Curvas de complacência ou curvas press?o-volume na insuficiência respiratória aguda

Abstract: the goal of this paper is to review the use of pressure-volume (p-v) curves or compliance curves in patients with acute respiratory failure (arf) - acute lung injury (ali), and acute respiratory distress syndrome (ards). the most important papers published in english language literature concerning p-v curves in arf were found in medline and reviewed. ideally, recommendation is made to calculate p-v curves in all patients with arf in order to achieve adequate monitoring and management considering data obtained from the curves, such as lower inflexion point, upper inflexion point, and static compliance. the methods used to obtain p-v curves are supersyringe, multiple inspiratory occlusions and continuous flow. all of them are similar if low continuous flow is used and if care is taken to allow lung emptying and to avoid intrinsic peep (positive end expiratory pressure) before accomplishing the curves. in this way continuous flow can be a simple, inexpensive, secure and reliable method to be carried out at bedside in order to obtain p-v curves that allow for a better monitoring and management of patients with arf.

Abstract: the goal of this paper is to review the use of ct scan in acute respiratory failure (arf), acute lung injury (ali) and acute respiratory distress syndrome (ards). the most important papers studying ct in arf, published in english language literature, were found in medline. the use of ct in patients with arf let allowed the authors to proceed to a qualitative evaluation of lung morphology looking for diffuse an/or localized hyperdensities, and for other concomitant abnormalities (pleural effusion, pneumothorax). it is also possible to evaluate lung density histograms and calculate the gas-tissue ratio within the lungs. likewise, one can also calculate total and regional lung volumes (aerated, poorly-aerated, non-aerated, and overdistension volumes). knowing those volumes it is possible to quantify alveolar recruitment and overdistension due to ventilatory strategies as the use of different positive end expiratory pressure (peep) levels.

Abstract: abstract: the article analyzes partial data from a survey made with young catholics in parishes and young people pentecostal churches in the baixada fluminense in rio de janeiro. the survey also discussed representation of politics and notions of development. the purpose of this article is to introduce elements that favor the outline of the profile of religious youth; provide understanding on ways of identity construction and even youth behavioral aspects such as sexuality and drugs. the results indicate similarities and differences of these young people in the two segments analyzed, and when compared to metropolitan youth in general.

Abstract: the article has for objective to reflect on the polypharmacy of the aged with emphasis in the adverse drug reactions and the drug-drug interactions. the vulnerability of the elderly to the problems related from the use as of drugs is pretty upswing, the one to in case that must the complexity of the problems clinical, at the must as of need pharmacotherapeutic interventions, and to the update pharmacokinetic and pharmacodynamic changes inherent to the aging. in this way, to rationalize the drugs utilization and to avoid the injury related to the polypharmacy will be, doubtless, one of the biggest challenges for public health in this century.

Abstract: nowadays drugs interactions constitute one the most important subjects of pharmacological for clinical practive of health professionals. the concomitant use of many drugs as therapeutic strategy and the growing number of agents available contribute to enlarge the benefical effects of therapy. besides, these factors can also permit the mutual interference of pharmacological actions that result in alteractions of therapeutics effects. the aim of this article is to review the principles pharmacological related to the mechanisms of drugs interactions, to describe the types of interactive drugs; the groups of risk patients and to suggest specific nursing interventions aiming the prevention of occurrence of adverse reactions from accidental interactions.

Abstract: this article analyzes brazilian recent research in public administration, through the review of papers published at thematic journals rap and rsp and enanpad's proceedings, from 1995 to 2002. it takes into consideration prior reviews, that pointed out the normative approach of the public administration research as its major weakness. the present work takes this issue in a different perspective: it considers the normative approach as inherent to this essentially applied field. other aspects are noteworthy, in order to improve the research is this field: the researchers community seems to be self-referred, the subjects are extremely dispersed, and the theories from other disciplines are acritically and too easily adopted. this article discusses the relevance of the field, considering the need of knowledge on governments and governance, to develop public reformers.

