Geometrical Evaluation of Cervical Cells. Fractal and Euclidean Diagnostic Methodology of Clinical Application  [PDF]

Abstract: Background: The concomitant use of fractal and Euclidian measurements has led to the development of new methodologies of cell evaluation, including a diagnosis of cervical cells that set up differences between normality and various degrees of lesion, to carcinoma. Aim: To confirm the diagnostic capacity of the methodology based on fractal and Euclidian geometry for the mathematical diagnosis through a blind study of normal cells and with different types of lesion, as atypia of undetermined significance (ASCUS), low grade squamous intra-epithelial lesion (LGSIL) and high grade squamous intra-epithelial lesion (HGSIL). Methods: 100 cells of Papanicolaou tests were analyzed and divided into 4 groups according to conventional parameters: 25 normal, 25 ASCUS, 25 LGSIL and 25 HGSIL. By means of the Box-counting Fractal Space, we calculated the fractal dimension and occupying spaces of the border and surface in pixels of the cell nucleus and cytoplasm. The diagnostic parameters of the previously developed methodology were applied and compared with the conventional diagnosis, setting up sensibility, specificity, negative likelihood ratio and Kappa coefficient. Results: The values of the occupation of the border and surface of the cell nucleus and cytoplasm were consistent with the values found by the diagnostic methodology previously found. The subtraction of the nucleus and cytoplasm frontiers presented values between: 189 and 482 for normality; 159 and 432 for ASCUS; 126 to 401 for LGSIL and 39 to 122 for HGSIL. A sensitivity and specificity of 100%, and a Kappa coefficient of 1 were obtained. Conclusions: The capacity of the methodology to diagnose quantitatively the different stages in the evolution of the cervical cells observed in Papanicolaou tests was confirmed, from normality to HGSIL.
Theoretical generalization of normal and sick coronary arteries with fractal dimensions and the arterial intrinsic mathematical harmony

Abstract: Starting from all the possibilities of space occupation in box-counting space, all arterial prototypes differentiating normality and disease were obtained with a computational simulation. Measures from 2 normal and 3 re-stenosed arteries were used as spatial limits of the generalization.A new methodology in animal experimentation was developed, based on fractal geometric generalization. With this methodology, it was founded that the occupation space possibilities in the stenotic process are finite and that 69,249 arterial prototypes are obtained as a total.The Intrinsic Mathematical Harmony reveals a supra-molecular geometric self-organization, where the finite and discrete fractal dimensions of arterial layers evaluate objectively the arterial stenosis and restenosis process.The fractal geometry, developed by Benoit Mandelbrot, allows irregular objects characterization, through fractal dimensions [1,2]. There are several fractal dimension definitions and different methodologies for calculation, applied according to measured object [2]. For wild fractals, such as those that characterize the morphology, box counting method is used. Fractal geometry have been used in experimental and clinical applications [3,4], for example to differentiate cardiac lesion degree in angiography, to characterize pre-neoplasic cervical cells or normal and abnormal erythrocyte morphology [5-7]. However, in some cases, isolated fractal measures not differentiate normality and disease, and could present limitations for their effective application [8].This problem was analyzed [9] in the restenosis phenomena. Although arteries present irregular form and structure, it is common to do Euclidean measurements of their shape for their posterior statistical analysis [10,11]. Rodríguez et al. Abstract: background: the professionals in the medical field are particularly exposed to psychological risk factors which may affect their quality of life. identification of stress coping strategies and the sources of professional rewards would afford the possibility of implementing preventive measures aimed a moderating the negative effect of these risk factors. the objectives of this study are to identify the stress coping strategies that are used by specialised doctors from six public hospitals of the valencian community, and to describe their sources of work reward and job satisfaction. methods: a qualitative study conducted by means of quasi-structured individual interviews of 47 specialists employed at six public hospitals in the province of valencia. the interviews were taped and subsequently transcribed. an analysis was made of the substance of the opinions expressed. results. the coping strategies mentioned most often by the physicians interviewed, analyzed overall, are those focused on the emotions, specifically, disconnecting conduct and seeking social emotional support. however, when confronted with specific stressful factors in daily practice, the strategies most often employed as those focused on the problem. a major part of the physicians surveyed state not having any work reward. among those who do state finding some aspects gratifying, top mention is made of their salary, personal satisfaction related to being in the medical field being ranked second. conclusions. including training related to active stress coping strategies in the professional curriculum of physicians could heighten their personal resources for dealing with stress. organizational changes should additionally be made to increase their work rewards.

