Surveillance of Fluoroquinolone-Resistant Clinical Isolates of Pseudomonas aeruginosa  [PDF]

Abstract: Ciprofloxacin (CPFX) and pazufloxacin (PZFX) have strong antibacterial activity against Pseudomonas aeruginosa. We investigated the sensitivity of P. aeruginosa to CPFX and PZFX in 373 strains isolated from inpatients (321 strains) and outpatients (52 strains) during September 2010 to September 2011 at Toho University Ohashi Medical Center. The percentage of CPFX-non-susceptible (≥3.91 μg/mL) among inpatients was 22.4%, but that among outpatients was 1.9%. As the major resistance mechanism to fluoroquinolones in P. aeruginosa involves modification of type II topoisomerases (DNA gyrase and topoisomerase IV), we examined mutations in the quinolone-resistance-determining regions (QRDRs) of gyrA and parC of P. aeruginosa isolates. Among the 373 isolates, 73 isolates had reduced CPFX-susceptibility and 88 had reduced PZFX-susceptibility. Sequencing of gyrA and parC revealed base substitutions that resulted in amino acid replacements in QRDR of GyrA in 70 P. aeruginosa isolates, while Thr83Ile (in GyrA) and Ser87Leu (in ParC) substitutions were found in 12 strains. These replacements were clearly associated with reduced susceptibility to CPFX and PZFX. However, we also found strains with high MICs to quinolones without mutations in either gyrA or parC. We then investigated the effect of efflux pumps in CPFX-resistance in these isolates. In the presence of an efflux pump inhibitor, MIC values in 12 of 66 strains decreased to 1/23. We also sequenced genes related to overexpression of efflux pumps, viz., mexZ, mexR, and nfxB. Eight of the strains without mutations in QRDRs had a mutation in mexZ, 7 strains had a mutation in mexR, but no mutation was identified in nfxB.

Abstract:
Brain abscesses are commonly associated with cranial trauma, a contiguous focus of infection, or hematogenous spread from a distant focus. However, no predisposing factors are identified in approximately 4% of the cases, being recognized as a cryptogenic brain abscess (CBA). Here we report a patient with a CBA in the left occipital lobe presumably caused by a periodontal disease. The patient displayed a patent foramen ovale (PFO), through which a spontaneous right-to-left shunt was revealed with transesophageal echocardiography. A literature review indicated that in contrast to cases of general brain abscesses, patients with CBA were older and mostly had dental disorders represented by periodontal diseases and a large PFO. In these patients, the abscess was located predominantly in the posterior circulation area, and their prognosis was worse than that of general cases. Consequently, we emphasize the significance of screening for PFO in cases of advanced age with CBA in the posterior circulation region. Furthermore, to avoid neurological sequelae, we suggest immediate surgical drainage with antibiotic administration and maintenance of oral hygiene.

Abstract: Stent insertion for cerebral aneurysm has been studied using ideal and realistic aneurysms in recent years. Stent insertion aims at reducing the flow in an aneurysm. To minimize the average velocity in an aneurysm, we applied optimization to the strut position in a realistic aneurysm based on computational fluid dynamics. The result shows the effect on velocity reduction of strut placement in the inflow area.
Algorithms to Evaluate Multiple Sums for Loop Computations

Abstract: We present algorithms to evaluate two types of multiple sums, which appear in higher-order loop computations. We consider expansions of a generalized hypergeometric-type sums, $\sum_{n_1,...,n_N} [Gamma(a1.n+c1) Gamma(a2.n}+c2) ... Gamma(aM.n+cM)] / [Gamma(b1.n+d1) Gamma(b2.n+d2) ... Gamma(bM.n+dM)] x1^n1...xN^nN $ with $ai.n=\sum_{j=1}^N a_{ij}nj$, etc., in a small parameter epsilon around rational values of ci,di's. Type I sum corresponds to the case where, in the limit epsilon -> 0, the summand reduces to a rational function of nj's times x1^n1...xN^nN; ci,di's can depend on an external integer index. Type II sum is a double sum (N=2), where ci,di's are half-integers or integers as epsilon -> 0 and xi=1; we consider some specific cases where at most six Gamma functions remain in the limit epsilon -> 0. The algorithms enable evaluations of arbitrary expansion coefficients in epsilon in terms of Z-sums and multiple polylogarithms (generalized multiple zeta values). We also present applications of these algorithms. In particular, Type I sums can be used to generate a new class of relations among generalized multiple zeta values. We provide a Mathematica package, in which these algorithms are implemented.

