隐裂牙临床特征分析及CBCT影像学研究

Abstract: 摘要 目的: 通过对临床上非外伤性隐裂牙的临床特征进行分析并对其锥形束CT(Cone beam computed tomography, CBCT)的影像学表现进行总结。方法: 对105名患者的105颗明确冠隐裂牙的临床特征及CBCT影像学特征进行统计,收集临床资料包括患者的性别、年龄、牙位,同时对其CBCT上牙根折裂情况,牙槽骨及根尖周骨质吸收情况进行评价和统计。结果: 105例患者中,男性49.52%,女性50.48%;在50岁之前,随着年龄增加,隐裂牙发生比例增加,其中41~50岁发生率最高(34.29%),之后下降。105例隐裂牙中有5例发生在前磨牙区,其余均发生在磨牙区,其中第一磨牙发生率最高(59.05%)。105颗隐裂牙,CBCT检查有16颗(15.24%)可观察到牙根存在不完全折裂,仅4颗(3.81%)可见牙根完全折裂。其余85颗(80.95%)隐裂牙牙根未见明显异常。结论: 隐裂牙好发于上下颌第一磨牙,CBCT检测下大多数的冠隐裂牙并没有发生牙根折裂

Abstract: 摘要 口腔鳞状细胞癌(简称鳞癌)是口腔颌面部常见的恶性肿瘤,现有的治疗方法以手术治疗为主,存在一定的风险,并可影响患者生活质量;而传统放化疗在治疗肿瘤的同时会对正常组织造成损害。纳米靶向给药技术是近年来针对恶性肿瘤的一种治疗技术,在一定程度上实现肿瘤的定点定向治疗,以提高治疗效果,目前该治疗技术在全身其他部位的实体瘤研究已取得重大进展,而在口腔鳞癌研究证据尚不足。随着治疗技术的不断发展,如何提高靶向给药的高效性与安全性以及如何在靶向治疗过程中实现针对肿瘤组织的持续监测成为新的问题,这对于即时评估治疗效果有重大意义。本文就近年来国内外纳米靶向药物在口腔鳞癌的诊断与治疗中的研究进展作一综述

Abstract: Uncoupling proteins (UCPs) are mitochondria carrier proteins,which are able to dissipate the proton gradient of the inner mitochondria membrane. The uncoupling procedure reduces the amount of ATP generated through an oxidation of fuels. Therefore, UCPs are suggested as candidate genes for human obesity or type II diabetes mellitus. Experimental evidences,that genetically engineered mice over expressing different UCP homologues were resistant to diet-induced obesity and 45 bp insertion polymorphism in the UCP2 3' untranslated region and C-55T in UCP3 promoter region were associated with obesity related phenotype, supported the hypothesis. The roles of UCP genes in polygenic obesity and type II diabetes are evaluated and discussed in this paper.
Characterization of PUD-1 and PUD-2, Two Proteins Up-Regulated in a Long-Lived daf-2 Mutant

Abstract: C. elegans PUD-1 and PUD-2, two proteins up-regulated in daf-2(loss-of-function) (PUD), are homologous 17-kD proteins with a large abundance increase in long-lived daf-2 mutant animals of reduced insulin signaling. In this study, we show that both PUD-1 and PUD-2 are abundantly expressed in the intestine and hypodermis, and form a heterodimer. We have solved their crystal structure to 1.9-? resolution and found that both proteins adopt similar β-sandwich folds in the V-shaped dimer. In contrast, their homologs PUD-3, PUD-4, PUDL-1 and PUDL-2 are all monomeric proteins with distinct expression patterns in C. elegans. Thus, the PUD-1/PUD-2 heterodimer probably has a function distinct from their family members. Neither overexpression nor deletion of pud-1 and pud-2 affected the lifespan of WT or daf-2 mutant animals, suggesting that their induction in daf-2 worms does not contribute to longevity. Curiously, deletion of pud-1 and pud-2 was associated with a protective effect against paralysis induced by the amyloid β-peptide (1-42), which further enhanced the protection conferred by daf-2(RNAi) against Aβ.

Abstract: 摘要 目的:对比CBCT法与数字化X线成像法(RVG)对下颌恒切牙根管形态评估的差异。方法:对101颗离体牙进行唇舌向及近远中向数字化X线片拍摄和CBCT高清牙列模式扫描,对图像进行根管数目、Vertucci根管分型、根管钙化和根尖口开口位置的评估,并通过CBCT图像建立偏角度投照的三角函数模型,计算偏向投照的角度。结果:CBCT法显示有30颗为双根管,RVG近远中向投照显示有36颗为双根管,卡方检验显示对根管数目和Vertucci根管分型两种方法均具有显著性差异(P<0.05)。两种方法对根管钙化和根尖口开口位置的评估均不具有显著性差异。30颗双根管牙中下颌恒切牙唇舌向双根管的最大距离(LaL)在0~1.5 mm之间的,85.7%的为Vertucci III型,LaL在1.5~2 mm之间的,66.7%为Vertucci IV和V型。以根尖片上双根管显示距离为1 mm计算,LaL在1~2 mm之间的,偏向投照角度为26.6°~45.0°,LaL在0.1~1 mm之间的,偏向投照角度为45.0°~84.3°。结论:CBCT高清牙列模式扫描和RVG投照对于根管数目和Vertucci根管分型的评估具有显著性差异,对于根管钙化和根尖口开口位置的评估不具有显著性差异。LaL在1~2 mm之间时,偏角度投照的临床可操作性较强
上颌窦底壁与上颌后牙根尖位置关系的临床与锥形束CT研究

