Introduction: Foreign body (FB) aspiration is a common cause of respiratory emergency in early childhood. Parents and care-givers are usually unaware of initial time of FB aspiration, and the varied symptoms may be confusing to them. Therefore, education on ways of avoiding the inci-dence is paramount, and prompt hospital presentation will reduce morbidity and mortality. Case Report: A 5 cm concrete nail, which was aspirated by a 2-year-old male Nigerian child of the Hausa ethnic group, was successfully removed by rigid bronchoscopy. We emphasize the need for more parental/care-givers’ education on foreign body aspiration among children. Conclusion: Infants commonly play with objects by inserting them in various orifices of their body. This, therefore, predisposes them to aspiration, which will go unnoticed at the time of aspiration. There is a need for repeated parental/care-givers’ education on dangers of allowing children to play alone. Parents/care-givers should be conversant with varied presentations of FB aspiration in children, as this will reduce the morbidity and mortality to the minimum.
Necrotizing Fasciitis of the Upper and Lower Limbs: Port Harcourt Experience  [PDF]

Abstract: Background: Necrotizing fasciitis (NF) is a rare but life-threatening soft-tissue infection characterized by rapidly spreading inflammation and subsequent necrosis of the fascial planes and surrounding tissue. Aim: To determine the clinical characteristics at presentation, causative pathogens and clinical outcome of NF after aggressive management. Patients and methods: We retrospectively reviewed case notes of patients with NF referred to the Teaching Hospital in Port Harcourt from January 2004 to December 2009. Results: The case notes of over 2,280 patients with history of cellulitis and/or infections of the upper and lower limbs were reviewed. These cases were seen in a five-year period from 2004 to 2009. Seventy five (3.29%) patients had a diagnosis of necrotizing fasciitis and were evaluated. Only those with infections on the lower and upper limbs were further analyzed. Thirty five (46.67%) patients had their infections in the lower limb while 40 (53.33%) patients had their infections in the upper limb. Twenty (26.67%) patients were found to be diabetic on admission. Thirty (30%) patients sustained injuries on their limbs while 5 (6.67%) patients could not give account of their infections. Eleven (14.66%) patients had fixed flexion deformities as their wounds healed and had to have further plastic surgery to extend their limbs and 7 (9.33%) patients died while on admission. Conclusion: Although an early diagnosis of NF can be difficult, a high index of suspicion is required in all patients presenting with unexplained warmth and/or cellulitis of the limbs, so that prompt and aggressive debridement can be carried out with commencement of broad spectrum antibiotics.

This article is a review and promotion of the study of solutions of differential equations in the neighborhood of infinity via a non traditional compactification. We define and compute critical points at infinity of polynomial autonomuos differential systems and develop an explicit formula for the leading asymptotic term of diverging solutions to critical points at infinity. Applications to problems of completeness and incompleteness (the existence and nonexistence respectively of global solutions) of dynamical systems are provided. In particular a quadratic competing species model and the Lorentz equations are being used as arenas where our technique is applied. The study is also relevant to the Painlevé property and to questions of integrability of dynamical systems.

Two significant findings compel a rethink of physical theories. First, using a 7-billion-year-old gamma-ray burst, Nemiroff (2012) showed that quantum foam could not exists. And second, Solomon (2011) showed that gravitational acceleration is not associated with the gravitating mass, that gravitational acceleration g is determined solely by τ the change in time dilation over a specific height multiplied by c2 or g = τc2. Seeking consistency with Special Theory of Relativity, as means to initiate this rethink, this paper examines 12 inconsistencies in physical theories that manifest from empirical data. Skin grafts have remained relatively unchanged since their introduction as a medical treatment for burns/wounds. This paper seeks to open an academic discussion as to whether their use-by date has now been passed. A skin graft substitute is described in a paradigm using fine leaf gelatine sheets which inherently possess several distinct advantages including, discarding the harvest of autologous tissue from patient donor sites. A clinical study will be needed to determine its suitability taken together with the understanding that experimental animal studies may not provide unequivocal answers to its in situ modus operandi.

