Giant Hepatic Carcinoid: A Rare Tumor with a Favorable Prognosis

Abstract: Primary hepatic carcinoids are rare tumors that are often diagnosed at a locally advanced stage. Their primary nature can only be ascertained after thorough investigations and long-term follow-up to exclude another primary origin. As with secondary neuroendocrine liver tumors, surgical resection remains the mainstay of therapy. Despite their large size and often central location liver resection is often feasible, offering long-term survival and cure to most patients. In selected patients liver transplantation appears to be a good indication for tumors not amenable to liver resection. An aggressive surgical attitude is therefore warranted. We report a large and unusually fast-growing liver carcinoid that appeared only marginally resectable in a patient who remains free of disease four years after surgery. 1. Introduction With less than 100 reports in the literature little is known of primary hepatic carcinoids. They often present as large centrally situated liver masses, characteristics that may discourage attempts at resection. Data is scarce on the outcome of surgical treatment but these tumors seem to be associated with a favorable prognosis, justifying an aggressive surgical approach. Presented here is a patient with a giant primary liver carcinoid that appeared only marginally resectable, who remains disease free four years after surgical resection. 2. Case Report A 52-year-old female complained of fatigue and intermittent fever of one-year duration. There was no history of liver failure, hematemesis, flushing, or diarrhea. She had undergone a previous appendectomy. Abdominal ultrasound revealed a heterogeneous 8?cm right lobe focal liver lesion. Contrast enhanced CT performed one month later showed a solid 15?cm liver mass with a hypodense core that was suggestive of an atypical hemangioma. The patient came to our attention two months after the initial ultrasound with palpable hepatomegaly. MR showed a hypervascularized solitary liver lesion measuring 20?cm (Figure 1). Malignancy was further suspected by FDG-PET scan showing increased uptake. Laboratory data showed normal liver function and hepatitis B and C serologies were negative. Serum tumor markers including CEA, AFP, CA 12.5, CA 19.9, and NSE were within normal range while chromogranin A was moderately elevated. 24-hour urinary 5-HIAA excretion was normal. Chest CT showed no signs of malignancy. No primary tumor was found at gastroscopy and colonoscopy. Abstract: We argue that the superpartner of the Dirac right-handed neutrino is a prime candidate for dark matter created from a 'mattergenesis' mechanism. We show that due to the smallness of the Yukawa couplings, a right-handed sneutrino density created in the early Universe would not be erased by annihilations, which remain out of thermal equilibrium. It would also not be drowned by a later, additional production of right-handed sneutrinos, as the relic density of the non-thermal right-handed sneutrinos is found to be generally negligible compared to the observed dark matter density. Mild constraints on sneutrino masses and trilinear SUSY-breaking couplings are obtained. Possible mattergenesis scenarios are also mentioned

Abstract: Si désormais les divers pays et cultures du monde hispanique figurent dans les programmes officiels de l'apprentissage de l'espagnol langue étrangère en France, l'approche qui en est proposée demeure encore souvent traditionnelle, une certaine vision de l'histoire et les "beaux" textes dominant largement. Comment amener les jeunes enseignants en formation initiale à porter un regard différent sur l'autre, à atteindre une véritable compétence culturelle dépassant la culture académique ? Nous rendrons compte dans notre étude de la spécificité de la formation initiale des professeurs d'espagnol qui, dorénavant, sont de plus en plus nombreux à être d'origine hispanophone. Nous exposerons leur parcours de formation sur et grace aux TICE afin de dépasser les clichés réducteurs initiaux comme en témoignent leur approche première sur Internet, et en vue d'atteindre un regard plus ouvert sur l'hispanité et des pratiques pédagogiques différentes. Despite the ongoing presence of several Hispanic countries and cultures in state programs concerning Spanish as a foreign language in French education, the suggested methodology remains very often traditional with a predominantly biased vision of history as well as texts that are considered "beautiful". How can we help young teachers in their initial training to have a different perception of the other, to reach a true cultural competence that surpasses academic culture? We will show in our study the details involved in a Spanish teacher's initial training since, as it appears, they tend to be more and more of Hispanic origin. We will describe their training history through and thanks to the TICE in order to overcome the reductive and stereotyping clichés as they move initially towards the use of Internet, and in order to reach out for a more open perception of the Hispanic World and of different pedagogical practices.

Abstract: The EDELWEISS II experiment is devoted to the search for Weakly Interacting Massive Particles (WIMP) that would constitute the Dark Matter halo of our Galaxy. For this purpose, the experiment uses cryogenic germanium detectors, cooled down at 20 mK, in which the collision of a WIMP with an atomic nucleus produces characteristic signals in terms of ionization and elevation of temperature. We will present the preliminary results of the first operation of the detectors installed in the underground laboratory of the Frejus Tunnel (LSM), attesting to the very low radioactive background conditions achieved so far. New detectors, with a special electrode design for active rejection of surface events, have been shown to be suited for searches of WIMPs with scattering cross-sections on nucleon well below 10-8 pb. Preliminary results of WIMP search performed with a first set of these detectors are presented.
First determination of $f_+(0) |V_{us}|$ from a combined analysis of $τ\to Kπν_τ$ decay and $πK$ scattering with constraints from $K_{\ell3}$ decays

Abstract: We perform a combined analysis of $\tau\to K\pi \nu_\tau$ decay and $\pi K$ scattering with constraints from $K_{\ell3}$ data using a $N/D$ approach that fulfills requirements from unitarity and analyticity. We obtain a good fit of the $I=1/2$ $\pi K$ amplitude in the $P$ wave using the LASS data above the elastic region while in this region data are generated via Monte Carlo using the FOCUS results based on $D_{\ell 4}$ decay. The spectrum and branching ratio of $\tau\to K\pi \nu_\tau$ constrained by $K_{\ell3}$ decays are also well reproduced leading to $f_+(0) |V_{us}|= 0.2163 \pm 0.0014 $. Furthermore, we obtain the slope of the vector form factor $\lambda_+=(25.56 \pm 0.40) \times 10^{-3} $ while the value of the scalar form factor at the Callan-Treiman point is $\ln C=0.2062 \pm 0.0089$. Given the experimental precision our results are compatible with the Standard model.
The unstable integral homology of the mapping class groups of a surface with boundary

Abstract: We construct a graph complex calculating the integral ho- mology of the bordered mapping class groups. We compute the ho- mology of the bordered mapping class groups of various surfaces. Using the circle action on this graph complex, we build a double complex and a spectral sequence converging to the homology of the unbordered mapping class groups. We compute the homology of the punctured mapping class groups associated to certain surfaces. Finally, we use Miller's operad to get the first Kudo-Araki and Browder operations on our graph complex. We also consider an unstable version of the higher Kudo-Araki-Dyer-Lashoff operations.

In this paper we show how to extend this construction to a topological conformal field theory of degree d. Abstract: In this paper, we give the expressions for the bounded spherical functions, or equivalently the spherical functions of positive type, for the free two-step nilpotent Lie groups endowed with the actions of orthogonal groups or their special subgroups. Next we deduce some results about the (Kohn) sub-Laplacian, and we compute the radial Plancherel measure.
