Erectile Dysfunction among Diabetic Men in Two Medical Centers in Burkina Faso: Epidemiological, Diagnosis and Therapeutic Aspects  [PDF]

Abstract: Objective: To study erectile dysfunction in diabetic patients seen in two clinics in the city of Bobo-Dioulasso, Burkina Faso. Materials and Methods: A prospective cross-sectional descriptive study was conducted at the Souro Sanou Teaching Hospital (CHUSS) and the Saint Leopold clinic in Bobo-Dioulasso, from March 1 to September 1, 2012. A total of 107 patients data were collated and analysed, which was then grouped into two: the ED group, designating patients with erectile dysfunction and the NED group consisting of those patients without. The sample comprised of 61 patients with types 1 and 2 diabetesand were aged between 25-70 years. The IIEF-5 was used to evaluate erectile dysfunction. Results: The prevalence of erectile dysfunction was 57%. The average age of patients was 54.4 ± 8.3 years. All patients with ED had type 2 diabetes. The mean disease duration of diabetes was 7.2 ± 6 years. Erectile dysfunction was severe in 32.8% of cases, moderate in 31.1% of cases and mild in 36.1%. Its severity was significantly associated with glycated hemoglobin, triglycerides and BMI. Phosphodiesterase types 5 (PDE5) inhibitors were found to be effective in the treatment of erectile dysfunction with a satisfactory therapeutic response in 77.4% of users. Conclusion: Erectile dysfunction is a common complication in diabetic patients. Its occurrence and severity are influenced by several factors. The potential presence of this disorder should be assessed due to its negative impact on quality of life. Phosphodiesterase type 5 inhibitors are an effective treatment modality in diabetic patients.
Stackelberg-Walras and Cournot-Walras Equilibria in Mixed Markets: A Comparison  [PDF]

Abstract: In this note, we compare two strategic general equilibrium concepts: the Stackelberg-Walras equilibrium and the Cournot-Walras equilibrium. We thus consider a market exchange economy embodying atoms and a continuum of traders. It is shown that, when the preferences of the small traders are represented by Cobb-Douglas utility functions, the Stackel-berg-Walras and the Cournot-Walras equilibria can coincide only if 1) the endowments and preferences of atoms are identical and 2) the elasticity of the followers’ best response functions are equal to zero in equilibrium.

Abstract: Software Product Line Development advocates software reuse by modeling common and variable artefacts separately across members of a family of products. Aspect-Oriented Software Development aims at separation of concerns with “aspects” to increase modularity, reusability, maintainability and ease of evolution. In this paper, we apply an as-pect-oriented use case modeling approach to product line system modeling. A use case specification captures stake-holders concerns as interactions between a system and its actors. We adapt our previous work with the introduction of a “variability” relationship for the expression of variabilities. This relationship is used to model variable and common behaviours across a family of products as use cases. A variability composition mechanism enables building of executa-ble behaviour models for each member of a product line family by integrating common elements with the applicable variable elements.
Trace Element Levels, Cytokine Profile and Immune Activation Status in Plasma among Repeat Blood Donors with Asymptomatic HIV-1, HBV and HCV Infection  [PDF]

Abstract: Imbalance of essential trace elements viz. Zinc, Selenium, Iron, Copper and Magnesium has been reported to influence disease course in HIV-1, HBV and HCV infections by altering immune status. A study was taken up to examine plasma levels of Th1 (IFN-γ and IL-2) and Th2 (IL-4 and IL-10) categories of cytokines and immune activation markers (TNF-α, TNFR I and TNFR II) in an asymptomatic group of HIV-1, HBV and HCV infected blood donors in relation to trace elements. Plasma levels of Zn, Se and Mg were depressed in all the three groups of blood donors (P < 0.001 for all). Levels of Cu and Fe were depressed in HIV-1 infection (P < 0.001 for both), but elevated in HBV and HCV infections (P < 0.015 and < 0.001 for Cu and < 0.001 for Fe in case of HBV and HCV infections respectively). IL-2 and IFN-γ were depressed in all the three groups of blood donors (P < 0.001). IL-4 and IL-10 levels were elevated in HBV and HCV infections (P < 0.001 for both). Immune activation markers were elevated in all the three groups of blood donors (P < 0.001 for all). HIV-1 infection showed positive correlations between Cu and IL-2, Zn and IFN-γ, and in HBV infection while positive correlations were found between Mg and TNFR I and TNFR II and Se with TNFR II. HCV infection showed a positive correlation between Se and IFN-γ (P < 0.001), Mg and IL-4 (P = 0.02), Fe and IL-10 (P < 0.01). The present study reveals possible relationship between trace element level alterations and alterations in cytokine and immune activation levels in HIV-1, HBV and HCV infection.
The Little ICU Book of Facts and Formulas

