The contribution of the medical history for the diagnosis of simulated cases by medical students

Abstract: Objectives: The case history is an important part of diagnostic reasoning. The patient management problem method has been used in various studies, but may not reflect the actual reasoning process because a list of choices is given to the subjects in advance. This study investigated the contribution of the history to making the correct diagnosis by using clinical case simulation, in which students obtained clinical information by themselves. Methods: A prospective study was conducted. Ninety-four fifth-year medical students from Chiba University who underwent supervised clinical clerkships in 2009 were surveyed. Each student randomly selected 1 of 4 test cases and attempted to make a diagnosis through medical interview, physical examination, and laboratory tests, while the teacher acted as a patient. The student ranked the disease(s) diagnosed at each stage of the process. Diagnostic accuracy rates were compared using analysis of the χ2-test. Results: Sixty students (63.8 made a correct diagnosis, which was based on the history in 43 students (71.7, physical findings in 11 students (18.3, and laboratory data in 6 students (10.0. Compared with students who considered the correct diagnosis in their differential diagnosis after taking a history, students who failed to do so were 5.0 times (95 = 2.5-9.8) more likely to make a final misdiagnosis (χ[sup]2[/sup][sub](1)[/sub] = 30.73; p<0.001). Conclusions: History taking is especially important for making a correct diagnosis when students perform clinical case simulation. To improve the diagnostic reasoning skills, medical students should be trained in methods for inferring the correct diagnosis from the case history.
First-principles calculation method for electron transport based on grid Lippmann-Schwinger equation

Abstract: We develop a first-principles electron-transport simulator based on the Lippmann--Schwinger (LS) equation within the framework of the real-space finite-difference scheme. In our fully real-space based LS (grid LS) method, the ratio expression technique for the scattering wave functions and the Green's function elements of the reference system is employed to avoid numerical collapse. Furthermore, we present analytical expressions and/or prominent calculation procedures for the retarded Green's function, which are utilized in the grid LS approach. In order to demonstrate the performance of the grid LS method, we simulate the electron-transport properties of the semiconductor/oxide interfaces sandwiched between semi-infinite metal electrodes. The results confirm that the leakage current through the (001)Si/SiO$_2$ model becomes much larger when the dangling-bond (DB) state is induced by a defect in the oxygen layer while that through the (001)Ge/GeO$_2$ model is insensitive to the DB state.
Genomic Profiling of Oral Squamous Cell Carcinoma by Array-Based Comparative Genomic Hybridization

Abstract: We designed a study to investigate genetic relationships between primary tumors of oral squamous cell carcinoma (OSCC) and their lymph node metastases, and to identify genomic copy number aberrations (CNAs) related to lymph node metastasis. For this purpose, we collected a total of 42 tumor samples from 25 patients and analyzed their genomic profiles by array-based comparative genomic hybridization. We then compared the genetic profiles of metastatic primary tumors (MPTs) with their paired lymph node metastases (LNMs), and also those of LNMs with non-metastatic primary tumors (NMPTs). Firstly, we found that although there were some distinctive differences in the patterns of genomic profiles between MPTs and their paired LNMs, the paired samples shared similar genomic aberration patterns in each case. Unsupervised hierarchical clustering analysis grouped together 12 of the 15 MPT-LNM pairs. Furthermore, similarity scores between paired samples were significantly higher than those between non-paired samples. These results suggested that MPTs and their paired LNMs are composed predominantly of genetically clonal tumor cells, while minor populations with different CNAs may also exist in metastatic OSCCs. Secondly, to identify CNAs related to lymph node metastasis, we compared CNAs between grouped samples of MPTs and LNMs, but were unable to find any CNAs that were more common in LNMs. Finally, we hypothesized that subpopulations carrying metastasis-related CNAs might be present in both the MPT and LNM. Accordingly, we compared CNAs between NMPTs and LNMs, and found that gains of 7p, 8q and 17q were more common in the latter than in the former, suggesting that these CNAs may be involved in lymph node metastasis of OSCC. In conclusion, our data suggest that in OSCCs showing metastasis, the primary and metastatic tumors share similar genomic profiles, and that cells in the primary tumor may tend to metastasize after acquiring metastasis-associated CNAs.
Delivery of Plasmid DNA into Tumors by Intravenous Injection of PEGylated Cationic Lipoplexes into Tumor-Bearing Mice  [PDF]

