Abstract: the organization of the health sector in brazil changed significantly after the ratification of the federal constitution in 1988 and the organic health laws in 1990. decentralization of health services has been the basis for the implementation of the organizational and management principles in the brazilian unified health system. this study presents some of the experiences in the health sector that have contributed decisively to the development of such principles, caused important changes in the present care model, and achieved significant results in social health production.

Abstract: the objective of this study was to provide access to and disseminate a questionnaire used as an instrument to assess the organizational elements and the performance of primary health care clinics regarding tb control in brazil, comparing selected organizational dimensions by health care clinic, by municipality and by actor (patients, health care workers and managers). the results show that municipalities where the coverage of supervised treatment was more extensive presented more favorable indicators regarding access to tb treatment. the organizational format of the health care clinics involved in tb treatment-family health programs and referral centers with specialized teams in tb control programs (tcps)-was not a factor that expanded access to diagnosis. the tcps involving a smaller number of patients presented better performance regarding health care professional-patient relationship. the majority of the patients faced economic and social difficulties, and most managers were unaware of the amount of resources allocated to tb control activities. the instrument proved to be viable and to have the potential to adequately assess the performance of health care clinics in the urban areas studied.

Abstract: objective: to determine the spatial distribution of tuberculosis in the city of ribeir?o preto, located in the state of s?o paulo, brazil, between 1998 and 2002, with a focus on the potential spatially-dependent nature of its occurrence. methods: the secondary epi-tb database of the ribeir?o preto municipal secretary of health. georeferencing of tuberculosis cases was performed using mapinfo 6.5 software, and the spring program was used for statistical analysis of spatial data. results: through analysis of the existing spatial pattern and those of the previous years analyzed, we found a consistent pattern of spatial distribution of tuberculosis in ribeir?o preto. although there are areas that were homogeneous in terms of risk, the highest concentrations of cases were found in one zone, consisting of middle- to lower middle-class neighborhoods, located in the northeast part of the city. conclusion: these results add to the store of knowledge regarding the spatial distribution of tuberculosis in ribeir?o preto in various periods, emphasizing the importance of space as a methodological alternative to aid the planning, monitoring and evaluation of health care programs, allowing interventions to be more appropriately directed in order to decrease inequalities in the allocation of health care resources.

Abstract: the insertion of the nursing profession in epidemiologic surveillance (es) at ribeir?o preto from 1988 to 1996 has been identified in the process of health care decentralization. the qualitative methodology is used based in bibliographic and documentation sources and by 8 semi-structured interviews. nurses are in greater number than all other health profissionals and their practice are based on the epidemiology and management of control side, represented by notifications and visits, and the prevention side, represented by immunization.

Abstract: this work aims at a critical reflection on social control as a legal-institutional achievement and its legitimacy conditions, pointing out strategies for information democratization in the local health system. the text was developed according to three thematic units: the first discusses social participation as a legal-institutional achievement; the second analyzes the essential conditions for legitimacy and the third presents information as a fundamental element for management and social control as an unfinished process that is still under construction.

Abstract: based on data available in the information system for notifiable diseases, predictive factors of favorable results were identified in the treatment of pulmonary tuberculosis, diagnosed between 2001 and 2004 and living in recife-pe, brazil. uni- and multivariate logistic regression methods were used. in multivariate analysis, the following factors remained: age (years), 0 to 9 (or=4.27; p=0.001) and 10 to 19 (or=1.78; p=0.011), greater chance of cure than over 60; education (years), 8 to 11 (or=1.52; p=0.049), greater chance of cure than no education; type of entry, new cases (or=3.31; p<0.001) and relapse (or=3.32; p<0.001), greater chances of cure than restart after abandonment; time (months) 2, 5-|6 (or=9.15; p<0.001); 6-|9 (or=27.28; p<0.001) and more than 9 (or=24.78; p<0.001), greater chances of cure than less than 5; health unit district, ds i (or=1.60; p=0.018) and ds iv (or=2.87; p<0.001), greater chances of cure than ds vi.

Abstract: this work presents a general view of the health situation in peru in the last few decades by presenting a short historic development of health and pointing out the changes that have taken place as a consequence of the social, economic and political transformations and affected the population's health. the process of change in public and private health services, the influence of conceptions and sanitary policies of international organizations, the health system and the health situation are mentioned. two problems of great impact are presented: the cholera epidemics and political violence.

Abstract: health surveillance (hs) framework is important to become a model best follow up health situation in a community. this reflective study aims to discuss the possibilities of contribution of this framework to tuberculosis control in brazil. previous studies in this theme allow confirming that hs is essential to identify heterogeneous area of risk to tuberculosis and also allow to do better interventions according to health needs of social vulnerable groups. thus is possible to contribute to decrease social inequalities. if the health surveillance is used in the strategy of family health, it becomes a useful means to transform the health work process, to provide a broadened understanding of the object of attention and stimulate action on the determinants of tuberculosis.
