Effectiveness of Medical Management in Spontaneous Expulsion of Lower Ureteral Calculi: A Prospective Study  [PDF]

Abstract: Purpose: We evaluated the efficacy of combined drug therapy with alpha1-adrenergic antagonist tamsulosin and corticosteroid deflazocort for conservative expulsive therapy in patients with lower ureteric calculi. Materials and Methods: A total of 100 consecutive symptomatic patients with stones located in the Lower ureter were selected for the study who received oral tamsulosin 0.4 mg daily for 4 weeks and deflazocort 30 mg daily for 10 days and diclofenac IM/Oral on demand. Ultrasound follow-up and medical visits and X ray KUB were performed weekly for 4 weeks. Stone passage rate and time, pain episodes and endoscopical intervention were evaluated. Results: The stone expulsion rate was 87.8%. Mean stone size was 6.5 ± 2.14 mm. Mean expulsion time was 8 ± 1.84 days. Mean pain episodes were 1.50 ± 0.87 days. 11 patients needed intervention for stone retrieval. There was no statistically significant difference between the expulsion rates of right and left ureteral stones (P value = 0.31). Conclusions: Medical therapy with a combination of α1 adrenergic blocker and corticosteroid achieve good stone expulsion rates, lower pain episodes and should be considered in all patients with lower ureteral stones of size less than 10 mm who are not having any contraindications for medical therapy.

Abstract: Meckel’s diverticulum is not commonly encountered surgical entity and presents unique challenges for a pediatric surgeon, as it is prone to varied complications. A 14-year-old boy was admitted with us with a 48-hour history of lower abdominal pain and multiple episodes of vomiting. Radiological imaging studies revealed a high-grade partial small bowel obstruction. A fleeting conservative management was tried. The diagnostic laparoscopy revealed a small bowel obstruction secondary to a Meckel’s diverticulum. The diverticulum was resected using an endovascular GIA stapler. The patient was discharged on postoperative day four, tolerating a regular diet. Laparoscopy is a useful diagnostic and therapeutic means for a patient with a small bowel obstruction due to an uncertain etiology.

Abstract: Background: Kashmir valley, being a known conflict zone keeps on receiving war victims due to scuffles between civilians and the government forces. Such upsurges have been noted in the recent past in 2008 and 2010; however, a new face of upheaval took place in the month of July 2016. Herein, we present an analysis of 500 abdominal trauma victims reported to our department at Government Medical College Srinagar, Kashmir, India. Methods: Government Medical College Srinagar Kashmir is a tertiary care teaching hospital situated in the heart of Kashmir valley. Being one of the referral hospitals, for trauma, we received a total figure of 6149 trauma victims out of which 500 were purely of abdominal in nature managed by the department of surgery. The study conducted was a short analysis of these abdominal trauma patients that reported from 8th July to 8th November 2016. Results: The result of analysis of 500 abdominal trauma patients is reported herein. The male female ratio of patients was 19.8:1 (male = 95.2%, female = 4.8%). The age range was from 5 years to 65 years with majority reported in the age range of 15 - 30 years. The number of bullet injuries was =85 and the pellet injuries were =349. The commonest organs involved were small intestines, spleen, large bowel and liver in descending order. Penetrating injuries were seen in =60.4%, non-penetrating were seen in =39.6%. The patients were grouped into conservative, non operative and operative group. The commonest morbidity observed was wound sepsis and a total mortality reached to a figure of 2%. Conclusion: This short study of analysis of trauma victims has explored the newer horizons of trauma management and we believe and conclude that specialized trauma hospitals with skilled manpower and modern gadgets of handling trauma is need of an hour especially in developing countries like ours.
Study of the Structural and Electrical Properties of Cr-Doped BiFeO3 Ceramic  [PDF]