Abstract: Este artigo discute a gênese das agências reguladoras (ARs) no Brasil, as propostas para revis o do modelo e as distintas formas de controle sobre estes novos entes, dotados de autonomia. No processo de cria o das ARs, entre 1996 e 2002, o modelo adotado para a área de infra-estrutura foi indevidamente estendido às demais agências. Tal inadequa o n o foi corrigida nas propostas de revis o do modelo, encaminhadas pelo novo governo ao Congresso. Após fazer as distin es necessárias entre a experiência dos EUA e o contexto brasileiro, o artigo recupera, da experiência norte-americana, a distin o entre várias formas de controle (hierárquico, político e social). No Brasil, no debate sobre o grau de autonomia das ARs s o confundidas as formas de controle, muito freqüentemente denominando "controle político" aquilo que é de fato controle hierárquico, e "controle social" como sin nimo de controle político. Para a autora, n o há antinomia entre independência da agência e controle político, mas sim entre controle hierárquico e independência; também n o é apropriado confundir controle social e controle político. A cria o das agências sob um modelo único e a indistin o entre as formas de controle podem ser explicadas pela combina o de características do sistema político-institucional brasileiro, com preferências e resistências de atores intragovernamentais - especialmente do Executivo federal. This article discusses the genesis of the regulatory agencies (RAs) in Brazil, the model modification proposals and the different forms of control over these new entities. During the RA creation process, between 1996 and 2002, the infrastructure sector model was improperly extended to the other agencies. This inadequacy was not corrected by the model reviews proposed to Congress by the new government. After comparing the American and the Brazilian contexts, the article retrieves from the American experience the differences between the forms of control: hierarchical, political, and social. In Brazil, in the discussion about the extent of AR autonomy the forms of control are confused, and very often what is called 'political control' is in fact hierarchical control, and 'social control' is a synonym for political control. The author believes that agency independence and political control are not antonymous, but hierarchical control and independence are. It is also wrong to confuse social control and political control. Abstract: O objetivo do presente trabalho é revisar o uso das curvas de complacência ou curvas press o-volume (P-V) de pacientes com insuficiência respiratória aguda (IRA) - les o pulmonar aguda ou acute lung injury (ALI) e síndrome da angústia respiratória aguda (SARA). Foram revisados os principais trabalhos publicados na literatura em língua inglesa, localizados por pesquisa via Medline, que abordavam o uso das curvas P-V na IRA. Idealmente as curvas P-V devem ser realizadas em pacientes com IRA visando uma monitoriza o e um manejo mais adequado dos mesmos a partir dos dados por elas fornecidos: ponto de inflex o inferior (Pinf), ponto de inflex o superior (Psup) e complacência estática. Todos os métodos disponíveis para a obten o das mesmas (superseringa, oclus es inspiratórias e fluxo contínuo) s o similares desde que fluxos contínuos baixos sejam utilizados e sejam tomados cuidados que permitam o esvaziamento pulmonar antes de realizar as curvas, evitando o surgimento de PEEP (press o expiratória final positiva) intrínseca. Com estes cuidados o fluxo contínuo torna-se um método simples, n o dispendioso, seguro e confiável, para realizar curvas P-V à beira do leito, facilitando a monitoriza o dos pacientes com IRA.

Abstract: this paper presents an analysis of the so-called passive-se constructions, compared with canonical passives and other se-constructions (inherent, reflexive, reciprocal, etc.) in texts written by portuguese authors born between the 16th and 19th centuries. the aim of the study is to show that the change from passive-se into indefinite-se may be related to the attested change that involves the subject position. according to galves and paix?o de sousa (2005, 2010), in the classical portuguese (16th and 17th centuries) grammar the left position of the verb is a position to where topicalized constituents move, including subjects, so the base position of the subject is post verbal. from the 18th century on, a shift occurs and the subject is preferably pre-verbal and topicalized constituents occupy the left periphery. the quantitative analysis of the evolution of se-constructions in comparison to canonical passives over time demonstrates that the change that occurs in se-constructions - from passive-se to indefinite-se - has been triggered by the change in subject position.
