Estudo rand?mico da corre??o cirúrgica do prolapso uterino através de tela sintética de polipropileno tipo I comparando histerectomia versus preserva??o uterina

Abstract: objectives: to compare surgical morbidity and time, as well as anatomical outcomes between vaginal histerectomy and uterine preservation in the treatment of uterine prolapse using a mesh kit (nazca ?). methods: randomized controled trial with 31 women with uterine prolapse pop-q stage 3 or 4 pelvic organ prolapse who underwent vaginal surgery using tipe i polypropilene mesh (nazca ?). they were randomized in two groups: group hv: hysterectomy and pelvic reconstruction floor with mesh (n=15); group hp: hysteropexy and pelvic reconstruction floor with mesh (n=16). race, miccional urgency, intestinal constipation, sacral pain were assessed as well as the amount of bleeding and time of operation. results: median follow-up was nine months on both groups. no difference was observed on complication rates and functional outcomes. operation time was 120 minutes on group hv, versus 58.9 minutes on group hp (x2 = 17.613*, p < 0.001 ) and intraoperative blood loss was 120 ml on group hv versus 20 ml on group hp (x2 = 19.425*; p < 0.001). there was no differences in relationship to anatomical cure rates. objective success rate was 86.67% to group hv and 75% to group hp (p=0,667) at nine months of follow-up. the anatomical results were similar between the two groups. conclusion: the anatomic results between histeropexy and hysterectomy were similar. however, surgical time and blood loss were greater in group with histerectomies. the erosion rate were also similar. vaginal surgery using mesh is an effective procedure for pelvic organ prolapse.

Abstract: objective: pelvic organ prolapse is a disorder caused by the imbalance between the forces responsible for supporting the pelvic organs in their normal position and those that tend to expel them from the pelvis. anterior vaginal wall prolapse, known as cystocele, is the most common form of prolapse and can result from lesions in different topographies of the endopelvic fascia. currently, a woman has an 11% risk of being submitted to a surgical procedure to correct pelvic floor disorder, and a 29% chance of being reoperated due to failure in the first surgery. methods: a prospective randomized study was conducted to compare the use of polypropylene mesh with site-specific repair in the surgical treatment of anterior vaginal prolapse. thirty-two patients aged between 50 and 75 years, who had previous vaginal prolapse at stage iii or iv, or prolapse recurrence, were operated. mean follow-up was 8.5 months. results: the results demonstrate the superiority of the anatomical outcomes with the use of polypropylene mesh over site-specific repair. regarding surgical morbidity, shorter operative time was observed for the mesh group. conclusion: the results observed in this study indicate the superiority of anatomical results obtained with the use of polypropylene mesh over site-specific repair.

Abstract: background. chronic pain represents a significant obstacle in maintaining function and independence. previous studies have shown that music can improve motivation and increase feelings of control in patients with chronic pain. objective. to examine the influence of a musictherapy program on chronic low back pain patients related to coping strategies. material an method. 14 patients from the pain unit in the san pedro hospital of logro?o were included in a music-therapy program during six weeks with a dignosis of chronic low back pain. subjects were weekly visited by a music therapist, 60 min per sesión. all the patients answered a questionnarie to evalúate the used coping strategies to pain (cad), in the first and the end visits. in adittion, a severity pain scale (vas) was answered twice in each interventional session. results. the pain coping strategy more frecuently used was information seeking. all strategies shown better results at the end of the program, but only two of them had significant results: information seeking, catarsis, and distraction. conclusión. the music-therapy program increased the more active pain coping estrategies that are possitively associated to a better degree of adjustment of the patient with chronic low back pain.
Freqüência de Neoplasia Intra-epitelial Cervical em Portadoras do Vírus da Imunodeficiência Humana

Abstract: Objetivo: verificar a freqüência de neoplasia intra-epitelial cervical (NIC) em mulheres infectadas pelo vírus da imunodeficiência humana (HIV). Métodos: foram estudadas 99 mulheres HIV-soropositivas; o diagnóstico da infecc o pelo HIV foi realizado por meio de dois testes ELISA, complementados por teste Western blot ou de imunofluorescência indireta. Como grupo controle foram analisadas 104 mulheres que n o apresentavam positividade no teste ELISA. Em ambos os grupos o rastreamento de NIC foi realizado por meio da associa o de colpocitologia oncológica e colposcopia. Nos casos em que a colposcopia revelou existência de zonas de transforma o anormal, o diagnóstico de NIC foi realizado mediante biopsia dirigida, complementada ou n o por coniza o. Resultados: em 15 das 99 pacientes do grupo de estudo (15,2%) foi encontrada neoplasia intra-epitelial cervical, sendo dez casos de NIC I, um de NIC II e quatro de NIC III. Entre as 104 mulheres do grupo controle, quatro (3,8%) eram portadoras de neoplasia intra-epitelial cervical, encontrando-se um caso de NIC I e três de NIC III. Conclus o: a análise comparativa dos resultados evidenciou que a freqüência de neoplasia intra-epitelial cervical foi significantemente mais elevada entre as pacientes infectadas pelo HIV.

