Metastasis to Paranasal Sinuses as the First Presenting Sign of Renal Cell Carcinoma  [PDF]

Abstract: The clear cell carcinoma is a relatively rare tumor, represented approximately 85% by renal cell carcinoma (RCC), but has a characteristic of early metastatic, which sometimes spread to unusual places. Metastatic tumors in the paranasal sinuses are very rare. This case report a 65 years-old patient who presented sinusitis and epitaxis as a first sign of renal cell carcinoma with metastasis to the right maxillary sinus, which was opereted at Lagoa Hospital—RJ and strengthen the inclusion of this disease in the differential diagnosis of sinus disease. The renal cell carcinoma is a relatively rare tumor, representing approximately 3% of malignant tumors. About a third of patients with renal cell carcinoma have metastasis at diagnosis. The sinuses are a rare location of metastasis. The maxillary sinuses and the paranasal sinuses are most commonly affected by metastatic tumors to the sinuses region. The most frequent primary tumor to metastasis to paranasal sinuses is the renal cell carcinoma. This report highlights the rarity of the case and the importance in the differential diagnosis of sinus disease.
Giant Bladder Stone and Renal Failure: A Case Report, Literature Review and Future Perspectives  [PDF]

Abstract: We present a male patient, 52 years old, with acute renal failure due to giant bladder stone. In first presentation with acute renal failure, tomography has shown a bladder stone with 10 cm in its largest diameter associated with bilateral ureterohydronephrosis. It was initially treated by bladder catheterization and indicated emergency dialysis and subsequently submitted to surgical treatment through open cistolitotomy. During the procedure it was possible to observe the giant calculus, adhered to the trigone of the bladder, occluding his neck. After the vesical catheter was deployed, the patient had clinical and laboratory improvement during hospitalization. The association between giant bladder stones and urologic kidney failure is a rare condition and once removed the causative factor, we expect the return to normal limits. Studies with higher levels of evidence are needed to increase knowledge about the subject.
Partial Nephrectomy as Treatment of an Atypical Metastasis from Prostate Cancer—A Case Report and Review of Literature  [PDF]

Abstract: Background: Prostate cancer is the second most common type of cancer in man and the second in cancer-specific deaths in this population in the world. Most of the causes of death related to prostate cancer are due to its distant metastases, with the most common sites being: skeleton, distant lymph nodes, liver and lung. Renal metastasis is rare, and studies suggest infiltration due to arterial microembolization of the tumor. A key point in this scenario is the clinical suspicion of differential diagnoses, to offer the patient an effective therapy in such a specific case. Aim: To report a case of a patient with prostate cancer undergoing partial nephrectomy whose histopathological report revealed a metastatic lesion of that primary site. Case Presentation: 74 years old man, referred in May 2015 due to high PSA level and lumbago. PSA 323.11 ng/dl, rectal examination cT3a; biopsy was performed and histopathological study reported bilateral prostate adenocarcinoma, Gleason’s score 8 (4 + 4). Patient’s staging showed multiple secondary implants on skeletal scintigraphy. Tomography revealed solid exophytic lesion in the lower pole of the right kidney (4.7 × 3.6 cm); prostate without cleavage planes with seminal vesicles and pelvic node enlargement. Hormone therapy was initiated, PSA levels dropped to 9.51 ng/dl and total testosterone < 50 ng/dl. Partial nephrectomy was planned, initially by laparoscopy, but converted to laparotomy in December 2015. Procedure lasting 3 hours, minimal blood loss, no perioperative complications, discharged on the 3rd postoperative day. Histopathological report described undifferentiated malignant neoplasm, requiring immunohistochemistry that confirmed prostate adenocarcinoma. Patient remains hormone therapy, with no progression of the disease so far. Conclusion: Given the rarity of these cases, it is not possible to presume that nephrectomy enhances the survival rates. However, we observed that partial nephrectomy was a good choice for our patient, being the first case described in the literature. More reports should be available and studies with higher levels of evidence should be conducted to assist us in patient orientation and decision making.

Abstract: an experiment was carried out in greenhouse to evaluate total content and distribution of heavy metals in seedlings of twenty different tree species growing in a soil contaminated by zn, cd, cu and pb. plantable size seedlings were transfered to 3.3 kg pots containing a contaminated soil mixture and to a control soil without contamination where they were allowed to grow for ninety days. metal foliar concentrations for plants grown in the contaminated soil were high, ranged from (mg kg-1): zn= 154 to 1177; cd = 0.6 to 54.6 and cu= 2.8 to 134. in most cases, these concentrations were superior to what has been considered as critical toxic levels, whereas foliar pb concentrations ranged from 0.1 to 4.3 mg kg-1, below the critical toxic level. in some species that were highly affected by contamination such as machaerium nictidans, myroxylon peruiferum, piptadenia gonoacantha, senna macranthera and trema micrantha it was found high translocation index for zn and/or cd. however, dendropanax cuneatum that was only slightly affected by contamination, exhibited high translocation of zn and cd to shoots. in contrast to others species, dendropanax cuneatum retained these elements in the stems. other group of plants that were only slightly affected by soil contamination such as acacia mangium, copaifera langsdorffi and cedrella fissilis accumulated more zn and cd in the roots than in the shoots, therefore indicating that reduced translocation is involved in their tolerance to the excess of heavy metal in soil. the proportional distribution pattern of zn and cd in the roots and shoots of the studied plant species is related to their behavior to the excess of heavy metals in soil.

Abstract: There is no enough information about metal transport parameters in residual soil. These soils are generally structured and still there is no answer to what happens with the contaminant transport parameters when an acid solution with metal percolates this material with different structure. The objective of this study was to determine the contaminant transport parameters for Cd, Ni, Cu and Zn metals in an acid multispecies solution to a residual soil from south Brazil with an undisturbed and remolded structure. Column tests were carried out to determine the kd, Rd, Dh transport parameters. It was possible conclude that the magnitude of the kd, Rd and Dh parameter did not vary significantly with the soil structure.

Abstract: this paper develops an index of global minimum variance portfolio (mvp) for the most liquid stocks in brazil. the unconstrained mvp shows no significant difference in performance over the ibovespa. the imposition of a ten percent ceiling on the mvp weights for each asset made it possible to beat the ibovespa. however, this strategy is comparable to an equally weighed portfolio and is beaten by a number of actively managed stock funds in out of the sample tests. nevertheless, these constraints facilitate the replication of the mvp by individual investors and by etfs and support the power of naive diversification strategies.
