Uso de dexmedetomidina em pacientes obesos mórbidos submetidos a gastroplastia: estabilidade cardiovascular e consumo de anestésicos venosos. Estudo retrospectivo

Abstract: background and objectives: the administration of powerful and short duration anesthetic agents is essential for patients undergoing bariatric surgical procedure. the dexmedetomidine, an a 2-adrenergic agonist, has appeared as an adjuvant option of the venous anesthesia technique. this study aimed at assessing the efficacy of dexmedetomidine associated with the venous anesthesia in morbid obese patients undergoing gastroplasty procedures. methods: retrospective analysis of morbid obese patients undergone open bariatric surgical intervention under anesthesia with propofol and alfentanil, with or without dexmedetomidine. patients were allocated into two groups: control (propofol and alfentanil) and dexmedetomidine (propofol, alfentanil and dexmedetomidine). for both groups, the anesthetic maintenance was as follows: propofol = 0.075 to 0.1 mg.kg-1.min-1 and alfentanil = 0.75 to 1 μg.kg-1.min-1; in the dexmedetomidine (dmd) group, initial dose of 1 μg.kg-1 in 10 min and maintenance with 0.4 to 0.7 μg.kg-1.h-1. the variables studied were: age, gender, body mass index (bmi), surgical time and recovery time, heart rate (hr), systolic and diastolic blood pressure (sbp and dbp), hemoglobin peripheral saturation (spo-2), propofol and alfentanil consumption and side effects. results: the dexmedetomidine group has shown a significant reduction in propofol and alfentanil consumption. the heart rate presented a significant variation only in the dexmedetomidine group. both spb and dbp presented a statistically significant reduction in both groups for the first 20 minutes, and subsequent stabilization. no side effects were observed in both groups of patients. conclusions: this study has shown the efficacy of dexmedetomidine administration in combination with venous anesthesia with propofol and alfentanil, thus promoting reduction in venous anesthetic drugs consumption, cardiovascular stability and time to recovery similar to that of the technique without dexmedetomidine. no side ef

Abstract: JUSTIFICATIVA E OBJETIVOS: A press o exercida pelo balonete do tubo traqueal contra a parede da traquéia deve permitir fluxo capilar adequado e prevenir escapes de ar ou aspira o pulmonar. Esta pesquisa procurou determinar as varia es de press o do balonete insuflado com ar ambiente ou com óxido nitroso a 100%. MéTODO: Trinta pacientes foram selecionados para receber anestesia geral balanceada com intuba o orotraqueal. O balonete foi insuflado conforme critérios clínicos. As medidas de base foram realizadas após 15 minutos do início da anestesia com um man metro aneróide calibrado em cm de H2O e forneceram os valores iniciais de press o e volume. Os pacientes foram divididos em dois grupos: balonete reinsuflado com ar ambiente, grupo A, ou com óxido nitroso, grupo B. As medidas de press o foram obtidas em intervalos até a primeira hora e os resultados comparados. RESULTADOS: Os grupos mostraram-se comparáveis para idade e sexo. Em ambos os grupos os valores basais médios para press o foram próximos de 40 cmH2O com 8 ml de volume. No grupo com ar ambiente, as press es aumentaram até 36 cmH2O em uma hora. No grupo de balonete insuflado com N2O, as press es diminuíram abaixo de 20 cmH2O entre 20 e 30 minutos de anestesia. CONCLUS ES: O uso de N2O a 100% para insufla o do balonete de sonda traqueal n o constitui método seguro, acarretando progressiva perda da capacidade de veda o. O uso de ar ambiente promove aumento de volume e de press o no balonete, aumentando possibilidade de les o da mucosa traqueal.

Abstract: background and objectives: different oxygenation techniques aim at promoting denitrogenation before apnea during induction. the main reason why cio2 = 100% cannot be reached is the lack of adequate face mask coupling, allowing the entry of room air. although anesthesiologists know this principle, not all of them apply it correctly, facilitating the entry of air in fresh gases flow and consequently diluting cio2. this prospective study was performed to comparatively evaluate, through the variation of oxygen expired concentration (ceo2), the efficacy of the oxygenation technique via face mask in the conditions routinely used by anesthesiologists, simulating situations of progressive leaks. methods: oxygen end-tidal concentrations of 15 volunteers, physical status asa i, were studied with 8 deep breaths (vital capacity) in 60 s with fresh gas flow of 10 l.min-1. the face mask was: tightly fitted with 100% cio2 (tf100) or varying from 50% to 90%, (tf50, tf60, tf70, tf80, tf90); gravity-coupled to face and 100% cio2 (grav) and moved 1 cm away from face with 100% cio2 (aw). ceo2 was recorded at 10 s intervals. p < 0.05 was considered statistically significant. results: ceo2 has increased for all groups (p < 0.001), but only tf100 reached values close to ideal (82.20 - 87). comparing mean ceo2 of grav and tf100 at the end of 60s, (82.20 and 65.87) there was a difference of approximately 20% between both techniques, since gravity-coupled mask only did not provide adequate oxygenation. there were no significant differences between groups tf70 and grav (65.87 and 62.67) in all studied moments, suggesting that the latter simulates a 70% cio2 at 60 s. mean aw group ceo2 increased to 47.20 at 60s showing that this technique may be associated to unacceptable risk of hypoxemia. conclusions: all situations of face mask coupling gradually increased ceo2, although with decreased oxygenation efficacy due to situations of face mask malposition. this study has shown the need for attenti

