The Role of Oropharyngeal Barotrauma as a Cause of Pneumomediastinum: Report of a Case  [PDF]

Abstract: Free air or gas in mediastinum is defined as pneumomediastinum. This is a rare condition which orginates from over distention of alveolus and alveolar rupture by barotrauma. A 6-year-old boy was admitted to our department with sudden onset, swelling of neck and face that developed during drinking water from tap by using his mouth. Physical examination revealed the presence of subcutaneous emphysema over the two side of the face that extended toward the neck bilaterally. The chest X-ray and CT of the thorax, when performed, revealed the diagnosis of pneumomediastinum and extrathoracic subcutaneous emphysema. Interestingly neither trachea-bronchial nor esophageal pathology was found by emergent rigid bronchoscopy and endoscopy for etiology of pneumomediastinum. Antibiotic treatment and oxygen therapy were given to the patient with chest pain and dyspnea. During the following days, the patient’s condition improved notably, with almost total resolution of the cervical emphysema and pneumomediastinum confirmed by daily chest X-ray and control thorax CT. He was discharged home after 6 days. The emphysema gradually resolved. Pneumomediastinum caused by barotrauma is a rare condition and only conservative treatment is required when the other causes are ruled out.

Abstract: The importance of the role of inflammation in allergic asthma and IgE has been better understood in recent years. Omalizumab is an anti IgE antibody which has demonstrated efficacy among patients with moderate to severe persistent allergic asthma, whose symptoms are inadequately controlled with other agents. This therapy is generally well tolerated but has a potentially life-threatening risk of anaphylaxis. In this case report we aimed to emphasize the mechanism, management and preventive precautions in omalizumab associated anaphylaxis.

Abstract: Nowadays, using the Internet, for academic and non-academic activities, is an integral part of students’ daily life. Studies showed that excessive use of the Internet has negative effects on physical and mental health. The present research was conducted in order to compare the psychological disorders of Internet addicted students and ordinary Internet users in Zabol University of medical sciences. It applied a causal-comparative method. Through applying purposive sampling method, 252 undergraduate and postgraduate students of Zabol University of Medical Sciences were selected in May 2015. Data collection was performed using standard questionnaires of Young’s Internet Addiction Test and the Symptom Check List 90-Revised (SCL-90-R). Data analysis was carried out using independent t-test with SPSS21. Results indicated that the Internet addicted students, compared with ordinary Internet users had more psychological disorders. Considering the high prevalence of psychological disorders in the Internet addicted students, teaching appropriate use of the Internet to students is an important factor in controlling the Internet addiction.

Abstract: Thirty-one samples of cigarettes have been collected from local markets of different types of origins. The samples were selected according to a survey distributed to smokers by paper and digital survey to see the most heavily traded among smokers and in addition to a number of questions to see how the awareness and the culture of smokers in diseases caused by smoking and considered this study the first survey in Iraq. The aim of this research is to assess the number of cancer cases due to cigarette smoking. Through the use of High-Purity Germanium system (HPGe) (efficiency 40%) we determinated the radionuclides in cigarette tobacco. The average values were (14.86 ± 3.76, 10.84 ± 3.13, 1050.64 ± 47.57) Bq/kg for Ra-226, Th-232 and K-40, respectively, and the excess lifetime of cancer risk values ranged from 0.54 to 130 at average of 76 per million person per year. Raeq values varied from 18.50 to 87.21.4 Bq/kg with an average value of 39.51 Bq/kg for tobacco samples. The annual effective dose (HE) varies from 16.38 μSv/y to 44.69 μSv/y with an average value of 24.97 μsv/y. The Annual Gonadal Dose Equivalent (AGDE) varies from 0.3 to 0.64 (mSv/y) with an average value of 0.42 for all tobacco samples under investigation.

Abstract:

In this paper, the problem of optimum allocation of repairable and replaceable components in a system is formulated as a Bi-objective stochastic non linear programming problem. The system maintenance time and cost are random variable and has gamma and normal distribution respectively. A Bi-criteria optimization technique, weighted Tchebycheff is used to obtain the optimum allocation for a system. A numerical example is also presented to illustrate the computational details.

