursing.
The relationships among critical thinking disposition, caring behavior, and learning styles in student nurses  [PDF]

The purpose of this study was to identify the relationships among disposition toward critical thinking, learning styles, and caring behaviors in student nurses enrolled in three 5-year junior nursing colleges in Southern Taiwan. Methods: This cross-sectional study used a convenience sample that comprised 777 nursing students from two cities. The data were obtained through the Critical Thinking Disposition Inventory, Chinese Version (CTDI-CV), Index of Learning Styles (ILS), and the Caring Behavior Scale (CBS). The sample ranged in age from 16 to 22 years (M = 18.21). Results: Students scored highest on the inquisitiveness subscale and lowest on the truth-seeking subscale. Additionally, some dimensions of disposition toward critical thinking differed significantly by learning style. There was a positive relationship between overall critical thinking dispositions and caring behaviors (r = 0.23, p < 0.01). Finally, dimensions of caring behavior, with the exception of the maturity dimension, were significantly positively related to most dimensions of the disposition toward critical thinking (p < 0.05). Conclusion: The findings indicate that not only is learning style related to critical thinking disposition but also to caring behaviors. However, the cause of these relationships warrants further study.

Re-Scaling the Socio-Ecological Italian Conflicts: Marginality as Arena of Practices  [PDF]

Abstract: The progressive raising of socio-ecological conflicts in Italy and the ordinary abuse of “emergency regimes” are progressively contributing to the fragmentation of public sphere. In Italy, “territorial public action” has long since abandoned indeed the ability to arrange the goods according to the model of the Nation State and Welfare State: recent cases like ILVA in Taranto, the earthquake in L’Aquila, and the improper management of the “environmental regimes” testifying the growing of regional conflicts around distribution of bads and production of risks. Abusing of emergency regimes is becoming both a structural constraints and a strategy for Italian policies, forcing institutional rationality to set up umbrella concept—sustainable development for instance—, a body of international Law in its own right, but de facto proceed intentionally avoiding implementation processes, broadening space-temporal misfit, favoring negative cascade effects even on democracy spaces. Moreover, the vertical shift of political authority produces an enforcement of functional differentiations reducing the policy efforts for a multisectorial and multistakeholder approach, the occasions of citizenship and inclusion of public policies, enhancing on the opposite “territorial conflict” in collective resistance. The aim is not to test the validity of the best technique to solve territorial conflicts. Rather it is to show the right path of a counter-apparatus and the role of polycentric perspectives and micro-territorial practices enhancing cognitive processes on territorial policies and breaking up the organizational constraints of “emergentism”. Using small case studies stands out the role of marginality as “creative approaches” that increase the governance (and government) effectiveness of public action, turning on the interstices that link different functional domains. Those capabilities are not usual to public action itself, belonging rather to community of practices. We name these “caring practices”, able to foster opportunity to de-structuring consolidated bodies of knowledge and set an unusual interaction processes together with the institutions. These practices do not re-balance the conflicts, but are able to overturn territorial conflicts in a new “codex” of learning opportunities, helping to set up renewed political spaces widening the intensity of citizenship and generating territorial innovation.
Caring Behaviour of Nurses in Orthopaedic Wards of Selected Health Institutions as Perceived by Patients  [PDF]

Abstract: Nurses are trained to provide hands-on care to patients in a variety of settings, which include physical care which can range from total care to helping a patient prevent illness. The health of a patient can significantly improve when they (patients) perceive caring nursing behaviours and are satisfied with the quality of care been received from their care giver. The aim of this study was to determine the caring behaviour of nurses as perceived by patients admitted to orthopaedic ward in some selected hospitals in Osogbo, Osun state, Nigeria. A descriptive comparative design was used. Convenient sample technique was used to select 616 respondents consisting of male and female patients admitted to orthopaedic wards of selected hospitals. The questionnaire on Caring Behaviour Assessment scale was adapted from CBI. The most highly ranked perceived nurses’ behaviour by patients rated “satisfactory” was the general perception of the patients and rendering of basic needs. The findings also revealed that patients rated nurses behaviour (care-giving) with respect to human needs assistance, the provision of safe environment, assisting in psychological care and giving physical comfort as average. Lastly, findings also revealed that patients rated perceived nurses behaviour (care-giving) as “not satisfactory” with regards to meeting their communication needs, nutritional needs and individualized care. In conclusion, care giving is an important branch of nursing and it depicts the quality of care received by a patient which in turn is capable of aiding speedy recovery.

