Nódulos de tireóide: valor da ultra-sonografia e da biópsia por pun o aspirativa no diagnóstico de cancer

Abstract: Os nódulos de tireóide s o um problema clínico comum e a pun o aspirativa por agulha fina (PAAF) é o principal exame utilizado para seu diagnóstico diferencial. Recentemente, foi demonstrado que algumas características ultra-sonográficas dos nódulos est o associadas a um maior risco de cancer. Entretanto, n o encontramos estudos que avaliassem o desempenho dos dois exames de forma comparativa e/ou complementar. OBJETIVO: Avaliar o desempenho da ultra-sonografia (US), da PAAF e de sua combina o no diagnóstico de cancer em nódulos de tireóide. MéTODOS: Oitenta pacientes com nódulos únicos ou dominantes, com indica o cirúrgica, foram selecionados dentre 289 pacientes com nódulos tireoidianos atendidos entre mar o de 1995 e julho de 1997. Os pacientes foram submetidos à PAAF e à US de tireóide por um dos autores. A PAAF foi considerada positiva quando os achados citológicos eram suspeitos ou diagnósticos de cancer. A US foi considerada positiva quando os nódulos apresentavam pelo menos um dos seguintes achados: microcalcifica es, hipoecogenicidade ou halo ausente. PAAF e US foram combinados em paralelo (o resultado positivo em qualquer um dos dois exames era considerado indicativo de malignidade) e em série (somente o resultado positivo de ambos os testes indicava presen a de malignidade). RESULTADOS: A sensibilidade e a especificidade da PAAF na detec o do cancer em nódulos de tireóide foram, respectivamente, 87% e 62%, e a sensibilidade e a especificidade da US foram, respectivamente, 81% e 70%. A combina o em série dos dois exames (PAAF e US) resultou em sensibilidade de 69% e especificidade de 91%, e a combina o em paralelo (PAAF ou US) apresentou sensibilidade de 94% e especificidade de 52%. CONCLUS O: A US pode ser útil na avalia o dos nódulos de tireóide, alternativamente ou em combina es com a PAAF. Como o resultado da US depende da experiência do operador e das características do equipamento, s o necessários estudos em diferentes centros para o seu uso na prática clínica.
Spatial Correlations in the Helium-Ionizing Background

Abstract: After quasars ionize intergalactic HeII at z~3, a large radiation field builds up above the HeII ionization edge. Unlike the background responsible for HI ionizations, this field should be highly variable, thanks to the scarcity of bright quasars and the relatively short attenuation lengths (~50 Mpc) of these high-energy photons. Recent observations of the HeII and HI Lyman-alpha forests show that this background does indeed vary strongly, with substantial fluctuations on scales as small as ~2 Mpc. Here we show that such spatial fluctuation scales are naturally expected in any model in which the sources are as rare as bright quasars, so long as the attenuation length is relatively small. The correlation length itself is comparable to the attenuation length (~10 Mpc) for the most plausible physical scenarios, but we find order-of-magnitude fluctuations on all scales smaller than ~6 Mpc. Moreover, aliasing along the one-dimensional skewers probed by the HeII and HI Lyman-alpha forests exaggerates these variations, so that order-of-magnitude fluctuations should be observed on all scales smaller than ~20 Mpc. Complex radiative transfer is therefore not required to explain the observed fluctuations, at least at the level of current data.
Clinical and biochemical findings in 7 patients with X-linked adrenoleukodystrophy treated with Lorenzo's Oil

Abstract: x-linked adrenoleukodystrophy (x-ald) is a hereditary disorder of the peroxisomal metabolism biochemically characterized by the accumulation of very long chain fatty acids (vlcfa) in tissues and biological fluids. the major accumulated acids are hexacosanoic acid (c26:0) and tetracosanoic acid (c24:0). the disorder is characterized clinically by central and peripheral demyelination and adrenal insufficiency closely related to the accumulation of fatty acids. the incidence of x-ald is estimated to be 1:25,000 males. at least six phenotypes can be distinguished. the most common phenotypes are childhood cerebral ald and adrenomyeloneuropathy (amn). the recommended therapy consists of the use of the glyceroltrioleate/glyceroltrierucate (gto/gte) mixture, known as lorenzo's oil, combined with a vlcfa-poor diet. there are alternative treatments such as bone marrow transplantation and immunosuppression, as well as the use of lovastatin and sodium phenylacetate. in the present study we report the clinical and biochemical course of 7 male patients with x-ald treated with lorenzo's oil and a vlcfa-restricted diet. treatment produced 50% reduction in c26:0 and 42.8% reduction in the c26:0/c22:0 ratio. most patients remained clinically well, although approximately 30% of them presented a rapid clinical deterioration. the results showed a poor biochemical-clinical correlation for treatment, indicating that new therapies for x-ald are needed in order to obtain a better prognosis for patients.
Fitonematoides associados a frutíferas na regi?o Noroeste do Paraná, Brasil

Abstract: a survey was carried out in order to study the occurrence of plant parasitic nematodes in fruit crops in the northwest of paraná, from december 2007 to february 2009. a hundred and twenty four soil and roots samples were collected from 19 species of fruit crops in 15 different municipalities. nematodes were extracted from the samples and identified under an optical microscope. nine genera of plant parasitic nematodes were found. in citrus, the most abundant species was tylenchulus semipenetrans, but it was also recovered meloidogyne, pratylenchus, helicotylenchus, xiphinema, trichodorus, mesocriconema and dolichodorus. in the other fruit crops, meloidogyne, pratylenchus, helicotylenchus and hemicycliophora were recovered. pratylenchus brachyurus was most abundant in pineapple crops, meloidogyne incognita in fig and persimmon, and helicotylenchus dihystera and h. multicinctus in banana. the main plant parasitic nematode genera were found in approximately 50% of the samples collected, which may represent a possible threat to regional fruit production if not managed adequately.

