Abstract: this article presents a process of investigation that was developed to identify life and health conditions of a community in order to propose changing strategies to the situation. it took place in distrito do grajaú (sp) - brazil. all families were interviewed through the application of a survey in order to find personal/life conditions, and a health disease process profile. data reveal a determined profile that was presented to the people to implement integrated actions to solve the main problems/issues at the community.

Abstract: this article presents a report about the participation in one program in the field of international health witch has been promoted by panamerican health organization. the author presents a synthesis of the concept of international health, describing the objectives of the program and the activities developed during the process to achieve it. beside this, the author indicates the importance of the participation of nurses in this kind of program in order to enlarge the visibility of nursing knowledge.
Sildenafil no tratamento da hipertens?o arterial pulmonar idiopática em crian?as e adolescentes

Abstract: objective: this study aims to provide data on the use of oral sildenafil in patients in new york heart association functional class iii or iv with severe idiopathic pulmonary arterial hypertension unresponsive to conventional therapy. method: in this series, six patients with idiopathic pulmonary arterial hypertension were prospectively treated with 2 to 8 mg of oral sildenafil in four to six doses a day. all pa-tients were submitted to physical examination, electrocardiogram and echocardiogram, chest computed tomography, ventilation and pulmonary perfusion scintigraphy, coagulation studies, and tests for collagen vascular disease, acquired immune deficiency syndrome and schisto-somiasis in order to rule out secondary causes of pulmonary arterial hypertension. all patients underwent cardiac catheterization for vasoreactivity tests using nitric oxide, o2 at 100% and oral nifedipine, and a 6-minute walking test was performed in those patients who were con-sidered able to exercise. results: all patients achieved a good therapeutic response, with improvement by at least one functional class, and presented an increase in systemic arterial oxygen saturation. five patients showed a decrease in the pulmonary systolic pressure to systemic systolic pres-sure ratio and improvement in the six-minute walking test. no major side effects were ob-served at 4 to 36 months of follow-up. one patient had sudden death after sildenafil had been withdrawn by mistake. conclusions: these data suggest that sildenafil may be useful in the manage-ment of idiopathic pulmonary arterial hypertension. patients should be advised against the withdrawal of sidenafil without medical supervision.

Abstract: this study was an investigation of the proposition and development of health surveillance as an instrument or model of health. a literature search was conducted in lilacs and medline databases using the key words population's surveillance and primary health care. the literature indicated the existence of different propositions and terminologies for health surveillance which are used in epidemiology and territorial surveillance. there was emphasis on the use of health indicators, geoprocessing, and analysis of network of health surveillance agencies. health surveillance can be used to promote health, social control of information, and to evaluate health policies and its implementation.

Abstract: the surveillance in health is constituted as an assistance model with potential to reorganize the working processes. this study was developed to identify and analyze surveillance practices in the primary health care field, in a region of s?o paulo city-brazil. interviews were performed with 14 individual, from february to april 2006: health workers and administrators who had been operating in the epidemiologic surveillance area of health basic units. the speeches were analyzed based on the social theory of the health-disease process and by the materialistic interpretation of the labor process. it was verified that their work was fragmented. the precariousness of the working structure, the lack of professional qualification and of management-political support, besides the lack of participation from the population were all identified as barriers that limit health surveillance practices. it was concluded that, in a local level, there is a potential to change the organization of the working process in health and meet the health needs of the population through a health surveillance model.
Tuberculose: tratamento supervisionado nas Coordenadorias de Saúde Norte, Oeste e Leste do Município de S?o Paulo

Abstract: this study had as objective to analyze the strengths and weaknesses of the directly supervised treatment strategy (dots) from the view of patients and workers from the north, west and east health departments of s?o paulo. the analysis of the statements was based on the social determination theory of the health-disease process. in general, the dots makes it possible to establish attachments and that treatment adherence is associated with the need of returning to work. the identified weaknesses were the health professionals' lack of involvement in the treatment and the irregular distribution of incentives. results show that treatment adherence transcends the biological and individual scope, and that it is central that health professionals recognize patients as people who have specific needs that are not limited to the tuberculosis treatment.

Abstract: this is a qualitative study performed in the theoretical framework of the theory of social determination of the health-disease process and the concept of compliance. the goal was to analyze meanings of dots in compliance with tuberculosis treatment, according to healthcare professionals of the technical healthcare supervision of butant? (suvis), a region of the s?o paulo city healthcare secretariat, brazil. the project was submitted to the ethics committee of the s?o paulo municipal health secretariat. all professionals (22 people) developing dots were interviewed, including service coordinators, healthcare professionals and the dots coordinator for the region. the statements were analyzed with an appropriate technique for discourse analysis. the results appoint that the strategy presents more potentialities than limits and is effective regarding compliance, since it allows the professionals to welcome and monitor the patients, considering their needs. the importance of increasing the understanding of compliance is also noted, so that it can go beyond the simple intake of medication, integrating the care for the sick person and his or her necessities by transcending those restricted to the biological dimension.
