Successful Treatment of Thyroid Carcinoma Invading the Trachea as a Single-Stage Procedure: A Case Report  [PDF]

Abstract:
Objective: Locally advanced thyroid carcinoma is relatively rare; extrathyroid extension, such as to the trachea, is one of the most important prognostic factors. Surgical resection is the mainstay of treatment, and appropriate surgical planning is crucial for obtaining a reasonable prognosis and quality of life of the patients. Case Report: Herein, we report a case of advanced thyroid carcinoma involving the trachea. Total thyroidectomy and window resection of the trachea were performed. The tracheal defect was approximately 2.5 × 3.0 cm in size, extending from the first to the third tracheal cartilage, and was reconstructed with auricular cartilage and covered with the sternohyoid muscles of both sides in a single-stage surgical procedure. The tracheal stoma was closed 2 weeks after the surgery. There were no postoperative complications. Conclusion: This reconstruction of the relatively small defect of the trachea provided excellent functional and cosmetic results.
The reversal of recurrence hazard rate between ER positive and negative breast cancer patients with axillary lymph node dissection (pathological stage I-III) 3 years after surgery

Abstract: This study investigated the assessment of variables in defining prognosis of 742 breast cancer women with pathological stage (pTNM) I-III diagnosed between 1980 and 2005 at the Kyoto University Hospital in Japan, by age, clinical stage (cTNM), pTNM, the numbers of positive lymph nodes (pN), and ER status.Multivariate analysis demonstrated that pTNM and ER status were the independent prognostic factors for overall survival, and that pTNM and pN were the independent prognostic factors for disease-free survival. For the 0- to 2-year interval, the hazard of recurrence was higher for the ER-negative patients than the ER-positive patients, and beyond 3 years the hazard was higher for ER-positive patients.The present study confirmed the previous reports which showed favorable prognosis of the patients with lesser pTNM or positive ER status. A reversal of recurrence hazard rate between ER positive and negative breast cancer patients beyond 3 years after operation was detected. The fact may indicate the importance of long term adjuvant hormone therapy for ER positive cancer patients.A prognostic factor is defined as a biological or clinical measurement that is associated with overall survival and/or disease-free survival [1]. The knowledge of prognosis forms an integral part of the decision-making process in medicine [2]. Moreover, prognostic factors are important in the treatment of cancer to help identify subgroups of patients who may need more aggressive approach to therapy [3]. Further, prognostic factors also play a critical role in designing clinical trial as stratification and allocation factors [4]. Impact of microRNA Expression in Human Atrial Tissue in Patients with Atrial Fibrillation Undergoing Cardiac Surgery

Abstract: Background Although microRNA (miRNA) regulates initiation and/or progression of atrial fibrillation (AF) in canine AF models, the underlying mechanism in humans remains unclear. We speculated that certain miRNAs in atrial tissue are related to AF, and evaluated the relationship of miRNA expression in human atrial tissue in cardiac surgery patients. Methods Right atrial tissues from 29 patients undergoing cardiovascular surgery were divided into 3 groups [A: chronic AF or unsuccessful maze, n=6; B: successful maze, n=10; C: sinus rhythm (SR) n=13]. miRNA expression was determined using high density microarrays and with Reverse transcriptase-polymerase chain reaction (RT-PCR). Fibrosis was examined using Masson trichrome staining. Results miRNA microarray analysis showed elevated miRNA-21, miRNA-23b, miRNA-199b, and miRNA-208b in AF as compared to SR groups. RT-PCR showed elevated miRNA-21 (1.9-fold) and miRNA-208b (4.2-fold) in AF as compared to the SR groups. miRNA-21 expression increased from Group C to A (A: 2.1-fold, B: 1.8-fold, C: 1.0-fold). Fibrosis increased from C to A (A: 43.0±12.9%, B: 21.3±6.1%, C: 11.9±3.1%). Percent fibrosis and miRNA-21 expression were correlated (r=0.508, p<0.05). The plasma levels of miRNA-21 in AF patients was significantly decreased as compared to the healthy volunteers (p<0.05). Conclusion The expression of miRNA-21 in human atrial tissue was found to be related to atrial fibrosis and might affect AF occurrence, indicating its usefulness as a biomarker for cardiac surgery management.
Suzaku Observation of the Anomalous X-ray Pulsar 1E 1841-045

Abstract: We report the results of a Suzaku observation of the anomalous X-ray pulsar (AXP) 1E 1841-045 at a center of the supernova remnant Kes 73. We confirmed that the energy-dependent spectral models obtained by the previous separate observations were also satisfied over a wide energy range from 0.4 to ~70 keV, simultaneously. Here, the models below ~10 keV were a combination of blackbody (BB) and power-law (PL) functions or of two BBs wit h different temperatures at 0.6 - 7.0 keV (Morii et al. 2003), and that above ~20 keV was a PL function (Kuiper Hermsen Mendez 2004). The combination BB + PL + PL was found to best represent the phase-averaged spectrum. Phase-resolved spectroscopy indicated the existence of two emission regions, one with a thermal and the other with a non-thermal nature. The combination BB + BB + PL was also found to represent the phase-averaged spectrum well. However, we found that this model is physically unacceptable due to an excessively large area of the emission region of the blackbody. Nonetheless, we found that the temperatures and radii of the two blackbody components showed moderate correlations in the phase-resolved spectra. The fact that the same correlations have been observed between the phase-averaged spectra of various magnetars (Nakagawa et al. 2009) suggests that a self-similar function can approximate the intrinsic energy spectra of magnetars below ~10 keV.
The Relationship between Athletic Abilities of Japanese High School Soccer Players and Their Psychological Skills in Both Athletic Club and School Life Settings  [PDF]