Abstract: background: thyroid nodules are a very common clinical problem and fine-needle aspiration biopsy (fnab) is its main diagnostic test. recently, it has been shown that some ultrasonographic features are associated to an increased risk of cancer, but a comparison between fnab and ultrasonography (us) or its combination is not available. the aim of this study was to evaluate fnab and us performance, isolated or combined, for cancer diagnosis in thyroid nodules. methods: eighty patients, with palpable solitary thyroid nodules or multinodular goiters, who were to undergo surgery, were selected among 289 patients seen in our clinic between march, 1995 and july, 1997. us and fnab were performed by one of us. us was considered positive for cancer when microcalcifications, absent halo or hypoechogenicity were seen. fnab was considered positive for cancer when it was either suspicious or compatible with malignancy. both methods (fnab and us) were evaluated in parallel combination (positive when fnab or us was positive) and in serial combination (positive when both, fnab and us, were positive). results: sensitivity and specificity of fnab and us for cancer diagnosis in thyroid nodules were, respectively, 87% and 62%, and 81% and 70%. serial combination of fna and us had sensitivity of 69% and specificity of 91%, and parallel combination resulted in sensitivity of 94% and specificity of 52%. conclusion: the us had good accuracy in the evaluation of thyroid nodules, and can help us take some decisions in the clinical practice, supplementary to fna. nevertheless, the quality of the ultrasound examination depends on the experience of the examiner and on the characteristics of the equipment, so new studies are necessary to evaluate these factors to the current usage of ultrasonographic characteristics in the evaluation diagnostic of thyroid nodules.

Abstract: this essay aims to verify if the notion of "hyperenunciator", proposed and developed by maingueneau (2005a, 2008) in his last works on discourse analysis, would have any correlation with the generic notion of "third" (investigated in its variants) in the theory of bakhtin"s circle. the proposition here is to examine an approximation or inspiration to review issues associated to established genres, including the authorship, seeking to establish their mode of operation and the implications as regards the process of authorship. it is noted, in conclusion, that the understanding of superaddressee (as a "third virtual person") goes from an instance bound to the ego, a double of the enunciator, up to an external body, regardless of the enunciator (the third with collective mark), as a set of rules that one must follow for the acceptance of the text (in a community of discourse), extending to a form of inner speech until it, eventually, coincides with the hyperenunciator.

Abstract: in this article the issue of the "primacy" of one of the terms of the literal sense/metaphorical sense dichotomy is discussed. beginning with a brief historical account, a few relevant points are considered to articulate the theme, and a thesis about the functioning of the metaphor is examined, from the viewpoint of french discourse analysis. from this perspective, we believe that a variety of phenomena converge to produce metaphors, which leads to the possibility of the dilution of the dichotomy. thus, metaphors originate from the circulation of linguistic-discursive contents among the discursive formations, here understood as instances of formation/repetition/transformation of knowledge elements -components of a memory of discourses -, a knowledge that is constantly reconfigured and that is lead to incorporate elements produced outside of a given formation, thus producin g varied meaning effects.

Abstract: in this work i present a tentative panel for the contextualization of the epistemological significance of the dialogical concept and of the extended concept to the genres of discourse in bakhtin, and their implications and consequences for communal life, with an emphasis on ethics. the panel unfolds to present a niche in which genres of discourse work as a unit of knowledge that only makes sense within social practice. from that point i call attention to some implications in relation to their relevance along our quotidian acts, especially those acts of professional character, based on the analysis and reflection on experiences of success and failure, considering the pragmatic categories of bakhtin's architectonics.

Abstract: this central theme of this theoretical essay revolves around institutions and their importance for the economic development of places and nations. initially, starting from a review of a theoretical perspective that relates organizations and institutions - institutionalist theory - we analyze both its contributions and some of the main criticisms that have been made of it, particularly with regard to economic orientation. with the objective of providing possible responses to some of its major and most emphatic critics (coming mostly from the social sciences), this is followed by the indication of an alternative route to the construction of efficient institutions, situated within studies of social capital and its influence on the relations amongst the diverse agents involved in a particular transaction. in our conclusions, our article reinforces the importance that institutions have for regional development and emphasizes the possibility of using social capital for the building of efficient institutions. we carry out a comparative analysis of both perspectives and conclude that the most intelligent option is to take advantage of the best that each can offer, thus recommending their complementary use whenever possible. hence, our conclusions reinforce the thesis of the importance of institutions and of all the social relations that are involved in their creation and consolidation.
