Treatment of thoraco-lumbar curves in adolescent females affected by idiopathic scoliosis with a progressive action short brace (PASB): assessment of results according to the SRS committee on bracing and nonoperative management standardization criteria

Abstract: The purpose of the present study was to effectiveness of Progressive Action Short Brace (PASB) in the correction of thoraco-lumbar curves, in agreement with the Scoliosis Research Society (SRS) Committee on Bracing and Nonoperative Management Standardisation Criteria.Fifty adolescent females (mean age 11.8 ± 0.5 years) with thoraco-lumbar curve and a pre-treatment Risser score ranging from 0 to 2 have been enrolled. The minimum duration of follow-up was 24 months (mean: 55.4 ± 44.5 months). Antero-posterior radiographs were used to estimate the curve magnitude (CM) and the torsion of the apical vertebra (TA) at 5 time points: beginning of treatment (t1), one year after the beginning of treatment (t2), intermediate time between t1 and t4 (t3), end of weaning (t4), 2-year minimum follow-up from t4 (t5). Three situations were distinguished: curve correction, curve stabilisation and curve progression.The Kruskal Wallis and Spearman Rank Correlation tests have been used as statistical tests.CM mean value was 29,30 ± 5,16 SD at t1 and 14,67 ± 7,65 SD at t5. TA was 12.70 ± 6,14 SD at t1 and 8,95 ± 5,82 at t5. The variation between measures of Cobb and Perdriolle degrees at t1,2,3,4,5 and between CM t5-t1 and TA t5-t1 were significantly different.Curve correction was accomplished in 94% of patients, whereas a curve stabilisation was obtained in 6% of patients.The PASB, due to its peculiar biomechanical action on vertebral modelling, is highly effective in correcting thoraco-lumbar curves.The treatment of adolescent idiopathic scoliosis (AIS) is aimed at stopping the progression of the deformity, and improving the aesthetic appearance, trunk balance and quality of life [1].However, the effectiveness of the various therapeutic approaches is still controversial, mainly because of the incomplete knowledge of the aetiology and pathogenesis of the deformity [2]. Determination of quality of life in adolescents with idiopathic scoliosis subjected to conservative treatment

Abstract: Data were collected in 108 consecutive patients (96 females, 16 males) affected by AIS admitted to the outpatient orthopaedic clinic of the Catholic University of the Sacred Heart in Rome (Italy). Patients were subjected to full-time (i.e., 22 hrs per day) conservative treatment with the progressive action short brace (PASB), the Lyon brace or a combination of PASB + Lyon brace. Three instruments were used for QoL determination: the Scoliosis Research Society 22 (SRS-22), Bad Sobernheim Stress Questionnaire (BSSQ) and the Brace Questionnaire (BrQ).A significant correlation was detected among the 3 scores (p < 0.001). The BrQ possesses a higher capacity to detect changes in QoL in relation to the patient gender, type of brace, curve severity at baseline and at the completion of treatment, and curve type. Overall, boys displayed a higher QoL than girls. In all 3 questionnaires, higher QoL scores were determined in patients treated with the PASB compared with those using the Lyon brace. QoL scores were significantly correlated with the curve severity. Higher QoL scores were obtained by participants with thoraco-lumbar curves as compared with those with other curves.The 3 questionnaires are effective in capturing changes in QoL in AIS patients subjected to conservative treatment. However, the BrQ possesses a higher discriminatory capacity compared with the other questionnaires tested. PASB-based treatment is associated with better QoL than the Lyon bracing.According to the WHO, "quality of life is defined as an individual's perception of their position in life in the context of the culture and value systems in which they live and in relation to their goals, expectations, standards and concerns" [1]. Abstract: the influence of prenatal events on the development of headaches at childhood has not been investigated and is the scope of our study. of 2,173 children identified as the target sample, consents and analyzable data were provided by 1,440 (77%). parents responded to a standardized questionnaire with a validated headache module and specific questions about prenatal exposures. odds of chronic daily headache (cdh) were significantly higher when maternal tabagism was reported. when active and passive smoking were reported, odds ratio (or) of cdh were 2.29 [95% confidence intervals (ci)=1.6 vs. 3.6)]; for active tabagism, or=4.2 (95% ci=2.1-8.5). alcohol use more than doubled the chance of cdh (24% vs. 11%, or=2.3, 95% ci=1.2-4.7). in multivariate analyses, adjustments did not substantially change the smoking/cdh association. prenatal exposure to tobacco and alcohol are associated with increased rates of cdh onset in preadolescent children.

