Abstract: purpose: to assess the performance of hybrid capture ii (hcii) hpv viral load in predicting the grade of cervical lesions. methods: between august 2000 to november 2002, 309 women admitted due to an abnormal pap smear result were recruited. histological disease confirmation was done in all women and cervical intraepithelial neoplasia (cin) grade 2 or above was considered as severe disease. hcii was done for high-risk hpv types and viral load was estimated in relative light units (rlu). receiver operating characteristics analysis was used to test the performance of hcii. results: histological findings included 140 (45.3%) cervicitis or cin 1 and 199 (54.7%) cin 2/3, in situ adenocarcinoma or invasive cancer. the best cutoff for hcii in detecting severe disease was 35 rlu, showing a sensitivity of 69% and a specificity of 70%. association of high-grade cervical lesions at pap smear and hcii at 35 rlu showed a positive predictive value of 88.2% in diagnosis of cin 2 or above. on the other hand, 95.7% of the women with low grade lesion at cytology and hcii below 1 rlu presented no severe histological disease. conclusions: the best performance of hcii in diagnosing cin 2 or above was found at 35 rlu. association of cytology and hcii in different settings provided very high positive and negative predictive values.

Abstract: purpose: to determine the prevalence of hpv 16, 18, 31 and 45 in cervical screening samples of women with cellular changes and/or colposcopy suggestive of persistent high grade or low grade lesion who were submitted to conization. methods: a total of 120 women were included in the study. histological analysis of the cervical cones revealed 7 cases of cervicitis, 22 of cin1, 31 of cin2, 54 of cin3, and 6 invasive carcinomas. the cervical screening samples were analyzed before conization for the presence of hpv-dna by pcr using the consensus primers pgmy09/11. hpv-dna-positive samples were tested for the presence of hpv16, 18, 31 and 45 using type-specific primers for these hpv. results: hpv-dna was detected in 67.5% of the studied women. hpv 16 (40%) was the most prevalent type in most ilesions, followed by hpv 31 (13.3%), 45 (13.3%), and 18 (4.1%). multiple infections occurred in 15% of the cases and infections with other hpv types were detected in 14% of the sample. conclusions: hpv 16 and 18 infections do not always occur as a single infection, and may be associated with other hpv types on different occasions.

Abstract: OBJETIVO: avaliar o desempenho da carga viral do HPV por captura de híbridos II (CHII) na predi o da gravidade das les es cervicais. MéTODOS: foram incluídas 309 mulheres admitidas por resultado anormal da colpocitologia oncológica (CO) entre agosto de 200 e novembro de 2002. Todas foram submetidas a avalia o histológica, sendo que a presen a de neoplasia intra-epitelial cervical (NIC) grau 2 ou mais (NIC 3, carcinoma invasor) foi considerada doen a grave. A CHII foi realizada para tipos de HPV de alto risco oncogênico e a carga viral medida em unidades relativas de luz (URL). O desempenho da CHII foi avaliado por curva receiver operating characteristics (ROC). RESULTADOS: na avalia o histológica, 140 (45,3%) mulheres apresentavam cervicite ou NIC 1 e 199 (54,7%), NIC 2/3, adenocarcinoma in situ ou cancer invasor. O melhor ponto de corte da CHII para a detec o de doen a grave foi 35 URL, com sensibilidade de 69% e especificidade de 70%. O valor preditivo positivo das altera es compatíveis com les o de alto grau na CO associado a CHII de 35 URL (unidades relativas de luz) foi de 88,2% para a detec o de NIC 2 ou mais. Já 95,7% das mulheres com les es de baixo grau na CO e CHII menor que 1 URL n o apresentaram les es histológicas graves. CONCLUS O: o melhor desempenho da CHII no diagnóstico de NIC 2 ou les o mais grave foi encontrado com 35 URL. A associa o da CO com a CHII em diferentes cargas virais mostrou valores preditivos positivos e negativos muito altos.

Abstract: Avaliar as altera es na citologia oncológica, na captura híbrida II e na inspe o visual com ácido acético (IVA) a 5% em 684 mulheres atendidas em uma unidade de saúde, e avaliar o desempenho e a concordancia entre os exames. Procurar descrever as características sócio-demográficas e reprodutivas (SDR) destas mulheres e sua associa o com a presen a de doen a. As mulheres foram submetidas à coleta de citologia, captura e IVA, e as com exame positivo foram submetidas à colposcopia com biopsia, se necessário. Foi calculado o desempenho dos testes e a associa o dos fatores SDR com o diagnóstico histológico foi avaliada por intermédio do cálculo do odds ratio. Embora 198 mulheres apresentassem pelo menos um teste alterado, apenas 21 apresentaram les o histológica. A sensibilidade dos testes foi semelhante enquanto as especificidades da IVA e da citologia foram maiores que a da captura híbrida II. Apenas a ausência de citologia esteve associada à presen a de doen a. O desempenho da citologia foi maior que o da IVA, que foi maior que o da captura. A ausência de citologia foi associada com doen a histológica.

Abstract: the objective of this study was to evaluate alterations in pap smear, hybrid capture ii (hcii), and visual inspection with acetic acid (via) in 684 women treated at a primary health care unit. the performance and agreement of the exams were evaluated. the study also described social, demographic, and reproductive factors and their association with uterine cervical lesions. women had specimens taken for pap smear, hcii, and via. when at least one of the tests was positive, colposcopy was performed and targeted biopsies were taken from any suspicious lesions. performance of tests was evaluated. women's distribution in relation to social, demographic, and reproductive factors and histological diagnosis was evaluated using the odds ratio. among 198 women with at least one positive screening test, only 21 showed histological disease. sensitivities of the tests were similar. via and pap smear presented higher specificity than hcii. only absence of a previous pap smear was associated with the presence of histological disease. pap smear performed better than via and hc ii. absence of previous cytology was associated with histological disease.

