Subcutaneous Trastuzumab (Herceptin®): A UK Time and Motion Study in Comparison with Intravenous Formulation for the Treatment of Patients with HER2-Positive Early Breast Cancer  [PDF]

Abstract:

Aim: Firstly, to quantify active healthcare professional (HCP) time and costs associated with subcutaneous (SC) administration of trastuzumab (Herceptin?) compared with the standard intravenous infusion (IV) in the treatment of patients with HER2-positive early breast cancer within the adjuvant PrefHer trial setting; secondly, to measure patient time in the care unit and patient infusion chair time for both routes of administration. Methods: A UK multi-centre prospective, observational Time and Motion study was conducted alongside the PrefHer trial (ClinicalTrials.gov id: NCT01401166). Trained observers measured the duration of each SC and IV related task that HCPs undertook and recorded patient time in the chemotherapy unit and infusion chair. The type and quantity of medical consumables used with each route of administration were also collected. Twenty-four patient episodes were recorded (12 SC, 12 IV). Mean total administration time was calculated as the mean sum of task times, both for IV and SC formulations. The mean cost of each route of administration was calculated as the mean cost of HCP time plus the mean cost of consumables used. HCP time was costed using Personal Social Services Research Unit. Consumables were costed using hospital pharmacy data and online sources. Results: Mean active HCP time for IV administration was 92.6 minutes compared with 24.6 minutes for SC administration. The mean cost for IV preparation and administration was £144.96 (£132.05 of HCP time and £12.92 of consumables) versus £33.15 (£31.99 of HCP time and £1.17 of consumables) for SC administration. Mean time spent in the care unit and in the infusion chair was 94.5 minutes and 75 minutes respectively for IV, and 30.3 minutes and 19.8 minutes for SC. Thus, effective education can occur only within an organization that is capable of analyzing the needs of future generations and that is capable of creating delivery systems that will meet the needs. In this article, the author explored the nature of public school districts and public school management in primary schools and secondary schools in U.S. to shed light on the pros and cons of the educational system.

Abstract: As the internet grows in popularity and therefore also in size more and more transmission takes place mainly because the technology is more readily available and applications have become more user friendly allowing entry to less sophisticated user over a broad spectrum. Most data transfer are mainly text based not secure and vulnerable to various forms of security risks. So the model that uses SSH for securing channel like intranet/internet which provides client authentication encryption and decryption with high degree of security by transferring the data in an encrypted format, up on this model enhances the efficiency of data transmission by encrypting or decrypting the data with AES in Counter Mode. AES is a symmetric key encryption standard. Moreover the permutation controlled by data can be performed at high speed in generic cpu. This scheme also expands the key space without costing more to run. And also finally through the combination of secure shell (ssh) and AES (Counter Mode) not only enhances the security of communication channel. It also provides various applications like remote user creation, remote user deletion, remote command execution, remote system shutdown, remote file transfer applications in a highly secure manner.

Abstract: The objective of this study is to analyze the ability of the public management of environmental problems faced by the municipalities in the state of Minas Gerais, Brazil. The theoretical elements that underlie this article are related to environmental management and municipal public administration. We used the results of the natural vulnerability and the social potential of the indicator of municipal environmental management, as well as data on sanitation and waste disposal. The weaknesses of public administration are located in the counties of low population group. These municipalities do not have suitable structure, formal organization and power of decisiveness in dealing with environmental problems.

Abstract: the purpose of the present study was to verify the correspondence between the administrative experience of the hospital "sofia feldman" and the theory on flexible management proposed by motta (1991) and médici & silva (1993). it also described innovation elements for institutional integration with the community, such as the substitute mother, ombudsman and systematic voluntary work, done by community association of friends and users of sofia feldman hospital. data were collected with a semi-structured interview, applied to employees of differents sectors, aiming at knowing their perception about the administrative model used. the flexible administration characteristics of the hospital were evidenced, however authors found elements of classical administration.

La autonomía de las instituciones prestadoras de servicios de salud (IPS): más un ideal que una vivencia institucional

Abstract: the issue of hospital autonomy has aroused considerable international interest since the mid-1990?s, especially with regard to its linkage to governmental decentralization processes and reform of the health field. non-profit hospitals have increasingly been obliged to become more autonomous as they compete for funding with private institutions. objective: to understand how autonomy expresses itself through key management processes in non-profit and for-profit hospitals both in medellín, the second largest city of colombia, and its metropolitan area. different institutional factors-political, administrative and economic-influencing autonomy are examined. methods: a grounded theory method was used, consisting of semi-structured interviews with managers in six hospitals. results: findings suggest that autonomy is perceived more as an ideal to aspire to, which would enable the hospital management to make free decisions, providing services in a manner that ensures financial sustainability. in practical terms, however, the degree of administrative and financial autonomy is circumscribed by both internal organizational factors associated with hospitals characteristics and external factors associated with institutional environment.