Abstract: Background: Many studies have focused on exploring the concept of care from patient and nurse perspectives, but knowledge is limited regarding student perceptions. Objective: To explore the meanings given to the concept of professional care from the perspective of graduate students in nursing and pastoral care. Research design: A qualitative study was employed with the formation of six focus groups. Data were analyzed via a thematic content analysis of the discussions. Participants and research context: Thirty-one students attending a University College in Oslo participated. Findings: Seven main themes and forty-four subthemes were identified. Major themes included reverence and respect for the dignity and value of human life, bonding, sensitive to self and other, communication, competence, willfulness and deep caring. Discussion: Different levels of intentionality, professional comportment and caring consciousness were revealed in the discussions. Findings also lend support to major beliefs and values in Watson’s Human Caring Theory. Conclusion: The focus groups generated valuable detail of complex experiences behind student’s perceptions, attitudes, beliefs and actions. Focus group methodology can enhance holistic nursing practice by providing opportunities to explore and clarify holistic care values, create opportunities for self-awareness and transformative learning in education, clinical practice, administration and research.
The Impact of Concept Mapping on Caring Efficacy of Palestinian Nursing Students at Clinical Setting  [PDF]

Abstract: Background: The rapid change of health care delivery affirms the need for a change in the culture of nursing education through identifying the challenge to nursing faculty that many teach in the way they were taught, and suggesting a focus on the exploration of new teaching methods and student learning, rather than only focus on content coverage. Objectives: To examine the impact of concept mapping on caring efficacy of Palestinian nursing students. Methods: A quasi-experimental design, pre-post test, two groups (intervention and control group) were used. Convenience sample of 80 (40 participants in each group) nursing students from Arab American University, Jenin. Caring Efficacy was measured by Caring Efficacy Scale. The statistical tests such as t-test (independent and paired t-test) were used to examine the effect of concept mapping on caring efficacy of the fourth year nursing students at their clinical training. Results: Independent-samples t-test revealed statistically significant differences between the two groups in caring efficacy (t = 2.589, df = 78, P = 0.011); students in the intervention group showed higher level of caring efficacy than students in the control group during their clinical training. Conclusion: Concept mapping was found to be an effective teaching method for nursing students. The use of concept mapping is useful in enhancing meaningful learning.

Abstract: every one hold analytical responsibility when a caring theory is shown be cause building and confirming or invalidating postulates contributes to the progress of science criticism, in its original analysis sense, it is part of science as well as confirming and enriching legitimately its khowledge and application. transcultural care formed and mainly transmitted by american nurses, it is originated within american anthropology. it is convenient to reflect, in order to elude privileging specific cultural group?s studies, impoverishing those groups of a western culture. this specific situation encourages analyzing some aspects of the american anthropology with a european perspective attaching those positions and proposing a universal and cultural care.
Perspectives of Patients and Nurses on Caring Behaviors of Nurses

Abstract: Background & Aim: Although caring is a complex concept with various definitions, there are no agreement on the exact meaning among researchers. The aim of this study was to compare caring behaviors of nurses from patients' and nurses' perspectives. Methods & Materials: The study was conducted among 150 patients hospitalized in Boushehr hospitals and 50 nurses caring for them. Data were collected using quota sampling method via Caring Behaviors Inventory (CBI). Results: Findings showed that there were significant differences between patients' and nurses' perspectives on the subscales of the instrument including assurance of human presence and attentiveness to others' experiences. Moreover, there was significant difference in total scale of caring behaviors of nurses between patients and nurses (t=2.559, P=0.011). Conclusion: Nurses believed that they provide care for patients without making estimations of their expectations. It seems that nurses should attend to standard care to fill the gap
