Hipovitaminose A em pré-escolares de creches públicas do Recife: indicadores bioquímico e dietético

Abstract: objective: to evaluate the extent of vitamin a deficiency among preschool children attending public day care centres in recife, state of pernambuco, brazil. methods: a cross-sectional survey involving 311 under 5-year-old children of both sexes, randomly selected, and assessed by biochemical (serum retinol), dietetic (vitamin a rich-food consumption) and anthropometric indicators (weight-for-height, weight-for-age and height-for-age). results: the prevalence of hyporetinolemia (serum retinol <0.70μmol/l) was 7.0%, which characterizes the vitamin a deficiency as a mild public health problem, according to world health organization criteria. approximately 78.0% of the children met the recommendation for vitamin a rich-food consumption proposed by the dietary reference intakes, 2001. the serum retinol levels and vitamin a rich-food intake were homogeneously distributed between both genders. however, children between 12 and 48 months old were liable to have lower vitamin a rich-food intakes as compared to the other age groups (p<0.05). the prevalence of underweight was 7.5%, stunting 8.1% and acute malnutrition 1.8%. vitamin a deficiency showed no correlation with protein-energy malnutrition (p>0.05). vitamin a rich-food intake showed low sensitivity (43.0%) and a very low positive predictive value (6.8%) as a screening diagnostic test for vitamin a deficiency. conclusion: the identification of vulnerable population groups, and the selection of reliable indicators for the assessment of vitamin a status, are both essential for the diagnosis and planning aimed at preventing and controlling vitamin a deficiency.

Abstract: objectives: to evaluate the nutritional status of zinc in children from two to five years old, institutio-nalised in daycare centers in the municipality of jo?o pessoa, paraíba. methods: a systematic random sample comprised of 377 children. the nutritional status of zinc was evaluated according to the following parameters: mineral concentrations in the hair and enzymatic activity levels of serum alkaline phosphatase. results: inadequate zinc concentrations prevalence in the hair was of 61,9% with predominance in the form of severe deficiency, 38,5%. prevalence of inadequate concentrations of levels of serum alkaline phosphatase activity was 40,1%. zinc concentrations in the hair did not correlatated with serum alkaline phosphatase (r = 0,01; p = 0,882). according to sex and age zinc distribution in the hair was homogenous. the alkaline phosphaase had identical behavior. conclusions: zinc deficiency is extremely high in the municipality of jo?o pessoa.

Abstract: OBJETIVO: Avaliar a magnitude, a distribui o espacial e a tendência temporal da anemia em pré-escolares no Estado da Paraíba, Brasil. MéTODOS: Corte transversal com amostra aleatória, do tipo multietapas, em oito cidades da zona urbana, de três mesorregi es do Estado da Paraíba, no ano de 1992, pela qual foram selecionados aleatoriamente 1.287 pré-escolares de ambos os sexos. A hemoglobina foi dosada pelo método da cianometa-hemoglobina em sangue venoso, empregando <11,0 g/dl como ponto de corte para anemia. A análise estatística de propor es incluiu o teste do qui-quadrado e a de médias, os de Mann-Whitney e Kruskal-Wallis, com intervalo de confian a de 95%. RESULTADOS: A prevalência da anemia no Estado da Paraíba foi de 36,4% (IC 33,7-39,1), maior (p= 0,00) do que a de 1982, que foi de 19,3% (IC 17,3-21,5). Apenas 1,0% (IC 0,6-1,8) e 6,8% (IC 5,5-8,3) dos casos de anemia foram categorizados nas formas grave e moderada, respectivamente. Crian as do sexo masculino apresentaram concentra es médias de hemoglobina mais baixas (p=0,00), e crian as menores de três anos constituíram o grupo biológico de maior suscetibilidade ao desenvolvimento do quadro carencial (p=0,00). O segundo ano de vida mostrou-se como o período vital mais crítico à exacerba o da deficiência nutricional (p=0,00). A mesorregi o do Agreste configurou-se como o espa o geográfico de maior risco (p=0,00), desenhando uma outra dinamica epidemiológica do problema, comparada àquela de 1982, em que a mesorregi o do Sert o representava a área geográfica de maior risco para a deficiência. CONCLUS ES: Os resultados mostraram que a anemia é um problema de saúde pública do tipo moderado, segundo os critérios internacionais para caracterizar a endemia em escala epidemiológica. Admitindo-se a comparabilidade entre os dois cortes transversais em análise, concluiu-se pelo caráter evolutivo ascendente na prevalência da anemia nutricional (+88,5%) em todas as três mesorregi es, no período de 10 anos, entre 1982-1992.

