Diagnostic Utility of Sago-Like Nodules on Gross Thoracoscopic Appearance in Tuberculous Pleural Effusion and Their Correlation with Final Histo-Microbiologic Findings  [PDF]

Abstract: Background: Distinguishing tuberculous pleural effusion (TPE) from other causes of exudative effusion is often challenging. Delay in treatment initiation can occur while awaiting histo-microbiologic confirmation owing to the paucibacillary nature of the disease. Sago-like nodules are the most common visual finding on gross thoracoscopic appearance. The primary objective was to determine the diagnostic utility of the presence of sago-like nodules on gross thoracoscopic appearance in TPE to help justify early initiation of tuberculosis (TB) treatment based on their finding while awaiting final histo-microbiologic confirmation. Secondary objective was to study the correlation between the presence of sago-like nodules and the final histo-microbiologic findings in pleural biopsy specimens. Methods: This was a retrospective-descriptive study of all patients with exudative pleural effusion who underwent diagnostic medical thoracoscopy (MT) at Hamad General Hospital during an eight-year period (from January, 2008 to December, 2015). Results: The presence of sago-like nodules on gross thoracoscopic appearance of the pleural surface had a sensitivity of 58%, a specificity of 89% and a positive predictive value of 97% for TPE with a diagnostic accuracy of 62%. There is significant association between the presence of sago-like nodules and demonstration of granulomatous inflammation in pleural biopsy specimens (P = 0.000). There is no association between sago-like nodules and positive TB smear and culture in biopsy specimens. Conclusion: The presence of sago-like nodules on gross thoracoscopic appearance has a high specificity and positive predictive value for TPE and significantly correlates with the presence of granulomatous inflammation. Patients from TB prevalent areas with exudative pleural effusion and sago-like nodules on gross thoracoscopic appearance may be commenced on TB chemotherapy while awaiting final histologic confirmation.
Diagnostic Delay and Associated Clinical Features of Tuberculosis among Adult Patients with Acute Myeloid Leukemia in Doha, Qatar  [PDF]

Abstract: Background: Tuberculosis is currently the world’s leading cause of death arising from a single infectious condition. While T cell mediated immunity is recognized to have a major contribution to tuberculosis activation, the present investigation confirmed that TB was more prevalent among patients with acute myeloid rather than lymphoid leukemia and such association was frequently overlooked. The primary objective of this study is to estimate the diagnostic delay of tuberculosis among patients with acute myeloid leukemia (AML) and compare it to the general population in Qatar. Secondary objective is to study the clinical and epidemiologic characteristics of tuberculosis in patients with AML. Methods: This is a retrospective study of tuberculosis cases diagnosed in subjects with AML during the period from January 2008 till December 2016. Results: Among 215 subjects with AML identified during the study period, 12 (5.58%) received the diagnosis of tuberculosis. The estimated incidence of tuberculosis among AML cases was 7.14 cases per 1000 per year. The mean delay in diagnosis of tuberculosis was 64.2 days (95% CI: 26.8 - 101.5) and the median was 45 days (interquartile range; Q1 - Q3, 29.5 - 97.5). Prolonged fever was the most common presentation (100% of cases). Parenchymal lung involvement was the most common radiologic abnormality (83.3% of cases). Three patients (25%) died and 8 patients completed 9 to 12 months of anti-tuberculous treatment with clinical and radiological remission. Conclusion: Infections caused by Mycobacterium tuberculosis are not uncommon in patients with AML especially in patients from tuberculosis endemic regions. It constitutes a diagnostic challenge so high index of suspicion is of paramount importance.
Preoperative Versus Postoperative Radiation Therapy in Patients with Soft Tissue Sarcomas