A regi o tem cerca de 380 mil habitantes, um alto índice de violência e um grande número de jovens sem acesso à educa o básica e sem oferta de emprego. Diante disso, em 2005, a Subprefeitura articulou-se com uma série de parceiros com quem desenvolveu a ideia da CAE-IT. Inicialmente, buscou compreender a realidade local, através da análise de dados secundários. Em seguida, houve consulta a agentes comunitárias de saúde para compreender quais eram as voca es econ micas da regi o e em qual público-alvo a política deveria focar esfor os. A decis o final foi dar ênfase à promo o de arranjos produtivos locais envolvidos com reciclagem de lixo. Abstract: A cDNA, named aleh1, was obtained by the RACE method following PCR with degenerate primers designed based on amino-acid sequences of FEHs from other plants. The aleh1 encoded a polypeptide of 581 amino acids. The relative molecular mass and isoelectric point (pI) of the deduced polypeptide were calculated to be 65,666 and 4.86. A recombinant protein of aleh1 was produced in Pichia pastoris, and was purified by ion exchange chromatography with DEAE-Sepharose CL-6B, hydrophobic chromatography with Toyopearl HW55S and gel filtration chromatography with Toyopearl HW55S. Purified recombinant protein showed hydrolyzing activity against β-2, 1 type fructans such as 1-kestose, nystose, fructosylnystose and inulin. On the other hand, sucrose, neokestose, 6-kestose and high DP levan were poor substrates.The purified recombinant protein released fructose from sugars extracted from burdock roots. These results indicated that aleh1 encoded 1-FEH.Fructans (polyfructosylsucrose) are important storage carbohydrates in plants such as Poaceae (e.g. wheat and barley), Asteraceae (e.g. burdock, chicory and Jerusalem artichoke), and Liliaceae (e.g. onion and asparagus) [1,2]. Inulin-type fructan [1F(1-β-D-fructofuranosyl)msucrose], which is a β-2, 1 linked fructose-oligomer or -polymer terminated by glucose, is mainly accumulated in Asteraceae plants (Figure 1). The inulin-type fuctan is synthesized from sucrose by sucrose:sucrose 1-fructosyltransferase (1-SST, EC 2.4.1.99) and fructan:fructan 1-fructosyltransferase (1-FFT, EC 2.4.1.100). 1-SST synthesizes 1-kestose (1-β-D-fructofuranosylsucrose, 1-kestotriose), an inulin-type trisaccharide, from two molecules of sucrose by fructosyltransfer [3-6]. 1-FFT elongates fructose chain of inulin-type fructans by fructosyltransfer from 1-kestose to another 1-kestose or fructan [3,7-9]. Immunohistochemical Evaluation of Insulin-like Growth Factor I Receptor Status in Cervical Cancer Specimens

Abstract: The insulin-like growth factor I receptor (IGF-IR) is exceptionally overexpressed in many cervicalcancer-derived cell lines. It is postulated that a decrease of p53 protein levels due to human papillomavirus (HPV) infection may contribute to the up-regulation of IGF-IR expression in cervical cancer cells because transcription of IGF-IR is strictly down-regulated by p53. To evaluate this fact in clinical cervical cancer specimens, we checked the expression levels and activated status of IGF-IR by immunohistochemistry. Formalin-fixed and paraffin-embedded specimens obtained by conization or hysterectomy were stained with anti-IGF-IR and with an antibody recognizing phosphorylated tyrosine at its c-terminus. The expression levels of IGF-IR were significantly high in cervical intraepithelial neoplasia (CIN) III and invasive cancer specimens. Phosphorylation of IGF-IR was promoted in all CIN and invasive cancer specimens, and its intensity was related to the promotion of lesions. Interestingly, IGF-IR overexpression was missing in the basal layer of CIN I and II lesions, whereas it was evenly distributed in CIN III and invasive cancer lesions. This IGF-IR overexpression pattern may be utilized in the diagnosis of HPV infection status in CIN lesions.
Operational Experiment of Seamless Handover of a Mobile Router using Multiple Care-of Address Registration

Abstract: NEMO Basic Support (NEMO BS) adds a mobility function to IPv6 routers, such a router is called as a mobile router. The network behind the mobile router (mobile network) becomes logically static. This function is considered useful when a network has a lot of nodes that do not have a mobility function but move with the network. The typical usage of this technology is the network provided in a transportation system like a bus or airplane. In the base specification of NEMO BS, a mobile router can use one IPv6 address as its attachment point at one time. Therefore, the mobile router and its mobile network will face service disruption of network connectivity while the mobile router is moving from one network to other network. In this paper, we explain two operational experiments of network mobility. One is an experiment using only the base NEMO BS function. The other is an experiment of seamless handover using multiple network interfaces to solve the problem. In these experiments, the mobile networks were actual conference networks which held a few hundred people. The result shows that the multiple interfaces usage provides one-third to one-tenth of the packet loss rate compared to the case that is not using the technology. Also most attendees of the conference did not notice the movement, which means the technology can be used for the realistic solution for the seamless handover.

Abstract: Traditional Chinese herbal medicines (Kampo) are used to improve flow characteristics of blood (Oketsu). We assumed that by preventing stagnation of blood, these medicines may be beneficial not only in venous but in arterial thrombotic conditions. The present study aimed to assess the antithrombotic effect of three Kampo, using well-established in vitro and animal models of thrombosis. Western-style highfat diet containing 1% Kampo (Shimotsuto, Juzentaihoto or Unseiin) was administered to C57BL/6 mice for 12 weeks. The effect on thrombus formation by laser irradiation of the carotid artery of mice was assessed. In addition the ex-vivo technique of shear-induced platelet reactivity measurement (haemostatometry) and the in vivo test of endothelial function (flowmediated vasodilation) were also used to assess the mechanism of antithrombotic effect. All three medicines have significantly inhibited arterial thrombus formation in mice. According to our studies, the mechanism of antithrombotic effect is based on the inhibition of shear-induced platelet reac- tivity and stimulation of endothelial function (Unseiin). It is assumed that the common ingredients Japanese Angelica Root, Cnidium Rhizome, Peony Root and Rehmannia Root could be responsible for the observed antithrombotic effect.