Abstract: The relation between the HRM and the firm performance is analyzed statistically by many researchers in the literature. However, there are very few nonlinear approaches in literature for finding the relation between Human Resource Management (FIRM) and firm performance. This paper exposes the relationship between human resource management and organizational performance through the use of nonlinear modeling technique. The modeling is proposed based on Radial Basis Function (RBF) which is nonlinear modeling technique in literature. The relation between 12 input and 9 output parameters is investigated in this research that is collected between 54 companies in Turkey which indicated that the relationship between organizational management performance and relationship management can be modelled through nonlinearly.
A Model of Happiness for Older Adults Living with Their Families

Abstract: Objective: Subjective well-being is important not only for adolescents or adults but also for older people. This study aimed to develop and to test a path model which consists of personality traits, loneliness, and perceived control for elderly individuals living with their families. Methods: In this study, cross-sectional and causal-comparative research designs were used. A total of 218 older adults, 108 females (49,5%) and 110 males (50,5%), with a mean age of 67,60 years (Sd: 6,43) completed the Adjective-Based Personality Test, UCLA Loneliness Scale, Domain General Perceived Control Scale, Life Satisfaction Scale, and PANAS Scale. Path analysis method and descriptive statistics were used, and a p value of 0.05 was considered statistically significant. Results: According to the results of the model analysis, standardized coefficients for the relationships between subjective well-being and independent variables were found between 0.10 and 0.38 (p<0.01). The values of fit index which related with the developed model (RMSEA .052, X2/Sd ;3,84; p <.001) indicate that the observed data fitted well to the developed model. Conclusion: The subjective well-being levels of the older adults decreased in parallel with the increase in the levels of loneliness, neuroticism and conscientiousness. On the other hand, the subjective well-being levels of elderly increased in parallel with the increment in the levels of perceived control. The model developed in this research may be useful for specialists such as psychiatrists, geriatricians, clinical psychologists, welfare workers, geriatrics nurses in order to increase subjective well-being levels of older adults. Using Stochastic Colored Petri nets for Designing Multi-Purpose Plants  [PDF]

Abstract: Designing of a multi-purpose plant as one of the well-known manufacturing systems is more challenging than other manufacturing systems. This paper applies a stochastic colored Petri net (CPN) to design and analyze multi-purpose plants. A simple approach is proposed to determine the utilization of shared resources and to reduce the equipment’s idle times. Three scenarios are presented to describe the proposed model. Generally, according to desire of a decision maker, different scenarios can be considered in the model to achieve to the expected design or plant configuration. The main characteristics of the proposed model are flexibility, the easiness of practical application and the simulation of the model in an easy way.
Prevalence of congenital hydrocephalus in the Hashemite kingdom of Jordan: A hospital-based study  [PDF]

Abstract: Background: Congenital hydrocephalus, an important cause of neurologic morbidity and mortality in children, is a medical condition characterized by an abnormal accumulation of cerebrospinal fluid in the brain. It can be caused by abnormal brain development, obstruction of the cerebral aqueduct flow, Chiari malformations, and Dandy-Walker malformation. The prevalence of congenital hydrocephalus is 2.2 to 18 per 10,000 live births. Objectives: To determine the prevalence of congenital hydrocephalus among livebirths in the Hashemite Kingdom of Jordan (HKJ). Methods: Clinical data were collected from medical records of all livebirths with congenital hydrocephalus born at Al Bashir Hospital, Amman/The Hashemite Kingdom of Jordan in 2004-2005 and 2008-2011. Descriptive analysis was carried out using the Statistical Package for the Social Science (SPSS 20). Results: The prevalence of hydrocephalus was 0.092% with a relative predominance among males. Conclusion: The study concluded that the prevalence of congenital hydrocephalus is comparable to that of developed countries. The efforts made by the Ministry of Health seem to have paid off. More research on stillbirths is recommended.
Local and Time Changes over a 66-Year Period and Annual Relocation of Saudi Arabian Subtropical High Pressure  [PDF]

Abstract: Saudi Arabian subtropical high pressure is a major system affecting general circulation of the atmosphere of west Asia. Its annual relocation affects the relocation of other systems in the area, such as Mediterranean cyclones, Sudanese low pressure areas, and west wind waves. This system is known to relocate to the south and north in response to outward solar relocation, but the reasons behind its eastern and western relocation have not been studied thoroughly. The present study examined 1000 and 850 HPa levels over the course of 66 years (1948-2015) to determine a pattern of latitudinal and longitudinal relocation of the system using synoptic maps. The research showed that, after 2008, high pressure latitudinal and longitudinal swings were larger than in previous years and the annual high pressure relocation was not in concord with the apparent motion of the sun. At the onset of autumnal moderation, the high pressure core was positioned to the north of Saudi Arabia (22°- 30°?north latitude and 42°- 50°?south longitude). Southern movement continued until the end of March, when the core again relocated to the north. These relocations first occurred slowly, but the northern relocation occurred very quickly from May to June, such that the core moved northward 22°?to 30°. After June, the core did not relocate much until the end of September. After September, it relocated strongly south in all time periods. It was noted that high pressure over Saudi Arabia had two cores from June to September in some years; in others the high pressure core was in southwestern Iran and Iraq. In still others, a southwest to northeast high pressure tab entered Iran from the southwest (Khuzestan) and continued northward with a core forming in the northern Caspian Sea.