Abstract: Purpose: To evaluate treatment outcome and to determine whether or not the timing of radiotherapy (RT) was associated with any difference in disease relapse, survival, or incidence of complications in patients with soft tissue sarcomas (STS). Methods: The medical files of 63 patients with a primary, nonmetastatic, STS, treated with surgery and irradiation were evaluated. Data regarding tumor stage, grade, site, dosage and timing of radiotherapy, treatment complications, disease relapse, and disease-free (DFS) and overall survival (OAS) rates were analyzed. Results: The median follow up was 47 months (range; 5-66 months). Four-year OAS and DFS rates were 82.6% and 78.8% respectively. There were significant higher 4-year OAS (p = 0.024) and DFS (p = 0.011) rates in patients with stage I and II diseases than those in patients with stage III disease. On the other hand, there were no significant differences in 4-year OAS (p = 0.83, HR: 0.743, 95% CI: 0.165 to 3.345) and DFS (p = 0.64, HR: 0.74, 95% CI: 0.21 to 2.61) rates between preoperative and postoperative RT patients. Disease relapse for preoperative versus postoperative RT patients was not statistically different (p = 0.41). Wound complications were more frequent in preoperative RT patients (25%) compared to postoperative RT patients (8%) (p = 0.0566 chi-square). Conclusions: Preoperative irradiation has not a positive impact on survival or disease relapse rates, but is associated with high wound complication rate.
Structural Deformation Monitoring and Analysis of Highway Bridge Using Accurate Geodetic Techniques  [PDF]

Abstract: As the new materials and technologies are increasingly applied to construction of civil infrastructures such bridges, dam and tunnels, the need for structural monitoring systems, maintenance and restoration becomes more important and vital. Bridges are widespread in every society and affect its human, social, economical and cultural aspects. Measurements and monitoring of the structural deformation of highway bridges have an essential role in structural safety. This paper investigates an integrated monitoring system for estimation of the deformation behavior of one of the important reinforced bridges in Egypt. The applied data for analysing the deformation of any structure from geodetic observations are the coordinates of several monitoring points distributed on the structure itself. The coordinates of these points are calculated with respect to control fixed points. So any deviations in the control points coordinates between the two successive epochs of observations will affect the values of structural deformation. To overcome this shortage, applying the multi-parameter transformation will be studied in this paper for structural health monitoring of bridges. Statistical tests using F-Fisher criterion with a confidence level of 98% of the geodetic observations for bridge deformation values are also presented. The results of the practical measurements, analysis of the interesting deformation technique and traffic flow around the studied highway bridge are also presented. The resulting structural deformation values from statistical tests provide a significant improvement of understanding and prediction the structure deformation values of highway bridges.

Abstract: Western blotting is a popular technique for examining expression levels of proteins using gel-based electrophoretic fractionation followed by blotting and antibody reactions. Although this is a mature technique, one of the major limitations is the need to prepare an individual electrophoretic gel for each of the protein species to be analyzed. Since most analyses require the detection of multiple protein species, a procedure that allows utilization of a single gel for detecting multiple protein species should significantly save time and resources. In this paper, we developed a novel multiprotein detection device, which enabled simultaneous detection of several proteins species from a single electrophoretic gel. In this device, a protein transfer unit utilized a multi-anode plate that generated a non-uniform voltage profile. This voltage profile enabled uniform transfer regardless of molecular mass of proteins. In vitro experiments using samples, isolated from boneforming osteoblast cells, showed that the expression levels of 5 - 7 different proteins were detectable in the presence and absence of mechanical stimulation that activated genes necessary for bone formation. The result supports the notion that through simultaneous detection of multiple protein species, the described device contributes to reduction in procedural time and sample amounts, as well as a removal of variations among multiple gels.
Monitoring Bridge Deformation Using Auto-Correlation Adjustment Technique for Total Station Observations  [PDF]

Abstract:

Bridges are omnipresent in every society and they affect its human, social, ecological, economical and cultural aspects. This is why a durable and safe usage of bridges is an imperative goal of structural management. Measurement and monitoring have an essential role in structural management. The benefits of the information obtained by monitoring are apparent in several domains. In deformation analysis, the functional relationship between the acting forces and the resulting deformations should be established. If time depending observations are given, a regression could be used as a functional model. In case of stochastic model uncorrelated observations with identical variance are assumed. Due to the high sampling rate, a small time difference arises between two observations. Thus the assumed stochastic model is not suitable. The calculation has to be effected by means of auto-correlated observations. This paper investigates an integrated monitoring system for the estimation of the deformation (i.e., static, quasi-static) behavior of bridges from total station observations and studies the effect of autocorrelation technique on the accuracy of the estimated parameters and variances. The results have shown that autocorrelation technique is reduced the standard deviation of X&Y-direction about 6.7% to 29.4% and 6.5% to 15.5% of the original value, respectively, but the situation was differ in Z direction; the standard deviation in vertical component Z was increased.

Research on the prevalence of conduct disorders among primary school pupils in Khartoum-Sudan  [PDF]

Abstract: The aim of this research was to investigate conduct disorder among pupils of primary schools in Khartoum, the capital of Sudan. School survey descriptive method was used and 384 pupils were selected from primary schools through systematic sampling technique. Age ranged from 5 to 17 years old with a mean of (9.34) years. The tools of data collection consisted of the Sutter-Eyberg Student Behavior Inventory. The statis-tical tests used to analyze the collected data involve frequency and percentage, Pearson co-efficient of correlation, mean, t-test for one sample and t-test for two independent samples. The results of this research revealed that that the prevalence of conduct disorder among pupils of primary schools in Khartoum was low. There were significant differences in conduct disorder between male and females pupils. There were no significant differences between pupils of pre-paratory classes and pupils of elementary classes. There was no significant correlation between conduct disorder and age.

Abstract: Difference equations or discrete dynamical systems is diverse field whose impact almost every branch of pure and ap- plied mathematics. Every dynamical system an+1=f(an) determines a difference equation and vise versa. We ob-tain in this paper the solution and periodicity of the following difference equation. xn+1=(xnxn-2xn-4)/(xn-1xn-3xn-5, (1) n=0,1,... where the initial conditions x-5,x-4,x-3,x-2,x-1 and x0 are arbitrary real numbers with x-1,x-3 and x-5 not equal to be zero. On the other hand, we will study the local stability of the solutions of Equation (1). Moreover, we give graphically the behavior of some numerical examples for this difference equation with some initial conditions.
Relationship between Stress and Psychosomatic Complaints among Nurses in Tabarjal Hospital  [PDF]

Abstract: This research was conducted to examine the relationship between stress and psychosomatic complaints among nurses in Tabarjal hospital. To achieve this aim, the researcher used the descriptive method and selected a sample consisting of (56) nurses working in hospital through the simple random sampling technique. The respondents were requested to complete a questionnaire for rating of stress and the most common psychosomatic diseases. The collected data were analyzed statistically by using SPSS. The results revealed that the stress was dominant among nurses in Tabarjal hospital, the prevalence of psychosomatic complaints was significantly higher in nurses, and there was significant correlation between stress and psychosomatic complaints among nurses. Conclusion: This research confirmed the effect of strains on the prevalence of psychosomatic symptoms among nurses in Tabarjal hospital. Further, implications of the results were discussed on the light of some related researches.

Abstract: The electrical performance of a photovoltaic (PV) silicon solar cell is described by its current–voltage (I–V) character-istic curve, which is in turn determined by device and material properties. In this study, an investigation of the performance and device parameters of photovoltaic single crystalline silicon (Si.) solar cell of the construction n+pp++ PESC(Passivatted Emitter Solar Cell) at different conditions of solar irradiance, title angle and mirror boosting effects had been studied. Also the paper reports on the performance data of the Si. cell, using standard I–V characteristic curves to obtain output parameters and to show that there are possible performance degrading defects presents.