Abstract: The book is a condensed version of 'The ICU Book' by the same author. Intended as a compact bedside reference, it is presented as 15 chapters, each with up to 5 sub-sections. Chapters cover diverse subjects with an appendix of conversion tables, normal values and formulae. Algorithms are well referenced using current evidence.However, these strengths are offset by some notable omissions and imbalances. The haemodynamic monitoring section is exclusively based on the pulmonary artery flotation catheter (PAFC), disregarding other forms of monitoring – for example, oesophageal Doppler, LiDCO? or PiCCO?. No reference is made to evidence regarding lack of clinical utility of the PAFC. The section on haemodynamic drugs lacks breadth, omitting discussion of epinephrine, vasopressin (mentioned briefly in the septic shock chapter), phosphodiesterase inhibitors or levosimendan. Sodium nitroprusside, however, receives four pages.The chapter on vascular access mentions the importance of aseptic technique, stating that handwashing, sterile gloves and full barrier precautions are 'recommended' for central venous catheter (CVC) insertion. A simple table mandating such measures would have greater impact, including the use of alcoholic chlorhexidine and a catheter cart facilitating prior preparation. There is no reference to the use of ultrasound guided CVC insertion. Conversely, there is an unnecessary page describing peripheral intravenous cannulation.The toxicology chapter omits important aspects of self-poisoning, namely tricyclic antidepressants and drugs of abuse. The chapter on antimicrobial therapy would have been improved by repeating the Surviving Sepsis Campaign recommendations in the septic shock section and by referencing the website, rather than the now outdated 2004 recommendations. Spontaneous Recanalization of Complete Internal Carotid Artery: A Clinical Reminder

Abstract: Spontaneous recanalization of atherothrombotic extracranial cerebral arteries is rare vis-à-vis recanalization of intracranial vessels. The time course is unknown. The question is the advisability and timing of surgery in a recanalized vessel. We describe a patient with spontaneous recanalization of a totally occluded left Internal Carotid Artery (ICA) who was monitored with periodic imaging and in time had partial recanalization of the ICA. We believe patients with total ICA occlusion with return of function should be followed up with periodic carotid ultrasound, Magnetic Resonance Angiography (MRA) or CT angiography, and when appropriate, be candidates for carotid vascular interventions.
The Burkinabè novel: heterogeneous writing, texts located between written and oral forms

Abstract: A French person perusing some Burkinabè novels would undoubtedly be surprised or even baffled by tense transitions. These are often not typical, and the purists of the French language would unquestionably condemn them. Yet, these transitions express the potentials inherent in the French tense system. Some linguistic theories, like Benveniste's, if not used mechanically and sketchily, may help account for these “turbulence zones,” as some people have labelled them. My paper aims to show how, in most Burkinabè novels, this tense transition game allows for switching from one tense to another: from the Past Perfect (PP) to the Simple Past (SP) and Pluperfect (PLP), from the Present PRES to the Imperfect (IMP), and from the Future (FUT) to the Conditional (COND). This takes place in all narrative spaces, in a perfect linguistic legality. Some have explained these often unpredictable transitions as being the result of a switch between, a telescoping, of two different types of narration, two enunciation points of view of the narrator (linked diegesis and autonomous diegesis). However, this switching-telescoping may also be interpreted as the narrator oscillating between a written narration and an oral narration. We see this phenomenon as a form of intrusion or irruption, not necessarily a conscious one, of orality into writing. Hence, Burkinabè novels are heterogeneous in nature, evolving between writing and orality, and this is one of the manifest trends of novel writing in Burkina Faso.
Ecriture romanesque burkinabè: une écriture hétérogène, des textesentre écrit et oral

Abstract: Le lecteur franco-fran ais qui parcourt un certain nombre de romans burkinabè sera certainement surpris voire dérouté par les transitions temporelles dont on pourrait dire qu'elles sont souvent atypiques et que les puristes de la langue stigmatiseraient sans appel. Or la plupart du temps, ces transitions sont des manifestations de potentialités inhérentes au système temporel du fran ais. Certaines théories linguistiques, comme celle de Benveniste, à condition de ne pas en faire une application mécanique un peu caricaturale comme c'est souvent le cas, permettent de rendre compte de ces zones de turbulence comme on a pu les nommer. Cet exposé a pour objectif de montrer comment, pour la grande majorité des romans burkinabè, dans ce jeu de transitions temporelles, le PC va jusqu'à flirter avec le PS et le PQP, le PRES avec l'IMP, le FUT avec le COND dans tous les espaces narratifs, et cela, dans la plus grande légalité linguistique. L'explication consiste à voir dans ces transitions souvent imprévisibles, le résultat d'une alternance, d'un télescopage entre deux modes de narration, deux postures énonciatives du narrateur (diégétisation liée, diégétisation autonome). Cette alternance-télescopage peut aussi secondairement être interprétée comme une oscillation du narrateur entre une narration écrite et une narration orale. On pourrait voir dans ce phénomène une forme d'intrusion, d'irruption, pas nécessairement consciente, de l'oralité dans l'écriture. D'où le caractère hétérogène de ces textes pris entre écrit et oral… une des tendances manifestes de l'écriture romanesque burkinabè.
The Strategic Transformation of Automobile Industry in China

Abstract: In the past few years, the global automobile industry is developing difficultly because of the influence from the financial crisis. In contrast, China's automobile production and sales are still having a blowout type growth, and jumped into the world's largest automobile production and sales market. At the same time, Chinese automobile companies continue to deepen and join with international brand cooperation; independent research and development of the independent brand production, and their technical also get greatly strengthened. Similarly, in the tide of industrial upgrading and international acquisitions, strategic transformation era of Chinese automobile industry has gradually started. This paper based on the world economic crisis brought both challenge and opportunity to the automobile industry in China, comprehensively analysis China's automobile industry development present situation and development trend in the transformation era, with case analysis of China's automobile companies, and then gives some suggestions on China's automobile industry strategy adjustment in transformation.
