A Rare Case of Pulmonary Mucormycosis Successfully Treated with Dual Antifungal Agents and Surgery in a Patient with Uncontrolled Diabetes  [PDF]

Abstract: Pulmonary mucormycosis is an emergent and life-threatening invasive fungal infection underdiagnosed by clinicians due to unspecific clinical picture and lack of awareness. Although the acknowledgment of risk factors, clinical and radiological findings may increase early recognition, the definite diagnosis is challenging to establish and the ideal treatment has never been delineated. The authors describe a rare case of pulmonary mucormycosis that was successfully treated with dual antifungal treatment and surgery in a patient with uncontrolled diabetes. When last evaluated, he was asymptomatic with no evidence of relapse. To our knowledge this is a rare report of a successfully treated diabetic patient with pulmonary mucormycosis with L-AmB, triazole posaconazole and surgery with no evidence of recurrence. We highlight the importance of clinicians’ awareness to early diagnosis, combined antifungal treatment and adjuvant surgery as the greatest chance of cure of a rapidly progressive disease with high mortality and morbidity.
Therapeutic implications of transesophageal echocardiography after transthoracic echocardiography on acute stroke patients

E., PortugalBackground: The role of transesophageal echocardiography (TEE) in the evaluation of acute stroke patients is still ill-defined. We conducted a prospective observational study to find the prevalence of TEE findings that indicate anticoagulation as beneficial, in acute ischemic stroke patients without indication for anticoagulation based on clinical, electrocardiographic and transthoracic echocardiography (TTE) findings.Methods: We prospectively studied all patients referred to our laboratory for TTE and TEE. Patients were excluded if the diagnosis was not acute ischemic stroke or if they had an indication for anticoagulation based on clinical, electrocardiographic, or TTE data. Patients with TEE findings that might indicate anticoagulation as beneficial were identified.Results: A total of 84 patients with acute ischemic stroke and without indication for anticoagulation based on clinical and electrocardiographic or TTE data were included in the study. Findings indicating anticoagulation as beneficial were found in 32.1%: spontaneous echo contrast (1.2%), complex aortic atheroma (27.4%), thrombus (8.3%), and simultaneous patent foramen ovale and atrial septal aneurysm (2.4%).Conclusions: The results of our study show that TEE can have therapy implications in 32.1% of ischemic stroke patients in sinus rhythm and with TTE with no indication for anticoagulation.Keywords: acute ischemic stroke, transesophageal echocardiography, transthoracic echocardiography, therapy
Analysis of the videolaparoscopy potentiality in the surgical treatment of the bowel obstruction

Abstract: context: laparotomy is the gold standard treatment of patients with intestinal obstruction without response to clinical management. nowadays, literature has been demonstrating the feasibility of videolaparoscopy in the treatment of intestinal obstruction. objectives: to report the clinical-epidemiological profile of patients with intestinal obstruction submitted to surgery and verify the presence of contraindications for laparoscopy. methods: it was done a observational, descriptive and retrospective study including adults patients with intestinal obstruction submitted to surgery at hospital de clínicas de porto alegre, rs, brazil, between january of 2004 and october of 2008. results: it was included 135 patients in the study, with a total of 126 patients submitted to open surgery and 9 to laparoscopy. there was similar distribution between gender and the mean age was 59 years (sd ± 16.9). the most frequent site of obstruction was the small bowel and the most frequent etiology was adhesions. among the patients submitted to laparotomy, 75.4% presented with abdominal distention, 68.3% previous abdominal surgery, 11.9% body mass index >30 kg/m2, 4.8% coagulopathy and 3.2% hemodynamic instability. among the 135 patients, only 5 of them presented with none contraindications for videolaparoscopy. conclusion: the epidemiological findings of this study are similar to the ones of the worldwide literature. indications of videolaparoscopy in retrospective analyses have the limitation of subjective evaluation of intestinal obstruction, which was included in this study as a relative contraindication to laparoscopy.

Abstract: puerperal psychosis (pp) is an exuberant clinical syndrome with an estimated frequency of 1 case per 1,000 childbirths that has been most consistently associated with the bipolar disorders spectrum. available evidence is scarce, namely regarding management and treatment. the authors present the clinical case of a 28 year-old first-time mother, with no psychiatric history, who developed a florid psychotic syndrome in the first weeks of puerperium. due to lack of improvement following pharmacological treatment, electroconvulsive therapy (ect) was applied, with substantial and quick response. in this context, the authors reviewed current literature on the use of ect for the treatment of puerperal psychosis.