Abstract: background and objectives: tracheal tube cuff pressure against tracheal wall should to prevent air leaked or pulmonary aspiration, allowing adequate capillary blood flow. this study aimed at determining pressure variations of tracheal tube cuffs filled with air or 100% nitrous oxide. methods: participated in this study 30 patients submitted to balanced general anesthesia with tracheal intubation. cuff was inflated according to clinical criteria. the intracuff pressure measurements were undertaken after 15 minutes of anesthesia by means of an aneroid manometer gaged in cm of h2o and have provided initial pressure and volume values. the patients was allocated in two groups: cuff inflated with air, group a, and with nitrous oxide, group b. measurements were recorded during one hour and results were compared. groups were homogeneous in gender and age. results: both groups have shown baseline pressure values close to 40 cmh2o with 8 ml of volume. group filled with air had pressures increased up to 36 cmh2o in one hour. group filled with nitrous oxide had pressures decreased below 20 cmh2o after 20 to 30 minutes of anesthesia. conclusions: 100% nitrous oxide to fill tracheal tube cuffs is not a safe method resulting in progressive air leakage. the cuff air promotes volume and pressure increase, with higher chances for tracheal mucosa injury.

Abstract: the article discusses a new brain theory drawn from neurosciences: the threefold or three-sided brain and its correspondent three fields of performance. the author worked out a method with some tools to apply the triadic approach to education: tq - the triadic quotient detector; the cybernetic cycle binding the three mental processes together thus going further than the scientific method; and the four developmental levels of each side of the brain as a matrix for curricula design.

Abstract: El artículo presenta una nueva teoría del cerebro en tres bloques anatómicos y tres procesos funcionales. En base a descubrimientos de las neurociencias, el autor trata de aplicar el nuevo enfoque triádico al proceso psicopedagógico y desarrolla su propuesta de herramientas para identificar, educar y organizar los tres procesos funcionales: el CT, o Revelador del Cuociente Triádico; el Ciclo Cibernético de Feedback, que va más allá del método científico tradicional, y los cuatro niveles de desarrollo y desempe o del cerebro como nueva matriz curricular. Abstract: the soil seed bank was studied in a gallery forest in southeastern brazil. samples were collected from edge to edge along transects perpendicular to the river during the wet (december, 1990) and dry (august, 1991) seasons. the number of seeds found in the seed bank samples was greater in the dry season than in the wet season indicating that there was important variation in the seed stocks in these two periods. the similarity values between the seed bank and the community composition were low. the high density of weed species might be reflecting forest fragmentation and indicating a degree of perturbation.

Abstract: In many regions of Brazil, lateritic clays are the natural candidates for the construction of compacted clay liners of waste disposal sites because of their availability and appropriate geotechnical characteristics. Lateritic soils have been extensively used in recent decades in dam and road construction, but little is known about the migration of pollutants through compacted layers of such soils. This paper describes the characteristics of a lateritic clay, representing a group of soils of significant occurrence in the State of São Paulo, to be employed in a clay liner of a waste disposal site. Laboratory tests to assess permeability, adsorption and diffusion of six metals through the compacted soil showed that permeability criterion may be met in the field, that the soil presents a modest capacity to retain cadmium and that constituent metal oxides may be dissolved from the soil grains by acidic solutions.

Abstract: O comércio exterior brasileiro nesses últimos quinze anos nos mostra que as exporta es brasileiras vêm crescendo cada vez mais e nossos produtos est o ficando mais competitivos no mercado mundial. Isso tudo quando se fala de commodities, porque no caso de produtos manufaturados o Brasil tem exportado novos produtos num ritmo bem acanhado, e mesmo assim vem conquistando novas oportunidades de mercados em outros países, até porque basicamente o mundo todo está se tornando comprador. a balan a comercial desde 1998 vem tendo um bom desempenho, com altos superávits, isso se deve ao excelente crescimento de nossas exporta es e a substitui o de alguns produtos que importávamos. Durante esses últimos quinze anos o governo brasileiro e os empresários têm fortalecido a marca Brasil no mercado internacional, proporcionando ao país um melhor desempenho e crescimento econ mico.
