START Triage Scale in Disasters

Abstract: Disasters are events that occur since the beginning of time. When previous records are investigated, information on various types of disasters occurred in different parts of the World are encountered. Disasters occur unexpectedly and require constant state of preparedness. No matter how extensive the precautions are, disasters cannot be prevented, but the precautions significantly contribute to the reduction of negative consequences to a minimum. In this context, response plans to events that may occur are as important as the preparedness plans. Responding to disaster victims on time, correctly and efficiently reduces the mortality rate to a minimum as well as increasing the survival rate. Correct classification of the victims based on the life threatening events or the priority of their injuries, providing transport, and ensuring each individual receives emergency aid should be the main goals of triage. That is, to be the most helpful to as many injured people as one can by using the limited opportunities available. A successfully performed triage not only increases the success of the healthcare team’s response, it also enables healthy execution of scene management. In this study, the aim is to provide information about START scale composed of the first letters of “Simple Triage And Rapid Treatment” that has been developed and accepted to be used in mass events.
Selenium in the Prevention of Anthracycline-Induced Cardiac Toxicity in Children with Cancer

Abstract: High cumulative doses of anthracyclines (300–500?mg/m2) used in the treatment of children with cancer may result in cardiotoxicity, a major long-term adverse effect that limits clinical usefulness of this class of chemotherapeutic agents. We assessed anthracycline-induced cardiotoxicity by measuring Pro-BNP levels and echocardiographic (ECHO) findings and investigated potential protective effect of selenium (Se) supplementation in a group of pediatric cancer patients. Plasma level of Pro-BNP was measured, and ECHO was performed in 67 patients (45 boys, 22 girls; ages 2–18 years; median age 12 years) after they completed anthracycline-containing chemotherapy. Serum Se level was measured in 37 patients. Eleven patients had high Pro-BNP levels and/or cardiac failure with Pro-BNP levels of 10–8,022?pg/mL (median 226.3?pg/mL; laboratory normal level is less than 120?pg/mL). Serum Se levels were low (20–129?mcg/L, median 62?mcg/L) in ten of these eleven patients. Eight of 10 patients with low Se and high Pro-BNP levels were supplemented with Se 100?mcg/day for a period of 4–33 months (median 6 months) which resulted in improvement in Pro-BNP and/or ECHO findings. These results suggest that Se supplementation may have a role in protection against anthracycline-induced cardiac toxicity. 1. Introduction High cumulative doses of anthracyclines (300–500?mg/m2) are frequently administered to children with cancer. Cardiac toxicity is a serious adverse effect that limits the therapeutic potential of anthracyclines and threatens the cardiac function of pediatric cancer patients leading to debilitating long-term effects resulting in poor quality of life in cancer survivors [1–5]. This is particularly devastating in children who are cured of their cancer because they have to endure the debilitating cardiac dysfunction for the rest of their lives with limited exercise capacity which may also lead to other chronic illnesses. B-type-natriuretic peptide (BNP) is a polypeptide hormone predominantly released from the cardiac ventricles in response to volume expansion and pressure overload. BNP is found in the circulation as BNP-32 and the NH2-terminal portion of ProBNP (Nt-proBNP). BNP levels are elevated in patients with left ventricular systolic dysfunction and correlate with the severity of symptoms and prognosis [6–14]. Measuring serum Pro-BNP levels is a reliable way to monitor the cardiac function of patients receiving cardiotoxic drugs such as anthracyclines. Abstract: A seasonal-spatial distribution and ecological risk assesment of the surface sediment are provided for Trabzon Harbour, located in the Eastern Black Sea Region. Active Optimal Control of the KdV Equation Using the Variational Iteration Method

Abstract: The optimal pointwise control of the KdV equation is investigated with an objective of minimizing a given performance measure. The performance measure is specified as a quadratic functional of the final state and velocity functions along with the energy due to open- and closed-loop controls. The minimization of the performance measure over the controls is subjected to the KdV equation with periodic boundary conditions and appropriate initial condition. In contrast to standard optimal control or variational methods, a direct control parameterization is used in this study which presents a distinct approach toward the solution of optimal control problems. The method is based on finite terms of Fourier series approximation of each time control variable with unknown Fourier coefficients and frequencies. He's variational iteration method for the nonlinear partial differential equations is applied to the problem and thus converting the optimal control of lumped parameter systems into a mathematical programming. A numerical simulation is provided to exemplify the proposed method.
Laparoscopic management of fallopian tube prolapse masquerading as adenocarcinoma of the vagina in a hysterectomized woman

Abstract: A 39-year-old woman with vaginal prolapse of the fallopian tube after total abdominal hysterectomy presented with an incorrect diagnosis of adenocarcinoma of the vaginal apex. The prolapsed tube and cystic ovary were removed by vaginal and laparoscopic approach. The postoperative course went well.Early or late fallopian tube prolapse can occur after total abdominal hysterectomy and vaginal hysterectomy. Symptoms consist of persistent blood loss or leukorrhea, dyspareunia and chronic pelvic pain. Vaginal removal of prolapsed tube with laparoscopic surgery may be a suitable treatment. The abdominal or vaginal approach used in surgical correction of prolapsed tubes must be decided in each case according to the patient's individual characteristics.Fallopian tube prolapse (FTP) is an unusual complication of both abdominal and vaginal hysterectomies. Sporadic cases have been added to the literature about predisposing factors and techniques of management [1]. The diagnosis of FTP needs postoperative observation and careful examination of the vaginal vault. A biopsy is necessary to differentiate between this lesion and the presence of vaginal granulation tissue. The presence of cellular atypias may result in an unnecessary additional operation. These changes are probably secondary to the severe underlying inflammatory reaction. Vaginal infection, postoperative bleeding, a defective surgical technique, and poor physical condition of the patient are the most important predisposing factors for intravaginal prolapse of the fallopian tube [2]. An excisional biopsy constitutes the definitive diagnostic procedure and adequate treatment for this posthysterectomic vaginal complication.We have managed a woman with FTP on combining a simple vaginal approach and laparoscopic total salpingectomy and oophorectomy. Abstract: Few concepts in instructional communication literature have received as much attention as teacher immediacy. However, educational communication scholars have thoroughly studied immediacy behaviors mainly in traditional classrooms and these studies are mostly related to student attitudes and learning. Thanks to some growing attempts, recent research has extended these findings to distance education. The difference of this study is to examine the relationship between teacher immediacy behaviors and participation in an online setting. Results indicated that affective and interactive indicators were the least used immediacy behaviors while cohesive indicators were mostly used by teacher in this case. Also data show that teachers’ interactive immediacy behaviors and immediate feedback determine students’ participation in asynchronous computer-mediated communication environment.

Abstract: Food irradiation is the process of exposing food to ionizing radiation at different doses in order to disinfest, sterilize, or preserve food. Administered hygienically at low doses, it extends shelf life and delays the ripening or sprouting of fruits and vegetables. At higher doses, it eliminates insects, moulds, bacteria and other potentially harmful micro-organisms that cause spoilage in foods. In today s increasingly densely populated world, where half of the global population lives in near-starvation conditions, the provision of sufficient and safe food is a high priority concern. Irradiation is a safe and effective method of food preservation used in many countries around the world. The present study examines the effects of the fast-growing irradiation technology on nutrition and health.