Abstract: austenitic stainless steels specimens were deformed by tension from 0.03 to 0.12 of true strain at -50oc and -10oc, in order to obtain different volumetric fractions of e and α' strain induced martensites. the morphology, distribution and volumetric fractions of the strain induced martensites were characterized by metallography and dilatometry analysis and quantified by ferrite detector measurements. the reverse transformation temperatures of ε→γ and α'→γwere detected by dilatometry tests. the e and α' martensites reversion was observed in the temperature range of 50-200oc and 500-800oc, respectively. the heating rate did not influence the reversing of ε→α' temperatures but did affect the α'→γtransformation temperatures.

Abstract: introduction: high plantar pressure is a proven risk factor for ulceration among individuals with diabetes mellitus. the harris and beath footprinting mat is one of the tools used in screening for foot ulceration risk among these subjects. there are no reports in the literature on the reliability of footprint analysis using print pattern criteria. objective: the aim of this study was to evaluate the inter- and intra-examiner reliability of the analysis of footprint patterns obtained using the harris and beath footprinting mat. methods: footprints were taken from 41 subjects using the footprinting mat. the images were subjected to analysis by three independent examiners. to investigate the intra-examiner reliability, the analysis was repeated by one of the examiners one week later. results: the weighted kappa coefficient was excellent (kw>0.80) for the inter- and intra-examiner analyses for most of the points studied on both feet. conclusions: the criteria for analyzing footprint patterns obtained using the harris and beath footprinting mat presented good reliability and high to excellent inter- and intra-examiner agreement. this method is reliable for analyses involving one or more examiners.

Abstract: parinaud's oculoglandular syndrome is a rare eye disease caused by different pathogens, including bacteria, viruses and fungi. it is characterized by a granulomatous conjunctivitis with adjacent preauricular lympha-denopathy and can cause sequelae if not promptly treated. we report a case of a young nurse assistant and veterinary student showing parinaud's oculoglandular syndrome caused by the fungus sporothrix schenkii after contamination with infected cats. its clinical presentation and negative outcome until the correct ethiological diagnosis, in addition to specific treatment, emphasize the importance of laboratory investigations in cases of atypical development of the disease.
An Eclipse Plugin to Support Code Smells Detection

Abstract: Eradication of code smells is often pointed out as a way to improve readability, extensibility and design in existing software. However, code smell detection in large systems remains time consuming and error-prone, partly due to the inherent subjectivity of the detection processes presently available. In view of mitigating the subjectivity problem, this paper presents a tool that automates a technique for the detection and assessment of code smells in Java source code, developed as an Eclipse plug-in. The technique is based upon a Binary Logistic Regression model and calibrated by expert's knowledge. A short overview of the technique is provided and the tool is described.

Abstract: the electrochemical behavior of the herbicide metribuzin was studied in an aqueous solution on glassy carbon, carbon paste/nujol oil and carbon paste/castor oil using cyclic voltammetry, square-wave voltammetry, differential pulse voltammetry, controlled-potential coulometry and electrolysis, for quantification and decontamination purposes. the main electrolytic products obtained from the reduction of metribuzin, after consumption of 8.26 mol electron mol-1, were deaminometribuzin and diketometribuzin. the anodic wave observed after electroreduction is associated with the oxidation of the methylthiolate generated in the electrolytic process. this wave was used to quantitatively determine metribuzin in a commercial sample by anodic stripping voltammetry. the electro-fenton method was employed to promote decontamination by eliminating the herbicide, resulting in 80% of mineralization of the metribuzin.

Abstract: acne fulminans is a rare manifestation that may occur during the evolution of acne vulgaris primarily in male adolescents. use of isotretinoin, testosterone, and exacerbated immune responses in the body are related triggers. signs, symptoms and laboratory findings such as fever, hepatomegaly, polyarthralgia, leukocytosis, plaquetose, increased inflammatory markers and transaminases, are characteristic. a bone scan can detect osteolytic lesions in multiple skeletal sites. the treatment is performed with prednisolone, isotretinoin and antibiotics if secondary infection is present. this case describes a male patient with a diagnosis of grade iii acne, who developed acne fulminans and bilateral sacroiliitis with inability to ambulate, after initiation of therapy with isotretinoin