Abstract: purpose: to evaluate the frequency of anxiety and depression in main caretakers of patients with terminal breast or gynecological cancer. methods: for this cross-sectional study, 133 informal caretakers of terminally-ill breast and gynecologic cancer patients were included. patients were hospitalized for palliative care in the oncology clinic of the "centro de aten??o integral à saúde da mulher (campinas, brasil) from august 2002 to may 2004. seventy-one of the patients had breast cancer and 62 had gynecological malignancies. hospital anxiety and depression scale (had) was applied to these informal caretakers, in order to detect anxiety and depression, and they were also interviewed to provide additional information regarding their age, gender, religion, relation to patient, current occupation, if they cared for other people, whether their routine had changed and whether other people helped them to care for the patient. logistic regression was used to calculate the odds ratio (or) and its confidence interval (ci), used to assess the relationship between the diagnoses of anxiety and depression among the informal caretakers. for multiple analyses, the stepwise criterion for variable selection was used. results: 43% percent of the patients identified their daughters as their main caretaker, and 24%, their husbands. most of the caretakers were over 35 years old (63%), 68% were female, 59% were unemployed, 47% cared for another person and 84% referred that his/her routine had changed because of caring. anxiety was detected in 99 caretakers (74.4%) and depression in 71 (53.4%). anxiety and depression were strongly correlated (odds ratio 5.6; 95% confidence interval 2.2 to 15.9). bivariate analysis disclosed that the patients' husbands were less affected by depression, but multivariate analysis revealed that only the fact of being male was related to a lower prevalence of anxiety. conclusion: caring for terminally-ill cancer patients led to high prevalence of anxiety and d

Abstract: objective: to assess the ability of pap smear and hybrid capture ii (hcii) to detect clinically significant cervical lesions (cin2/3) in women referred to hospital due to atypical squamous cells of unknown significance (ascus) or low-grade squamous intraepithelial lesions (lsil). methods: a cross-sectional study comprising 161 women referred to the taubaté university hospital due to ascus/lsil, between august 2000 and september 2002. all women responded to a questionnaire regarding sociodemographic and reproductive characteristics and were subjected to gynecological examination with specimen collection for pap test and hcii, along with colposcopy and eventual cervical biopsy. the relationship between hcii results and age, use of condom, oral hormonal contraception, and smoking were evaluated by the chi-square test. the sensitivity, specificity, positive and negative predictive values of both pap test and hcii were calculated. all calculations were performed within 95% confidence intervals. results: sixty-seven percent of the women that tested positive for hpv were less than 30 years old. pap smear and hcii showed the same 82% sensitivity in detecting cin2/3 when the threshold for a positive pap result was ascus, lsil or hsil. pap smear specificity and positive predictive values were substantially increased when only hsil results were considered as positive (from 29 to 95% and 12 to 50%, respectively). these figures were superior to those of hcii, but at the expense of an expressive loss of sensitivity (from 82% to 41%). conclusions: our results substantiate the potential of hcii in detecting cin2/3 among women referred due to ascus/lsil.

Abstract: purpose: to evaluate the detection of high oncogenic risk human papillomavirus dna (hpv-dna) immediately before and 4±1.25 months after large loop excision of the transformation zone (lletz) in the treatment of cervical intraepithelial neoplasia (cin). methods: in this clinical prospective study, 78 patients submitted to lletz from february to december 2001 were enrolled. all patients were submitted to colposcopic evaluation and had pap smear and hybrid capture ii (hc ii) specimens collected immediately before lletz and four months after the procedure. statistical analysis was made through odds ratio (or) calculations with 95% confidence interval (95% ci). results: before excision, 67 (86%) women had positive hc ii for high oncogenic risk hpv-dna, while four months afterwards only 22 (33%) maintained positive hc ii results. positive results of hpv-dna after treatment were not associated with previous viral load, compromised margins in the surgical specimen or age. four months after the procedure, detection of hpv-dna was significantly associated with cytological abnormalities (or = 4.8; ci 95%: 1.7-13.7) but not with histological residual disease or relapse (or = 6.0; ci 95%: 0.8-52.3). conclusion: hpv-dna detection was significantly reduced after treatment of cin, but was not associated with the presence of histologic residual disease or relapse.

Abstract: objective: to assess the neoplastic invasion of superficial and deep inguinal lymph nodes of women with invasive vulvar squamous carcinoma smaller than 5 centimeters with a clinically normal inguinal region. methods: the medical records of 59 women cared at the state university of campinas with invasive vulvar squamous carcinoma t1 and t2 and who presented clinically normal inguinal regions (n0) were reviewed. clinical characteristics of both tumor and patients were evaluated as well as the follow-up data. odds ratios and fisher's exact test were used to assess the correlations between the invasion of inguinal lymph nodes and tumor size, grade, relapses and clinical complications. confidence limits of 95% were used. results: age of the patients ranged from 34 to 91 years (mean 67 years), and follow-up time ranged from 3 days (peri-operatory death) to 252 months (mean 27 months). clinically, 22 (37%) women had lesions t1 lesions and 37 (63%) t2. histological analysis showed unilateral lymphatic invasion in six (10%) women and bilateral in three (5%). there was no significant association between tumor size and lymph node invasion. also, pathologic tumor size and grade were not associated with lymph node neoplastic involvement. relapses and late complications were not correlated with lymph node neoplastic invasion. conclusions: superficial and deep inguinal dissection disclosed clinically undetectable lymph node neoplastic invasion, although tumor size and histological grade, relapses and late complications were not associated with node involvement.