Abstract: Las empresas actualmente se enfrentan a un entorno con innumerables cambios, haciendo que tengan que basar cada vez más sus ventajas competitivas en recursos intangibles y, entre ellos, en sus recursos humanos. Por ello, la formación de los trabajadores está recibiendo una especial atención por parte de las empresas. La formación ha de partir de los objetivos empresariales, y se debe planificar para cubrir las necesidades existentes de la misma. En el presente trabajo analizamos de forma cualitativa y cuantitativa cómo se desarrolla dicho proceso formativo en la práctica empresarial, para ello hemos utilizado los datos recogidos en una encuesta realizada a las 572 empresas castellano-manchegas de más de 50 trabajadores, de las que obtuvimos 118 cuestionarios válidos (20,6%).

Abstract: Fundamento. Los profesionales de la medicina están particularmente expuestos a factores de riesgo psicosocial que pueden afectar su calidad de vida. La identificación de estrategias de afrontamiento frente al estrés y las fuentes de recompensa profesional permitiría instaurar medidas preventivas tendentes a moderar el efecto negativo de estos factores de riesgo. Los objetivos del presente trabajo son identificar las estrategias de afrontamiento ante el estrés que utilizan los médicos especialistas de seis hospitales públicos de la provincia de Valencia, y describir sus fuentes de recompensa y satisfacción profesional. Métodos. Estudio cualitativo realizado por entrevistas semiestructuradas individuales a 47 médicos especialistas de seis hospitales públicos de la provincia de Valencia. Las entrevistas han sido grabadas en audio y transcritas posteriormente. Se ha realizado un análisis del contenido del discurso. Resultados. Las estrategias de afrontamiento referidas con más frecuencia por los facultativos entrevistados, analizadas de manera general, son las centradas en las emociones y en concreto la desconexión conductual y la búsqueda de apoyo social emocional. Sin embargo, ante factores estresantes puntuales concretos de la práctica diaria, las estrategias más frecuentes son las centradas en el problema. Una parte importante de los médicos entrevistados manifiesta no tener ninguna recompensa laboral. Entre los que sí expresan elementos de recompensa, refieren en primer lugar el salario, seguido de la satisfacción personal por la dedicación a la medicina. Conclusiones. La inclusión de formación sobre estrategias de afrontamiento activo frente al estrés en el currículo profesional de los médicos, podría aumentar sus recursos personales para hacer frente al mismo. Además se deberían instaurar cambios organizacionales que aumenten las recompensas laborales.

Abstract: objective: knowing how a person deals with and adapts to a colostomy along a two years period. methods: qualitative case study. the case shows a colostomyzed patient due to a rectal cancer. data collected through semistructured interviews to the patient and a relative at two different moments: during hospitalization and two years later. the four interviews were recorded, transcribed and analyzed according to taylor-bogdan method. results: confrontation strategies aimed at alleviate negative emotions by delegating the cares of the stoma to relatives are key during hospitalization. two years later, there is a higher concern for practical questions and the patient can finally reach autonomy. the report shows the aspects leading to adaptation and the changes the ostomy brings about in the patient's life. conclusions: ostomyzed people require individualized care based on their biopsycosocial dimension.
Aislamiento de Staphylococcus epidermidis portador de integrón clase 1 en un paciente con sepsis neonatal

Abstract: objective: to determine the presence and the sequence of the class one integron in a staphylococcus epidermidis strain isolated from a septic neonatal patient. materials and methods: the s. epidermidis strain was isolated from a blood culture of a newborn. the microbiological identification, test of sensitivity and molecular characterization of the integrase gene int/1, the cassette gene aac6; aac6′aph2′′ resistant gene to aminoglycoside antibiotics and meca gene resistant to ?-lactamics were realized. results: the nucleotide sequence of the integrase gen intl1 in s. epidermidis is reported, which showed a 78% similarity to the reported sequence of the ncbi data base in gram negative bacteria. the gene cassette aac6 aac6′ (aminoglycoside acetilation) was also identified within the class one integron confering aminoglycoside resistance. through molecular characterization we also found the aminoglycoside ?-lactamics resistance genes (aac6′aph2′y meca ′); the susceptibility tests showed resistance to ampicilin, oxacilin and gentamicin. conclusions: the nucleotide sequence of the integrase gen int/1 is reported for the first time in colombia in a s. epidermidis strain isolated from a septic neonatal patient, at the neonatal care unit of a third level hospital in bogotá. this integrase in class one integron was reported, which has shown multiresistance association in clinical isolates in gram negative bacteria. the resistance mechanism found during this approach realized in gram negative bacteria showed the evidence of an interspecies horizontal transfer especially by gene transfer or by moving elements such as integrons and gene cassettes.the molecular character of the causing agent of sepsis is important for epidemiological control of the infection and treatment in the newborn.