Abstract: There is growing body of literature which offers reviews of the concepts of organised crime and political vio- lence, while documenting the official efforts to address such concepts jointly and treat them as a single issue. It would be intriguing to investigate how members of organised criminal groups and violent political groups re- spectively react to such official efforts. In my own memory, when the ‘mafiosi’ happened to share a prison in- stitution with members of the Red Brigades, they would steer away from those idealist Communists who got nothing out of killing. The former, when overcoming the disgust they felt in the presence of those who in their eyes adopted an incomprehensible political stance, and perhaps even a despicable sexual lifestyle, would simply suggest: “don’t make revolution, make money, you cretin!”. The latter, in their turn, would deal with the former as one deals with yet a different version of the economic and political power against which they fought. Echoes of this are found in an example coming from Greece itself, where the Courts have attempted to term ‘common’ rather than ‘political’ the offences attributed to the Revolutionary Organisation November 17. The fact that or- ganised crime is guided by material motivations and terrorism by political ones may be seen as irrelevant by of- ficial agencies pursuing the objective of degrading the ‘enemy’ whoever that might be. Therefore, the ceremo- nies of degradation, including the choice of an ad hoc vocabulary, may well serve the task, as the mad, the drug user and the terrorist constitute an undistinguishable mob in the face of which quibbling differences may just obstruct the criminal justice process. The purpose of this paper is to try and clarify a number of issues that we encounter when dealing with organised crime and political violence respectively.

Abstract: Economic initiative, by definition, is a human and social effort devoted to the collective wellbeing. However, economic development in general, along with more or less measurable benefits, may cause harm to people and the environment. This aspect tends to be obscured by advocates of the currently predominant philosophy (or ideology) known as neo-liberalism. This paper sets off with a critical analysis of the axioms of neo-liberalism focusing on the variable risk and on the harm provoked by economic development on the environment. Straddling legality and illegality, harmful economic conduct is then equated to a specific form of white collar crime, while an attempt is made to trace in the history of economic thought itself the embryo of a destructive rationale. After considering how harm to individuals and the environment is hidden through ideological strategies, the paper looks at some refreshing proposals to turn the threats of economic development into human opportunities.

Abstract: Photonic crystal point-defect cavities were fabricated in a GaN free-standing photonic crystal slab. The cavities are based on the popular L3 design, which was optimized using an automated process based on a genetic algorithm, in order to maximize the quality factor. Optical characterization of several individual cavity replicas resulted in an average unloaded quality factor Q = 16,900 at the resonant wavelength {\lambda} $\sim 1.3$ {\mu}m, with a maximal measured Q value of 22,500. The statistics of both the quality factor and the resonant wavelength are well explained by first-principles simulations including fabrication disorder and background optical absorption.

Abstract: Gestalt Psychology as a School of Respect - Respect as “phenomenological principle” means that one must respect phenomenal reality in the same way as physical reality. Respect as “epistemological principle” means that studying the self of the other the researcher must establish an intimate and equal relation with this person. This is not an accidental aspect, but one of the basic epistemological factors of the “field” in the sense of Lewin’s “action-research”. Respect as “educational principle” means that in the caring for a living being we must respect its inner factors and conditions and its rhythm of development.

Abstract: Desde hace más de 20 a os la imagenología cerebral funcional ha avanzado hasta llegar a elaborar la presunción diagnóstica, el diagnóstico diferencial, el pronóstico, la respuesta terapéutica y el estudio de los neurotransmisores y receptores en Neurología y Psiquiatría. Los trastornos psiquiátricos son denominados funcionales hace más de una centuria, y hoy pueden ser visualizados en imágenes cerebrales.Las imágenes funcionales en PET (Tomografía por Emisión de Positrones) y el NEUROSPET (Tomografía por Emisión de Fotón único), demuestran en forma no invasiva el estado de la función cerebral. El PET lo hace fundamentalmente evaluando el metabolismo de la glucosa y el NEUROSPET, visualizando el estado del flujo sanguíneo cerebral.La prevalencia de este trastorno es relativamente baja (2 a 3%) pero la comorbilidad con psicosis, la depresión, el trastorno bipolar y la esquizofrenia es alta. Además no es infrecuente en el autismo, en el trastorno de la atención, en la tricotilomania, en la personalidad fronteriza, en los compradores compulsivos, en la ludomania, en la compulsión sexual, en los tics y en la enfermedad de Gilles de la Tourette’s. Al NEUROSPET encontramos una hiperperfusión en ambos lóbulos frontales, en su cara dorso lateral prefrontal, en su zona inferior y en la corteza pre-motora, hiperperfusión en cíngulo posterior e hipoperfusión en los ganglios de la base (núcleo caudado).Como dice Cummings, la hiperactividad del sistema límbico estaría implicada en el TOC.Así pues, la imagenología cerebral nos permite en el TOC: una ayuda diagnóstica, la evaluación de la evolución clínica imagenológica, la respuesta terapéutica y la posible utilización como predictor terapéutico.In the past twenty years functional brain imaging has advanced to the point of tackling the differential diagnosis, prognosis and therapeutic response in Neurology and Psychiatry. Psychiatric disorders were rendered "functional" a century ago; however nowadays they can be seen by means of brain imaging.Functional images in positron emission tomography (PET) and single photon emission tomography (NEUROSPET) show in non-invasive fashion the state of brain functioning. PET does this assessing glucose metabolism and NEUROSPET by putting cerebral blood flow in images.Prevalence of OCD is clearly low (2 to 3%), but comorbidity with depression, psychoses, bipolar disorder and schizophrenia is high.Furthermore, it is not infrequent with autism, attention disorder, tichotillomany, borderline personality disorders, in pathological compulsive spending, sexual compulsion and in pathologic
