Stroke Patients with a Past History of Cancer Are at Increased Risk of Recurrent Stroke and Cardiovascular Mortality

Abstract: Background and Purpose Cancer patients are at increased risk of cardiovascular and cerebrovascular events. It is unclear whether cancer confers any additional risk for recurrent stroke or cardiovascular mortality after stroke. Methods This was a single center, observational study of 1,105 consecutive Chinese ischemic stroke patients recruited from a large stroke rehabilitation unit based in Hong Kong. We sought to determine whether patients with cancer are at higher risk of recurrent stroke and cardiovascular mortality. Results Amongst 1,105 patients, 58 patients (5.2%) had cancer, of whom 74% were in remission. After a mean follow-up of 76±18 months, 241 patients developed a recurrent stroke: 22 in patients with cancer (38%, annual incidence 13.94%/year), substantially more than those without cancer (21%, 4.65%/year) (p<0.01). In a Cox regression model, cancer, age and atrial fibrillation were the 3 independent predictors of recurrent stroke with a hazard ratio (HR) of 2.42 (95% confidence interval (CI): 1.54–3.80), 1.01 (1.00–1.03) and 1.35 (1.01–1.82) respectively. Likewise, patients with cancer had a higher cardiovascular mortality compared with those without cancer (4.30%/year vs. 2.35%/year, p = 0.08). In Cox regression analysis, cancer (HR: 2.08, 95% CI: 1.08–4.02), age (HR: 1.04, 95% CI 1.02–1.06), heart failure (HR: 3.06, 95% CI 1.72–5.47) and significant carotid atherosclerosis (HR: 1.55, 95% CI 1.02–2.36) were independent predictors for cardiovascular mortality. Conclusions Stroke patients with a past history of cancer are at increased risk of recurrent stroke and cardiovascular mortality.

Tuberculosis of the breast with erythema nodosum: a case report

Abstract: We report the case of a 33-year-old Chinese woman with tuberculosis of the left breast accompanied by erythema nodosum on the anterior aspect of both lower legs. Due to her poor clinical response to conventional therapy, and the histopathological findings of fine needle aspiration cytology, there were strong indications of tuberculosis. Her clinical diagnosis was confirmed by molecular detection of Mycobacterium tuberculosis complex by polymerase chain reaction. The diagnosis was further confirmed by a second polymerase chain reaction test of erythema nodosum which tested positive for Mycobacterium tuberculosis complex. She received anti-tuberculous therapy for 18 months, and finally underwent residual lumpectomy. During her follow-up examination after 12 months, no evidence of either residual or recurrent disease was present.Histopathological features and a high index of clinical suspicion are necessary to confirm a diagnosis of tuberculosis of the breast. Anti-tuberculous therapy with or without simple surgical intervention is the core treatment.Tuberculosis (TB) is one of the leading infectious diseases worldwide. Extrapulmonary TB involving the breast is extremely rare. Clinical examination usually fails to differentiate breast TB from breast carcinoma. Vulnerability to breast TB is increased in women who are young, married, multiparous and who breast-feed [1]. Histopathological examination using fine needle aspiration cytology (FNAC) may reveal caseating epithelioid cell granulomas and acid-fast bacilli (AFB). Although the presence of an acid-fast stain or culture is essential to confirm diagnosis, it does not give a positive result in most patients [2,3]. Molecular detection of Mycobacterium tuberculosis by polymerase chain reaction can be particularly useful in the validation of a diagnosis of tuberculosis in clinical settings where the diagnosis is uncertain [3,4]. istar: A Web Platform for Large-Scale Protein-Ligand Docking

Abstract: Protein-ligand docking is a key computational method in the design of starting points for the drug discovery process. We are motivated by the desire to automate large-scale docking using our popular docking engine idock and thus have developed a publicly-accessible web platform called istar. Without tedious software installation, users can submit jobs using our website. Our istar website supports 1) filtering ligands by desired molecular properties and previewing the number of ligands to dock, 2) monitoring job progress in real time, and 3) visualizing ligand conformations and outputting free energy and ligand efficiency predicted by idock, binding affinity predicted by RF-Score, putative hydrogen bonds, and supplier information for easy purchase, three useful features commonly lacked on other online docking platforms like DOCK Blaster or iScreen. We have collected 17,224,424 ligands from the All Clean subset of the ZINC database, and revamped our docking engine idock to version 2.0, further improving docking speed and accuracy, and integrating RF-Score as an alternative rescoring function. To compare idock 2.0 with the state-of-the-art AutoDock Vina 1.1.2, we have carried out a rescoring benchmark and a redocking benchmark on the 2,897 and 343 protein-ligand complexes of PDBbind v2012 refined set and CSAR NRC HiQ Set 24Sept2010 respectively, and an execution time benchmark on 12 diverse proteins and 3,000 ligands of different molecular weight. Results show that, under various scenarios, idock achieves comparable success rates while outperforming AutoDock Vina in terms of docking speed by at least 8.69 times and at most 37.51 times. When evaluated on the PDBbind v2012 core set, our istar platform combining with RF-Score manages to reproduce Pearson's correlation coefficient and Spearman's correlation coefficient of as high as 0.855 and 0.859 respectively between the experimental binding affinity and the predicted binding affinity of the docked conformation. istar is freely available at http://istar.cse.cuhk.edu.hk/idock.
Determination of Acceptor Concentration, Depletion Width, Donor Level Movement and Sensitivity Factor of ZnO on Diamond Heterojunction under UV Illumination

