Associated or Concomitant Diseases Influence Significantly the Health-Sickness Concept in Celiacs  [PDF]

Abstract: It is well-known that feeling to be a healthy or sick person most probably results from the mind than from the body. We all know healthy people who feel sick and vice versa. We were interested in the health and sickness feeling of celiac people, their autorating of these feelings and its conditioning factors as well as their expectations. In this paper we present the results of an inquiry to evaluate these situations. We performed a descriptive, transversal and prospective study for 2 years to groups of celiacs and their families. They received a closed inquiry to be completed before the beginning of the talk. The inquiry included personal data and the co-existence of associated or concomitant (AoC) diseases. Most of the sample’s patients felt to be a healthy person (86.8%). Mothers see their children as healthy and the auto rated criteria is significantly better than the adult celiac person (“t”= -6.024 (p = 0.000)). AoC diseases influenced negatively in the feeling of being healthy and strongly decreased the autorating. Longer time passed on treatment reflects an increase feeling of health and of the autorating. In people with AoC diseases and who feel sick, the increased time of treatment did not show significant differences. People with “gluten sensitivity” felt sicker and auto rated themselves with a lower number than celiacs. Many pediatric gastroenterologists notice that the newly agreed definition of celiac disease, referring it as “autoimmune, chronic, incurable, and multisystemic”, results in a very negative character of the condition which might compromise the future labour of this people as well as their admittance to different health insurance systems.

Abstract: Chronic spontaneous urticaria (CSU) is defined as the spontaneous appearance of wheals, angioedema, or both, for at least 6 weeks, due to known or unknown causes [1]. In some patients who present a CSU with daily or almost daily symptoms a type I allergy could be the underlying cause. We present one adult patient with chronic urticaria who was finally diagnosed as a non-occupational case of IgE-mediated wheat allergy manifested following exposure by 3 different routes: inhalation (rhinitis and bronchial asthma), dermal absorption (contact urticaria) and ingestion (systemic chronic urticaria). We were able to detect the culprit proteins by immunoblotting. Serum IgE binds mainly to alpha-amylase/trypsin inhibitors and, to a lesser extent, to other proteins associated with food allergy to grains (e.g. beta-glucanase, serpin, peroxidase). In our opinion, skin prick tests with a food standard battery could help in the etiological diagnosis of chronic urticaria. The identification of responsible allergens could be difficult because only a few complex in vitro techniques allow detecting the allergy to several proteins.

Abstract: SonoVue® (Bracco; Milan, Italy) is an ultrasound contrast medium which contains polyethylene glycol (PEG) or macrogol, an excipient responsible for severe allergic reactions, especially through the parenteral route, and widely used, especially in the health field. We present an adult patient who experienced an IgE-mediated anaphylactic shock due to macrogol contained in SonoVue®, as demonstrates the skin tests realized. The allergy study has been useful in our case because it has guided us in the preventive measures to advice to the patient, who has to avoid not only SonoVue® but also other products that contain macrogol. We consider this case of interest because it is an allergic adverse reaction to SonoVue® in which its excipient, macrogol, has acted like the allergen responsible. We have to add SonoVue® to the list of the products that contain macrogol in their composition and keep in mind the possible allergenic capacity of this excipient for suspecting its responsibility, especially in those patients with multiple drug allergy and/or reactions to products of different natures like ultrasound gels, topical products (e.g. toothpastes, antiseptics, sunscreens, etc.), plasma expanders and others.
Estudio de marcadores fenotípicos y de susceptibilidad antimicrobiana en cepas de Escherichia coli entéricas Study of phenotypical and antimicrobial susceptibility markers in enteric Escherichia coli strains