Abstract: objective: to assess the magnitude, geographical distribution and trends of the prevalence of nutritional anemia among preschoolers and to identify risk groups in the state of paraiba, brazil. methods: a cross-sectional survey with multistage random sampling procedure was conducted in 8 urban municipalities of three mid-regions (zona da mata, agreste and sert?o) in the state of paraíba in 1992. a total of 1,287 preschoolers of both sexes were selected. hemoglobin was determined by the cyanometahemoglobin method in venous blood, employing <11 g/dl as the cut-off for anemia. statistical analysis of proportions employed the chi square test, whereas for means mann-whitney and kruskal-wallis were the choice, all with confidence interval of 95%.. results: the prevalence of anemia was 36.4% (ci 33.7?39.1) in the state of paraiba, greater (p=0.00) than that observed (19.3%, ci 17.3?21.5) in 1982. only 1.0% (ci 0.61?1.8) and 6.8% (ci 5.5?8.3) of anemia cases were classified as severe and moderate, respectively. boys presented lower mean hemoglobin concentration (p=0.00), and children under age 3 comprised the biological group of highest susceptibility for deficiency status (p= 0.00). it was found that the second year of life is the most critical for developing nutritional deficiency (p= 0.00). the agreste mid-region revealed to be the geographical area of highest risk (p= 0.00), outlining a new epidemiological dynamics when compared to the year 1982, when the drought-ridden sert?o region were the geographical area at greatest risk of deficiency. conclusions: according to international epidemiological criteria, anemia in the studied regions represents a public health problem of moderate degree assuming the analytical comparability of the 1982 and 1992 cross-sectional surveys, it suggests an increasingly prevalence of nutritional anemia (+88.5%) in all 3 mid-regions in a 10-year-period (1982-92).

Abstract: this text indicates essential elements needed for an analysis of direct democracy, materialized in processes of decentralization and civil society participation in spaces for deliberating public policies, in a context of reform of these policies in the 1990's. it analyzes the national policy for the elderly and the experiences of elderly rights councils. it concludes that despite the fact that spaces for participation are contradictory and the orientations of participation of the various subjects are in conflict, the correlation of forces favorable to conservative forces, redirect social policies, giving them a new rationality and new legitimacy that distribute responsibilities to civil society. these changes reinforce a culture of privation in light of the clashes of the refractions of the social issue and transmute popular participation into consent and legitimization of the given order.

Abstract: The present paper aims at portraying and analyzing the role of the family in the history of the Brazilian social protection system, both in the light of its rights as a group and/or of its active role in the social protection of its members. It also reveals the contradictions of the recent prominence of the family on the public sphere and its inclusion in the governmental actions, as it assumes a central position in the social policies, and especially in the Social Welfare agenda.

Abstract: The dislike food preferences that children registered regarding vegetables and fish compromise the exposure to those iodine sources and may contribute to cause a nutritional deficiency in this micronutrient, causing serious damage of the brain and the central nervous system. Functional foods are becoming well-established in the market, focused on by an adult consumer more interested in healthy eating habits but without the motivation to optimize natural food preparation or to promote a wide variety of food in their diet. This article presents evidence of a new functional product concept, 100% vegetable, fermented soya based and salted by a red algae, that was approved by 900 parents who chose the sensorial features: distinctive aroma (57.9%), soft texture (38.4%), sausage shape (21%) and attractive color (77.9%). Three different varieties were developed: carrot (50% w/w), sugar-beet (50% w/w) and tomato (40% w/w) mini sausages, with tofu (20% w/w) and 0.8 % (w/w) of seaweed, reflecting trends that the younger consumer demands. The sensorial tests performed, after quality control assays, received very satisfactory acceptance by the potential buyer.

Abstract: objective: to assess nutritional iron status and anemia prevalence in children less than 5 years old at public daycare centers in the city of recife, pe, brazil. methods: a cross-sectional study, with a systematic random sampling of 162 children aged 6 to 59 months. nutritional iron status was assessed in terms of body iron reserves (serum ferritin), transferrinemia (serum iron, total iron binding capacity, and transferrin saturation %), erythropoiesis (free erythrocyte protoporphyrin) and hemoglobin production (hemoglobin). results: the prevalence of anemia (hemoglobin < 11.0 g/dl) was 55.6% (95%ci 47.3-63.5), evidence was found of depleted iron stocks (serum ferritin < 12.0 ng/ml) in 30.8% (95%ci 22.9-39.3), low transferrinemia levels (transferrin saturation % < 16) in 60.1% (95%ci 51.7-68.0) and deficient erythropoiesis (free erythrocyte protoporphyrin > 40 μmol/mol heme) in 69.6% (95%ci 61.0-77.1) of the children. iron parameters were not correlated with sex (p > 0.05). however, children < 24 months exhibited lower hemoglobin concentrations (p < 0.00) and higher levels of free erythrocyte protoporphyrin (p < 0.000) and total iron binding capacity (p < 0.001) when compared with children > 24 months. the significant correlation observed between reserves, transferrinemia and erythropoiesis is a finding that is compatible with the expected lifecycle of iron in the body. conclusions: iron deficiency and anemia appear to be an important public health problem among children less than 5 years old at public daycare centers in recife. therefore, effective actions aimed at the prevention and control of this deficiency are strongly recommended in this ecological context.
