Avalia??o antropométrica e consumo alimentar em crian?as menores de cinco anos residentes em um município da regi?o do semiárido nordestino com cobertura parcial do programa bolsa família

Abstract: objective: the objective of this study was to assess the health and nutritional status of children under five years of age and to associate the quality of the foods consumed with the bolsa família program in a city located in the brazilian semi-arid region. method: a total of 189 children from a sample of 411 households in the city of jo?o camara (rn) were assessed. weight and height were measured and socioeconomic and food habits were determined with the use of questionnaires. the nutritional status of the children was determined with the weight-for-age, height-for-age and weight-for-height indicators. univariate analyses were done and bivariate and multivariate logistic regression models were constructed to test the hypothesis of the study. results: of the studied children, 4.3% were underweight, 9.9% were stunted and 14.0% were overweight. the nutritional status of children whose families receive the bolsa família financial aid was not significantly different from those whose families do not receive the aid. in both groups, the consumption of fruits and non-starchy vegetables was low and similar. children from families who receive the aid were 3 times more likely to eat junk food (or 3.06 - ci 1.35-6.95). conclusion: the food intake patterns of this population indicate that they are at food and nutritional risk. health professionals need to intervene in order to promote healthier eating habits.

Abstract: objectives: to describe the strategy of the "sisvan-sp" applicative implementation in the state of s?o paulo and analyze the information generated by the system concerning the magnitude of nutritional deviations in children under five years old. methods: primary health units in the municipalities were in charge for collecting and entering data using an information system named "sisvan-sp" an applicative developed by the health institute of the health secretariat of the state of s?o paulo (ses/sp). monthly available data of the system are: health regional management, municipality, primary health unit, and medical file number, date of birth, date of visit, gender and classification of the children's nutritional condition. children with weight/age < percentile 3 were considered undernourished and children with age/weight > 97 of the reference curve from the national center of health statistics were considered in risk of overweight. results: malnutrition prevalence varied from 3.1% to 4.2% and overweight risk from 4.0 to 4.8% in the different municipalities; information quality and system coverage, even when variable were consistent with epidemiological information. conclusions: the "sisvan-sp" applicative developed by ses/sp and used to monitor the nutritional condition of children up to five years old seen at the primary health units was considered useful to assess and discuss aspects of the food and nutrition surveillance system (sisvan), such as data coverage and quality. results support decision making aimed at children's weight prevention and control.

Abstract: this study assessed the prevalence of overweight in public school first graders and the association between overweight and socio-environmental factors in a city in greater metropolitan s?o paulo, brazil. children enrolling in public school first grade in cajamar in 2005 were assessed. overweight was defined according to body mass index, by gender and age. socio-environmental information was obtained using a questionnaire applied to 58.6% of the children's mothers. poisson regression models were used to evaluate associations between overweight in schoolchildren and socio-environmental variables. seventeen percent of the children were overweight. factors associated with overweight were: mother's obesity (pr = 3.54; ci: 2.16-5.80), high consumption of junk food (pr = 2.12; ci: 1.30-3.45), more than 4 hours a day watching tv (pr = 2.08; ci: 1.03-4.20), and daily household per capita availability of vegetal oil (around 3 tablespoons) (pr = 1.52; ci: 1.01-2.31). the family environment has a strong influence on overweight in children entering first grade in public schools. prevention of childhood overweight should preferably target the parents and other caregivers.

Abstract: this study aims to evaluate the situation with breastfeeding in the state of piauí, brazil, at the time of the 2006 vaccination campaign. the sample consisted of 1,963 infants (< 1 year of age) from 45 municipalities. medians and probabilities of breastfeeding categories were calculated by probit analysis. multivariate logistic analysis was used to analyze the association between independent variables and partial and exclusive breastfeeding. the probabilities were: 16% for exclusive breastfeeding and 18% for predominant breastfeeding at 180 days of age and 58% for breastfeeding at 361 days. median duration was 67 days for exclusive breastfeeding and 200 days for breastfeeding. the breastfeeding rate was higher for children whose mothers belonged to social classes c, d, and e (p = 0.024) and lived in the countryside (p = 0.009). not nursing during the first 24 hours of life, using a pacifier, and bottle-feeding reduced the prevalence of breastfeeding (p < 0.001). the prevalence rates are below the recommended standards and reflect the need to prioritize actions to promote breastfeeding in the state.

Abstract: objective: to analyze indicators of structure, processes and results of healthcare to adults, specifically to diabetes mellitus, arterial hypertension, taking into consideration the regionalization of the unified health system in the state of s?o paulo. methods: the analyses were performed for five health districts. structure indicators (human development index, hospital index, intensive care bed index, outpatient index, human resources index, type of management) process indicators (family health program-fhp/strategy coverage, basic and specialized appointment coverage, invasion/evasion index, and management effort) and outcome indicators (% of early mortality by stroke, acute myocardial infarction, heart failure, and diabetes mellitus complications) were analyzed. results: the district of mogi das cruzes presented the highest mortality rates for the fours aspects assessed and also for early death. s?o josé do rio preto achieved the lowest values for early death, among all of health districts studied, which could be related to the adequacy of health offer, coverage by fhp, basic and specialized appointment coverage, and more efficient integration mechanism of regional health services. final considerations: the comprehensiveness in healthcare to diabetes mellitus and hypertension seems to depend on a design technically suitable for the effectiveness of the regional health planning of the services and adequate practices of management and care.
Efeitos do clampeamento tardio do cord?o umbilical sobre os níveis de hemoglobina e ferritina em lactentes aos três meses de vida