Abstract: For systemic injection of cationic liposome/plasmid DNA (pDNA) complexes (cationic lipoplexes), polyethylene glycol (PEG)-modification (PEGylation) of lipoplexes can enhance their systemic stability. In this study, we examined whether intravenous injection of PEGylated cationic lipoplexes into tumor-bearing mice could deliver pDNA into tumor tissues and induce transgene expression. PEGylation of cationic liposomes could prevent their agglutination with erythrocytes. However, when PEGylated cationic lipoplexes were injected intravenously into tumor-bearing mice, they accumulated in tumor vascular vessels and did not exhibit transgene expression in tumors with both poor and well-developed vascularization. Furthermore, PEGylated cationic lipoplexes of CpG- free pDNA could not increase transgene expression in tumors after intravenous injection. These results suggested that PEGylation could not extravasate cationic lipoplexes from vascular vessels in tumors and abolished transgene expression although it enhanced the systemic stability of cationic lipoplexes by avoiding interactions with blood components such as erythrocytes. Successful delivery of pDNA to tumors by PEGylated cationic liposomes will require a rational strategy and the design of liposomal delivery systems to overcome the issue associated with the use of PEG.
Questioning the Weber Thesis: Capitalist Ethics and the Hebrew Bible?  [PDF]

Abstract: Weber’s thesis proposed that it was ascetic Protestantism which supported the emergence of modern capitalism in 17th and 18th century Europe, and that this was a completely new and unique phenomenon in the history of mankind up to that point in time. This paper casts doubt on the Weber thesis by examining findings from an economic reconstruction of the Hebrew Bible, and proposing that modern capitalism the way Weber understood it is already visible in the ancient religious text of the “Hebrew Bible”. By means of institutional economic reconstruction, I show that the Hebrew Bible and particularly the stories involving Jacob and Joseph reveal a conceptual structure that can be compared with ideas of modern constitutional and institutional economics. Through this reconstruction, I find myself in agreement with one of Weber’s early but largely forgotten adversaries, Werner Sombart, who suggested, in a behavioral tradition, that other religions, and more specifically Jewish thought, contributed to the emergence of modern capitalism long before the advent of Protestantism.
Complexity Reduced MIMO Interleaved SC-FDMA Receiver with Iterative Detection  [PDF]

Abstract: In this paper, we propose the receiver structure for Multiple Input Multiple Output (MIMO) Interleaved Single Carrier-Frequency Division Multiple Access (SC-FDMA) where the Frequency Domain Equalization (FDE) is firstly done for obtaining the tentative decision results and secondly using them the Inter-Symbol Interference (ISI) is cancelled by ISI canceller and then the Maximum Likelihood Detection (MLD) is used for separating the spatially multiplexed signals. Furthermore the output from MLD is fed back to ISI canceller repeatedly. In order to reduce the complexity, we replace the MLD by QR Decomposition with M-Algorithm (QRD-M) or Sphere Decoding (SD). Moreover, we add the soft output function to SD using Repeated Tree Search (RTS) algorithm to generate soft replica for ISI cancellation. We also refer to the Single Tree Search (STS) algorithm to further reduce the complexity of RTS. By examining the BER characteristics and the complexity reduction through computer simulations, we have verified the effectiveness of proposed receiver structure.
Downregulation of SAV1 plays a role in pathogenesis of high-grade clear cell renal cell carcinoma

