Importancia da albumina sérica na avalia??o nutricional e de atividade inflamatória em pacientes com doen?a de Crohn

Abstract: background - the serum albumin concentration has frequently been regarded as an indicator of nutritional status, although the hypoalbuminemia may reflect an acute phase protein response during inflammation mediated by cytokines. both hypoalbuminemia and malnutrition are observed in crohn's disease. objectives ? to correlate the serum albumin values to disease activity and also to nutritional status in patients with crohn's disease. patients/methods ? thirty six patients were studied. nutritional status was assessed by anthropometry measures and inflammatory activity determined by harvey's simple clinical index and erythrocyte sedimentation rate. results - no correlation was found between malnutrition and hypoalbuminemia. the serum albumin levels correlated inversely with the disease activity. hypoalbuminemia was 100% sensitive for detection of disease activity. conclusion - this study suggests that serum albumin concentration is a very sensitive marker of inflammatory activity and not good indicator of the nutritional status in crohn's disease. it is necessary a suitable laboratorial parameter for routine nutrition assessment in patients with this inflammatory bowel disease.

Abstract: Racional - A albumina sérica é freqüentemente definida como um indicador de estado nutricional. No entanto, hipoalbuminemia pode refletir rea o de fase aguda mediada por citocinas durante um evento inflamatório. Tanto desnutri o, como hipoalbuminemia s o achados relevantes na doen a de Crohn. Objetivo - Estudar a rela o da albumina sérica com atividade inflamatória e com estado nutricional em pacientes com doen a de Crohn. Casuística/Método - Trinta e seis pacientes avaliados quanto ao estado nutricional por parametros antropométricos e a atividade inflamatória da doen a intestinal definida por índice clínico e velociodade de hemossedimenta o. Resultados - N o se encontrou associa o entre desnutri o e hipoalbuminemia. A utiliza o dessa proteína como parametro bioquímico na avalia o nutricional foi dispensável para se diagnosticar desnutri o. Houve significativa rela o entre atividade inflamatória e hipoalbuminemia, demonstrando 100% de sensibilidade na identifica o de inflama o ativa. Conclus o - Estes resultados sugerem a dosagem de albumina sérica como marcador sensível de doen a de Crohn ativa e a necessidade de um parametro laboratorial mais adequado para avalia o nutricional rotineira nos portadores desta doen a inflamatória intestinal.

Abstract: background: the pathogenesis of ulcerative colitis (uc) and its extraintestinal manifestations remain uncertain, although involvement of the immune system is emphasized. the likely importance of neutrophils is demonstrated by detection of the antineutrophil cytoplasmic antibody (anca) in this inflammatory bowel disease. pyoderma gangrenosum (pg) is an idiopathic skin condition and a rare cutaneous manifestation of uc. anca has also been reported in the latter dermatosis. objectives: to invetigate the relationship between clinical features of uc and the appearance of pg and its association with anca. patients and methods: anca was determined in sera from eight patients with pg. four out of eight patients had pyoderma gangrenosum associated-uc, and in four cases no identified systemic disease was associated. results: the search for anca yielded negative results in sera from all four patients with pyoderma not associated with systemic disease. two cases with active and extensive colitis associated with pg and primary sclerosing cholangitis (psc) were positive for anca. sera from two other patients with both uc and pg had negative test results. conclusions: the presence of anca in patients with pg associated with uc and psc suggests that its association with psc is responsible for anca positivity in this subset of patients.

Abstract: objective: a strong association has been observed between celiac disease, generally its silent clinical form, and autoimmune disorders. a potential correlation with inflammatory bowel disease has also been suggested. anti-tissue transglutaminase antibodies have been detected in crohn's disease. we investigated the prevalence of celiac disease in patients with autoimmune diabetes and in crohn's disease patients and also evaluated the correlation between anti-transglutaminase antibody positivity and the clinical status of these diseases. methods: anti-tissue transglutaminase and anti-endomysium antibodies were assessed by enzyme-linked immunosorbent assay and indirect immunofluorescence, respectively. upper digestive endoscopy and duodenal biopsy were indicated for cases with positive serology. results: anti-transglutaminase antibodies were detected in five diabetic patients (prevalence of 11.1%), only one serum sample was positive for igg isotypes. nine of thirty-three patients with crohn's disease had low positive levels for iga anti-transglutaminase. anti-endomysium antibodies were detected only in celiac patients. celiac disease was confirmed in all diabetic patients submitted to duodenal biopsies who presented both anti-transglutaminase and anti-endomisyum antibodies positivity. in crohn's disease, its clinical status and the diagnosis of celiac disease were not associated with positive anti-transglutaminase result. conclusions: the prevalence of celiac disease was high in diabetic patients. anti-tissue transglutaminase antibodies were sensitive and specific markers of celiac disease in this diabetic group, while these antibodies were of limited value for celiac disease screening in patients with crohn's disease.

