Laparoscopic Surgery for Meckel’s Diverticulum Presenting as Small Bowel Obstruction: A Case Report  [PDF]

Abstract: Meckel’s diverticulum is not commonly encountered surgical entity and presents unique challenges for a pediatric surgeon, as it is prone to varied complications. A 14-year-old boy was admitted with us with a 48-hour history of lower abdominal pain and multiple episodes of vomiting. Radiological imaging studies revealed a high-grade partial small bowel obstruction. A fleeting conservative management was tried. The diagnostic laparoscopy revealed a small bowel obstruction secondary to a Meckel’s diverticulum. The diverticulum was resected using an endovascular GIA stapler. The patient was discharged on postoperative day four, tolerating a regular diet. Laparoscopy is a useful diagnostic and therapeutic means for a patient with a small bowel obstruction due to an uncertain etiology.

Abstract: Background: Kashmir valley, being a known conflict zone keeps on receiving war victims due to scuffles between civilians and the government forces. Such upsurges have been noted in the recent past in 2008 and 2010; however, a new face of upheaval took place in the month of July 2016. Herein, we present an analysis of 500 abdominal trauma victims reported to our department at Government Medical College Srinagar, Kashmir, India. Methods: Government Medical College Srinagar Kashmir is a tertiary care teaching hospital situated in the heart of Kashmir valley. Being one of the referral hospitals, for trauma, we received a total figure of 6149 trauma victims out of which 500 were purely of abdominal in nature managed by the department of surgery. The study conducted was a short analysis of these abdominal trauma patients that reported from 8th July to 8th November 2016. Results: The result of analysis of 500 abdominal trauma patients is reported herein. The male female ratio of patients was 19.8:1 (male = 95.2%, female = 4.8%). The age range was from 5 years to 65 years with majority reported in the age range of 15 - 30 years. The number of bullet injuries was =85 and the pellet injuries were =349. The commonest organs involved were small intestines, spleen, large bowel and liver in descending order. Penetrating injuries were seen in =60.4%, non-penetrating were seen in =39.6%. The patients were grouped into conservative, non operative and operative group. The commonest morbidity observed was wound sepsis and a total mortality reached to a figure of 2%. Conclusion: This short study of analysis of trauma victims has explored the newer horizons of trauma management and we believe and conclude that specialized trauma hospitals with skilled manpower and modern gadgets of handling trauma is need of an hour especially in developing countries like ours.

Abstract: This study attempts to investigate the effect of trade openness, GDP and population on the investment in Syria over the period 1980-2010. The cointegration test indicates that investment is positively and significantly related to the trade openness, GDP and population. Population has the biggest effect on the investment. The Granger causality test indicates bidirectional short-run causality relationships between trade openness, GDP, population and investment. There are also unidirectional long-run causality relationship running from trade openness to investment, and bidirectional long-run causality relationships between GDP, population and investment. The study result indicates that it is vital for the Syrian government to still adopt the economic policy in liberalizing foreign trade in order to motivate the investment in the country.
Relating localized nanoparticle resonances to an associated antenna problem

Abstract: We conceptually unify the description of resonances existing at metallic nanoparticles and optical nanowire antennas. To this end the nanoantenna is treated as a Fabry-Perot resonator with arbitrary semi-nanoparticles forming the terminations. We show that the frequencies of the quasi-static dipolar resonances of these nanoparticles coincide with the frequency where the phase of the complex reflection coefficient of the fundamental propagating plasmon polariton mode at the wire termination amounts to $\pi$. The lowest order Fabry-Perot resonance of the optical wire antenna occurs therefore even for a negligible wire length. This approach can be used either to easily calculate resonance frequencies for arbitrarily shaped nanoparticles or for tuning the resonance of nanoantennas by varying their termination.

Abstract: Study aimed to determine the rate and prediction of infection in diabetes mellitus patients ≥18 year, with diabetic ketoacidosis (DKA). Retrospective cohort study design was adopted to achieve the objectives. Universal sampling technique was employed for data collection among Diabetes ketoacidosis patients, over a period of 3 years (Jan 2008-Dec 2010). Statistical package for social sciences used to analyze data. Over a 3-year period, total of 967 admissions were identified. Of it, 112 (11.6%) with no infection, 679 (70.2%) with bacterial infection and 176 (18.2%) with presumed viral infection. The mean WBC for all the patients was 18,177 (±9431). 721 (74.6%) had leukocytosis, as defined by a WBC ≥ 15,000/mm3. WBC, differential, leukocytosis, as well as sex, temperature were not significant predictors (p > 0.05) of bacterial infection. There was significant (p < 0.05) difference of age between the 3 groups, age above 57 years have high rate of infection as compared to age below and equal 57 years. The infection rate in elderly patients with DKA was high and majority of them had lack of clinical evidence. Major bacterial infections with potential serious sequel were particularly common (33.3%), among every third patient being presumed to have serious consequences.