Abstract: We performed array CGH and gene expression analysis of 8 RCC cell lines (786-O, 769-P, KMRC-1, KMRC-2, KMRC-3, KMRC-20, TUHR4TKB, and Caki-2), and expression level of mRNA was confirmed by quantitative RT-PCR (qRT-PCR) analysis. We next re-expressed SAV1 in 786-O cells, and analyzed its colony-forming activity. Then, we transfected siRNAs of SAV1 into the kidney epithelial cell line HK2 and renal proximal tubule epithelial cells (RPTECs), and analyzed their proliferation and apoptosis. Furthermore, the activity of YAP1, which is a downstream molecule of SAV1, was evaluated by western blot analysis, reporter assay and immunohistochemical analysis.We found that SAV1, a component of the Hippo pathway, is frequently downregulated in high-grade ccRCC. SAV1 is located on chromosome 14q22.1, where copy number loss had been observed in 7 of 12 high-grade ccRCCs in our previous study, suggesting that gene copy number loss is responsible for the downregulation of SAV1. Colony-forming activity by 786-O cells, which show homozygous loss of SAV1, was significantly reduced when SAV1 was re-introduced exogenously. Knockdown of SAV1 promoted proliferation of HK2 and RPTEC. Although the phosphorylation level of YAP1 was low in 786-O cells, it was elevated in SAV1-transduced 786-O cells. Furthermore, the transcriptional activity of the YAP1 and TEAD3 complex was inhibited in SAV1-transduced 786-O cells. Immunohistochemistry frequently demonstrated nuclear localization of YAP1 in ccRCC cases with SAV1 downregulation, and it was preferentially detected in high-grade ccRCC.Taken together, downregulation of SAV1 and the consequent YAP1 activation are involved in the pathogenesis of high-grade ccRCC. Some OFF bipolar cell types make contact with both rods and cones in macaque and mouse retinas

Abstract: This study compared the types of OFF bipolar cells found in the macaque retina with those found in the mouse retina and determined whether these OFF bipolar cells make direct contacts with both rods and cones by serial section transmission electron microscopy. We performed scatter plots and cluster analysis of the morphological variables of their axon terminals such as the stratification level, the arbor thickness, the arbor area, and the number of ribbons. Five OFF bipolar cell types, including the recently discovered DB3b type, were identified in the macaque retina. The macaque OFF bipolar cell types FMB, DB1, DB2, DB3a, and DB3b corresponded to the mouse OFF bipolar cell types 2, 1, 4, 3a, and 3b, respectively. In addition to contacting rod bipolar cells, approximately 7% of rods in the macaque retina made basal contacts exclusively with one cell type, DB3b, whereas 18% of rods in the mouse retina made basal contacts with one or two of types, 3a, 3b, and 4. Approximately 3% of mouse rods were divergently connected to two OFF bipolar cells of different types, but macaque rods were solely connected to one OFF bipolar cell. Rod-rod gap junctions were localized at rod cell bodies and axons in the outer nuclear layer in both macaque and mouse retinas. The direct rod-OFF bipolar connection system is slightly more developed in the mouse retina than in the macaque retina, possibly as a fine-tuned adaptation to nocturnal conditions. This one-step direct synaptic pathway from rods to OFF bipolar cells may enhance the response speed to OFF light stimuli compared with more indirect pathways via rod-cone gap junctions (a two-step pathway) and via rod bipolar and AII amacrine cells (a three-step pathway).

The cross-section measurement of the single-W process is found to give complementary bounds on the anomalous couplings to those obtained from W-pair analysis at LEP2.
The self gravity effect on the orbital stability of Twotinos

Abstract: We investigate how the self gravity changes the orbital stability of Twotinos using N-body simulations in which the gravitational interaction between planetesimals are fully calculated. We show the timescale in which the half of Twotinos becomes unstable, t_{half}, obey the formula, t_{half} = 4 \times 106(Mtot0.1M\oplus)-1(mp7.6 \times 1023g)-1(hi2i1/20.002)(years), if we assume the primordial planetesimals disk have the power low surface mass density, \Sigma = \Sigma_0 \times r-2/3. Where Mtot, mp, hi2i1/2 are the total mass of large bodies of Twotinos, the maximum mass of planetesimals, and the inclination dispersion. With this formulae, we conclude the total mass of Twotinos is reduced to the order of 0.01 M\oplus by the self gravity and secular perturbation of the Planets even if there was huge mass such as several order of the earth mass in 1:2 MMR of Neptune at the early age of the solar system. These results will invoke reexamination to many previous works which tried to explain the dynamical evolution of TNOs.