Abstract: this study assessed the effect of delayed (1 minute after delivery) clamping of the umbilical cord on hemoglobin and ferritin levels in infants at three months of age. mothers and their infants born through vaginal delivery, at term, and without congenital anomalies (325 pairs) were recruited at a hospital in s?o paulo, brazil, in 2006 (164 in the delayed clamping subgroup and 161 in the early clamping subgroup). maternal hemoglobin at delivery, umbilical cord hemoglobin, and ferritin were recorded. at three months follow-up, venous blood samples were drawn from 224 (69%) infants for hemoglobin and ferritin measurement. socioeconomic, maternal reproductive, anthropometric, and infant feeding variables were studied. multiple linear regression models were used to analyze the data. the effect of delayed clamping at birth, measured at three months, was only significant for ferritin (p = 0.040), and the concentration was higher (23.29ng/ml) in this subgroup as compared to the early clamping subgroup. delayed umbilical cord clamping can serve as a strategy to improve infant iron status and prevent iron deficiency.

Abstract: redemocratization in the 1970s brought about changes in the public health system in brazil. the unified national health system (sus) was implemented in 1988, based on local and regional administrative systems. this was an important step that resulted in greater local control and the introduction of new technologies. this study focuses on the local health system in campinas, a relatively affluent and technologically advanced region some 100km from the city of s?o paulo in southeast brazil. qualitative and quantitative methods were used to evaluate the work of 233 nurses in the local health system. the first nurse was hired in 1977, and now there are 53 nurses working in specific areas: 10 in the central health administration, 36 in district administrations, and the rest in local services. nursing services are organized in six main areas: administration, coordination of human resources, education, information technology, health surveillance, and health administration. nursing intervention may be found in various areas, but it generally focuses on medical consultations, and there is little emphasis on collective activities and health promotion. local health objectives can only be achieved by creating new practices that promote collective health.

Abstract: Desde a década de 70 o sistema de saúde vem se transformando com a redemocratiza o do Estado Brasileiro. O SUS representou um importante passo para o fortalecimento dos sistemas de administra o locais e regionais. Tal situa o tem contribuído para o aumento do controle local e para as mudan as no processo de trabalho. Este estudo considera essas mudan as no sistema local de saúde em Campinas, S o Paulo, e analisa, quantitativa e qualitativamente, as práticas de 233 enfermeiras da rede municipal em rela o às políticas de saúde locais. Cerca de 58% trabalham em servi os locais e 42% em servi os especializados, em níveis distritais e central de administra o. Os servi os de enfermagem organizam-se em seis áreas principais: administra o, coordena o de recursos humanos, educa o de pessoal, informática, administra o em saúde e vigilancia em saúde. A interven o das enfermeiras tem se dirigido às diversas áreas, mas continua centrada nas consultas médicas, com pequena ênfase em atividades coletivas e de promo o à saúde. Concluímos que novas práticas de promo o à saúde coletiva precisam ser fomentadas de acordo com os objetivos de desenvolvimento da saúde em ambito local.
Influência regional no consumo precoce de alimentos diferentes do leite materno em menores de seis meses residentes nas capitais brasileiras e Distrito Federal

Abstract: the aim was to assess regional influences on food consumption in infants less than six months of age. a cross-sectional study was conducted in a sample of 18,929 infants participating in the second survey on breastfeeding prevalence in brazilian state capitals and the federal district in 2008. consumption rates for tea, fruit juices, formula milk, and porridge were calculated for the state capitals from the five geographic regions of the country. food consumption was estimated by logit analyses and poisson models. differences in food consumption profile were observed between the different regions: tea was more common in state capitals in the south (rp = 2.37), while non-maternal milk (rp = 1.50 and 1.47) and juices (rp = 1.57 and 1.55) were more frequent in the northeast and southeast, respectively. porridge was more common in the northeast (rp = 3.0). brazil's geographic regions thus display different infant feeding patterns. public policy should take cultural diversity into account when planning strategies to improve infant nutrition and health.

Abstract: objective: the objective of the study was to analyze label conformity of foods for infants and lactating women according to ethical principles and current regulations. methods: a total of 86 labels acquired by free access and intentional sampling were analyzed, distributed as follows: infant formulas (n=11), follow-on formulas (n=5), complementary foods (n=07), cereal products (n=11), milk and foods containing vegetables (n=52), and food commonly used to feed this population (n=13). structured forms were filled out with items from the resolu??es da diretoria colegiada (graduated board resolutions) 222/02, 977/98, 40/01, 40/02, 259/02, 23/00 and of the portarias (rules) 34/98 and 36/98 for each food. the chi-square test, fisher's exact test and correlation test were used for data analysis. results: the rate of specific label non-conformity was much greater than that of general labeling. the highest rate of non-conformity was observed in the presentation of illustrations of the foods (images of breastfeeding infants or children with humanized images). other non-conformities were: the presence of words such as humanized milk, baby or phrases that give a false idea of advantage or safety; the absence of mandatory phrases and non-conformity regarding the composition of the food. foods that did not have as sales designation any of the characteristics of common use in this age group, that is, that were not marketed as such, such as lacteal flour, cereal flakes and paps presented warning phrases that were not necessarily for that kind of product. conclusion: labels of foods for breastfeeding infants and toddlers are irregular in many ways, especially regarding the specific labeling of the product. this practice may influence breastfeeding. more inspection is needed as well as more explanations to the food industries and to the consumers in general.