Abstract: The concentration of acceptor carriers, depletion width, magnitude of donor level movement as well as the sensitivity factor are determined from the UV response of a heterojunction consisting of ZnO on type IIb diamond. From the comparison of the I-V measurements in dark condition and under UV illumination we show that the acceptor concentration (~1017 cm?3) can be estimated from p-n junction properties. The depletion width of the heterojunction is calculated and is shown to extend farther into the ZnO region in dark condition. Under UV illumination, the depletion width shrinks but penetrates both materials equally. The ultraviolet illumination causes the donor level to move closer to the conduction band by about 50 meV suggesting that band bending is reduced to allow more electrons to flow from the intrinsically n-type ZnO. The sensitivity factor of the device calculated from the change of threshold voltages, the ratio of dark and photocurrents and identity factor is consistent with experimental data.
The Developmental Impact of Reverse-Migration Separation on Low-Income Chinese-American Children—Provider Perspectives  [PDF]

Abstract: Family service professionals, child development providers, and school teachers who work with American-born Chinese children of reverse-migration face great challenges especially when the specific needs of these children and their immigrant families are unknown to them. This study explored the experiences and perspectives of service providers on the possible developmental impacts of reverse-migration separation on returning children of Chinese immigrant families in New York City. Focus groups and key informant interviews were conducted with 20 healthcare providers, school teachers, social workers, and child and family service practitioners. Thematic analysis approach was used to analyze and encode qualitative information and to discover patterns and themes. The analytical process consisted of data immersion, taking notes, sorting data into codes, and comparing the themes across interviews. Findings revealed specific themes that included: 1) provider’s observations of child’s attachment, health, learning, and behavioral issues; 2) provider’s views concerning parenting methods, skills, and challenges; and 3) suggestions for support and resources for Chinese immigrant families. Implications for social policy, intervention services, and future research on this needy immigrant population were discussed. Knowledge derived from this study is instrumental in raising awareness, broadening knowledge base, and increasing effectiveness among service professionals who serve this population.

Abstract: Casino capitalism has its dialectical tendencies in Macau. On the one hand, it stimulates economic growth, provides employment, and strengthens the post-colonial state in Macau during the period of economic boom. On the other hand, casino capitalism can widen the income gap between the rich and the poor, generate addictive gambling, and de-legitimize the post-colonial state in Macau during the global and regional economic downturn. The weaknesses of the politico-administrative state in Macau, including the absence of institutional checks and balances, the frail civil society and the relatively docile mass media, have magnified the negative impacts of casino capitalism on Macau. In response to the negative ramifications, the Macau government has taken measures to be more interventionist, to enhance social welfare, and to prepare contingency plans that would tackle the sudden bankruptcy of any casinos. The central government in Beijing also displays contradictory considerations when it deals with Macau’s casino development, supporting the casino industry while simultaneously encouraging the Macau government to diversify its economy. Overall, casino capitalism not only has contradictory impacts on the Macau city-state but also reveals the inherent contradictions of Beijing’s policy toward the territory’s over-dependence on the casino economy. Der Kasino-Kapitalismus ruft in Macao widersprüchliche TEndenzen hervor: Einerseits stimuliert er das wirtschaftliche Wachstum und st rkt den post-kolonialen Staat in wirtschaftlichen Boomphasen, andererseits kann er aber auch dazu beitragen, dass sich die Einkommensunterschiede zwischen rmeren und wohlhabenderen Schichten verst rken und damit in Krisenphasen zur Deligitimierung des postkolonialen Staates beitragen.

Dessa forma, o Brasil conquistou parte do seu antigo desejo de grandeza. The close military relations between the Brazil and the United States underwent strains in the early post-war era. Cold War divisions created national and international pressures that led to a less precise and often reactive relationship between the hemisphere giants. When there was a convergence of goals, military relations reflected the previous smooth cooperation. When political and institutional divisiveness emerged in the officer corps over bilateral issues, Brazil refused to follow the U.S. lead. To a large degree, the post-war foreign policy forced Brazil and its military establishment to develop the confidence and ability to end the traditional practice of acting as a surrogate for a more powerful patron. The process began with U.S. assistance in creating the Escola Superior de Guerra and culminated in 1977 with the end of formal military relations with the United States. In so doing, Brazil achieved part of the long-held desire for grandeza.
The Chinese ba as a Verb: A Constructional-Cognitive Approach  [PDF]

Abstract: Although there have been various studies on the Chinese ba-construction, this article presents some unconventional views. Most studies do not treat ba as a verb. This article argues that ba is a verb, although it has some non-typical verbal properties. Firstly, the argument establishes that it is a verb based upon the “gradient” analysis, which has not been applied to ba before. Secondly, having been treated as a verb, ba shows special properties of its own such as marking subordination. Apart from this non-traditional view, ba is shown to be highly transitive, which is also a verbal feature. Different from previous analyses, this article argues that ba is a radial category showing various associated meanings. Finally, ba is shown to produce blending with other verb complements, such as the resultative construction, through its ability to create mental spaces.