Abstract:

To examine the relationship between students’ athletic ability and their psychological skills in the athletic club and the school life setting, 99 players from high school soccer teams were administered with the Psychological Skills Inventory for Athletic Clubs and the Psychological Skills Inventory for School Life. Five coaches of the soccer teams evaluated the players’ athletic ability, using the Technique, Intelligence and Insight, Personality, and Speed (TIPS) evaluation framework. Structural equation modeling was conducted to test a series of structural models. Psychological skills in the school life setting have a direct effect on athletic ability. In addition, psychological skills in an athletic club setting have an indirect effect on athletic ability through psychological skills in the school life setting. In conclusion, the coach’s belief that players who have life skills and can control their daily life must have a high athletic ability was not wrong.

The Effect of Participation in a Game of Tag, an Activity Promoting Helping Behavior, on Children’s Helping Behavior-Related Self-Efficacy  [PDF]

Abstract: With a breakdown in classroom discipline, bullying and social withdrawal have become social problems in recent years; there is a heightened expectation toward physical education for developing proper social attitudes and cultivating morality. In this study, children were instructed to play “Nakama-oni (helping tag)”, a tag game variant promoting helping behavior through physical education. Thereafter, the frequency of helping behavior in Nakama-oni was checked, and the effect of participation in physical education through Nakama-oni on children’s helping behavior-related self-efficacy was discussed. Sixty elementary school children in the fifth grade played both Nakama-oni and normal tag. The results suggested that: 1) children experienced helping behavior more often in Nakama-oni, compared to normal tag; and 2) participation in Nakama-oni enhanced children’s helping behavior-related self-efficacy. In conclusion, results suggest that incorporating Nakama-oni that promotes helping behavior in physical education enhances children’s helping behavior-related self-efficacy.
Treatment for Depression with Chronic Neck Pain Completely Cured in 94.2% of Patients Following Neck Muscle Treatment  [PDF]

Abstract: We report three patients with cervical neuromuscular syndrome (CNMS) who followed similar courses. Autonomic imbalance may occur following neck muscle pain, and a wide variety of somatic symptoms including headache and vertigo appear and a generalized poor condition may continue for long periods. If many such somatic symptoms persist for months to years, symptoms of depression are exacerbated. The patients end up in psychiatric clinics, where they are diagnosed with depression, but they do not respond to antidepressants. Thus, they continue to suffer for many years. These patients eventually were completely cured with the resolution of neck pain by neck muscle treatment, using two types of special low-frequency therapy equipment, far-infrared radiation and acupuncture. When treatment for the neck muscles is initiated, symptoms of depression are quickly relieved, and diverse somatic symptoms disappear one after another as neck muscle tension is gradually alleviated (the number of abnormal neck muscle checkpoints decreases). Such a course suggests that neck muscle tension and chronic pain are closely related to depression. Neck muscle-related depression due to CNMS clearly differs from psychiatric conditions such as major and bipolar depression. In patients with neck muscle-related depression, symptoms of depression are not accompanied by ungrounded anxiety, a sense of emptiness, apathy, or self-rejection. Neck muscle abnormalities leading to CNMS are caused by head injury, whiplash injury, and a prolonged forward-bent-posture due to using a personal computer, playing computer games, texting, and engaging in machine-paced work such as assembly-line operation.
Suppression of Methane Gas Emissions and Analysis of the Electrode Microbial Community in a Sediment-Based Bio-Electrochemical System  [PDF]

Abstract:

The effects of bioelectrochemical systems (BESs) for the suppression of methane gas emissions from sediment were examined using a laboratory-scale reactor system. Methane gas emissions from acetate were suppressed by approximately 36% from control based on the installation of a BES in which carbon-graphite electrodes were buried in sediment and arbitrarily set at certain oxidative potentials (+300 mV vs Ag/AgCl) using a potentiostat. Meanwhile, methane gas emissions increased in the BES reactor where the electrode potential was set at -200 mV. Results obtained from pyrotag sequencing analysis of the microbial community on the surface of the buried electrodes targeting 16S rRNA genes demonstrated that the genusGeobacterhad drastically propagated in a sample from the reactor where the electrodes were buried. Quantitative analysis of 16S rRNA genes of archaea also revealed that the archaeal population had decreased to approximately 1/6 of its original level on the electrode of the BES set at +300 mV. This implied that the oxidation-reduction potential (ORP) in the sediment was raised to the inhibition level for methanogenesis in the vicinity of the buried electrode. Analysis of electron flux in the experiment revealed that electrons intrinsically used for methanogenesis were recovered via current generation in the sediment where a potential of +300 mV was set for the electrode, although most electrons donated from acetate were captured by oxygen respiration and other electron-accepting reactions. These results imply that BES technology is suitable for use as a tool for controlling re-dox-dependent reactions in natural environments, and that it also brought about changes in the microbial population structure and methanogenic activity in sediment.
