Abstract: Metastasis to the abdominal wall including port sites after laparoscopic surgery for colorectal carcinoma (CRC) is rare. Recently the rate was reported as 1.3% in a randomized clinical trial by the Colon Cancer Laparoscopic or Open Resection Study Group [1] and 2.4% in the CLASSIC trial [2]. Although the prognosis is not clearly defined in the literature, resection of metastatic lesions may lead to greater survival benefit if the abdominal wall metastasis is the only manifestation of recurrent disease. However, it can be difficult to diagnose a lesion in the abdominal wall as recurrence of disease on the basis of clinical characteristics alone. Approximately between 5% to 15% of CRCs are mucinous adenocarcinomas [3-7]. Patients with colorectal mucinous adenocarcinoma are reported to have a poorer prognosis compared to patients with non-mucinous adenocarcinoma because the greater frequency of lymph node involvement and peritoneal dissemination seen with mucinous adenocarcinoma [7-10]. Therefore, Patients with mucinous adenocarcinoma should be followed carefully after surgery, and receive rapid diagnosis and treatment if recurrence is suspected. We report a case in which 18F-fluorodeoxyglucose positron emission/computed tomography (18F-FDG PET/CT) was very useful for early diagnosis and planning a theraupetic strategy for a case of mucinous adenocarcinoma metastasis at a laparoscopic port site.A 57-year-old man received curative laparoscopic ileocecal resection and lymph node dissection for carcinoma of the cecum in May 2004. Morphologically, the tumor was type I (45 mm by 30 mm). The histological examination revealed a mucinous adenocarcinoma which invaded the cecal subserosa. Tumor cells were not identified histologically in the 20 regional lymph nodes, surgical margins, lymph vessels, or veins of the surgical specimens (pT3 N0 M0). Background: To explore the effects of cholestasis on whole blood concentration of tacrolimus (TAC), an immunosup-pressant, we investigated the relationship among blood TAC concentration, bile flow, and TAC metabolites in bile, as well as the relationship between total bilirubin (T-Bil), an index of cholestasis, and blood TAC concentration, in liver transplant recipients. Methods: Data were collected retrospectively from 16 male and 19 female patients (mean age: 38 years; range: 12 -59 years) who had undergone a living-related liver transplantation at Kyoto Prefectural University of Medicine from 2004 through 2008. Analysis of TAC, demethyl-TAC, and hydroxy-TAC in bile was performed by LC-MS/MS. Results: There was no correlation between the ratio of TAC metabolite to TAC in bile (M/P) of demethyl-TAC and post operation days (POD), whereas a weak linear correlation was demonstrated between M/P of hydroxy-TAC and POD (r = 0.345). Moreover, linear correlations were not observed between M/P and the TAC trough level normalized dose (TLTAC/dose), and between TLTAC/dose and POD. A negative linear correlation was demonstrated between bile flow and T-Bil in blood (r = 0.495). Abstract: We study the asymptotic behaviour of tame harmonic bundles. First of all, we prove a local freeness of the prolongation by an increasing order. Then we obtain the polarized mixed twistor structure. As one of the applications, we obtain the norm estimate of holomorphic or flat sections by weight filtrations of the monodromies. As other application, we establish the correspondence of semisimple regular holonomic $D$-modules and polarizable pure imaginary pure twistor $D$-modules through a tame pure imaginary harmonic bundles, which is a conjecture of Sabbah. Then the regular holonomic version of Kashiwara's conjecture follows from the results of Sabbah and us. A characterization of semisimple local system by tame pure imaginary pluri-harmonic metric

Abstract: Let $L$ be a local system on a smooth quasi projective variety over $\cnum$. We see that $L$ is semisimple if and only if there exists a tame pure imaginary pluri-harmonic metric on $L$. Although it is a rather minor refinement of a result of Jost and Zuo, it is significant for the study of harmonic bundles and pure twistor $D$-modules. As one of the application, we show that the semisimplicity of local systems are preserved by the pull back via a morphism of quasi projective varieties. This paper will be included as an appendix to our previous paper, ``Asymptotic behaviour of tame harmonic bundles and an application to pure twistor $D$-modules'', math.DG/0312230. Abstract: Let $X$ be a smooth projective complex variety with an ample line bundle $L$, and let $D$ be a simple normal crossing divisor. We establish the Kobayashi-Hitchin correspondence between tame harmonic bundles on $X-D$ and $\mu_L$-stable parabolic $\lambda$-flat bundles with trivial characteristic numbers on $(X,D)$. Especially, we obtain the quasiprojective version of the Corlette-Simpson correspondence between flat bundles and Higgs bundles.
The structure of the cohomology ring of the filt schemes

Abstract: Let $C$ be a smooth projective curve over the complex number field $\cnum$. We investigate the structure of the cohomology ring of the quot schemes $\Quot(r,n)$, i.e., the moduli scheme of the quotient sheaves of $\nbigo_C^{\oplus r}$ with length $n$. We obtain a filtration on $H^{\ast}(\Quot(r,n))$, whose associated graded ring has a quite simple structure. As a corollary, we obtain a small generator of the ring. We also obtain a precise combinatorial description of $H^{\ast}(\Quot(r,n))$ itself. For that purpose, we consider the complete filt schemes and we use a `splitting principle'. A complete filt scheme has an easy geometric description: a sequence of bundles of projective spaces. Thus the cohomology ring of the complete filt schemes are easily determined. In the cohomology ring of complete filt schemes, we can do some calculations for the cohomology ring of quot schemes. Abstract: We study a certain type of wild harmonic bundles in relation with a Toda equation. We explain how to obtain a classification of the real valued solutions of the Toda equation in terms of their parabolic weights, from the viewpoint of the Kobayashi-Hitchin correspondence. Then, we study the associated integrable variation of twistor structure. In particular, we give a criterion for the existence of an integral structure. It follows from two results. One is the explicit computation of the Stokes factors of a certain meromorphic flat bundle. The other is an explicit description of the associated meromorphic flat bundle. We use the opposite filtration of the limit mixed twistor structure with an induced torus action.
Asymptotic behaviour and the Nahm transform of doubly periodic instantons with square integrable curvature

Abstract: We study the asymptotic behaviour of doubly periodic instantons with square-integrable curvature. Then, we establish the equivalence given by the Nahm transform between the doubly periodic instantons with square integrable curvature and the wild harmonic bundles on the dual torus.