Abstract: The present research is aimed to measure the porosity of anodes in solid oxide fuel cell through water Archimedeans method. There are various alternatives available to replace fossil fuel cells like nuclear power, wind energy, solar energy, bio fuel, and geothermal and fuel cells. Among all the alternatives of fossil fuel, one form of energy production that stands out from the rest and promises a sustainable future energy is fuel cell. Moreover, it offers many advantages in contrast to other forms of energy generation. An Archimedean approach for water immersion porosimetry is carried out. Some of the results are beyond rational limits, and given negative and sometime above 100 percent porosity. The reasons for these unacceptable results are either due to water ingress into the sample or the sample turns into buoyant due to air in the cling film. The results from Archimedean porosimetry should only be used qualitatively due to errors associated with the results. It is also noted that Archimedean porosimetry is not the ideal technique for measuring the porosity of coated samples. It is suggested that larger samples should be analyzed that will help to minimize the weighing errors.

Abstract: This study investigated the nature of relationships of demographic factors (age and job tenure) and job satisfaction facets with organizational commitment. The study also sought to determine the impact of demographic factors and job satisfaction facets on organizational commitment. A sample consists of 128 employees from service industry selected randomly. Employees were given a Job Descriptive Index (JDI) questionnaire and the Organizational Commitment questionnaire (OCQ). Pearson’s product moment correlation coefficient and multiple regression analyses were used to analyze the data. The Results of the study show that the mean values of job satisfaction and organizational commitment are at moderate side. A moderate significant positive relationship was found among job satisfaction facets, demographic factors, and organizational commitment. Supervision, pay, overall job satisfaction, age, and job tenure were the significant predictors of organizational commitment.

Abstract: The purpose of this paper is to study the effect of presence of fluid within and around a poroelastic circular cylindrical shell of infinite extent on axially symmetric vibrations. The frequency equation each for a pervious and an impervious surface is obtained employing Biot’s theory. Radial vibrations and axially symmetric shear vibrations are uncoupled when the wavenumber is vanished. The propagation of axially symmetric shear vibrations is independent of presence of fluid within and around the poroelastic cylindrical shell while the radial vibrations are affected by the presence of fluid. The frequencies of radial vibrations and axially symmetric shear vibrations are the cut-off frequencies for the coupled motion of axially symmetric vibrations. The non-dimensional phase velocity as a function of ratio of thickness to wavelength is computed and presented graphically for two different types of poroelastic materials for thin poroelastic shell, thick poroelastic shell and poroelastic solid cylinder.
The Missing Piece in Glaucoma?  [PDF]

Abstract: Glaucoma is defined as an optic disc neuropathy meaning the nerve fibers are being atrophied similar to the fate occurring in non-glaucomatous optic atrophies. Furthermore, the nerve fibers are always being destroyed randomly in all the non-glaucomatous optic atrophies. In contrast, the nerve fibers in glaucoma are invariably destroyed in an orderly tandem fashion, from peripheral to central, never randomly. Is glaucoma really an optic disc neuropathy in light of orderly destruction of nerve fibers in glaucoma? The current prevailing theories in glaucoma such as posterior bowing of the lamina cribrosa or cupping can’t explain the orderly destruction of nerve fibers occurring in glaucoma. In fact, there is no biological mechanism acting directly on the nerve fibers or their RGCs which could lead to their orderly destruction. Therefore, there should be some mechanical way, which could result in the orderly destruction of nerve fibers even though this mechanical scenario may have resulted from the direct biological effect of raised IOP on some important component of the optic disc. It is proposed that the border tissue of Elschnig (BT) atrophies due to chronic ischemia caused by raised IOP, and as a result, the lamina cribrosa (LC) begins sinking in the scleral canal—a mechanical problem. Due to sinking of the LC, the nerve fibers get stretched and broken starting with the most peripheral nerve fibers being closest to the edge of the scleral opening and ending with the most central nerve fibers in an orderly tandem fashion. Therefore, in view of the orderly destruction of nerve fibers, glaucoma may not be an optic disc neuropathy but an optic disc axotomy.