Abstract: Se analizaron 41 cepas de Escherichia coli, aisladas en ni os menores de 5 a os con diarreas agudas, procedentes de las diferentes provincias del país. Fueron probados 4 importantes determinantes fenotípicos: sorbosa, sorbitol, enterohemolisina y la serología de O157: H7, para seleccionar las cepas de la categoría enterohemorrágica o productora de toxina shiga. De igual modo fueron caracterizadas por el método de biotipaje y de susceptibilidad antimicrobiana. El uso de las pruebas fenotípicas mostró 6 cepas con características presuntivas, de estas, 4 mostraron mayor probabilidad de pertenecer a la categoría productora de toxina shiga. En el ensayo de susceptibilidad antimicrobiana las cepas mostraron elevada resistencia, sobre todo para la ampicilina y el trimetropin-sulfametoxasol. Otro hallazgo de interés fueron los valores de resistencia y de susceptibilidad intermedia obtenidos para el augmentin, aztreonan y ceftriaxona. Se alcanzaron 12 patrones de resistencia antimicrobiana y de ellos 10 resaltaron por ser multirresistentes. Forty strains of Escherichia coli isolated from children under 5 years of age with acute diarreas, coming from different provinces of the country , were analyzed. Four important phenotypical determinants were tested: sorbosa, sorbitol, enterohemolysin and 0157: H7 serology, in order to select those strains from enterohemorrhagic or Shiga toxin-producing category. Likewise, they were characterized by biotyping and antimicrobial susceptibility methods. The use of phenotypical tests showed six strains with presumptive characteristics, four of which were most likely to be Shiga toxin-producing strains. In antimicrobial susceptibility test, the strains showed high resistance mainly to ampicillin and trimethrophin-sulfamethoxasole. Another interesting finding were intermediate resistance and susceptibility values to augmentin, aztreonan and ceftriaxone. There were 12 antimicrobial resistance patterns of which 10 were multi-resistant.
Flexural Performance of I-Joist Fabricated with Glue-Laminated Bamboo and Gmelina arborea Plywood  [PDF]

Abstract:

The search for efficient and versatile structural elements, leads to the fabrication of I-joists (6.5 cm × 18.5 cm × 600 cm (width × depth × length) with glue-laminated bamboo (Guada angustifolia) in the flanges and Gmelina arborea 12-mm structural plywood in the web. The results showed a modulus of rupture (MOR) of 39.45 MPa and an effective modulus of elasticity (MOE) of 17.05 GPa. Shearing in the glue line was 5.95 MPa and the lamination strength was 6.45 MPa. Structural design values averaged 9.43 MPa for bending and 4.72 MPa in shear according to Costa Rican structural standards. Both resistance value (flexure and shear) were considered satisfactory for structural proposes and I-joists fabricated with bamboo and G. arborea plywood are comparable with the Andean classification group “C” structural grade. The use of this I-joist was also shown in roofing and flooring systems. This beam can be used in allowable spans from 2 to 4 m in span for flooring systems and from 5 to 7 m for roofing applications.

Características de personalidad infantil asociadas al riesgo ambiental por situación de pobreza

Abstract: an individual's context affects the way in which their biological and psychological subsystems function and interacts with each other. while individuals are normally able to manipulate their environment, personality factors increase or diminish their context characteristics, which at the same time influence personality. heredity and socio-environmental personality factors should not be considered separate elements, but rather a holistic approach as an individual-environment system that functions as a whole. therefore, an unfavorable environment is a socio-environmental variable that interacts with children's heredity, thus influencing the development of certain personality traits. based on the above, the specific goal of our research was to compare personality characteristics of children at risk due to poverty and children without risk due to poverty. we applied the child personality questionnaire for argentina (cuestionario argentino de personalidad infantil - capi) for children aged 6 to 8 (lemos, 2005). we compared personality profiles from both groups through manova, as well as comparisons of means and graphic profiles. based on this analysis, we observed significant differences regarding personality factors [f de hotelling (5, 168) = 2.47; p = .035], as well as facets [f de hotelling (14, 159) = 2.607; p = .002], among children at risk due to poverty and those who were not. there were significant differences particularly in conscientiousness [f(1) = 4.35; p = .038] and the following facets: competence [f(1) = 4.652; p = .032], vulnerability [f(1) = 9.732; p = .002], gregariousness and positive affect [f(1) = 8.338; p = .004], order [f(1) = 6.798; p = .010] and action [f(1) = 4.233; p = .041]. the tendencies of the results are as follows: the group at risk scored lower in conscientiousness, and regarding facets: competence, gregariousness, positive affect, order, action and vulnerability. with regards to conscientiousness, which includes order, organization and