Insulin resistance index (HOMA-IR) in the differentiation of patients with non-alcoholic fatty liver disease and healthy individuals

Abstract: context: due to its good correlation to glycemic clamp, homa-ir has been widely utilized as insulin resistance index in clinical and epidemiological studies involving non-alcoholic fatty liver disease carriers. however, values used for this parameter have shown large variability. objective: to identify the homa-ir cut value that best distinguishes non-diabetic non-alcoholic fatty liver disease patients from a control group. methods: one hundred sixteen non-alcoholic fatty liver disease patients were studied, diagnosed by clinical, biochemical, and liver image or biopsy criteria, and 88 healthy individuals, without any liver disease and testing for oral glucose tolerance within normality. these groups did not differ in age and gender. all were submitted to oral glucose tolerance test and blood samples were collected for glucose and insulin measurements by immunofluorometric method. homa-ir was calculated according to the formula: fasting insulin (μu/l) x fasting glucose (nmol/l)/22.5. results: nafld patients showed higher insulin, glycemia, and homa-ir values than control group, even when excluding glucose intolerant and diabetes mellitus patients by their glycemic curves. homa-ir 75th percentile for control group was 1.78 and the best area under the curve index was obtained for homa-ir values of 2.0 [auc= 0.840 (0.781-0.899 ci 95%), sensitivity (se): 85%, specificity (sp): 83%] while value 2.5 showed best specificity without important loss in sensitivity [auc=0,831 (0.773-0.888) se = 72%, sp = 94%]. conclusion: homa-ir values above or equal to 2.0 or 2.5 show enhanced diagnostic value in distinguishing non-alcoholic fatty liver disease carriers from control group individuals.

Abstract: A longevidade é uma característica do mundo atual. O envelhecimento da popula o, que ocorreu ao longo de muitas décadas nos países desenvolvidos, tornou-se rapidamente uma realidade nos países menos desenvolvidos, caracterizando uma transi o demográfica em um nível sem precedentes. O impacto previsto sobre os setores econ mico, social e de saúde transformou a velhice em um problema social. Diante disso, a gerontologia, tendo como referência a cidadania, vem investindo na constru o de novo lugar social e uma nova mentalidade de cuidado para a velhice, através de um discurso que se apóia na educa o, no trabalho e na família. Neste artigo, sob a luz da psicanálise, apresentamos uma breve historiciza o da constru o social da velhice no ocidente, considerando sua inspira o no envelhecimento biológico, e problematizamos a sua influência nos discursos atuais, nas rela es familiares e suas ressonancias no mosaico de modos pelos quais a velhice, em si mesma, será bem ou mal percebida pelos sujeitos. Longevity is a characteristic of the present world. The aging of the population, which had happened during many decades in the developed countries, became quickly a reality in the less developed countries, marking a demographic transition in an unexampled level. The foreseen impact on the economic, social and health sectors transformed old age into a social problem. In such case, gerontology, with reference to citizenship, has been investing in the building of a new social place and a new mentality of care for the elder, through a discourse based on education, work and family. In this article, under the light of psychoanalysis, we introduce a brief historical development of the social construction of old age in the western world, considering its inspiration in biological aging. We also put in doubt its influence in the present speeches, in the family relationship and identify its resonances in the mosaic of manners by which old age itself will be well or badly perceived by the subject.
Atividade antioxidante de Jacaranda decurrens Cham., Bignoniaceae

Abstract: since many diseases and degenerative processes have been associated to the overproduction of reactive oxygen species (ros) many research groups are motivated to investigate the antioxidant potential of substances produced by several families of the world flora. the aim of this work was to evaluate the antioxidant activity of crude hydroalcoholic extract of jacaranda decurrrens leaves and fractions obtained from that extract, using the free radical dpph (2,2-difenyl-1-picrylhydrazyl) through spectrometric discoloration method. results confirmed that either the crude extract or fractions of the extract presented antioxidant activity. the antioxidant activity (91%) demonstrated by the crude extract was equivalent to the antioxidant activity determined for fraction jd-2 in the concentration of 10 mg/l, and similar to the activity showed by the standard rutin, in the concentration of 1 mg/l (92,56%). fraction jd-1 in the concentration of 10 mg/l and 5 mg showed 84% and 86% antioxidant activity, respectively. those values are significantly different and inferior if compared to the standard rutin. compared to the other fractions the jd-3 presented higher antioxidant activity. the antioxidant potential of jacaranda decurrens crude extracts and fractions jd-1 and jd-2 is probably related to the production of the triterpenes ursolic and oleanolic acids and also to the glycosylated flavonoids produced in the fraction jd-3. this is the first report on the antioxidant activity of crude hydroalcoholic extract of j. decurrrens leaves and fractions obtained from that extract.

Abstract: the policy of continuing healthcare education (che) aims to develop healthcare workers. with the objective of analyzing the process of implementing and developing the policy in paraná, a qualitative study involving the six regions of this state is being concluded. this paper relates to the results from the northern region, focusing on the "experiencing che" category. in december 2006, two focus groups were conducted involving representatives from management, training, attendance and participation. the data underwent thematic content analysis. the first che encounters aroused feelings of mistrust and resistance, and the center was understood as a means of enabling courses and funding sources. there was a diversity of interests and little negotiating capacity. during the process, the study participants began to talk, reflect and participate. their teamwork was a positive experience. this experience allowed them to recognize the power of che for linking and mobilizing different players.

Abstract: in this paper we present some theoretical presuppositions with respect to the qualification of old-age caregivers proposed by two courses of the city of rio de janeiro. we adopted a qualitative methodology, using as instruments for data collection direct observation, tape recording of the ministered classes and interviews with the course managers. documental sources were the course programs, texts and lists of source materials. the data were ordered in descriptive categories that were compared with the literature. we emphasize: the difference between invisible care, visible care and technical care; the idea of care as a natural value with no need for technical qualification and the concepts of social care and health care. we believe that on giving emphasis to social care (moving away from the figure of disease) or to health care (whose core subject is disease) without paying attention to the importance of technical care, we are not considering the complexity of the subjective and physical aspects of the concrete existence of the old-aged sick individual.