Abstract: Multiferroic BiFe1-xCrxO3 (x = 0.2 and 0.4) ceramics were synthesized in a single phase. The effects of Cr3+ substitution on the crystal structure, dielectric permittivity and leakage current were investigated. Preliminary X-ray structural studies revealed that the samples had a rhombohedral perovskite crystal structure. The dielectric constant ε' significantly increased while the dielectric loss tanδ was substantially decreased with the increase in Cr3+ substitution. The temperature effect on the dielectric properties exhibited an anomaly corresponding to magneto-electric coupling in the samples and was shifted to lower temperatures with the increase in Cr3+ substitution. The leakage current density also reduced in magnitude with the increase in the Cr3+ substitution.
A Study of Students and Teachers' Preferences and Attitudes towards Correction of Classroom Written Errors in Saudi EFL Context

Abstract: It is no doubt that teacher written feedback plays an essential role in teaching writing skill. The present study, by use of questionnaire, investigates Saudi EFL students' and teachers' preferences and attitudes towards written error corrections. The study also aims at identifying the difficulties encountered by teachers and students during the feedback process. The questionnaire findings reveal that both teachers and students have positive attitudes towards written error correction. The study also shows that while teachers and students share such common preferences as the importance of error correction and the types of errors, there are considerable discrepancies as to the techniques of error correction. For instance, students favor the overall correction, whereas most teachers do not. However, the results show that students prefer teacher correction to peer and self-correction. Moreover, both the teachers and students have encountered a number of difficulties, so suggestions have been investigated so as to deal with the problems.

Sex steroids, also known as sex hormones are synthesized naturally by the gonads (ovaries or testes). The two main classes of sex steroids, androgens and estrogens, are crucial hormones for the proper development and function of the body; they regulate sexual differentiation, the secondary sex characteristics, and sexual habits. They are also well known to influence many common cancers, especially hormonally driven cancers such as breast and prostate cancer. In this report, we review the association of sex steroids with cancer and the potential use of endocrine manipulation in cancer therapy as well as the limitations and challenges faced in this field.

A Fast Pattern Matching Algorithm Using Changing Consecutive Characters  [PDF]

Abstract: Pattern matching is a very important algorithm used in many applications such as search engine and DNA analysis. They are aiming to find a pattern in a text. This paper proposes a Pattern Matching Algorithm Using Changing Consecutive Characters (PMCCC) to make the searching pro- cess of the algorithm faster. PMCCC enhances the shift process that determines how the pattern moves in case of the occurrence of the mismatch between the pattern and the text. It enhances the Berry Ravindran (BR) shift function by using m consecutive characters where m is the pattern length. The formal basis and the algorithms are presented. The experimental results show that PMCCC made enhancements in searching process by reducing the number of comparisons and the number of attempts. Comparing the results of PMCCC with other related algorithms has shown significant enhancements in average number of comparisons and average number of attempts.

Abstract: The charmonium-like states, Y(4260), Y(4350), produced via initial state radiation, as well as the X(3872), and Y(3940), produced in B meson decays from the BABAR B-factory are reviewed. These mesons do not seem consistent with conventional charmonium models, and several alternate hypotheses have been proposed to explain these new discoveries.

Abstract: Recently, several charmonium-like states above $D\bar{D}$ threshold have been discovered at the BELLE and BABAR $B$-factories. Some of these states are produced via Initial State Radiation ({\it e.g.} Y(4260) and Y(4350)) and some are observed in $B$ meson decays ({\it{e.g.}} X(3872), Y(3940)). The BELLE observation of the enhancement in the $\psi(2S)\pi^-$, {\it {i.e.}} the $Z(4430)^-$ state, has generated a great deal of interest, because such a state must have minimum quark content ($c\bar{c}d\bar{u}$), so that it would represent the unequivocal manifestation of a four-quark meson state. Here we report recent BABAR results on the Y(4260), X(3872), Y(3940), and a search for the $Z(4430)^-$.

Abstract: Neutral-current ({NC}) and charged-current ({CC}) deep inelastic scattering ({DIS}) reactions have been studied in $e^+p$ and $e^-p$ collisions using the {H1} and {ZEUS} detectors at {HERA I}. Following the upgrade of the {HERA} accelerator, the {HERA II} program recently started with the first data in $e^+p$ scattering with longitudinally polarised positrons. In this paper, a summary of the electroweak results from {HERA I} and the first measurement of the cross section for $e^+p$ CC {DIS} at a longitudinal polarisation value of 33% from the {ZEUS} collaboration are presented.