Abstract: 摘要 目的:通过锥形束CT影像分析无症状人群上颌窦底壁与上和后牙根尖位置关系,为上颌后牙区临床治疗提供理论依据。方法:选取53例98侧无症状人群的上颌窦锥形束CT影像,三维重建后在选取的截面上测量784个根尖与上颌窦底壁的距离,上颌后牙根尖的位置据此分为4类,并将各例上颌窦底壁与上颌后牙根尖距离的均值进行95%置信区间估计。结果:上颌第一磨牙远中颊根根尖距离上颌窦底壁的平均距离最短,为(0.44±0.61) mm(置信度95%).上颌磨牙根尖广泛存在于上颌窦底壁密切接触的情况,其中上颌第二磨牙与上颌窦底壁的关系最为密切。结论:上颌磨牙根尖与上颌窦底壁位置关系密切,锥形束CT可精确了解上颌后牙区的解剖情况,从而降低医源性上颌窦炎发生的概率

Abstract: 摘要 目的:运用锥形束CT研究年龄因素对拔除下颌第三磨牙的影响,评价早期拔除下颌第三磨牙的风险性。方法:筛选2015年6~12月在我院就诊患者中符合纳入标准的锥形束CT图像,共选取150颗下颌第三磨牙,其中16~18岁组47颗牙齿,19~24岁组60颗牙齿,24岁以上43颗牙齿,测得3个年龄组中患者的下颌骨体高(a)、下颌骨体剩余高度(b)与下颌骨体高(a)的比值、牙齿至下颌神经管最短距离数据,并分析不同年龄组之间的差异性。结果:下颌骨体高3个年龄组间差异无统计学意义。3组间b/a比值以24岁以上组最大,其次19~24岁组,各组间两两比较均有统计学意义。在牙齿至下颌神经管最短距离方面,19~24岁组和24岁以上组均比16~18岁组大,有统计学意义;而19~24岁组和24岁以上组间无统计学意义。但是,3个年龄组间牙齿与下颌神经管紧密接触比率无统计学差异。结论:3个年龄组中19~24岁组拔除下颌第三磨牙可能相对最合适、风险最小

Abstract: 摘要 目的: 探讨角化型成釉细胞瘤的影像学特征,并结合文献资料讨论其影像学鉴别诊断。方法: 搜集2011~2017年我院经病理证实罕见的角化型成釉细胞瘤3例,回顾性分析并总结3例角化型成釉细胞瘤的影像学特征,并结合现有文献对于其鉴别诊断进行探讨。结果: 3例角化型成釉细胞瘤中,发病部位均位于左下颌骨体和升支部,共同的影像学特征为膨胀性不均匀密度影,边界呈扇贝样及虫蚀状,其内见不均匀性稍高密度影,部分伴有钙化致密影。结合文献,其主要与原发性骨内鳞状细胞癌、恶性成釉细胞瘤及颌骨中央性巨细胞病变进行鉴别。结论: 角化型成釉细胞瘤影像学上同时兼具良性及恶性特征表现。识别典型角化型成釉细胞瘤的影像学特征有助于本病的诊断及手术方案制定,此外更多的病例报道对于可以准确诊断这一罕见的角化型成釉细胞瘤至关重要

Abstract: 摘要 目的:研发基于CBCT容积数据的体绘制可视化技术,并对拟行上颌磨牙区种植的患者的种植区域进行三维演示,以期提高对此类患者种植区骨量及上颌窦情况术前评估的直观性。方法:将经临床筛选的13例上颌后牙种植患者使用CBCT扫描所获得的原始DICOM数据分别导入NNT 2.19(J.MoritaManufacturing Corp.)软件及本项目组研发的三维可视化软件(TooMax 1.0),分别测量牙槽嵴顶至上颌窦底的高度(H1)、种植区牙槽骨颊腭向宽度(H2),对两种软件测量的骨量进行统计学分析,同时在TooMax中使用体绘制可视化技术显示上颌窦形态及位置以及二维骨量测量值在牙槽骨内的空间位置。结果:用两种软件测量的13例上颌后牙种植患者的骨量测量不具有显著性差异,同时可视化技术可以将不同测量值在上颌骨内的三维空间位置实时显示及变换,便于医患交流,并辅助种植方案的设计。结论:体绘制可视化技术提升了CBCT技术对种植骨量评估的直观性,具有一定的种植模拟的效果,有助于实现可视化临床治疗目标

Abstract: 摘要 目的: 探讨如何使用Simplant软件对上颌骨骨量不足的患者进行颧种植体设计,并在手术导航中实施。方法: 5例上颌后牙区骨量不足的患者使用Simplant软件基于CBCT数据进行种植体设计,并在导航下实行手术。结果: 5例患者植入种植体后,位置与术前设计方案比较发现,种植体与Fankfort平面所成角度(angle,A)偏差小于1°,颧骨内种植体的长度(length in zygoma,LZ)距眶壁距离(distance to orbit wall,D)、牙槽骨内种植体长度(length in alveolar bone, LA)的偏差均小于2 mm,术后12个月随访无种植体失败。结论: 通过Simplant软件进行颧种植体设计,可以使种植方案更加完善,与手术导航系统的联合使用可以实现精准手术,大大提升了复杂颧种植手术的安全性和准确性
