Diagnostic Utility of Sago-Like Nodules on Gross Thoracoscopic Appearance in Tuberculous Pleural Effusion and Their Correlation with Final Histo-Microbiologic Findings  [PDF]

Abstract: Background: Distinguishing tuberculous pleural effusion (TPE) from other causes of exudative effusion is often challenging. Delay in treatment initiation can occur while awaiting histo-microbiologic confirmation owing to the paucibacillary nature of the disease. Sago-like nodules are the most common visual finding on gross thoracoscopic appearance. The primary objective was to determine the diagnostic utility of the presence of sago-like nodules on gross thoracoscopic appearance in TPE to help justify early initiation of tuberculosis (TB) treatment based on their finding while awaiting final histo-microbiologic confirmation. Secondary objective was to study the correlation between the presence of sago-like nodules and the final histo-microbiologic findings in pleural biopsy specimens. Methods: This was a retrospective-descriptive study of all patients with exudative pleural effusion who underwent diagnostic medical thoracoscopy (MT) at Hamad General Hospital during an eight-year period (from January, 2008 to December, 2015). Results: The presence of sago-like nodules on gross thoracoscopic appearance of the pleural surface had a sensitivity of 58%, a specificity of 89% and a positive predictive value of 97% for TPE with a diagnostic accuracy of 62%. There is significant association between the presence of sago-like nodules and demonstration of granulomatous inflammation in pleural biopsy specimens (P = 0.000). There is no association between sago-like nodules and positive TB smear and culture in biopsy specimens. Conclusion: The presence of sago-like nodules on gross thoracoscopic appearance has a high specificity and positive predictive value for TPE and significantly correlates with the presence of granulomatous inflammation. Patients from TB prevalent areas with exudative pleural effusion and sago-like nodules on gross thoracoscopic appearance may be commenced on TB chemotherapy while awaiting final histologic confirmation.

Abstract: Pulmonary arterial hypertension (PAH) is a disease that leads to characteristic vascular wall remodeling and hemodynamic alterations. Consequently, this pulmonary vascular disease contributes to substantial morbidity and mortality in afflicted patients. PAH may be idiopathic in nature or associated with connective tissue disease, chronic liver disease, human immunodeficiency virus, congenital heart disease, and a growing list of other conditions. There are currently nine Food and Drug Administration-approved therapies for specific PAH treatment. Therapeutic targets include prostacyclin replacement, endothelin-1 antagonism, and phosphodiesterase-5 inhibition. This article focuses on the prostanoid treprostinil and explores its role in the management of patients with PAH.
NLO Uncertainties in Higgs + 2 jets from Gluon Fusion

Abstract: A central ingredient in establishing the properties of the newly discovered Higgs-like boson is to isolate its production via vector boson fusion (VBF). With the typical experimental selection cuts, the VBF sample is contaminated by a 25% fraction from Higgs + 2 jet production via gluon fusion (ggF) which has large perturbative uncertainties. We perform a detailed study of the perturbative uncertainties in the NLO predictions for pp -> H+2 jets via ggF used by the ATLAS and CMS collaborations, with the VBF selection cuts of their current H -> gamma gamma analyses. We discuss in detail the application of the so-called "ST method" for estimating fixed-order perturbative uncertainties in this case, and also consider generalizations of it. Qualitatively, our results apply equally to other decay channels with similar VBF selection cuts. Typical VBF selections include indirect restrictions or explicit vetoes on additional jet activity, primarily to reduce non-Higgs backgrounds. We find that such restrictions have to be chosen carefully and are not necessarily beneficial for the purpose of distinguishing between the VBF and ggF production modes, since a modest reduction in the relative ggF contamination can be easily overwhelmed by its quickly increasing perturbative uncertainties.

Abstract: In this paper, a novel harmonic modeling technique by utilizing the concept of multi-terminal components is presented and applied to frequency scan analysis in multiphase distribution system. The proposed modeling technique is based on gathering the same phase busses and elements as a separate group (phase grouping technique, PGT) and uses multi-terminal components to model three-phase distribution system. Using multi- terminal component and PGT, distribution system elements, particularly, lines and transformers can effectively be modeled even in harmonic domain. The proposed modeling technique is applied to a test system for frequency scan analysis in order to show the frequency response of the test system in single and three-phase conditions. Consequently, the effects of mutual coupling and transformer connection types on three-phase frequency scan responses are analyzed for symmetrical and asymmetrical line configurations.

Abstract: This Article discusses a comprehensive review of biomass energy sources, environment and sustainable development. This includes all the biomass energy technologies, energy efficiency systems, energy conservation scenarios, energy savings and other mitigation measures necessary to reduce emissions. The current literature is reviewed regarding the ecological, social, cultural and economic impacts of biomass technology. This article gives an overview of present and future use of biomass as an industrial feed-stock for production of fuels, chemicals and other materials. However, to be truly competitive in an open market situation, higher value products are required. Results suggest that biomass technology must be encouraged, promoted, invested, implemented, and demonstrated, but especially in remote rural areas.

Abstract: People to rely upon oil for primary energy and this for a few more decades. Other conventional sources may be more enduring, but are not without serious disadvantages. The renewable energy resources are particularly suited for the provision of rural power supplies and a major advantage is that equipment such as flat plate solar driers, wind machines, etc., can be constructed using local resources and without the advantage results from the feasibility of local maintenance and the general encouragement such local manufacture gives to the build up of small-scale rural based industry. This article comprises a comprehensive review of energy sources, the environment and sustainable development. It includes the renewable energy technologies, energy efficiency systems, energy conservation scenarios, energy savings in greenhouses environment and other mitigation measures necessary to reduce climate change. This article gives some examples of small-scale energy converters, nevertheless it should be noted that small conventional comma i.e, engines are currently the major source of power in rural areas and will continue to be so for a long time to come. There is a need for some further development to suit local conditions, to minimise spares holdings, to maximise interchangeability both of engine parts and of the engine application. Emphasis should be placed on full local manufacture. It is concluded that renewable environmentally friendly energy must be encouraged, promoted, implemented and demonstrated by full-scale plant especially for use in remote rural areas.
Partial Quality Assessment of 60Co-Teletherapy Machine Performance  [PDF]

Abstract: The aim of this study was to assess the performance of 60Co-teletherapy unit at Radiation and Isotopes Center in Khartoum, using the common tools used for quality control (Front pointer, Graphic paper, Pin, Non-screen film, meter and Check plate). The collected and analyzed data revealed that: there was a difference between the calculated theoretical field size (FS) and the measured one relative to the change of SSD; and the average FS shift was 0.9 cm relative to the standard one, while the machine isocenter was almost fixed at 0° with a shift of 0.003 cm, which was within the standard limit (0.2 cm). The diaphragm isocenter showed a shift of ±0.36 cm (i.e. in clock and anti clock wise) in average relative to standard limit (±0.3 cm) and the couch vertical isocenter (CVI) relative to the SSD changes was exceeded the standard limit (0.2 cm) by a factor of 0.9 cm. Also the SSD determined by the optical distance indicator was greater than the actual SSD (determined by Front pointer and measured by scale meter) by an average of 0.8 cm; which exceeded the limit (0.3 cm) by an average factor of 0.5 cm. The study also showed that the penumbra profile (1.5 cm) was less than the actual specified for 60Co-teletherapy machine, while the radiation beam was so homogeneous across the field size.