Abstract: Introducción. El dolor crónico supone un importante obstáculo para mantener la funcionalidad y la independencia. Estudios previos han demostrado que la música puede mejorar la motivación y los sentimientos positivos de autocontrol frente al dolor. Objetivos. Descubrir qué estrategias de afrontamiento frente al dolor crónico son las más utilizadas por los pacientes y cómo se modifican al aplicar un programa de musicoterapia, en una muestra de pacientes con dolor lumbar crónico Material y métodos. 14 pacientes de la Unidad de Dolor del Hospital San Pedro de Logro o con dolor lumbar crónico fueron admitidos a un programa de musicoterapia durante seis semanas. Las sesiones fueron semanales, de una hora de duración, dirigidas por un musicoterapeuta que aplicó diferentes protocolos. El Cuestionario de Afrontamiento al Dolor CAD fue autoadministrado a los pacientes al inicio y al término del programa de musicoterapia. La intensidad del dolor se midió al principio y al final de cada sesión mediante la escala EVA. Resultados. La estrategia más utilizada frente al dolor fue la búsqueda de información. Todas las estrategias fueron más utilizadas al finalizar el programa de musicoterapia, aunque las diferencias fueron significativas sólo en tres de ellas: Búsqueda de información, Catarsis y Distracción. Conclusiones. Nuestro programa de musicoterapia resultó una herramienta efectiva para disminuir el dolor lumbar crónico al ser capaz de afectar a la percepción del dolor en su componente cognitivo, permitiendo a los pacientes desviar el foco del dolor del estimulo aversivo y mostrando una mayor destreza en el uso de estrategias de afrontamiento que permiten una adaptación positiva al dolor crónico. Abstract: Lab-on-a-chip immuno assays utilizing superparamagnetic beads as labels suffer from the fact that the majority of beads pass the sensing area without contacting the sensor surface. Different solutions, employing magnetic forces, ultrasonic standing waves, or hydrodynamic effects have been found over the past decades. The first category uses magnetic forces, created by on-chip conducting lines to attract beads towards the sensor surface. Modifications of the magnetic landscape allow for additional transport and separation of different bead species. The hydrodynamic approach uses changes in the channel geometry to enhance the capture volume. In acoustofluidics, ultrasonic standing waves force μm-sized particles onto a surface through radiation forces. As these approaches have their disadvantages, a new sensor concept that circumvents these problems is suggested. This concept is based on the granular giant magnetoresistance (GMR) effect that can be found in gels containing magnetic nanoparticles. The proposed design could be realized in the shape of paper-based test strips printed with gel-based GMR sensors.
Tunnel magnetoresistance in alumina, magnesia and composite tunnel barrier magnetic tunnel junctions

Abstract: Using magnetron sputtering, we have prepared Co-Fe-B/tunnel barrier/Co-Fe-B magnetic tunnel junctions with tunnel barriers consisting of alumina, magnesia, and magnesia-alumina bilayer systems. The highest tunnel magnetoresistance ratios we found were 73% for alumina and 323% for magnesia-based tunnel junctions. Additionally, tunnel junctions with a unified layer stack were prepared for the three different barriers. In these systems, the tunnel magnetoresistance ratios at optimum annealing temperatures were found to be 65% for alumina, 173% for magnesia, and 78% for the composite tunnel barriers. The similar tunnel magnetoresistance ratios of the tunnel junctions containing alumina provide evidence that coherent tunneling is suppressed by the alumina layer in the composite tunnel barrier.

Abstract: In the 2000s, the financial crisis in Argentina and several nationalizations carried out by governments in South America have spawned a large number of claims before International Centre for Settlement of Investment Disputes (ICSID). Latin Americans began to look at ICSID critically, there have been complaints about ICSID's connections with the World Bank; non-commercial interests, such as health or environmental protection, have not received adequate attention in the arbitration cases; a lack of transparency by arbitration panels; and the absence of an appeals process, but only a limited annulment procedure. This situation generates impacts upon the overall due process of ICSID arbitrations and Latin American hostility against ICSID, such the Bolivarian Alliance for the Peoples of our America Peoples'Trade Treaty (ALBA-TCP) Declaration to denounce ICSID Convention and the firm intention of Union of South American Nations (UNASUR) countries to create a regional arbitration centre to replace ICSID arbitration.