This paper sets out to demonstrate that scraping of the flat dorsal surface of human dermis with a scalpel blade and cell plating without centrifugation can lead to the recognition and identification of the individual packing micro pattern of dermal reticular fibroblasts at confluence. The characteristic alignment of papillary and reticular fibroblasts at right angles to each other led to the positive identification of reticular fibroblasts. A non-enzymatic means of sub-culturing (passaging), which yields fully functional, healthy cells with normal, phenotypic morphology is also described. Implications for published subcutaneous wound healing studies are discussed as well as the confluent reticular fibroblast configuration, interpreted as ananatomic site identity code,which may be the address of a specific fibroblast gene pattern expression.

The human dermis presents an ongoing problem for regenerative medicine. Current medical management uses various acellular dermal matrices on wound sites. The challenge for scientists is to examine, then to question accepted conventional wisdom and to present new concepts. In this paper, Autologous Cell Therapy will be described by using cell culture of autologous dermal fibroblasts and their extracellular matrix as a foundation for rebuilding the dermis in conditioned wound beds. This proposal seems to create a conflict with the medical approach to keeping a wound bed moist.

We study the stability of accretion disc around magnetised stars. Starting from the equations of magnetohydrodynamics we derive equations for linearized perturbation of geometrically thin, optically thick axisymmetric accretion disc with an internal dynamo around magnetized stars. The structure and evolution of such discs are governed by an evolution equation for matter surface density ∑(R,T). Using the time-dependent equations for an accretion disc we do a linear stability analysis of our steady disc solutions in the presence of the magnetic field generated due to an internal dynamo.

Abstract: We have analyzed photon statistics and quadrature squeezing of the signal mode produced by one-mode subharmonic generator. It is found that the mean photon energy of the signal mode is twice of a twin signal light beam. And the energy fluctuations of the two signal light beams are four times that of one of the signal light beams. We have also shown that the photon energy quadrature squeezing of the output signal mode is exactly equal to that of the cavity signal mode. In light of this, the global quadrature squeezing of the signal mode is independent of the mean photon energy. On the basis of the above results, the quantum analysis of a one-mode sub-harmonic generating system should somehow be modified.
Soft Sediment Deformation Structures in the Maastrichtian Patti Formation, Southern Bida Basin Nigeria: Implications for the Assessment of Endogenic Triggers in the Maastrichtian Sedimentary Record  [PDF]

Abstract: Detailed fieldwork carried out in the southern part of Bida Basin, Nigeria, allowed the documentation of soft sediment deformation structures (SSDS) in the Maastrichtian Patti Formation. The aim of this study is to examine the sedimentary successions, describe and analyse these deformation features, discuss their deformation mechanisms and potential triggers. The Maastrichtian Patti Formation is composed of lithofacies interpreted to have been deposited in tidal and fluvial sedimentary environments. Soft sediment deformation structures recognised in the tidal sediments were clastic dykes, load cast, isolated sand balls, dish-and-pillar structures, convolute lamination, diapiric structures and recumbent folds. Severely deformed cross beds, ring structures, associated sand balls, normal folds and recumbent folds were identified in the fluvial sediments. SSDS recognised were interpreted to have been caused by effects of liquefaction and fluidization. Field observations, facies analysis and morphology of the SSDS indicate that there are relationship between the depositional environments and SSDS. Endogenic processes are considered as the trigger agents and they are represented by rapid sedimentation and overloading, impact of breaking waves, pressure fluctuations caused by turbulent water flow, cyclic stress and current generated by storm waves and changes in water table. The present study did not identify exogenic processes as trigger agent. The occurrence of SSDS in southern Bida Basin strongly favoured a non-tectonic origin but a clear relationship high energy processes in tidal and fluvial depositional environments.
